<body><![CDATA[<p style="text-align: justify; text-autospace: none"> <font face="Verdana" size="2"><b>INTRODUCCIÓN</b></font></p>     <p style="text-align: justify; text-autospace: none"> <font face="Verdana" size="2">La hepatitis C afecta alrededor de 170 millones de  personas en el mundo. Un 80% de las personas infectadas por el virus de  hepatitis C (VHC) se convierten en portadores crónicos de la infección, con  secuelas graves como cirrosis y carcinoma hepatocelular (1). No se dispone de  vacuna contra este virus y el tratamiento actual es sólo parcialmente efectivo.  Recientemente se han incorporado drogas antivirales de acción directa con el VHC,  lo cual ha sido un avance significativo en el éxito terapéutico. Varios blancos  de acción antiviral están siendo actualmente explorados y éstos podrían  conllevar a la erradicación de esta enfermedad, si no fuera por el alto costo  asociado a estos tratamientos emergentes (2). Esta revisión pretende describir  el estado del arte del tratamiento contra la hepatitis C y explorar los nuevos  potenciales tratamientos antivirales, incluyendo la terapia enfocada a blancos  celulares.</font></p>     <p style="text-align: justify; text-autospace: none"> <font face="Verdana" size="2"><b>Epidemiología de la hepatitis C</b></font></p>     <p style="text-align: justify; text-autospace: none"> <font face="Verdana" size="2">Aproximadamente 170 millones de personas (3% de la  población mundial) se encuentran infectadas con el VHC. Cada año más de 350.000  personas mueren por enfermedades del hígado relacionadas a la infección por VHC,  inclusive el carcinoma hepatocelular (3). En países de África y Asia se ha  reportado una alta prevalencia de anticuerpos anti-VHC, mientras que en Norte  América, Europa occidental y Australia la prevalencia es baja (4). En  Latinoamérica cerca de 7 millones de personas están infectadas con este virus y  la prevalencia promedio de anticuerpos anti-VHC es aproximadamente del 1% (5).</font></p>     <p style="text-align:justify;line-height:normal"><font face="Verdana" size="2"> Las poblaciones en riesgo de adquirir la infección por el VHC son los usuarios  de drogas por vía endovenosa, las personas expuestas a transfusiones sanguíneas,  los pacientes en hemodiálisis y las poblaciones bajo condiciones especiales como  los inmunosuprimidos, entre otros (6). La principal medida de prevención ha sido  enfocada en evitar el contagio de las <span style="color:black">poblaciones de  alto riesgo, dado que hasta la fecha no existe vacuna contra el VHC (4). </span> </font></p>     <p style="text-align: justify; text-autospace: none"> <font face="Verdana" size="2"><b><span style="color: black">Ciclo de replicación  del VHC </span></b></font></p>     <p style="text-align:justify"><font face="Verdana" size="2"> <span style="color: black">La replicación del VHC ocurre principalmente en el  citoplasma de los hepatocitos (<a href="#fig1">Fig. 1</a>). Los viriones se unen a las células  hospedadoras a través de una interacción específica entre las glicoproteínas de  la envoltura del VHC y glicosaminoglicanos presentes en la membrana celular, al  permitir concentrar el virus en una primera etapa de adhesión (7). La  interacción del VHC con los receptores celulares es un proceso complejo y se han  propuesto diversos receptores que intervienen en el mismo. Uno de ellos es el  receptor de lipoproteínas de baja densidad que se ha descrito como un  correceptor en la entrada del virus (8). Posteriormente, el VHC interacciona con  el receptor “scanvenger” B1 (SR-B1), la tetraspanina CD81 y las proteínas de las  uniones estrechas: claudina-1 y ocludina (9). El receptor del factor de  crecimiento epidermal y el receptor “ephrin” A2, regulan la asociación  co-receptora CD81- claudina-1 y la fusión a la membrana dependiente de la  glicoproteína del virus (10). Luego del proceso de unión-adhesión, los viriones  son internalizados vía endocitosis dependiente de clatrina, donde ocurren  cambios conformacionales en la membrana viral que permiten la liberación del  genoma al citoplasma (11).</span></font></p>     <p style="text-align:center"><a name="fig1"> <img border="0" src="/img/fbpe/ic/v57n1/art10fig1.gif" width="576" height="628"></a></p>     
