Estos son normalmente clasificados en base a su selectividad por los receptores beta. Los beta-bloqueantes no selectivos, como propanolol, pindolol, nadolol, timolol y labetalol antagonizan los receptores Î²1 y Î²2. Los beta-bloqueantes selectivos, como el metoprolol, atenolol, esmolol y acebutolol tienen mayor afinidad por los receptores b-1. Los beta-bloqueantes selectivos estÎ±n indicados en pacientes en quienes el bloqueo b-2 puede asociar un aumento en el riesgo de efectos adversos. Como en pacientes asmÎ±ticos o diabÎ¹ticos, asÎ½ como en pacientes con enfermedad vascular perifÎ¹rica o enfermedad de Raynaud. Algunos beta-bloqueantes tienen actividad agonista parcial (actividad simpÎ±tico mimÎ¹tica intrÎ½nseca). Los beta-bloqueantes, como monoterapia o combinados con otras drogas han probado que tienen efectividad en la reducción de síntomas de angina de pecho y reducción de la morbi-mortalidad después del infarto al miocardio. Además son las drogas de elección en pacientes con hipertensión arterial e infarto al miocardio o angina. Muchos beta-bloqueantes son comúnmente usados en el tratamiento de pacientes con migraña recurrente, cardiomiopatía hipertrófica obstructiva, ansiedad aguda y en casos de glaucoma. The beta-blockers are normally distinguished based on their selectivity for beta-receptors. The nonselective beta-blockers, including propanolol, pindolol, nadolol, timolol and labetalol each antagonize both beta-1 and beta-2 adrenergic receptors. The Selective antagonists including metoprolol, atenolol, esmolol and acebutolol, each has much greater binding affinity for the beta-1 adrenergic receptor. The selective beta-blockers are normally indicated for patients in whom beta-2 receptor antagonism might be associated with an increased risk of adverse effects. Such patients include those with asthma or diabetes, or patients with peripheral vascular disease or Raynaudâ€˜s disease. Some beta-blockers have partial agonist (intrinsic sympathomimetic) activity. Beta-blockers, as monotherapy or combined with others drugs, have proven effective in reducing the symptoms of angina pectoris and in reducing morbidity and mortality after myocardial infarction. Therefore, are the drugs of choice in patients with elevated blood pressure and myocardial infarction or angina. Many are commonly used in the treatment of patients with frequent recurrent migraine, hypertrophic obstructive cardiomyopathy, acute anxiety, and glaucoma. <body><![CDATA[<P ALIGN="JUSTIFY">Los beta-bloqueantes son f&aacute;rmacos que han sido ampliamente utilizados en la pr&aacute;ctica m&eacute;dica como antiarr&iacute;tmicos y antianginosos, comprob&aacute;ndose posteriormente su efecto antihipertensivo.</P>        <P ALIGN="JUSTIFY">El primer beta-bloqueante utilizado como f&aacute;rmaco antihipertensivo fue el propanolol, desarroll&aacute;ndose posteriormente un gran n&uacute;mero de derivados que difieren entre s&iacute; por sus propiedades farmacol&oacute;gicas, tales como su cardioselectividad, actividad simpaticomim&eacute;tica intr&iacute;nseca, actividad estabilizante de la membrana, liposolubilidad o bloqueo alfa asociado, lo que confiere a esta familia de f&aacute;rmacos una gran heterogeneidad. Es importante se&ntilde;alar la relevancia del conocimiento detallado del sistema nervioso aut&oacute;nomo y de los sitios de acci&oacute;n de las drogas que act&uacute;an sobre los receptores adren&eacute;rgicos para el conocimiento adecuado de la farmacolog&iacute;a y los usos terap&eacute;uticos de esta importante clase de drogas en la terap&eacute;utica antihipertensiva.</P>        <P ALIGN="JUSTIFY">En el presente cap&iacute;tulo revisaremos los aspectos fundamentales para el manejo de este grupo de medicamentos como herramienta de primera l&iacute;nea en la terap&eacute;utica antihipertensiva.</P>        <P ALIGN="JUSTIFY"><B>AGENTES BETA-BLOQUEANTES</P>    </B>    <P ALIGN="JUSTIFY">Los beta-bloqueantes son drogas que inhiben competitivamente los receptores beta-adren&eacute;rgicos en varios &oacute;rganos y modulan la actividad de el sistema nervioso simp&aacute;tico; actuando sobre dos tipos de receptores que son los beta-1 y los beta-2, produciendo bloqueo de distinta magnitud y tipo dependiendo del f&aacute;rmaco especifico utilizado y del tipo de receptor inhibido.