Se observó el efecto de la sustitución del aceite de girasol que se utiliza corrientemente en la preparación de las comidas en ese comedor, por un aceite obtenido de la mezcla de aceite de girasol y oleína de palma, en la proporción 70/30 (v/v) respectivamente. Después de 40 días continuos de la sustitución no hubo cambios significativos en las concentraciones de colesterol total (CT), ni del colesterol de las lipoproteínas de baja densidad (LDL) y muy baja densidad (VLDL). La concentración del colesterol de las lipoproteínas de alta densidad (HDL) aumentó significativamente (p<0,05). Los triglicéridos (TG) del plasma aumentaron en un 30%. La resistencia a la oxidación de las LDL aumentó considerablemente (p< 0,01). Hoy se considera a esta resistencia como un factor protector de gran importancia en la prevención del inicio del proceso aterogénico. Tomando en cuenta las modificaciones favorables como el aumento de colesterol de HDL sin modificación de la LDL y el claro aumento de la resistencia a la oxidación de la LDL, se considera que la oleína de palma es un aceite vegetal que puede ser utilizado sin mayores riesgos en mezcla con otros aceites que tengan una relación linoleico/palmítico más elevada como los aceites de girasol, maíz, soja y otros.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[We analyzed in 31 subjects, regular guests of the University food service of the Central University of Venezuela (UCVFS), in Caracas, the effects of replacing sunflower oil, commonly used in the preparation of meals, by a mix of sunflower oil and palm olein 70/30 (v/v) respectively. Plasma concentrations of total cholesterol, low and very low density lipoproteins were not changed after 40 days of the substitution. On the contrary, concentrations of high density lipoprotein and total triglycerides increased. The resistance to the oxidation of low-density lipoproteins increased considerably (p<0, 01). Today this resistance is considered as a protective factor of great importance in the prevention of the initiation of the atherogenic process. Taking into account the favorable modifications of HDL cholesterol and the clear increased resistance to the oxidation of LDL, we think that palm olein, mixed with other oils with a high ratio linoleic/palmític (sunflower, corn, soya an the likes), can be used as a healthy alternative in human nutrition.]]></p></abstract>

Aceptado: 18-04-2013</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>INTRODUCCI&#211;N</B></FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> La mayor&#237;a de las poblaciones actuales ingieren alimentos que suministran  de 20 a 40% de las calor&#237;as totales del d&#237;a en forma de grasas. Buena parte  de esas grasas, provienen de los aceites derivados de unas pocas oleaginosas  de alta producci&#243;n, como son: la soya, la palma aceitera, el girasol y  el olivo con composiciones de &#225;cidos grasos (AG) muy diferentes. El efecto  del consumo de aceite de palma y sus derivados, en la salud y la nutrici&#243;n  humana, es controversial por su alto porcentaje de grasa saturada de hasta  50%, fundamentalmente acido palm&#237;tico, lo cual se ha asociado con hipercolesterolemia  y aumento de riesgo de enfermedades cardiovasculares (ECV) (1-4). Este  aceite contiene tambi&#233;n un 44% de &#225;cido oleico considerado el AG m&#225;s favorables  para la salud, ya que su consumo disminuye la concentraci&#243;n de las lipoprote&#237;nas  de baja densidad (LDL-C) en plasma y las hace menos susceptibles a la peroxidaci&#243;n  (5).</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Por otro lado se ha comprobado que el aceite de palma y sus derivados contienen  altas concentraciones de antioxidantes como tocoferoles y tocotrienoles  que evitan la modificaci&#243;n oxidativa de las LDL (6). En un trabajo previo  realizado en la Secci&#243;n de Lipidolog&#237;a del Instituto de Medicina Experimental  de la UCV, se estudi&#243; en humanos, las modificaciones de las lipoprote&#237;nas  del plasma, al consumir alimentos (ole&#237;na y margarina) derivados, de la  palma africana, con los respectivos alimentos derivados del girasol (7-8).  