<p style="text-align: justify; text-autospace: none"> <font face="Verdana" size="2"><span style="color: black">El ARN viral liberado  se traduce directamente, dando lugar a una poliproteína que es procesada por  proteasas celulares y virales, para generar 10 proteínas virales maduras. Las  proteínas virales junto a factores celulares forman un complejo de replicación,  asociado a una red de membranas intracelulares, localizado cerca del retículo  endoplasmático (RE). Dentro de este complejo, el ARN de polaridad positiva es  copiado a un ARN complementario de polaridad negativa, que sirve de molde para  la síntesis de la progenie de ARN genómico. Este ARN genómico es utilizado para  la traducción de poliproteínas, para la síntesis de nuevos intermediarios de  replicación o para ser encapsidado dentro de las nucleocápsides formadas en el  citoplasma. Posteriormente adquiere las glicoproteínas de la envoltura a través  del proceso de migración por el RE, siendo liberadas a través de la vía de  secreción celular (11). </span></font></p>     <p style="text-align: justify; text-autospace: none"> <font face="Verdana" size="2"><b><span style="color: black">Terapia antiviral  estándar y antivirales de acción directa </span></b></font></p>     ]]></body>
<body><![CDATA[<p style="text-align: justify; text-autospace: none"> <font face="Verdana" size="2"><span style="color: black">La agencia reguladora  de drogas y alimentos de Estados Unidos (de sus siglas en ingles FDA), ha  aprobado aproximadamente 50 compuestos y sus combinaciones para su uso en la  terapia antiviral. Sin embargo, en años recientes la demanda por nuevas  estrategias antivirales ha aumentado, impulsado por el incremento en la  prevalencia de enfermedades crónicas y a la aparición de nuevas patologías  asociadas a agentes virales, en especial el virus de la inmunodeficiencia humana  (VIH). </span></font></p>     <p style="text-align: justify; text-autospace: none"> <font face="Verdana" size="2"><span style="color: black">En el caso de la  hepatitis C, el tratamiento ha ido evolucionando desde la monoterapía con IFN a  la biterapia utilizada en los últimos años, de IFN alfa pegilado (IFN-PEG) y  ribavirina (RBV) (<a href="#fig2">Fig. 2</a>, <a href="#tab1">Tabla I</a>), obteniendo el control de la infección  (basados en la no detección del ARN viral en el suero) en el 50% de los  pacientes (18). En la actualidad el tratamiento varía dependiendo del genotipo  (G) viral que infecte al paciente. Para los pacientes infectados con los G2, G3  y G4 consiste en la combinación de IFN-PEG más RBV, mientras que desde el 2011 a  los pacientes infectados con el G1 se les incluye los inhibidores de la proteasa  viral NS3/4A, telaprevir y boceprevir (<a href="#fig2">Fig. 2</a>,  <a href="#tab1">Tabla I</a>). Con esta triple terapia  (IFN-PEG y RBV, más telaprevir ó boceprevir) se ha obtenido una mayor respuesta  antiviral y una disminución en la duración de la terapia en comparación con la  administración de la terapia estándar, con tasas de respuesta virológica  sostenida (RVS), de 67-75% (en pacientes sin tratamiento previo) y de 59-64% (en  pacientes tratados previamente) (19). Aunque el telaprevir ha mostrado actividad  clínicamente significativa contra el G2 (20) y G4 (21), y el boceprevir parece  tener cierta eficacia contra el G3, hasta la fecha su uso está prescrito  solamente para pacientes infectados con el G1.</span></font></p>     <p style="text-align: center; text-autospace: none"> <a name="fig2"> <img border="0" src="/img/fbpe/ic/v57n1/art10fig2.gif" width="575" height="702"></a></p>

