Analizaremos las drogas ansiolíticas y antidepresivas de uso común y las más recientes; ya que el médico debe conocer las indicaciones precisas de los distintos medicamentos, la farmacocinética y farmacodinamia de los mismos, dosis, efectos adversos, interacciones con otros fármacos y complicaciones; para así poder aplicarlos al paciente en forma individual, de manera que éste se beneficie lo mejor posible.]]></p></abstract>

La intensidad con la cual se presentan, varia ampliamente desde reacciones normales de tristeza y pesar hasta estados graves e incapacitantes que pueden culminar en la muerte. Entre los desordenes psiqui&aacute;tricos, la ansiedad representa la de mayor incidencia en la comunidad en general, y esta presente en cerca del 15 al 20% de los pacientes(1). Este trastorno puede ser un s&iacute;ntoma cardinal de una condici&oacute;n psiqui&aacute;trica primaria o una reacci&oacute;n a una patolog&iacute;a m&eacute;dica. La literatura indica que su frecuencia es tres veces mayor en mujeres que en hombres(2).</P>     <P ALIGN="JUSTIFY">Con respecto a los trastornos emocionales, podemos mencionar que muchas de las enfermedades por las cuales se consulta diariamente, tienen un alto riesgo de cursar con depresi&oacute;n y por esta raz&oacute;n el m&eacute;dico debe estar alerta para realizar el diagn&oacute;stico adecuado y as&iacute; disminuir las posibilidades de subestimar problemas afectivos, que a&uacute;n siendo graves tienen una alta tasa de estar subtratados, y de esta manera instaurar la terap&eacute;utica m&aacute;s conveniente para el paciente en particular.</P> <B>    ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">ANSIEDAD</P> </B>     <P ALIGN="JUSTIFY">Desde el punto de vista fisiol&oacute;gico se define como un sistema que alerta al organismo ante sucesos que lo pueden poner en desventaja; es un sentimiento displacentero que se acompa&ntilde;a de sensaciones som&aacute;ticas como n&aacute;useas, palpitaciones, sudoraci&oacute;n, cefalea, necesidad de vaciamiento vesical e inclusive diarrea, entre otras(3).</P>     <P ALIGN="JUSTIFY">Desde el punto de vista patol&oacute;gico se caracteriza por una autonom&iacute;a relativa, sin causa externa o interna aparente, con intensidad, duraci&oacute;n y conducta asociada al cuadro cl&iacute;nico; implica dos respuestas, una de hiperalerta continuo, y una respuesta de tipo vegetativo, mediada por el sistema simp&aacute;tico. Se manifiestan alteraciones a nivel cognoscitivo; a nivel f&iacute;sico, neurol&oacute;gico, y por &uacute;ltimo, a nivel conductual. Tambi&eacute;n pueden considerarse factores ex&oacute;genos como contribuyentes, entre ellos, ingesti&oacute;n de estimulantes, xantinas, abstinencia de depresores del SNC y abstinencia de opi&aacute;ceos(3,4,5).</P>     <P ALIGN="JUSTIFY">Las entidades psiqui&aacute;tricas cuyo s&iacute;ntoma cardinal es la ansiedad se subdividen seg&uacute;n DSM-IV en(1,4,6):</P>     <P ALIGN="JUSTIFY"></P><DIR>      <P ALIGN="JUSTIFY">1)&#9;Estados ansiosos (neurosis de angustia).</P><DIR>      <P ALIGN="JUSTIFY">&#9;a)&#9;Trastorno por p&aacute;nico (ataques de p&aacute;nico con o sin agarofobia).</P>     <P ALIGN="JUSTIFY">&#9;b)&#9;Trastornos por ansiedad generalizada.</P>     <P ALIGN="JUSTIFY">&#9;c)&#9;Trastornos obsesivos-compulsivos. </P></DIR>      <P ALIGN="JUSTIFY">2)&#9;Trastornos f&oacute;bicos (agarofobias, fobia social, fobia simple, etc).</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">3)&#9;Conversi&oacute;n (trastorno conversivo). </P>     <P ALIGN="JUSTIFY">4)&#9;Alteraci&oacute;n postraum&aacute;tica (trastorno por estr&eacute;s postraum&aacute;tico).</P>     <P ALIGN="JUSTIFY"></P></DIR>      <P ALIGN="JUSTIFY">Entre los m&aacute;s frecuentes tenemos a la fobia, que es un miedo fuerte e irracional a un objeto o situaci&oacute;n dada, mientras que en el trastorno de ansiedad generalizada el s&iacute;ntoma cardinal es la ansiedad o sensaci&oacute;n de &quot;miedo flotante&quot; por un per&iacute;odo de al menos varios meses(7).</P>     <P ALIGN="JUSTIFY"></P> <B>    <P ALIGN="JUSTIFY">Neurobiolog&iacute;a de la ansiedad</P> </B>    <P ALIGN="JUSTIFY">Entre los sustratos neurobiol&oacute;gicos de la ansiedad se incluyen: Locus coeruleus (piso del IV ventr&iacute;culo), la elevada actividad de las c&eacute;lulas de Noradrenalina en este lugar se asocia a temor y ansiedad. Se ha vinculado al sistema de &quot;receptor benzodiacepinico&quot; que se encuentra en altas concentraciones en el sistema L&iacute;mbico, a nivel de las estructuras septohipoc&aacute;mpicas; al receptor GABA-A, cuya activaci&oacute;n produce efecto inhibidor postsin&aacute;ptico y al complejo receptor GABA/benzodiazepina (GABA-A), sitio de uni&oacute;n para benzodiazepinas, que seg&uacute;n propuestas, pueden tener participaci&oacute;n en los mecanismos que originan la ansiedad tanto patol&oacute;gica como normal(3,8).</P>     <P ALIGN="JUSTIFY">Tambi&eacute;n hay evidencia de la existencia de ligandos naturales en el cerebro de mam&iacute;feros que modulan el complejo receptor GABA/Benzodiazepina actuando como agonista, lo cual hace plantear la posibilidad de que los estados de ansiedad y el insomnio puedan ser consecuencia del d&eacute;ficit en la disponibilidad de dichos ligandos(9).