</P>        <P ALIGN="JUSTIFY">En el coraz&oacute;n predominan los receptores beta-1, que tienen efectos cronotr&oacute;pico, inotr&oacute;pico, dromotr&oacute;pico y batmotr&oacute;pico positivos. Algunos producen solamente bloqueo beta-1, en tanto que otros como el propanolol muestran una inhibici&oacute;n tanto beta-1 como beta-2, otros producen bloqueo de los receptores alfa y beta como el labetalol. <body><![CDATA[<P ALIGN="JUSTIFY">Hoy en d&iacute;a estos f&aacute;rmacos constituyen herramienta de elecci&oacute;n en el manejo del paciente hipertenso con ansiedad intensa, con hipertensi&oacute;n hipercin&eacute;tica y con cardiopat&iacute;a isqu&eacute;mica coexistente. Los beta-bloqueantes son considerados f&aacute;rmacos de primera l&iacute;nea, por su efectividad probada, en la reducci&oacute;n de los s&iacute;ntomas de la angina de pecho y en la reducci&oacute;n de la morbilidad y mortalidad despu&eacute;s del infarto al miocardio, como en la reducci&oacute;n de su recurrencia<SUP>(3,4)</SUP>, recomend&aacute;ndose para este fin aquellos beta-bloqueantes sin actividad simp&aacute;tico mim&eacute;tica intr&iacute;nseca.</P>        <P ALIGN="JUSTIFY">Seg&uacute;n el VI informe del comit&eacute; nacional conjunto sobre prevenci&oacute;n, detecci&oacute;n, evaluaci&oacute;n y tratamiento de la hipertensi&oacute;n arterial (JNC VI) de los EEUU, se concluyo que los diur&eacute;ticos y los beta-bloqueantes son los f&aacute;rmacos de primera l&iacute;nea en el tratamiento de la hipertensi&oacute;n arterial en pacientes que no presenten contraindicaciones para su uso<SUP>(5)</SUP>. Los beta-bloqueantes han demostrado ampliamente su eficacia, tanto en monoterapia como asociados, en el tratamiento de la hipertensi&oacute;n arterial ligera-moderada, as&iacute; como en la prevenci&oacute;n de sus complicaciones cardio-vasculares. Estudios han informado que los bloqueantes beta producen un mejoramiento sintom&aacute;tico en pacientes con insuficiencia cardiaca, incremento de la fracci&oacute;n de eyecci&oacute;n, disminuci&oacute;n de internaciones por descompensaci&oacute;n y disminuci&oacute;n en la mortalidad de pacientes con insuficiencia cardiaca leve a moderada<SUP>(6,7,8)</SUP>.</P>        <P ALIGN="JUSTIFY">En general, la respuesta hipotensora a los beta-bloqueantes es mayor en los pacientes hipertensos j&oacute;venes, los varones y los sujetos de raza blanca. Estos f&aacute;rmacos est&aacute;n especialmente indicados en la hipertensi&oacute;n que cursa con renina elevada, gasto cardiaco elevado, taquiarritmias, cardiopat&iacute;a isqu&eacute;mica, insuficiencia cardiaca, miocardiopat&iacute;a hipertr&oacute;fica, estr&eacute;s, ansiedad, migra&ntilde;a o hipertiroidismo. Por otro lado, su eficacia antihipertensiva es inferior en sujetos ancianos<SUP>(9-10)</SUP>, o de raza negra.</P>        <P ALIGN="JUSTIFY"><B>FARMACOCIN&Eacute;TICA</P>    </B>    <P ALIGN="JUSTIFY">La mayor&iacute;a de los medicamentos beta-bloqueantes tienen buena absorci&oacute;n cuando se administran por v&iacute;a oral, estos son absorbidos del tracto gastrointestinal por difusi&oacute;n pasiva; su absorci&oacute;n no es considerada estereoselectiva<SUP>(11)</SUP>.</P>        <P ALIGN="JUSTIFY">La absorci&oacute;n oral del nadolol y el atenolol es de aproximadamente del 45%, alcanzando concentraciones plasm&aacute;ticas efectivas a los 30 minutos de su administraci&oacute;n oral, obteni&eacute;ndose los m&aacute;ximos niveles plasm&aacute;ticos entre la 1 a 3 horas de su ingesti&oacute;n.</P>        <P ALIGN="JUSTIFY">Con relaci&oacute;n a la biod&iacute;iponibilidad oral, el propanolol por ejemplo, sufre un intenso metabolismo de primer paso que elimina antes de entrar a la circulaci&oacute;n sist&eacute;mica alrededor del 70% de la dosis terap&eacute;utica del f&aacute;rmaco administrado. Para datos individuales de la farmacocin&eacute;tica de cada beta-bloqueante, (ver la <a href="#tab3">Tabla 3</a>).</P>        <P ALIGN="JUSTIFY">Los beta-bloqueantes son drogas que se unen a la albumina y a la <FONT FACE=Symbol>a</FONT>1-glicoproteina &aacute;cida (AAG) en el plasma<SUP>(11)</SUP>. Casi todos los antagonistas beta se distribuyen r&aacute;pidamente por todos los tejidos teniendo vol&uacute;menes de distribuci&oacute;n grandes y pudiendo algunos de ellos pasar f&aacute;cilmente la barrera hematoencef&aacute;lica, como en el caso del propanolol.