Introducción: Aunque han sido descritos diversos síndromes relacionados con el procesamiento cognitivo del dolor, como consecuencia de lesiones cerebrales en áreas específicas, los estudios en demencias sólo han sido dirigidos a la investigación del umbral y la tolerancia. Sin embargo, existen diversas manifestaciones similares a dolor en algunos pacientes con demencia, lo cual no ha sido reportado previamente. Métodos: Se presenta un estudio descriptivo transversal de una serie de casos. Se seleccionaron 14 de 27 residentes en un centro geriátrico, con diagnóstico de demencia según criterios de DSM-IV y NINCDS-ADRDA. 6 presentaron conductas similares a dolor sin estímulos nociceptivos evidentes. Se utilizó una escala analógica para dolor con dibujos de caras (FPS), tanto por los pacientes como por el cuidador, antes y después del tratamiento con Gabapentin. Resultados: Los puntajes en FPS mostraron diversos patrones perceptivos del dolor y disminuyeron en los pacientes tratados. Conclusión: Las conductas similares a dolor en pacientes con demencia se ajustaron a un modelo cognitivo y el Gabapentin fue efectivo para disminuir su intensidad.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[To demonstrate that like-pain behaviour in demented patient is a cognitive disorder following dysfunction in cerebral mechanisms of pain regulation and therefore treatable with a neuromodulator. Background: although various syndromes of cerebral processing of information about pain following to brain lesions in specific areas has been described, studies in demented patients has just been focused on both tolerance and threshold. However, there are several like-pain behaviors in some demented patients, specially in late stages that have not been reported previously. Methods: A transversal descriptive study of a clinical series is shown. 14 out of 27 patients from a geriatric center which fulfilled both DSM-IV and NINCDS-ADRDA dementia criteria and 6 out of these 14 which showed like-pain behaviours, without any noxious stimulus and one caregiver in charge of all these patients. A Facial Pain Scale (FPS) was asked to both patient and caregiver before and after treatment with Gabapentin. Results: scores in FPS showed various perceptive patterns for pain and fell down in patients treated with Gabapentin. Conclusion: Like-pain behaviours in demented patients fitted into a cognitive pattern and Gabapentin was effective in release them.]]></p></abstract>

Es el s&iacute;ntoma m&aacute;s frecuente de enfermedad. Casi todas las enfermedades muestran el dolor como s&iacute;ntoma en alguna de sus etapas.</P>     <P ALIGN="JUSTIFY">Una gran proporci&oacute;n de las neuronas nociceptivas medulares env&iacute;an sus axones a centros supraespinales, bulbares y tal&aacute;micos. La mayor parte de la informaci&oacute;n nociceptiva se transmite por v&iacute;as cruzadas ascendentes, tradicionalmente se consideran los fasc&iacute;culos espino-tal&aacute;mico, espino-reticular y espino-mesencef&aacute;lico; pero la mayor&iacute;a de las v&iacute;as espinales ascendentes contienen axones de neuronas nociceptivas. La integraci&oacute;n de los componentes sensoriales, discriminativos y afectivos del dolor requieren de la participaci&oacute;n del t&aacute;lamo, los n&uacute;cleos diencef&aacute;licos subtal&aacute;micos y la corteza cerebral(2). El componente discriminativo-sensorial est&aacute; mediado fundamentalmente por el complejo ventrobasal del t&aacute;lamo y por la corteza somatosensorial. El componente afectivo est&aacute; mediado por los n&uacute;cleos tal&aacute;micos mediales y zonas corticales multimodales, especialmente la corteza supraorbitaria de las regiones prefrontales(3,4,5).