</P>     <P ALIGN="JUSTIFY">El sistema serotonin&eacute;rgico modula la conducta humana, as&iacute; el descenso en la transmisi&oacute;n serotonin&eacute;rgica facilita o desinhibe algunas actividades, plante&aacute;ndose por lo tanto que la ansiedad este relacionada a la serotonina(10).</P> <B>     <P ALIGN="JUSTIFY">AGENTES ANSIOL&Iacute;TICOS</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Benzodiazepinas</P> </B>    <P ALIGN="JUSTIFY">El t&eacute;rmino benzodiazepina (BZD) se refiere a la estructura compuesta por un anillo de benceno fusionado con un anillo de diazepina(9,11). En la actualidad para el tratamiento de la ansiedad se recomienda entre los derivados de la BZD el clordiazep&oacute;xido, diazepam, oxazepam, clorazepato, lorazepam, prazepam, alprazolam, halazepam y clonazepam(12). Ser&aacute;n &uacute;tiles en casos de significativa disforia; si la ansiedad es incapacitante o disfuncional(11). Otros usos: desintoxicaci&oacute;n alcoh&oacute;lica, como sedantes, hipn&oacute;ticos, anticonvulsivantes, en la medicaci&oacute;n preanest&eacute;sica y anest&eacute;sica y como relajantes musculares(4,11). En concentraciones m&aacute;s altas, incluyen adem&aacute;s la inhibici&oacute;n de la captaci&oacute;n de adenosina y la potenciaci&oacute;n resultante de este depresor(11). Se han detectado dos tipos de receptores para BZD en el cerebro: el tipo I, independiente del GABA, donde al parecer se ejerce el efecto ansiol&iacute;tico, y el tipo II dependiente del GABA donde la BZD tienen sus efectos anticonvulsivos e hipn&oacute;ticos(3).</P>     <P ALIGN="JUSTIFY"> Los efectos de las benzodiazepinas por acciones en el Sistema Nervioso Central son: sedaci&oacute;n, hipnosis, disminuci&oacute;n de la ansiedad, relajaci&oacute;n muscular, amnesia anter&oacute;grada y actividad anticonvulsiva. Al incrementar la dosis, la sedaci&oacute;n progresa hasta la hipnosis y luego a estupor(11).</P> <B>    <P ALIGN="JUSTIFY">Respiraci&oacute;n: </B>En altas dosis deprimen levemente la ventilaci&oacute;n alveolar y causan &aacute;cidosis respiratoria, pueden producir hipoxia alveolar, narcosis por CO2 o ambos(11,12).</P> <B>    ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Aparato Cardiovascular: </B>En sujetos normales son menores, a menos que haya intoxicaci&oacute;n grave. En dosis preanest&eacute;sicas disminuyen la presi&oacute;n arterial e incrementan la frecuencia cardiaca. El diazepam aumenta el flujo sangu&iacute;neo coronario y parece disminuir el trabajo del ventr&iacute;culo izquierdo y el gasto cardiaco(11,12).</P> <B>    <P ALIGN="JUSTIFY">Tubo Digestivo: </B>Mejoran los trastornos gastrointestinales relacionados con la ansiedad. El diazepam disminuye la secreci&oacute;n nocturna de gastrina(1).</P> <B>    <P ALIGN="JUSTIFY">M&uacute;sculo Estriado: </B>Ocurre cierta relajaci&oacute;n muscular(13).</P> <B>    <P ALIGN="JUSTIFY">Farmacocin&eacute;tica: </B>A excepci&oacute;n del diazepam, todas las BZD se absorben err&aacute;ticamente si se administran por v&iacute;a intramuscular, pero se absorben con rapidez por v&iacute;a oral, se fijan estrechamente a prote&iacute;nas plasm&aacute;ticas (85 a 95%). Algunas BZD no poseen metabolito activo y se eliminan r&aacute;pidamente, al contrario de otras. Se metabolizan en el h&iacute;gado, y se excretan en mayor parte por orina en forma de metabolitos oxidados y conjugados con el &aacute;cido glucur&oacute;nico. Los f&aacute;rmacos activos a nivel del receptor de BZD se pueden clasificar seg&uacute;n su vida media en: <B>1)</B> BZD de acci&oacute;n ultrabreve, <B>2)</B> agentes de acci&oacute;n breve con tl/2 menor de seis horas, <B>3)</B> agentes de acci&oacute;n intermedia con tl/2 de 6 a 24 horas y <B>4)</B> agentes de acci&oacute;n prolongada con tl/2 de m&aacute;s de 24 horas(4,5,11). La dosificaci&oacute;n y propiedades farmacol&oacute;gicas se especifican en el <a href="#c1"> cuadro 1</a>. El tiempo de tratamiento no debe exceder 2 a 6 semanas(13,14,15).</P> <B>    <P ALIGN="JUSTIFY">Reacciones Adversas: </B>La probabilidad de producir efectos adversos se incrementa con el uso prolongado de BZD. A dosis hipn&oacute;ticas pueden producir aturdimiento, laxitud, incremento del tiempo de reacci&oacute;n, trastorno de las funciones mentales y motoras, confusi&oacute;n, agitaci&oacute;n, psicosis, debilidad, cefalalgia, v&eacute;rtigo, n&aacute;useas, v&oacute;mitos, diarrea. Puede haber somnolencia residual durante el d&iacute;a. Hay otras menos frecuentes como reacciones al&eacute;rgicas, hepatot&oacute;xicas y hematol&oacute;gicas, reacciones de desinhibici&oacute;n o descontrol que pueden acompa&ntilde;arse tambi&eacute;n de paranoia, depresi&oacute;n e ideaci&oacute;n suicida, estas &uacute;ltimas son raras y pueden relacionarse con la dosis. Est&aacute;n contraindicadas en: Hipotonia muscular, miastenia gravis, glaucoma de &aacute;ngulo cerrado, hipersensibilidad a la droga, insuficiencia respiratoria grave, durante la lactancia materna y en el primer trimestre del embarazo; ya que se han asociado a malformaciones, entre ellas labio leporino(4,11,16,17). </P> <B>    <P ALIGN="JUSTIFY">Intoxicaci&oacute;n: </B>Depresi&oacute;n respiratoria, hipotensi&oacute;n, choque, coma y muerte. En casos de intoxicaci&oacute;n aguda, esta indicado el flumazenil (Lanexat) en dosis de 0,2 Mg endovenosos(18).