<p style="text-align: justify; text-autospace: none"> <font face="Verdana" size="2"><span style="color: black">Desde la aprobación de  los inhibidores de proteasa de primera generación en el 2011, se ha impulsado la  investigación para el desarrollo de nuevos fármacos de acción antiviral directa,  enfocados en lograr una terapia libre de IFN. Actualmente se encuentran en  estudios de fase clínica aproximadamente 25 fármacos, inhibidores de la NS5A, de  la NS5B (inhibidores nucleósidos y no nucleósidos) y de la NS3/4A. En la  <a href="#fig2">Fig. 2</a>  se muestran los fármacos de acción directa aprobados por la FDA (Administración  de Medicamentos y Alimentos, FDA, por sus siglas en inglés) hasta la fecha y  algunos de los que se encuentran actualmente en fase clínica. </span></font></p>     <p style="text-align: justify; text-autospace: none"> <font face="Verdana" size="2"><span style="color: black">Para noviembre del  2013, la FDA aceptó el uso de simeprevir en combinación con IFN-PEG y RBV (16).  Este es el tercer inhibidor de proteasa aprobado hasta la fecha y es  administrado únicamente en pacientes infectados con el G1 sin tratamiento previo  o que hayan fallado a la terapia con IFN. Ha remplazado al telaprevir y  boceprevir por ser una terapia más corta y presentar menos efectos secundarios. </span></font></p>     <p style="text-align:justify;line-height:normal"><font face="Verdana" size="2"> <span style="color: black">Ese mismo año la FDA aprobó el sofosbuvir, un análogo  nucleótido de la polimerasa NS5B, un fármaco potente con una excelente  tolerabilidad por vía oral, actividad pan-genotípica y una alta barrera a la  resistencia, el cual es administrado en regímenes combinados (17). Su eficacia  ha sido comprobada en pacientes infectados con todos los genotipos del VHC,  inclusive los que presentan carcinoma hepatocelular. Se utiliza en pacientes  infectados con G1 y G4 en combinación con IFN-PEG más RBV y en aquellos  infectados con el G2 ó G3 es el primer tratamiento libre de IFN (<a href="#tab1">Tabla I</a>).  Recientemente se ha aprobado su uso con inhibidores de proteasa (Noviembre 2014,  simeprevir) ó con inhibidores de la NS5A (daclatasvir ó ledipasvir en Octubre  2014) (17). Al administrar sofosbuvir con daclastavir, un inhibidor específico  de la proteína NS5A, con y sin RBV en 211 pacientes, de los cuales 167 estaban  infectados con el G1, se obtuvo un 98% de RVS, y 92% y 89% para los pacientes  infectados con el G2 y G3 respectivamente (22). Por otro lado, en fase III  sofosbuvir en conjunto con ledipasvir (inhibidor de la NS5A) generaron una tasa  de cura del 98% en pacientes infectados con el VHC que no había recibido  tratamiento alguno. En el estudio “ION- 1”, se demostró que la combinación de  ambos (sofosbuvir/ledipasvir) con o sin RBV, genera una tasa de RVS de 98% en  pacientes infectados con VHC G1, en tan sólo 12 semanas, con una toma única  oral, sin uso de IFN-&#945; (23). Estos resultados clínicos fueron los que impulsaron  su aprobación en octubre 2014, siendo el primer régimen terapéutico para el  tratamiento crónico de la infección que no requiere la administración de IFN-&#945; ó  RBV (<a href="#tab1">Tabla I</a>). </span></font></p>     <p style="text-align:justify;line-height:normal"><font face="Verdana" size="2"> <span style="color: black">A principios del año 2014, fueron publicados los  resultados del estudio “SHAPPIRE-I”, donde se evaluó la combinación de 4 drogas:  un inhibidor de proteasa NS3/4A del VHC (ABT-450 ó paritaprevir), con el de la  proteasa de VIH (ABT-450/r ó ritonavir), con un inhibidor de la NS5A (ABT-267 ó  ombitasvir) y un inhibidor no nucleósido de la polimerasa (ABT- 333 ó  dasabuvir), con