En el presente trabajo, se aprovech&#243; la circunstancia de que en el Comedor  Universitario de la Universidad Central de Venezuela (CUUCV) se iba a introducir  el uso de la ole&#237;na de palma (OP) en reemplazo del aceite de girasol (G),  como una oportunidad de agregar a la experiencia anterior, el estudio de  otro biomarcador de gran importancia de riesgo de ECV tal como es la susceptibilidad  a la oxidaci&#243;n de las LDL (9-12).</FONT></P> </MULTICOL> <MULTICOL GUTTER="31" COLS="2">     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>MATERIALES Y M&#201;TODOS</B></FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> A treinta y un voluntarios (catorce mujeres y diecisiete hombres), usuarios  regulares (3/d&#237;a) del CUUCV, que ven&#237;an consumiendo al menos 30 d&#237;as continuos  una dieta preparada con aceite de girasol (G), se les suministr&#243; durante  cuarenta d&#237;as (40&nbsp;d&#237;as) la misma dieta pero preparada con un aceite conformado  por 70% de aceite de girasol y 30% de ole&#237;na de palma (GO). Se utilizaron  como criterios de exclusi&#243;n la presencia de diabetes, enfermedad cardiovascular,  valores plasm&#225;ticos de colesterol total (CT) y de triglic&#233;ridos (TG) mayores  de 200 mg/dL y de 150 mg/dL respectivamente (13). Las caracter&#237;sticas de  las variables antropom&#233;tricas de los voluntarios fueron: edad: 40 &#177; 10  a&#241;os, peso corporal: 70,83 &#177; 10,9 Kg, talla: 164,86 &#177; 9,5 cm, circunferencia  de cintura (CC) 85,72 &#177; 9,43 cm, e &#205;ndice de Masa Corporal (IMC): 26,08  &#177; 2,86 Kg/cm<SUP>2</SUP>. Todos los participantes firmaron previamente el consentimiento  informado.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Para los efectos de la presentaci&#243;n de los resultados, las denominaciones  <I><B>PERIODO</B></I> <I><B>G </B></I>y <I><B>PERIODO GO </B></I>correspondieron<I><B> </B></I>a los tiempos: previo y durante  la incorporaci&#243;n de la ole&#237;na de palma.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>Caracter&#237;sticas de la dieta</B></FONT></P>     ]]></body>
<body><![CDATA[<P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> La f&#243;rmula diet&#233;tica del CUUCV, era prote&#237;nas 15%, carbohidratos 55%, l&#237;pidos  30%, con un contenido cal&#243;rico de 2300 Kcal.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> El dise&#241;o global de la dieta se hizo conforme al requerimiento energ&#233;tico  del grupo, seg&#250;n las recomendaciones (FAO/ OMS/ONU) (14), repartido en  tres comidas diarias: desayuno, almuerzo y cena, de lunes a viernes.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Los fines de semana y d&#237;as feriados del <I><B>PERIODO GO</B></I>, los voluntarios ten&#237;an  absoluta libertad de consumir cualquier alimento.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>Variables bioqu&#237;micas</B></FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Inmediatamente antes del <I><B>PERIODO GO</B></I> y al final del mismo y luego de un  ayuno de 14 horas, a los voluntarios se les tomaron muestras de sangre,  para las determinaciones en plasma de CT, TG y en las lipoprote&#237;nas VLDL,  LDL y HDL, mediante m&#233;todos enzim&#225;ticos-colorim&#233;tricos (Chemoroy Texas,  EEUU).</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>Determinaci&#243;n de la composici&#243;n de AG</B></FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> La composici&#243;n de AG de las comidas y de los aceites G y GO fue determinada  por la t&#233;cnica de cromatograf&#237;a en fase gas/l&#237;quido como se explic&#243; en  publicaci&#243;n anterior (15).</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>Separaci&#243;n de las fracciones  de lipoprote&#237;nas</B></FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> El aislamiento de las diferentes fracciones de lipoprote&#237;nas, se hizo por  el m&#233;todo de Havel, modificado por Bosch y col. (16) y el de las LDL, para  el estudio de la susceptibilidad a la oxidaci&#243;n por el m&#233;todo de Havel  (17). En ambos casos se utiliz&#243; una ultracentr&#237;fuga Beckman L5-55 y un  rotor Spinco 50 Ti.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>Oxidaci&#243;n de las fracciones de LDL</B></FONT></P>     ]]></body>