</P> <B>    <P ALIGN="JUSTIFY"></P>     <P ALIGN="JUSTIFY">&nbsp;</P>     <P>INTOLERANCIA, DEPENDENCIA F&Iacute;SICA E INTERACCIONES CON OTRAS DROGAS</P> </B>    <P ALIGN="JUSTIFY"></P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Generalmente tiene una baja incidencia de abuso y dependencia, pero esta puede ser una complicaci&oacute;n del uso cr&oacute;nico(13); la tolerancia y reacciones de abstinencia se observan en mucho menor grado que con opi&aacute;ceos y barbit&uacute;ricos. No deben interrumpirse de manera repentina luego de un uso prolongado(4,5). Tienen interacci&oacute;n m&iacute;nima con m&uacute;ltiples drogas. <body><![CDATA[<P ALIGN="JUSTIFY">Farmacocin&eacute;tica: </B>Se absorben con rapidez por v&iacute;a oral, tambi&eacute;n pueden administrarse por v&iacute;a intramuscular y rectal. La v&iacute;a endovenosa se reserva para tratar el estado epil&eacute;ptico (fenobarbital s&oacute;dico) o para la inducci&oacute;n o conservaci&oacute;n de la anestesia general (tiopental, metohexital). El metabolismo es hep&aacute;tico y su excreci&oacute;n es renal. Est&aacute;n contraindicados en pacientes con porfiria y en insuficiencia pulmonar, embarazo reciente y bloqueos cardiacos(11).</P> <B>    <P ALIGN="JUSTIFY">Tolerancia y dependencia:</B> Con la administraci&oacute;n cr&oacute;nica de dosis crecientes se crea tolerancia en semanas a meses, esto ocurre con mayor frecuencia que con BZD(4,12).</P> <B>    <P ALIGN="JUSTIFY">Intoxicaci&oacute;n: </B>Se produce coma, con respiraci&oacute;n lenta o r&aacute;pida y superficial, con hipotensi&oacute;n, irritabilidad, hipo o arreflexia y nistagmus. La sobredosis es letal por la depresi&oacute;n respiratoria, en especial si se combina con alcohol y antihistam&iacute;nicos; el tratamiento se basa en medidas de sost&eacute;n, con lavados g&aacute;stricos si han pasado menos de 24 horas, administrando carb&oacute;n activado y cat&aacute;rtico(11,17,19).</P> <B>    ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Usos Terap&eacute;uticos:</B> En situaciones de emergencia para controlar la agitaci&oacute;n; en caso de que las BZD o buspirona causen efectos adversos severos; en pacientes que no respondan adecuadamente a estos &uacute;ltimos y en personas que hayan recibido en el pasado barbit&uacute;ricos y no acepten BZD o buspirona(17).</P>     <P ALIGN="JUSTIFY"></P> <B>    <P ALIGN="JUSTIFY">Azapironas</P> </B>    <P ALIGN="JUSTIFY">Las azapironas son una clase de agentes psicoterap&eacute;uticos relativamente nueva, con propiedades ansiol&iacute;ticas y antidepresivas. La buspirona es actualmente la &uacute;nica azapirona en uso cl&iacute;nico(12).</P> <B>    <P ALIGN="JUSTIFY">Propiedades farmacol&oacute;gicas:</B> Producen disminuci&oacute;n del disparo de las neuronas serotonin&eacute;rgicas y disminuci&oacute;n de la s&iacute;ntesis y descarga de serotonina por su acci&oacute;n sobre ciertos receptores serotonon&eacute;rgicos; tambi&eacute;n tienen interacciones moderadas con los sistemas dopamin&eacute;rgico y noradren&eacute;rgico cerebrales(20).</P> <B>    <P ALIGN="JUSTIFY">Farmacocin&eacute;tica: </B>Poca biodisponibilidad, escasamente un 5%, fijaci&oacute;n a prote&iacute;nas plasm&aacute;ticas en un 95%, vida media de 2,5 horas. Se metaboliza en el h&iacute;gado y se excreta en la orina como metabolito oxidado(12). La dosis utilizada es de 15 a 45 mg/d&iacute;a dividida en tres dosis por v&iacute;a oral(4). Ejerciendo su efecto en dos a seis semanas(1,17,21). Esta relativamente libre de efectos secundarios y entre estos tenemos: cefalea, n&aacute;useas y mareos(22).</P> <B>    <P ALIGN="JUSTIFY">Tolerancia y dependencia:</B> No parecen producir tolerancia, adicci&oacute;n ni reacciones de abstinencia, tampoco manifiestan tolerancia cruzada con BZD y otros depresores del SNC(4,21,22).</P> <B>    <P ALIGN="JUSTIFY">Interacciones medicamentosas: </B>Aumenta las concentraciones de haloperidol. No debe administrarse con IMAO sin un intervalo de al menos 2 semanas luego de la suspensi&oacute;n de estos(17).</P> <B>    <P ALIGN="JUSTIFY">Usos terap&eacute;uticos:</B> En ansiedad generalizada y en trastornos obsesivos-compulsivos sin utilidad en casos de ansiedad con ataques de p&aacute;nico y trastornos de hiperactividad con d&eacute;ficit de atenci&oacute;n(10,12,16,22). Juega un papel importante en el tratamiento del alcoholismo y en la ansiedad del anciano(23).</P>      <P>&nbsp;</P> <B>    ]]></body>
<body><![CDATA[<P>OTROS F&Aacute;RMACOS IMPLICADOS EN EL TRATAMIENTO DE LA ANSIEDAD</P> </B> <B>    <P>Hipn&oacute;ticos no barbit&uacute;ricos</P> </B>    <P>Tienen propiedades sedantes e hipn&oacute;ticas, entre ellas el hidrato de cloral, paraldehido, glutetimida y metacuolona. El uso cl&iacute;nico de estos medicamentos ha disminuido considerablemente y son &uacute;tiles solo en casos espec&iacute;ficos como en el delirium tremens (paraldehido)(11,12).</P> <B>     <P>Carbamatos</P> </B>    <P>Se asemejan a las benzodiazepinas, siendo sus efectos ansiol&iacute;ticos menos selectivos, al igual que los hipn&oacute;ticos no barbit&uacute;ricos actualmente su uso ha disminuido(11).</P> <B>     <P>Antihistam&iacute;nicos</P> </B>    <P>Su efecto ansiol&iacute;tico no es tan eficaz como el de las benzodiazepinas, no obstante, se utilizan como sedantes en ancianos y ni&ntilde;os(3).</P>  <B>    <P>Propanolol</P> </B>    <P>El propanolol y otros antagonistas de los receptores beta-adren&eacute;rgicos son importantes en el alivio de los s&iacute;ntomas neurovegetativos de la ansiedad, mediados por el sistema simp&aacute;tico, que acompa&ntilde;an a las fobias aisladas, pero no parecen ser eficaces en la ansiedad generalizada o en el trastorno de p&aacute;nico(17).</P>  <B>    <P>Clonidina</P> </B>    ]]></body>