RBV, obteniéndose una tasa de RVS de 96%, en un grupo de 631  pacientes infectados con el G1 (24). Este tratamiento no es efectivo contra el  G3 del VHC. A finales del 2014, la FDA aprobó esta combinación, denominada  comercialmente ViekiraPak, con ó sin RBV, para pacientes infectados con G1 y la  duración va de 12 a 24 semanas, dependiendo de la presencia o no de cirrosis  (<a href="#fig2">Fig. 2</a>). </span></font></p>     <p style="text-align:justify;line-height:normal"><font face="Verdana" size="2"> <span style="color: black">En contraste con enfermedades crónicas como la  causada por el VIH y el virus de la hepatitis B, la causada por el VHC puede ser  erradicada de los pacientes con infección crónica mediante el tratamiento con  antivirales. Sin embargo, la terapia estándar tiene la desventaja de generar  resultados variables en respuesta al tratamiento por factores relacionados al  hospedador como la edad, el grado de fibrosis hepática, el índice de masa  corporal, la esteatosis, la resistencia a la insulina, polimorfismos puntuales  en el cromosoma 19 (IL-28B) y factores relacionados al virus como la carga viral  y el genotipo (25), que junto a los efectos secundarios asociados pueden  conducir al abandono de la misma. Por su parte, los nuevos regímenes  terapéuticos son más seguros, tienen una alta eficacia y una baja tasa de  aparición de efectos adversos (26), que podrían permitir la erradicación de esta  enfermedad, si no fuera por los altos costos asociados al tratamiento y la  eventual presencia de resistencia a los fármacos empleados. </span></font></p>     <p style="text-align: justify; text-autospace: none"> <font face="Verdana" size="2"><b><span style="color: black">Variabilidad  genética y fallas en el tratamiento de la hepatitis C </span></b></font></p>     ]]></body>
<body><![CDATA[<p style="text-align: justify; text-autospace: none"> <font face="Verdana" size="2"><span style="color: black">El VHC posee una ARN  polimerasa dependiente de ARN que carece de un mecanismo de corrección, que  junto a la alta capacidad replicativa de este virus se traduce en una gran  variabilidad genética. Dicha diversidad viral ha conllevado a lo largo de la  historia evolutiva, a la diversificación del virus en genotipos y subtipos y  durante la historia natural de la infección en un individuo a la generación de  cuasiespecies, es decir variantes virales estrechamente relacionadas pero que  pueden tener características fenotípicas distintas. </span></font></p>     <p style="text-align:justify;line-height:normal"><font face="Verdana" size="2"> <span style="color: black">Hasta la fecha se han confirmado siete genotipos y 67  subgenotipos del VHC (27), cada uno con una distribución geográfica distinta.  Los G1, G2 y G3 tienen distribución mundial y su prevalencia varía entre las  regiones del mundo. El G1 predomina en Europa, las Américas y Japón, el G2 en  los países del Mediterráneo y el Lejano Oriente, y el G3 es común en el  subcontinente Indio y recientemente ha incrementado en Europa (28). Se ha  encontrado que los genotipos del VHC poseen relevancia clínica y son un  parámetro predictivo de la RVS al tratamiento. Los G1 y G4 responden en menor  grado a la terapia basada en IFN al compararlos con los G2 y G3 mientras que los  pacientes infectados con el G3 tienen más probabilidades de desarrollar  esteatosis hepática y eventualmente carcinoma hepatocelular (29). </span></font> </p>     <p style="text-align:justify"><font face="Verdana" size="2"> <span style="color: black">Se han descrito mutaciones en algunas de las regiones  del genoma del VHC, como en la región de la cápside, NS3, NS5A y NS5B (<a href="#fig3">Fig. 3</a>),  que han producido una disminución en la susceptibilidad o resistencia a los  tratamientos actuales. En este sentido, es interesante tomar en cuenta nuevos  blancos de acción, específicamente los de tipo celular, que podrían ser  incorporados en los regímenes de tratamiento, bajo la premisa de aumentar la  barrera genética de resistencia viral.</span></font></p>     <p style="text-align:center"><a name="fig3"> <img border="0" src="/img/fbpe/ic/v57n1/art10fig3.gif" width="548" height="553"></a></p>     