<body><![CDATA[<P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Mediante una selecci&#243;n aleatoria se tomaron 17 fracciones de LDL, las que  fueron desalinizadas con una columna Pharmacia PD-10 empacada con Sephadex  G-25M (Sigma Chemical Co. St. Louis, USA) y una soluci&#243;n buffer (Tris 10  mM-NaCl 0,14 M, pH 7,2). La concentraci&#243;n de prote&#237;nas se determin&#243; de  acuerdo al m&#233;todo de Lowry modificado por Shacterlec y Pollack (18). La  oxidaci&#243;n se indujo seg&#250;n el m&#233;todo de Thomas y col. (19). El grado de  oxidaci&#243;n expresado en nmol de MDA/ mg de prote&#237;na, fue determinado cada  30 min. Por un tiempo de 3 horas, por el m&#233;todo propuesto por Kosugi y  col (20).</FONT></P> </MULTICOL> <MULTICOL GUTTER="31" COLS="2">     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> La curva del transcurso de la reacci&#243;n de oxidaci&#243;n de las LDL en <I><B>PERIODO  G </B></I>y <I><B>PERIODO</B></I> <I><B>GO,</B></I> y los par&#225;metros: fase de retardo, velocidad de la reacci&#243;n  y m&#225;xima oxidaci&#243;n fueron determinados como indica Puhl y col. (21).</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>An&#225;lisis estad&#237;stico</B></FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Los resultados son presentados como promedio &#177; desviaci&#243;n est&#225;ndar (DE).  La prueba &#147;t&#148; de Student para datos pareados se utiliz&#243; para realizar las  comparaciones estad&#237;sticas entre los valores de CT y TG, en el plasma y  en las fracciones HDL, LDL, y VLDL, as&#237; como el grado de oxidaci&#243;n en <I><B>PERIODO  G </B></I>y <I><B>PERIODO GO.</B></I> La distribuci&#243;n de los datos para los TG previo al an&#225;lisis  estad&#237;stico se normaliz&#243; a trav&#233;s de una transformaci&#243;n logar&#237;tmica. Las  dem&#225;s variables ten&#237;an una distribuci&#243;n normal.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>RESULTADOS</B></FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> La composici&#243;n de AG de los aceites G y GO es mostrada en la  <a href="#TABLA_I">Tabla I</a>. Esta  aparente contradicci&#243;n se puede aclarar tomando en cuenta, que la posici&#243;n  estereoqu&#237;mica de los &#225;cidos grasos con respecto a los tres &#225;tomos de carbono  del glicerol, influye considerablemente en el comportamiento de los &#225;cidos  grasos saturados en su potencial acci&#243;n sobre la concentraci&#243;n de LDL-C  (22-23). Seg&#250;n informe t&#233;cnico de la ole&#237;na de palma utilizada, el &#225;cido  palm&#237;tico est&#225; predominantemente en posici&#243;n 1 y 3 (99%), mientras que  en las grasas animales predomina en posici&#243;n 2, lo que hace que el comportamiento  de estos dos tipos de grasas sea distinto (24, 25).</FONT></P> </MULTICOL> <MULTICOL GUTTER="31" COLS="2">     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Por otro lado, las f&#243;rmulas predictivas de la concentraci&#243;n de colesterol  en plasma y de los &#225;cidos grasos de los alimentos se establecieron en dietas  con una presencia importante grasas animales como fuente de &#225;cido palm&#237;tico,  lo que explica que en esas condiciones el coeficiente del &#225;cido palm&#237;tico  en las f&#243;rmulas de predicci&#243;n de la concentraci&#243;n de LDL-C sea positivo  y alto. Por otra parte, las grasas animales tienen una importante proporci&#243;n  de los &#225;cidos grasos saturados l&#225;urico y mir&#237;stico que tienen una acci&#243;n  m&#225;s intensa que el palm&#237;tico en la elevaci&#243;n de la concentraci&#243;n de LDL-C  (26). En este sentido es muy ilustrativo el trabajo de Sundram y col. (27),  donde muestran experimentalmente, en un estudio doble ciego, que una dieta  rica en acido palm&#237;tico proveniente de la palma, en contraste con una dieta  rica en los &#225;cidos grasos l&#225;urico y mir&#237;stico &#150;manteniendo los dem&#225;s componentes  de la dieta iguales&#150; observaron que la dieta rica en palm&#237;tico no produjo  aumento de LDL-C, en tanto que la rica en los &#225;cidos grasos l&#225;urico y mir&#237;stico  s&#237;, en conformidad con las f&#243;rmulas de predicci&#243;n ya mencionadas.</FONT></P>     ]]></body>