</P>     <P ALIGN="JUSTIFY">Las alteraciones del procesamiento de la informaci&oacute;n nociceptiva han sido asociadas a lesiones cerebrales en &aacute;reas espec&iacute;ficas. Sin embargo, en la literatura revisada, no hemos encontrado referencias a lesiones difusas, como es el caso de las demencias. Los estudios realizados en este tipo de pacientes han sido enfocados en el umbral y la tolerancia al dolor(6,7), pero no a los aspectos cognitivos de &eacute;ste. En nuestra pr&aacute;ctica cl&iacute;nica, observamos 32 pacientes con demencias de diversas etiolog&iacute;as, quienes mostraban conductas que habitualmente son asociadas a estimulaci&oacute;n nociceptiva. Estos pacientes presentaban las siguientes caracter&iacute;sticas, ya sea en forma aislada o en combinaci&oacute;n: Manifestaci&oacute;n verbal de dolor sin localizaci&oacute;n anat&oacute;mica; dolor con localizaci&oacute;n anat&oacute;mica, m&aacute;s frecuentemente en extremidades, sin asociaci&oacute;n a una noxa; dolor intenso como respuesta a est&iacute;mulos habitualmente no dolorosos; entre &eacute;stos, un grupo de pacientes mostraba manifestaciones conductuales de gran intensidad, negando inmediatamente sentir alg&uacute;n dolor. 15 de estos pacientes fueron tratados con Gabapentin, en dosis de 300 a 900 mg diarios y los resultados fueron evaluados mediante la impresi&oacute;n sujetiva de cambio, tanto por los cuidadores como por los m&eacute;dicos tratantes. Todos los sujetos presentaron mejor&iacute;a.</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">El Gabapentin es un neuromodulador, cuyo mecanismo de acci&oacute;n principal se debe a la interacci&oacute;n en la subunidad alfa2 de los receptores de &aacute;cido gamma-aminobut&iacute;rico (GABA) de los canales de calcio voltaje dependiente del tipo L(8); puede aumentar los niveles de GABA y disminuir los de glutamato(9,10), entre otras acciones. Ha sido utilizado en el tratamiento de epilepsias(11-13), dolor neurop&aacute;tico(14,15) y ciertas manifestaciones psiqui&aacute;tricas(16,17,18), particularmente en trastornos bipolares(17,18). Los estudios llevados a cabo tanto en animales de experimentaci&oacute;n como con im&aacute;genes cerebrales funcionales muestran que el Gabapentin act&uacute;a en diversas &aacute;reas cerebrales, entre ellas el hipocampo y el sistema mesol&iacute;mbico, las cuales juegan un papel determinante en las manifestaciones neurocognitivas de las demencias(19,20).</P>     <P ALIGN="JUSTIFY">El presente trabajo es un estudio descriptivo transversal, cuyo objetivo es demostrar que las conductas similares a dolor en pacientes con demencia corresponden a un trastorno cognitivo como consecuencia de una disfunci&oacute;n en los mecanismos cerebrales de la regulaci&oacute;n del dolor y que pueden ser tratadas con Gabapentin.</P> <B>    <P ALIGN="JUSTIFY">Materiales y m&eacute;todos</P>     <P ALIGN="JUSTIFY">Sujetos</P> </B>    <P ALIGN="JUSTIFY">El estudio se llev&oacute; a cabo en un centro geri&aacute;trico de Caracas (Venezuela) con una poblaci&oacute;n de 27 residentes ancianos. 14 de estos sujetos llenaron los criterios para el diagn&oacute;stico de demencia(21,22) y, finalmente, 6 fueron seleccionados de acuerdo a los criterios de inclusi&oacute;n y exclusi&oacute;n propuestos. Los pacientes fueron evaluados mediante un examen m&eacute;dico general y neurol&oacute;gico. Se realizaron: una prueba breve del estado mental: &quot;Minimental State Examination&quot; (MMSE)(23); una escala funcional: &quot;Blessed Dementia Rating Scale&quot; (BDRS)(24); una escala de severidad de la demencia: &quot;Global Dementia Scale&quot; (GDS)(25); una escala anal&oacute;gica de severidad de los s&iacute;ntomas y/o signos medidos como percepci&oacute;n de dolor: &quot;Facial Pain Scale&quot; (FPS), aplicada en situaci&oacute;n pretratamiento y a las 48 horas de tratamiento con 300 mg/d&iacute;a de Gabapentin, al Cuidador (FPSC) y a los Pacientes capaces de colaborar (FPSP).</P> <B>     <P ALIGN="JUSTIFY">Criterios de inclusi&oacute;n</P> </B>    <P ALIGN="JUSTIFY">Fueron incluidos los sujetos que llenaron los criterios de demencia establecidos en el DSM-IV y NINCDS-ADRDA; puntaje menor o igual a 21 puntos en el MMSE; con manifestaciones conductuales o verbales de dolor, con o sin localizaci&oacute;n anat&oacute;mica, sin una causa m&eacute;dica o traum&aacute;tica demostrable, con duraci&oacute;n de al menos 30 d&iacute;as para el momento del examen. Para ser incluidos en este estudio los familiares debieron firmar un consentimiento.</P> <B>     <P ALIGN="JUSTIFY">Criterios de exclusi&oacute;n</P> </B>    <P ALIGN="JUSTIFY">Pacientes con alteraciones cognitivas asociadas a otras enfermedades mentales, tales como: retardo mental, epilepsia o esquizofrenia; pacientes con polineuropat&iacute;as de cualquier etiolog&iacute;a; pacientes con diagn&oacute;stico de diabetes mellitus; traumatismos o lesiones locales asociadas temporal y/o topogr&aacute;ficamente a los s&iacute;ntomas o conductas similares a dolor; sospecha de delirium; pacientes con medicamentos que mostraran alguna asociaci&oacute;n temporal con las conductas evaluadas. </P> <B>    <P ALIGN="JUSTIFY">Resultados</P> </B>     ]]></body>