<p style="text-align: justify; text-autospace: none"> <font face="Verdana" size="2"><b><span style="color: black">Terapia antiviral  enfocada a blancos celulares </span></b></font></p>     <p style="text-align:justify"><font face="Verdana" size="2">Los virus dependen  de la célula blanco para replicarse de manera efectiva y comparten diversas vías  metabólicas con su hospedador. Se han identificado diversos factores del  hospedador que ejercen una función importante en el ciclo de replicación del  VHC, como son: receptores celulares para la entrada del virus, vías de  señalización moduladas para favorecer la replicación viral, procesamiento y  maduración proteica para el ensamblaje y la secreción viral, entre otros.  Plantear la inhibición de blancos celulares en la terapia antiviral es hacer una  especulación rápida que cuestiona el paradigma tradicional de la misma, que ha  sido enfocado en la premisa de la toxicidad selectiva “los fármacos deben estar  dirigidos al blanco viral”. Aunque el enfoque de inhibir blancos celulares puede  representar un avance interesante, no todos los blancos celulares relacionados  con el proceso de replicación viral serían candidatos para su inhibición, ya que  podría interferir con funciones vitales del hospedador o son blancos para los  cuales no se ha descrito un fármaco inhibitorio. A continuación se describen  algunos blancos celulares que han sido considerados como potenciales dianas para  la inhibición de la replicación del VHC y sus respectivos compuestos inhibidores  (<a href="#fig4">Fig. 4</a>) (34-39).</font></p>     <p style="text-align:center"><a name="fig4"> <img border="0" src="/img/fbpe/ic/v57n1/art10fig4.gif" width="548" height="462"></a></p>     
<p style="text-align: justify; text-autospace: none"> <font face="Verdana" size="2"><b>Inhibidores de la entrada y etapas tempranas </b></font></p>     <p style="text-align:justify"><font face="Verdana" size="2">Como fue discutido  anteriormente, la entrada del VHC a los hepatocitos es un proceso de múltiples  etapas donde las glicoproteínas virales interactúan con diversos receptores.  Existen evidencias de que el VHC induce la activación del receptor del factor de  crecimiento epidermal (EGFR) mediado por la unión con CD81, siendo un paso clave  para la entrada viral. El bloqueo por fármacos como el erlotinib, utilizado  actualmente en la terapéutica del cáncer, es capaz de inhibir la activación del  EGFR y genera un efecto inhibitorio sobre la entrada del VHC en modelos <i>in  vitro </i>(10).</font></p>     <p style="text-align:justify;line-height:normal"><font face="Verdana" size="2"> <span style="color: black">En el año 2012 Zhu y col. (40), describieron la  actividad antiviral de pequeñas moléculas antagonistas del receptor SR-BI,  capaces de inhibir <i>in vitro </i>la replicación del VHC. El mecanismo de  acción planteado involucra el bloqueo de la entrada del virus a la célula  blanco, encontrándose actualmente el compuesto ITX-5061 en estudios de fase  clínica I. Se ha encontrado que péptidos humanos derivados de la apolipoproteína  E (apoE) también son capaces de bloquear la entrada del VHC en modelos <i>in  vitro </i>en concentraciones submicromolares (41). </span></font></p>     ]]></body>