<body><![CDATA[<P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Ahora bien, obs&#233;rvese la estructura de la dieta de este trabajo, en el  <I><B>PERIODO</B></I> <I><B>GO</B></I> (<a href="#TABLA_II">Tabla II</a>), n&#243;tese que el &#225;cido graso saturado predominante  es el palm&#237;tico (26% de la grasa), en buena parte aportado por la ole&#237;na  (<a href="#TABLA_I">Tabla I</a>); por el contrario, la suma de los &#225;cidos grasos l&#225;urico y mir&#237;stico  alcanza s&#243;lo el 3,6%. Por lo dem&#225;s, la presencia de16% de &#225;cido linoleico,  que baja la concentraci&#243;n de LDL-C, es importante.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Esto se ha logrado porque en este trabajo no se us&#243; la ole&#237;na de palma  para sustituir toda la grasa utilizada en la preparaci&#243;n de los alimentos,  sino solo 30%, de tal forma que en las condiciones de esta observaci&#243;n,  hay suficientes razones para que se obtenga este resultado.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> El aumento de la concentraci&#243;n de los TG del plasma de 87 a 115 mg/dL en  promedio, contrasta con lo encontrado en un estudio previo (7). No se tienen  argumentos que permitan explicar estas diferencias. Se hace destacar que  en el grupo de estudio, predominaron sujetos con un &#237;ndice de masa corporal  alto, de tal forma que alguna de las modificaciones metab&#243;licas que produce  el exceso de peso, podr&#237;a ser responsable de este resultado, lo que requiere  investigaciones posteriores para aclararlo.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Un hallazgo muy notorio e importante de este trabajo, fue el aumento de  la resistencia a la oxidaci&#243;n de las LDL, analizadas despu&#233;s del <I><B>PERIODO</B></I>  <I><B>GO,</B></I> en comparaci&#243;n con el <I><B>PERIODO</B></I> <I><B>G. </B></I>Todo esto, de acuerdo con el aumento  del &#225;cido oleico en la dieta, con el tipo de antioxidantes de los aceites  utilizados (28), y con el tama&#241;o de las LDL de los voluntarios. Es de resaltar  que los valores de la relaci&#243;n TG/HDL-C &lt;1,35 antes y despu&#233;s de la administraci&#243;n  de la dieta, sugiere que los sujetos presentaban LDL grandes y poco densas  como ha sido reportado en trabajos previos (29).</FONT></P> </MULTICOL> <MULTICOL GUTTER="31" COLS="2">     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> La ole&#237;na de palma es una fuente abundante de vitamina E en la forma de  tocotrienoles y de tocoferoles (30, 31), lo que le confiere un alto poder  antioxidante (32-33) y por lo tanto un efecto cardioprotector a trav&#233;s  de su capacidad de suprimir la inflamaci&#243;n (34), inhibir la HMG-CoA reductasa  (35) y reducir la expresi&#243;n de mol&#233;culas de adhesi&#243;n y la adhesi&#243;n monocito-c&#233;lula  endotelial (36).</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Los resultados presentados est&#225;n de acuerdo con la idea, de que la ole&#237;na  de palma puede ser una alternativa saludable en la alimentaci&#243;n humana  sobre todo si se combina con aceites que tengan una alta proporci&#243;n de  &#225;cido linoleico como el girasol, ma&#237;z, soya y similares.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> El presente trabajo muestra que se ha logrado una notable resistencia a  la oxidaci&#243;n de la LDL con s&#243;lo un 30% de incorporaci&#243;n de la ole&#237;na de  palma en el aceite utilizado y con poca modificaci&#243;n del patr&#243;n de las  lipoprote&#237;nas del plasma.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>AGRADECIMIENTOS</B></FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Al Consejo de Desarrollo Cient&#237;fico y Human&#237;stico (CDCH) por el financiamiento  a trav&#233;s del Proyecto de Grupo N&#186; 09-6086-2005.</FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>REFERENCIAS</B></FONT></P>     ]]></body>