Localiza el dolor en forma variable: cuello, regi&oacute;n lumbar, frente y, m&aacute;s frecuentemente, en los test&iacute;culos. </P> <B>    <P ALIGN="JUSTIFY">Caso 2: </B>mujer de 87 a&ntilde;os. Demencia Mixta (DM): Demencia Tipo Alzheimer (DTA) + DV. Facies de dolor, sin localizaci&oacute;n anat&oacute;mica.</P> <B>    <P ALIGN="JUSTIFY">Caso 3: </B>mujer de 80 a&ntilde;os. DTA. Facies de dolor, localiza el dolor en la cabeza, piernas y brazos; adicionalmente, quejas de fr&iacute;o excesivo a&uacute;n vistiendo ropa abrigada.</P> <B>    <P ALIGN="JUSTIFY">Caso 4: </B>mujer de 86 a&ntilde;os. Las estructuras supraespinales que han sido relacionadas a la modulaci&oacute;n del dolor son: la sustancia gris periacueductal (SGPA), el bulbo rostral ventromedial (VRVM), el tegmento pontino dorsolateral y las l&aacute;minas superficiales del asta dorsal. Los aspectos afectivos del dolor son, al menos en parte, modulados por las v&iacute;as espinoreticulares, que tambi&eacute;n conducen el dolor difuso de los tejidos profundos; haciendo conexiones difusas a trav&eacute;s del tallo cerebral y t&aacute;lamo, hasta llegar al sistema l&iacute;mbico y l&oacute;bulos frontales(4).</P>     <P ALIGN="JUSTIFY">La participaci&oacute;n de estas &uacute;ltimas estructuras refuerza nuestro planteamiento de hacer un abordaje del dolor desde un punto de vista cognitivo, ya que ellas son puntos cr&iacute;ticos para la traslaci&oacute;n activa, repetici&oacute;n, almacenamiento y evocaci&oacute;n, involucrando aspectos de la experiencia que requieren pensamiento y memoria(27). Aunque el dolor en pacientes con demencia ha sido estudiado por varios autores, las investigaciones han estado enfocadas en el umbral y tolerancia(6,7), mientras que las alteraciones en el procesamiento cognitivo de la informaci&oacute;n dolorosa s&oacute;lo han sido reportada para lesiones focales(28,29). En el presente trabajo se describen estos fen&oacute;menos cognitivos para la informaci&oacute;n nociceptiva en pacientes con lesiones difusas, lo cual no ha sido reportado en la literatura consultada. La presencia de conductas similares a dolor sin que haya percepci&oacute;n del mismo (caso 2), o que la propia percepci&oacute;n de da&ntilde;o del paciente sea superior a lo evidenciado por el cuidador (casos 1 y 3) son consistentes con la noci&oacute;n de doble disociaci&oacute;n de las conductas similares a dolor, como funci&oacute;n cognitiva, cuya alteraci&oacute;n hemos denominado s&iacute;ndrome cognitivo doloroso. Las desconexiones progresivas entre diferentes sistemas funcionales cerebrales y la reorganizaci&oacute;n de los mapas sensitivos y motores pueden ser la fuente de estos hallazgos, como ha sido descrito en trastornos cognitivos por lesiones espec&iacute;ficas(30,31).</P>     <P ALIGN="JUSTIFY">Aunque el tratamiento con Gabapentin fue posible s&oacute;lo en tres pacientes, fue efectivo en todos ellos. Por lo tanto, &eacute;sta puede ser una manera racional de tratamiento para aquellos pacientes con demencia que muestran esta forma espec&iacute;fica de agitaci&oacute;n, mejorando la calidad de vida, tanto de ellos como la de los familiares y cuidadores. Una posible explicaci&oacute;n para la efectividad del tratamiento con Gabapentin es su efecto en hipocampo y sistema mesol&iacute;mbico(32).</P>     <P ALIGN="JUSTIFY">Ser&aacute;n necesarios otros estudios controlados, con muestras m&aacute;s grandes, para demostrar la efectividad del Gabapentin en el tratamiento de este tipo de trastorno, as&iacute; como tambi&eacute;n el estudio con neuroim&aacute;genes, convencionales y funcionales, para determinar las &aacute;reas funcionales cr&iacute;ticas para tales manifestaciones. </P> <B>     <P ALIGN="JUSTIFY">Agradecimientos </P> </B>    ]]></body>