<body><![CDATA[<p style="text-align:justify;line-height:normal"><font face="Verdana" size="2"> <span style="color: black">Se ha evaluado bloquear las etapas tardías de la  entrada del virus, principalmente las relacionadas con endosomas-lisosomas.  Compuestos como la sibilina generan una inhibición de la entrada viral a través  del bloqueo de la vía endocítica asociada a clatrina (42). Chamoun-Emanuelli y  col. en el año 2013 (43) identificaron compuestos derivados de fenotiazina que  inhiben la entrada del VHC, a través de una disrupción de los dominios ricos en  colesterol presentes en la membrana de la célula blanco. Asimismo, compuestos  como la cloroquina y el cloruro de amonio generan una inhibición en el cambio  conformacional y procesamiento enzimático de las proteínas virales dentro de los  lisosomas (44). </span></font></p>     <p style="text-align:justify;line-height:normal"><font face="Verdana" size="2"> <span style="color: black">El uso de inhibidores de la entrada del VHC dirigidos  contra los receptores de la célula hospedera representa una opción terapéutica  prometedora. El bloqueo farmacológico de varios receptores podría generar un  efecto sinérgico en la inhibición y un menor desarrollo de resistencia. El  desafío farmacológico consiste en que ese bloqueo simultáneo genere el menor  efecto negativo sobre la homeostasis celular. </span></font></p>     <p style="text-align:justify;line-height:normal"><font face="Verdana" size="2"> <b><span style="color: black">Inhibidores de la replicación </span></b></font> </p>     <p style="text-align:justify"><font face="Verdana" size="2"> <span style="color: black">La inhibición de la síntesis del ARN viral es otro  blanco, que de hecho probablemente ya es abordado con el uso RBV, la cual genera  una disminución de la concentración intracelular de guanosina trifosfato (GTP),  como se ha descrito anteriormente (<a href="#tab1">Tabla I</a>). Se encuentra actualmente en  evaluación el compuesto denominado A3, de bajo peso molecular, que ha demostrado  inhibir la deshidrogenasa dihidroorotato (DHODH), enzima clave en la síntesis de  uridín monofosfato, observándose <i>in vitro</i>, un efecto sobre el VHC (45).</span></font></p>     <p style="text-align: justify; text-autospace: none"> <font face="Verdana" size="2"><span style="color: black">Los avances en la  farmacología genética han impulsado el desarrollo de compuestos que protegen a  los oligonucleótidos de la acción de nucleasas y facilitan su transporte a las  células blanco. Los microARN (miARN) están constituidos por ARN de cadena simple  con una extensión de entre 19 a 25 nucleótidos y tienen la capacidad de regular  la expresión de genes. El bloqueo de miARN asociados a la expresión génica viral  a través de oligonucleótidos antisentido ha generado resultados promisorios. El  miravirsen, prototipo de fármaco dirigido contra miARN, actualmente se encuentra  en estudios de fase clínica 2. Este fármaco fue diseñado para el tratamiento de  la infección por el VHC y su mecanismo de acción se basa en el bloqueo del miARN  miR122, el cual está presente en las células hepáticas y es un factor clave para  que se lleve a cabo la replicación del VHC (46). </span></font></p>     <p style="text-align: justify; text-autospace: none"> <font face="Verdana" size="2"><b><span style="color: black">Inhibidores de las  modificaciones post-traduccionales de proteínas virales </span></b></font></p>     <p style="text-align:justify;line-height:normal"><font face="Verdana"> <span style="color: black"><font size="2">Las ciclofilinas son enzimas  celulares, específicamente peptidil-prolil isomerasas, que catalizan la  isomerización de los enlaces peptídicos de los residuos de prolina de la  configuración <i>trans </i>a las <i>cis</i>, facilitando el plegado de la  proteína (47). La acción peptidil-propil isomerasa es esencial para que se  desarrolle completamente la replicación del VHC, lo cual está asociado a la  interacción con la proteína NS5B y su incorporación al complejo de replicación  viral (48). Este blanco celular ha sido ampliamente estudiado como posible  