<body><![CDATA[<P>No se ha demostrado su utilidad en los trastornos graves de ansiedad(12,16).</P>  <B>    <P>Calcioantagonistas</P> </B>    <P>Al igual que los beta bloqueantes, podr&iacute;an mejorar los s&iacute;ntomas atribuibles a la inestabilidad auton&oacute;mica. Tienen uso particularmente potencial en los pacientes con prolapso de v&aacute;lvula mitral asociado a trastornos de p&aacute;nico. Los datos disponibles de ensayos controlados son limitados(16).</P>  <B>    <P>Antipsic&oacute;ticos y antidepresivos</P> </B>    <P>Existen estudios que han revelado la eficacia de imipramina, clomipramine en los ataques de p&aacute;nico y entre las drogas antipsic&oacute;ticas la clozapina esta bajo investigaci&oacute;n como posible droga &uacute;til en los casos refractarios de ataque de p&aacute;nico(16).</P> <B>     <P ALIGN="JUSTIFY">DEPRESI&Oacute;N</P> </B>     <P ALIGN="JUSTIFY">Los trastornos afectivos o emocionales, se expresan principalmente por alteraciones del estado de &aacute;nimo, conducta y afecto. Los trastornos del &aacute;nimo se subdividen en: trastorno depresivo mayor, trastorno bipolar y depresi&oacute;n asociada con otras enfermedades y drogas(24,25).</P>     <P ALIGN="JUSTIFY">En el DSM-IV-R se presenta la siguiente clasificaci&oacute;n de trastorno depresivo: <B>1)</B> episodios man&iacute;acos, <B>2)</B> episodio depresivo mayor, <B>3)</B> melancol&iacute;a, <B>4)</B> trastorno bipolar, <B>5)</B> ciclotimia, <B>6)</B> distimia(26).</P>     <P ALIGN="JUSTIFY"></P> <B>    <P ALIGN="JUSTIFY">Trastorno depresivo mayor</P> </B>    ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Se define como un estado de &aacute;nimo depresivo diariamente con un m&iacute;nimo de duraci&oacute;n de 2 semanas. Su incidencia aumenta con la edad y su prevalencia es el doble en mujeres que en hombres(24). La Organizaci&oacute;n Mundial de la Salud, clasifica este trastorno como la cuarta enfermedad con respecto a carga global de incapacidad y para el a&ntilde;o 2002 se estima que este ubicado en el segundo lugar.</P> <B>    <P ALIGN="JUSTIFY">Caracter&iacute;sticas cl&iacute;nicas:</B> Sentimientos profundos de tristeza y desesperaci&oacute;n, deseos de no seguir viviendo, abandono del aseo personal, culpa, autodevaluaci&oacute;n, lentitud de las funciones mentales, p&eacute;rdida de la concentraci&oacute;n, pesimismo y autodesprecio(25,27). Alteraciones som&aacute;ticas y vegetativas como: anorexia, estre&ntilde;imiento, anhedonia, disminuci&oacute;n de la libido, retardo psicomotriz o agitaci&oacute;n, problemas del sue&ntilde;o, insomnio e hiperinsomnio(24).</P> <B>    <P ALIGN="JUSTIFY"></P>     <P ALIGN="JUSTIFY">Trastorno bipolar</P> </B>    <P ALIGN="JUSTIFY">Se define como la fluctuaci&oacute;n del estado de &aacute;nimo entre episodios de man&iacute;a o hipoman&iacute;a y depresi&oacute;n(24). Su Incidencia es de: 0,4 a 1,2% de la poblaci&oacute;n general. El primer episodio ocurre entre los 20 y 40 a&ntilde;os de edad, casi siempre de tipo man&iacute;aco(26).</P>     <P ALIGN="JUSTIFY">Las caracter&iacute;sticas de la man&iacute;a son: Aumento de la actividad psicomotora, disminuci&oacute;n de la necesidad de sue&ntilde;o, sensaci&oacute;n de elaci&oacute;n y de sentirse &quot;mejor que nunca&quot;. Deterioro del lenguaje en forma gradual, pueden experimentar ideas delirantes de grandeza, megaloman&iacute;acas o paranoides. Los individuos con cuadros de man&iacute;a tienen 80% de probabilidad de recurrencia(24,26).</P>     <P ALIGN="JUSTIFY"></P> <B>    <P ALIGN="JUSTIFY">Depresi&oacute;n asociada con otras enfermedades y drogas</P> </B>    <P ALIGN="JUSTIFY">Entre las drogas que inducen s&iacute;ntomas depresivos tenemos: los antihipertensivos (bloqueantes de canales de calcio y B-bloqueantes); anticolesterolemicos y antiarr&iacute;tmicos(24).</P>     <P ALIGN="JUSTIFY">Las enfermedades que inducen s&iacute;ntomas depresivos son m&uacute;ltiples entre ellas est&aacute;n: Cardiopat&iacute;as (20 a 30%); C&aacute;ncer (40 a 50%); Diabetes mellitus (8,5 a 7,3%) y otras como enfermedades cerebrovasculares, Parkinson, esclerosis m&uacute;ltiple, hipertiroidismo e hipotiroidismo y traumatismo craneal(24). La depresi&oacute;n es un s&iacute;ndrome com&uacute;n entre las personas de edad avanzada, debido a diferentes enfermedades y a tensiones psicosociales que afligen a los ancianos(28).</P> <B>    ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Etiolog&iacute;a y fisiopatolog&iacute;a: </B>Por medio del estudio del metabolismo de la glucosa cerebral se ha determinado la disminuci&oacute;n de la tasa metab&oacute;lica a nivel del n&uacute;cleo caudado y l&oacute;bulo frontal, estableci&eacute;ndolos como los sitios primarios de d&eacute;ficit; con la tomograf&iacute;a con emisi&oacute;n de positrones (TEP) se ha demostrado la disminuci&oacute;n de la captaci&oacute;n de Tc99 a nivel parietal bilateral, temporal derecha y frontal superior izquierda(29).</P>     <P ALIGN="JUSTIFY">En cuanto a las anormalidades neuroendocrinas est&aacute;n: El aumento de la secreci&oacute;n de cortisol, aumento del tama&ntilde;o adrenal, disminuci&oacute;n de la respuesta inhibitoria de los glucocorticoides y respuesta inapropiada de los niveles de TSH a la infusi&oacute;n de TRH(24,30).