estrategia terapéutica en el tratamiento de la infección por el VHC debido a los  resultados promisorios obtenidos. La ciclosporina A y sus análogos inhiben la  actividad de las ciclofilinas y han sido evaluados como inhibidores de la  replicación del VHC, describiéndose que compuestos inhibidores de la  ciclofilinas generan una alteración en el tráfico celular de lípidos y en la  secreción del virus (49). Qing y col. en el año 2009 (50) evaluaron en modelos <i>in vitro</i>, el efecto inhibitorio producido por la ciclosporina en virus  del género flavivirus, observando que el efecto antiviral estaba asociado al  bloqueo de la replicación ya que interfiere con la interacción de la proteína  NS5 de los flavivirus con la ciclofilina A. Actualmente se encuentran en  estudios de fase clínica por lo menos tres inhibidores de ciclofilina (alisporivir,  NIM811 y SCY-635), los cuales han demostrado seguridad y eficacia en el  tratamiento de pacientes con hepatitis C (51). Alisporivir se encuentra en  estudios de fase III, observándose una disminución marcada de la viremia en los  pacientes tras su administración (52). Además de ello, se ha evaluado <i>in  vitro </i>el desarrollo de resistencia, observándose que es necesaria la  aparición de múltiples mutaciones en el dominio II de NS5A en lugar de una sola  mutación para hacer el VHC significativamente resistente a los inhibidores de  ciclofilinas (53). Recientemente, se demostró que una mutación única en NS5A  (D2292E ó D2303H) confiere resistencia a ciclosporina A y está mutación en  combinación con otras tres confiere resistencia a NIM811. Sin embargo, es  necesaria una mutación adicional en NS3 para restablecer la capacidad  replicativa del virus (54). </font></span></font></p>     <p style="text-align:justify;line-height:normal"><font face="Verdana" size="2"> <b><span style="color: black">Modificaciones postraduccionales </span></b> </font></p>     <p style="text-align:justify;line-height:normal"><font face="Verdana" size="2"> <span style="color: black">La glicosilación de proteínas está relacionada  directamente con su migración dentro de los distintos organelos celulares y su  maduración. El bloqueo de la maduración de las proteínas virales, asociado a los  procesos de modificación post-traduccional, en específico la glicosilación, ha  sido considerado como blanco para la inhibición viral (55). Las proteínas E1 y  E2 de la envoltura del VHC, son altamente modificadas por N-glicosilación; dicha  modificación es crucial para la maduración de la progenie y la posterior entrada  viral. Diverso grupos de investigación han estudiado en modelos <i>in vitro </i> del VHC el efecto de derivados de iminoazúcares como la 1-deoxinojirimicina,  inhibidores de las glicosidasas tipo I y II presentes en el RE, observando que  poseen actividad inhibitoria sobre la glicosilación de las proteínas virales, lo  cual ejerce un efecto negativo sobre la replicación viral (56, 57). </span> </font></p>     <p style="text-align:justify;line-height:normal"><font face="Verdana" size="2"> <span style="color: black">La producción de mevalonato por la coenzima A 3-hidroximetilglutaril  reductasa (de sus siglas en ingles HMG-CoA reductasa) es el paso limitante en la  biosíntesis del colesterol. Las estatinas son potentes inhibidores de la HMG-CoA  reductasa y son beneficiosas en la prevención de la enfermedad cardiocoronaria.  Las estatinas también inhiben la prenilación de las proteínas. Diversos autores  han descrito que el VHC requiere elementos de las vías de biosíntesis del  colesterol y de los ácidos grasos para desarrollar una replicación eficiente  (58,59). Por esta razón, la inhibición de dichas vías a distintos niveles ha  sido evaluada como blanco potencial antiviral contra el VHC. En modelos <i>in  vitro </i>e <i>in vivo </i>del VHC se ha observado que las estatinas tienen un  efecto negativo sobre la prenilación de proteínas virales, produciendo un efecto  inhibitorio sobre el ciclo viral (60,61). En modelos <i>in vitro </i>del VHC se  ha observado que la combinación de estatinas y la terapia estándar genera un  efecto sinérgico en la actividad antiviral de dichos compuestos (62). Atsukawa y  col. (63) en el año 2013 publicaron el efecto de la administración de la terapia  estándar con 20 mg de fluvastatina a pacientes infectados con VHC genotipo 1b,  observando que la adición de fluvastatina aumenta la respuesta virológica  sostenida. Esta premisa fue confirmada por Zhu y col. en el año 2013 (64), al  concluir que la adición de estatinas a la terapia estándar mejora la RVS sin  generar un aumento significativo en la aparición de efectos adversos. Por lo que  dichos autores sugieren que pueden ser considerados como un adyuvante de la  terapia con IFN-&#945; y RBV. </span></font></p>     ]]></body>
<body><![CDATA[<p style="text-align:justify;line-height:normal"><font face="Verdana" size="2"> <b><span style="color: black">CONCLUSIONES Y PERSPECTIVAS </span></b></font></p>     <p style="text-align:justify;line-height:normal"><font face="Verdana" size="2"> <span style="color: black">La hepatitis C es una enfermedad de gran importancia  en salud pública. El hecho de que no exista una vacuna contra esta enfermedad y  que un alto número de los pacientes infectados no responden a la terapia  estándar, ha fomentado la búsqueda de nuevos compuestos antivirales que puedan  ser utilizados en la terapéutica del VHC. La introducción de los antivirales de  acción directa ha contribuido significativamente en la mejora clínica de los  pacientes infectados por el VHC. El éxito y el corto tiempo de tratamiento de  las terapias combinadas recientes, parece sugerir que la aparición de  resistencia no será un problema de gran impacto para la mayoría de los pacientes  que mantengan adherencia al tratamiento (65). La baja toxicidad de estas nuevas  terapias las hace igualmente prometedoras. Sin embargo, ciertos grupos podrían  no beneficiarse de estos adelantos, como pacientes con insuficiencias renales o  hepáticas o con resistencia previa a los antivirales (66). El costo actual de  estas terapias es muy elevado, aunque es de esperar que estos costos disminuyan,  en particular para los países en desarrollo, tal como ha estado ocurriendo con  las drogas antirretrovirales (67). Por todas las razones anteriores, los blancos  celulares podrían jugar un papel significativo como complemento de la terapia  antiviral emergente, ya que aumentan el número de opciones para la terapia de  combinación de fármacos y la barrera genética de la resistencia puede ser mayor  que para las terapias dirigidas contra el virus. Existen al menos diez blancos  celulares donde se ha evidenciado actividad antiviral a través de su inhibición  farmacológica, siendo la inhibición de ciclofilinas uno de los blancos más  prometedores, encontrándose actualmente el fármaco alisporivir bajo estudios de  fase clínica III. Sin embargo, el desarrollo de terapias antivirales enfocadas  en blancos celulares, debe estar sometido a un proceso de investigación  exhaustivo en el que se evalúen los diferentes efectos tóxicos que le pudiesen  producir al hospedador. Otro punto que no debe ser descartado, dado el enorme  potencial de variación de los virus, es la aparición de variantes virales  resistentes que usen mecanismos metabólicos alternativos en su replicación y así  evadan la terapia basada en el blanco celular afectado. </span></font></p>     <p style="text-align:justify;line-height:normal"><font face="Verdana" size="2"> <b><span style="color: black">AGRADECIMIENTO </span></b></font></p>     <p style="text-align:justify;line-height:normal"><font face="Verdana" size="2"> <span style="color: black">A la Dra.