</P>     <P ALIGN="JUSTIFY">Diversos estudios han revelado que la depresi&oacute;n mayor se acompa&ntilde;a de desordenes en amino&aacute;cidos excitatorios (glutamato y aspartato) y de alteraciones de niveles s&eacute;ricos de amino&aacute;cidos como serina, glicina y taurina(31).</P>     <P ALIGN="JUSTIFY">Existe evidencia de que durante la depresi&oacute;n hay disminuci&oacute;n de la concentraci&oacute;n de moduladores positivos del GABA como el 3 alfa, 5 alfa THP y 5 beta THP(32).</P>     <P ALIGN="JUSTIFY">Los aspectos bioqu&iacute;micos m&aacute;s relevantes en la fisiopatolog&iacute;a de la depresi&oacute;n son los siguientes: deficiencia funcional de la noradrenalina, bajas cantidades en l&iacute;quidos perif&eacute;ricos de &aacute;cido monovain&iacute;lico (HVA), principal catabolito de la dopamina, disminuci&oacute;n del precursor de la serotonina, menor frecuencia de las descargas de las neuronas de serotonina, descenso en la recaptaci&oacute;n plaquetaria de serotonina y predominio del sistema colin&eacute;rgico con aumento del n&uacute;mero de sus receptores(24,26).</P> <B>     <P ALIGN="JUSTIFY">TRATAMIENTO DE LA DEPRESI&Oacute;N</P> </B>     <P ALIGN="JUSTIFY">Las alternativas del tratamiento de la depresi&oacute;n son: Psicof&aacute;rmacos, electroconvulsi&oacute;n y Psicoterapia; siendo los Psicof&aacute;rmacos la terapia de elecci&oacute;n(25). En el &eacute;xito del tratamiento juega un papel decisivo la educaci&oacute;n de la poblaci&oacute;n, debido a que en la mayor&iacute;a de los casos hay una demora de aproximadamente 2 meses entre el inicio de la depresi&oacute;n y la instauraci&oacute;n de la terapia(33).</P> <B>    <P ALIGN="JUSTIFY">F&Aacute;RMACOS ANTIDEPRESIVOS</P>     <P ALIGN="JUSTIFY">Clasificaci&oacute;n</P><DIR>  </B>    <P ALIGN="JUSTIFY">1) Antidepresivos tric&iacute;clicos (ADTs) y drogas cl&iacute;nicamente similares.</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">2) Nuevos antidepresivos: <B>1)</B> Inhibidores selectivos de la recaptaci&oacute;n de serotonina (ISRSs). <B>2)</B> Inhibidores de la recaptaci&oacute;n de dopamina. <B>3)</B> Inhibidores de la recaptaci&oacute;n de noradrenalina. </P>     <P ALIGN="JUSTIFY">3)&#9;Inhibidores de la monoaminooxidasa (IMAO)(26,27).</P> </DIR>  <B>    <P ALIGN="JUSTIFY">Antidepresivos tric&iacute;clicos</P> </B>    <P ALIGN="JUSTIFY">Todos tienen un centro molecular de 3 anillos, sus estructuras son similares a las fenotiacinas, algunos tienen estructura de aminas secundarias: disipramina, nortriptilina y protriptilina. Los de amina terciaria son la imipramina, amitriptilina, trimipramina, doxepina y cloremipramina, (<a href="#c4">Ver cuadro 4</a>).</P> <B>    <P ALIGN="JUSTIFY">Mecanismo de acci&oacute;n:</B> Potencian acciones de las aminas bi&oacute;genas al bloquear sus medios de inactivaci&oacute;n fisiol&oacute;gica que comprenden: transporte o recaptaci&oacute;n en las terminaciones nerviosas(25,27,34). Para explicar este proceso debemos recordar que en las terminaciones nerviosas la tirosina se oxida hasta DOPA, la cual a continuaci&oacute;n se descarboxila hasta Dopamina (DA) y luego se almacena en ves&iacute;culas en las cuales la DA se convierte en Noradrenalina (NA), esta noradrenalina es descargada interactuando posteriormente con subtipos de receptores alpha 1, B-adren&eacute;rgicos posin&aacute;pticos y autoreceptores alpha 2 presin&aacute;pticos, produci&eacute;ndose trifosfato de Inositol y diacilglicerol, AMPc y modulaci&oacute;n de la s&iacute;ntesis de descarga de NA respectivamente(25).</P>     <P ALIGN="JUSTIFY">La inactivaci&oacute;n se produce principalmente por transporte activo (&quot;recaptaci&oacute;n&quot;) hacia terminaciones presin&aacute;pticas (inhibida por la mayor&iacute;a de los ADTs), con desaminaci&oacute;n secundaria por la MAO mitocondrial (bloqueado por la IMAO)(25). Luego de producir su efecto se degradan por hidroxilaci&oacute;n y glucoronizaci&oacute;n mediada por enzimas micros&oacute;micas hep&aacute;ticas para finalmente ser eliminadas por v&iacute;a renal(25,26).</P> <B>    <P ALIGN="JUSTIFY">Efectos colaterales:</B> Por su acci&oacute;n sobre el sistema muscarinico: sequedad de boca, sabor amargo o met&aacute;lico, estre&ntilde;imiento, visi&oacute;n borrosa, trastornos miccionales, cefalea, n&aacute;useas, tinitus, insomnio, mareos, taquicardia, palpitaciones, nerviosismo(25,26,27). Aumento de peso, impotencia, eyaculaci&oacute;n retr&oacute;grada y disosgarmo(27). A nivel cardiovascular hay hipotensi&oacute;n postural(25), disminuci&oacute;n de la conductividad cardiaca lo que significa un riesgo en pacientes con bloqueo de rama o bloqueo AV(26,27), inversi&oacute;n o aplanamiento de la onda T y tiempo de conducci&oacute;n prolongados(25). En cuanto al sue&ntilde;o, suprimen SMOR, experimentan sue&ntilde;os vividos y alteraciones en la continuidad del sue&ntilde;o, por esto se recomienda su reducci&oacute;n en forma gradual(26). </P> <B>    <P ALIGN="JUSTIFY">Contraindicaciones:</B> Pacientes con antecedentes de glaucoma de &aacute;ngulo cerrado. Las aminas terciarias y protriptilina en pacientes despu&eacute;s de infarto agudo del miocardio, con defectos de conducci&oacute;n o cuando se administran otros depresores card&iacute;acos(26).</P> <B>    <P ALIGN="JUSTIFY">Intoxicaci&oacute;n con antidepresivos:</B> Entre las manifestaciones cl&iacute;nicas tenemos: disminuci&oacute;n de lucidez, alucinaciones, delirio, agitaci&oacute;n, coma, midriasis, hipertermia, convulsiones, arritmia, bloqueo de rama, extras&iacute;stoles ventriculares. Durante esta etapa el paciente debe ser trasladado a una unidad de cuidados intensivos con apoyo respiratorio apropiado(25,26).</P>     <P ALIGN="JUSTIFY">Interacci&oacute;n con otras drogas: La fijaci&oacute;n de los ADTs a la alb&uacute;mina plasm&aacute;tica puede reducirse por competencia con diversos f&aacute;rmacos, entre ellos fenilhidantoina y fenilbutazona, &aacute;cido acetilsalic&iacute;lico, aminopirina, escopolamina y fenotiazinas. F&aacute;rmacos que interfieren en el metabolismo hep&aacute;tico como neurol&eacute;pticos, metilfenidato, algunos esteroides y anticonceptivos orales pueden potenciar el efecto de los ADTs. A la inversa, los barbit&uacute;ricos, otros anticonvulsivos (excepto las benzodiacepinas) y el tabaco, pueden aumentar el metabolismo hep&aacute;tico al inducir a los sistemas enzim&aacute;ticos microsomicos. Los antidepresivos potencian el efecto del alcohol y probablemente de otros sedantes(26,35).</P> </FONT><B><FONT SIZE=2>    ]]></body>

Todos bloquean la recaptura de serotonina, aumentando su disponibilidad y modificando la sensibilidad de los receptores 5-HT2b y 5-HT1-A. Se metabolizan en h&iacute;gado y excretan en orina(26).</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">&#9;Se debe cuidar su interacci&oacute;n con antidepresivos tric&iacute;clicos y con IMAO; con los primeros se produce aumento de sus niveles t&oacute;xicos y con los segundos se produce el s&iacute;ndrome serotonin&eacute;rgico(26). Los ISRSs tienen alto riesgo de inducir n&aacute;useas, v&oacute;mitos, insomnio, mareos, cefalea, disfunci&oacute;n sexual, trastornos de eyaculaci&oacute;n(26). Tiene poco efecto sobre el sue&ntilde;o y se han reportado varios casos de angina(27,36,37,38). </P>     <P ALIGN="JUSTIFY">&#9;La venlafaxina es un antidepresivo de primera elecci&oacute;n tiene mayor selectividad para serotonina, eficaz para tratar la depresi&oacute;n mayor, melancol&iacute;a, depresiones resistentes y depresi&oacute;n grave en el anciano. Se unen poco a prote&iacute;nas, por lo cual act&uacute;a directamente sobre el SNC y no produce efectos secundarios. No debe combinarse con IMAO porque trae consecuencias fatales(26).</P>     <P ALIGN="JUSTIFY">2)&#9;<B>Inhibidores de la recaptaci&oacute;n de Dopamina:</B> El bupropi&oacute;n, es el m&aacute;s importante, y se caracteriza por ser un antidepresivo unic&iacute;clico inhibidor leve de la captaci&oacute;n neuronal de monoaminas, el cual se absorbe bien en tracto gastrointestinal, se metaboliza en el h&iacute;gado, sus metabolitos se excretan en ri&ntilde;&oacute;n(27); a veces produce agitaci&oacute;n, insomnio, anorexia e incluso psicosis, adem&aacute;s de hipoman&iacute;a o man&iacute;a en enfermos bipolares(26). A dosis t&oacute;xicas mayores de 500 mg tiene riesgo de producir convulsiones t&oacute;nico cl&oacute;nicas(25).</P>     <P ALIGN="JUSTIFY">3) <B>Inhibidores de la recaptaci&oacute;n de Noradrenalina:</B> La Reboxetina es el primer inhibidor no tric&iacute;clico selectivo de la recaptaci&oacute;n de Noradrenalina. No tiene afinidad significativa por la serotonina, por el transportador adren&eacute;rgico,  transportador dopamin&eacute;rgico, colin&eacute;rgico o de receptores histamin&eacute;rgicos; sin embargo, regula los b-adrenoceptores m&aacute;s temprano que otros antidepresivos; mientras que se caracteriza por una latencia terap&eacute;utica acortada, aumentando los niveles de normetanefrina en el plasma de manera aguda, el cual es un metabolito de la norepinefrina. Esta droga, a una dosis oral de 8-10 mg/d&iacute;a, ha demostrado ser m&aacute;s eficaz que el placebo y se compara con 200 Mg de desipramine oral, con 75-100 Mg de los imipramines orales y con 20-40 mg/d&iacute;a de fluoxetine oral; sin embargo, los efectos colaterales son menores con el reboxetine con respecto a los antidepresivos tric&iacute;clicos. Se demostr&oacute; su eficacia en 4-8 experimentos realizados a corto plazo, as&iacute; como en un estudio con una duraci&oacute;n de 1 a&ntilde;o. Se observaron: dificultad con la micci&oacute;n, taquicardia, diaforesis, boca seca, sue&ntilde;o y perturbaciones er&eacute;ctiles con el reboxetine m&aacute;s a menudo que con el placebo, considerando que los efectos de la suspensi&oacute;n no fueron significativos. No hubo interacci&oacute;n con otros antidepresivos a nivel de isoenzymas microsomales hep&aacute;ticas P450. La Reboxetine representa un antidepresivo eficaz en la base moderna de la teor&iacute;a de la depresi&oacute;n noradren&eacute;rgica(39,40). </P>     <P ALIGN="JUSTIFY"></P>     <P ALIGN="JUSTIFY">4)<B> Inhibidores de la recaptaci&oacute;n de Noradrenalina y Serotonina: </B>El milnacipran se usa con preferencia en ancianos, menor incidencia de efectos indeseables y no empeora la habilidad cognitiva(41).</P>     <P ALIGN="JUSTIFY"></P></DIR>  <B>    <P ALIGN="JUSTIFY">Inhibidores de la monoaminooxidasa (IMAO)</P> </B>    <P ALIGN="JUSTIFY">Tenemos dos grupos seg&uacute;n su estructura: Hidracinas y no hidracinas(25,26,34). Existen 2 tipos de IMAO: la de tipo A representa el 20% y se encarga de degradaci&oacute;n de la NA, serotonina, dopamina y tiramina y la de tipo B, que representa el 80% y se encarga de la degradaci&oacute;n de las fentolaminas y dopamina. Los IMAO bloquean el efecto catal&iacute;tico de las catecolaminas en diferentes regiones del cuerpo, aumentando los niveles de las mismas en plasma(26). Son drogas de primera elecci&oacute;n en depresi&oacute;n at&iacute;pica, des&oacute;rdenes de p&aacute;nico o depresi&oacute;n refractaria(27). </P>     <P ALIGN="JUSTIFY">Existen IMAO irreversibles eficaces en los episodios depresivos mayores, no son &uacute;tiles para las depresiones psic&oacute;ticas. Existe una nueva familia de IMAO reversibles que ocasionan menos efectos secundarios y son menos hipertensos que los irreversibles(26).</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Los IMAO se absorben con facilidad por v&iacute;a oral. Producen inhibici&oacute;n m&aacute;xima en 5 a 10 d&iacute;as(25). Estas drogas pueden inducir sedaci&oacute;n o excitaci&oacute;n de la conducta, hipotensi&oacute;n postural y crisis hipertensivas, aumentando esta probabilidad al combinarlas con ISRSs. Otros efectos: insomnio, somnolencia diurna, aumento de peso, disfunci&oacute;n sexual, mioclonus y edema en miembros inferiores, todo lo cual los convierten en f&aacute;rmacos de segunda elecci&oacute;n(26). </P>     <P ALIGN="JUSTIFY">La intoxicaci&oacute;n<B> </B>se presenta en horas y entre las reacciones tenemos: agitaci&oacute;n, alucinaciones, hiperreflexia, hiperpirexia, convulsiones, hipotensi&oacute;n e hipertensi&oacute;n. Se ha sugerido que la crisis hipertensiva asociada a la inhibici&oacute;n de la MAO es producto de que la tiramina escapa a la desaminaci&oacute;n oxidativa, descarg&aacute;ndose catecolaminas de las terminaciones nerviosas y m&eacute;dula suprarrenal. Una fuente rica en tiramina, entre los alimentos, son los quesos envejecidos(25,26,27).</P> <B>    <P ALIGN="JUSTIFY">Aplicaciones terap&eacute;uticas:</B> Depresi&oacute;n mayor del adulto, supresi&oacute;n r&aacute;pida, pero moment&aacute;nea de la enuresis en ni&ntilde;os y pacientes ancianos. Hiperactividad con d&eacute;ficit de atenci&oacute;n y trastornos de tic (Imipramina, desipramina, nortriptilina). Trastornos graves de ansiedad. En el stress postraum&aacute;tico y trastornos del p&aacute;nico se utilizan la Imipramina y fenelzina, pero tambi&eacute;n muestran eficacia los ISRSs, que hoy se prefieren para tratar la enfermedad obsesivo compulsiva. Otras indicaciones incluyen trastornos psicosom&aacute;ticos como bulimia nerviosa, enfermedades con dolor cr&oacute;nico, s&iacute;ndrome de colon irritable, fatiga cr&oacute;nica, apnea del sue&ntilde;o, entre otros, (ADTs, IMAO, o ISRSs)(25,26).</P>     <P ALIGN="JUSTIFY"></P> <B>    <P ALIGN="JUSTIFY">&nbsp;</P>     <P ALIGN="JUSTIFY">F&Aacute;RMACOS ANTIMANIACOS</P>     <P ALIGN="JUSTIFY"></P> </B>    <P ALIGN="JUSTIFY">Actualmente el Litio es el tratamiento de elecci&oacute;n para tratar la man&iacute;a y prevenir crisis recurrente de la enfermedad man&iacute;aco depresiva. En cuanto a su mecanismo de acci&oacute;n, lo que se conoce es que en el tejido encef&aacute;lico, inhibe la secreci&oacute;n de NA y dopamina desde las terminaciones nerviosas, pudiendo intensificar la descarga de serotonina y alterando la recaptaci&oacute;n y almacenamiento de catecolaminas(25).</P>     <P ALIGN="JUSTIFY">Se absorbe bien por v&iacute;a oral con concentraci&oacute;n plasm&aacute;tica m&aacute;xima a las 4 horas, se distribuye primero en el liquido extracelular y luego se acumula en los tejidos, no se fija en forma apreciable a prote&iacute;nas plasm&aacute;ticas; se elimina por orina en 95%. La vida media es de 12 a 24 horas, con un &iacute;ndice terap&eacute;utico bajo. Sus cifras eficaces y seguras est&aacute;n entre 0,75 y 1,25 meq/l y suelen lograrse con dosis de 900 a 1500 Mg de carbonato de Li+/d&iacute;a en pacientes externos y 1200 a 2400 mg/d&iacute;a en hospitalizados(25).</P> <B>    <P ALIGN="JUSTIFY">Intoxicaci&oacute;n Aguda:</B> N&aacute;useas, v&oacute;mitos, diarrea, temblor, ataxia, convulsiones y coma. Los efectos m&aacute;s graves son confusi&oacute;n, hiperreflexia, temblor burdo, disartria, convulsiones, y signos neurol&oacute;gicos de pares craneales y focales hasta el coma y muerte(25).</P> <B>    ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Interacciones:</B> Los diur&eacute;ticos aumentan la retenci&oacute;n de Li+, en especial la tiazidas; el triamtireno aumenta su excreci&oacute;n. Algunos AINES aumentan la resorsi&oacute;n tubular y las concentraciones plasm&aacute;ticas a niveles t&oacute;xicos. Tambi&eacute;n ocurre interacci&oacute;n con los IECA(25,26).</P> <B>    <P ALIGN="JUSTIFY">Indicaciones</B>: La terap&eacute;utica de la man&iacute;a aguda y prevenci&oacute;n de las recurrencias de la enfermedad man&iacute;aco depresiva bipolar en adultos o adolescentes, por lo dem&aacute;s sanos, son las &uacute;nicas aplicaciones aprobadas en la actualidad por la FDA. En ocasiones se utiliza como equivalente o auxiliar de los antidepresivos en la depresi&oacute;n recurrente grave, como complemento del tratamiento en la depresi&oacute;n mayor aguda o como coadyuvante cuando es insastifactoria la reacci&oacute;n tard&iacute;a a un antidepresor administrado solo(42).</P> <B>    <P ALIGN="JUSTIFY">CONCLUSIONES Y RECOMENDACIONES</P> </B>    <P ALIGN="JUSTIFY">En cuanto a la ansiedad, los desordenes de p&aacute;nico usualmente no responden a BZD, est&aacute;n indicados para su manejo terap&eacute;utico los antidepresivos tric&iacute;clicos, inhibidores de la MAO e inhibidores selectivos de la recaptaci&oacute;n de serotonina(1,43).</P>     <P ALIGN="JUSTIFY">En los trastornos de ansiedad generalizada las BZD tipo diazepam y la buspirona son los ansiol&iacute;ticos de elecci&oacute;n. Los betabloqueantes, como el propanolol, pueden reducir los s&iacute;ntomas som&aacute;ticos neurovegetativos que acompa&ntilde;an al cuadro de ansiedad. La glutetimida, el meprobamato y los barbit&uacute;ricos (a excepci&oacute;n del fenobarbital) son f&aacute;rmacos que deben evitarse debido al elevado potencial de adicci&oacute;n y al escaso margen de seguridad.</P>     <P ALIGN="JUSTIFY">En lo que respecta a las BZD, est&aacute;n indicadas para la ansiedad y el insomnio. Las de acci&oacute;n prolongada se utilizan para calmar la ansiedad y el s&iacute;ndrome de abstinencia alcoh&oacute;lica, mientras que las de acci&oacute;n corta se utilizan como sedantes en la terap&eacute;utica del insomnio (Lorazepam) y procedimientos m&eacute;dicos como endoscopias (midazolam). No deben prescribirse por m&aacute;s de 4 a 6 semanas a causa del desarrollo potencial de tolerancia y el riesgo de abuso y dependencia(5,12,13). Por otra parte, las azapironas (buspirona) son drogas prometedoras, ya que no tienen efecto sedante, no producen tolerancia o dependencia, no tienen efecto aditivo con depresores del SNC y no tienen riesgo potencial de abuso, adem&aacute;s, parecen ser bien toleradas con su uso prolongado y pueden ser &uacute;tiles en el paciente con depresi&oacute;n sobrea&ntilde;adida(15).</P>     <P ALIGN="JUSTIFY">Los des&oacute;rdenes obsesivos compulsivos responden a la terapia serotonin&eacute;rgica, como fluoxetine y fluvoxamine y a la buspirona(1).</P>     <P ALIGN="JUSTIFY">Cabe mencionar que los trastornos de ansiedad son un problema com&uacute;n en el paciente geri&aacute;trico. A causa de que el paciente anciano es m&aacute;s vulnerable a efectos secundarios como sedaci&oacute;n e hipotensi&oacute;n ortost&aacute;tica, se recomienda utilizar las azapironas, las BZD de acci&oacute;n corta y los ISRSs si son necesarios, pero primordialmente debe emplearse la terapia psicosocial y deben evitarse los agentes sedantes(44,45,46,47,48).</P>     <P ALIGN="JUSTIFY">Es importante seguir varias recomendaciones para la prescripci&oacute;n de BZD y sedantes e hipn&oacute;ticos en general: </P>     <P ALIGN="JUSTIFY"></P><DIR>      ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">1)&#9;Evaluar la posibilidad de un enfoque terap&eacute;utico no farmacol&oacute;gico para el paciente.</P>     <P ALIGN="JUSTIFY">2)&#9;Considerar los factores psicosociales o de personalidad que impliquen mayor riesgo de abuso o adicci&oacute;n.</P>     <P ALIGN="JUSTIFY">3)&#9;Individualizar el tratamiento.</P>     <P ALIGN="JUSTIFY">4)&#9;Evitar un tratamiento prolongado.</P>     <P ALIGN="JUSTIFY">5)&#9;En pacientes de edad avanzada comenzar con la mitad de la dosis usual, evita los ajustes frecuentes de las dosis y estar alerta para detectar signos de confusi&oacute;n, sedaci&oacute;n y ataxia.</P>     <P ALIGN="JUSTIFY">6)&#9;Si se requiere aumento de dosis evaluar cuidadosamente la cl&iacute;nica y la probabilidad de abuso (la mayor&iacute;a de las BZD no requieren aumento de la dosis durante varios a&ntilde;os).</P>     <P ALIGN="JUSTIFY"></P></DIR>      <P ALIGN="JUSTIFY">En cuanto a la depresi&oacute;n son importantes varias recomendaciones:</P>     <P ALIGN="JUSTIFY"></P><DIR>      <P ALIGN="JUSTIFY">1)&#9;Si un sujeto est&aacute; gravemente deprimido con ideaci&oacute;n suicida, lo sensato ser&aacute; vigilarlo, administr&aacute;ndole medicaci&oacute;n para 1 semana cada vez o elegir el f&aacute;rmaco menos peligroso.</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">2)&#9;Los nuevos antidepresivos deben pasar a ser drogas de primera l&iacute;nea en pacientes deprimidos en vista de que carecen de los efectos adversos de los ADTs. De ellos la venlafaxina es de primera elecci&oacute;n, sobre todo en casos de depresi&oacute;n mayor, melancol&iacute;a, depresiones resistentes y depresi&oacute;n grave en el anciano. Los ISRSs se deben utilizar tambi&eacute;n en depresi&oacute;n mayor, pero son de elecci&oacute;n en trastornos obsesivo compulsivos y de p&aacute;nico.</P>     <P ALIGN="JUSTIFY">3)&#9;Se debe evitar los ADTs en pacientes post infarto, con defectos de conducci&oacute;n o con el uso de otros presores card&iacute;acos. Los ISRSs se han usado con seguridad en este tipo de pacientes.</P>     <P ALIGN="JUSTIFY">4)&#9;En pacientes con glaucoma no usar ADTs, se prefieren los ISRSs.</P>     <P ALIGN="JUSTIFY">5)&#9;Los ISRSs y venlafaxina no deben combinarse con IMAO por su efecto letal, y se debe tener cuidado ya que tambi&eacute;n se han visto reacciones graves cuando se combinan IMAO y ADTs.</P>     <P ALIGN="JUSTIFY">6)&#9;Aunque el litio se usa en caso de trastorno bipolar en ocasiones se debe tener presente como complemento en algunos casos de depresi&oacute;n.</P> <B></DIR>      <P ALIGN="JUSTIFY">REFERENCIAS BIBLIOGR&Aacute;FICAS</P> <DIR> <DIR>  </B>    <!-- ref --><P ALIGN="JUSTIFY">1.