</P>        <P ALIGN="JUSTIFY">Las vidas medias del grupo var&iacute;an, (ver <a href="#tab3">Tabla 3</a>). Siendo el betabloqueante de menor vida media el esmolol, el cual tiene una vida media de alrededor de 8 minutos y un volumen aparente de distribuci&oacute;n de aproximadamente 2 litros/kg. La droga contiene una uni&oacute;n &eacute;ster y es hidrolizada r&aacute;pidamente por estereasas en los eritrocitos. La vida media del metabolito &aacute;cido carbox&iacute;lico del esmolol es mucho m&aacute;s prolongada (4 horas) y se acumula durante la infusi&oacute;n prolongada de esmolol. Sin embargo, este metabolito tiene poca potencia como antagonista betaadren&eacute;rgico.</P>        <P ALIGN="JUSTIFY">La excreci&oacute;n de la mayor&iacute;a de los beta-bloqueantes ocurre v&iacute;a metabolismo hep&aacute;tico y/o excreci&oacute;n renal. Mientras los beta-bloqueantes lipof&iacute;licos como el propanolol y el metoprolol, son eliminados en su mayor parte por metabolismo hep&aacute;tico, la mayor&iacute;a de los hidrof&iacute;licos como el atenolol y nadolol, son casi exclusivamente eliminados sin cambios en la orina<SUP>(11)</SUP>. Se prolonga significativamente el tiempo de eliminaci&oacute;n en presencia de insuficiencia renal o hep&aacute;tica, aumentando la vida media plasm&aacute;tica y evidenci&aacute;ndose signos de toxicidad con intensa bradicardia e hipotensi&oacute;n arterial.</P>        ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Los f&aacute;rmacos beta-bloqueantes se unen a los beta-adrenoreceptores pero no activan la enzima adenilatociclasa. La uni&oacute;n es especifica y el complejo formado es diferente al que ocurre con los agonistas permaneciendo en estado de baja afinidad e incapaz de formar un complejo ternario con la prote&iacute;na G. Como dijimos anteriormente algunos f&aacute;rmacos del grupo bloquean tanto a los receptores beta 1 como beta 2, en cambio otros antagonizan preferiblemente a los beta 1, siendo denominados estos compuestos cardioselectivos, debido a la gran riqueza de los receptores beta 1 ubicados en el coraz&oacute;n. Entre ellos se han implicado:</P><DIR>            <P ALIGN="JUSTIFY">1) Reducci&oacute;n del gasto cardiaco: La reducci&oacute;n de la contractilidad del miocardio contribuye a disminuir el gasto cardiaco y el consumo de oxigeno de estos tejidos. Inicialmente esta reducci&oacute;n del gasto cardiaco no se acompa&ntilde;a de una reducci&oacute;n paralela de la presi&oacute;n arterial, ya que el efecto de los beta-bloqueantes sobre los vasos sangu&iacute;neos impide la vaso dilataci&oacute;n mediada por los receptores beta-2, evidenci&aacute;ndose un aumento reflejo de la resistencia vascular perif&eacute;rica inicialmente. El descenso de la presi&oacute;n arterial se produce tras varios d&iacute;as de tratamiento, cuando las resistencias vasculares vuelven a los valores basales y se mantiene el gasto card&iacute;aco disminuido.</P>        <P ALIGN="JUSTIFY">2) Inhibici&oacute;n de la actividad de la renina plasm&aacute;tica (ARP): Los beta-bloqueantes antagonizan la liberaci&oacute;n de renina mediada a trav&eacute;s de la activaci&oacute;n de receptores beta-1 ubicados en las c&eacute;lulas yuxtaglomerulares renales. Algunas evidencias apuntan a que esta inhibici&oacute;n jugar&iacute;a un papel importante en su efecto hipotensor<SUP>(12)</SUP>, sin embargo el efecto hipotensor de estos f&aacute;rmacos no puede atribuirse &uacute;nicamente a este mecanismo. En primer lugar, no existe correlaci&oacute;n entre el efecto antihipertensivo de los beta-bloqueantes y los niveles de ARP, adem&aacute;s las dosis necesarias para inhibir la ARP son inferiores a las dosis precisas para obtener un efecto hipotensor.</P>        ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">3) Aumento de los niveles plasm&aacute;ticos y excreci&oacute;n urinaria de prostaglandinas: Se ha observado que el tratamiento prolongado con beta-bloqueantes produce un incremento de los niveles plasm&aacute;ticos y de la excreci&oacute;n urinaria de prostaciclina y PGE2, las cuales poseen conocido efecto vasodilatador. Por otra parte, el uso de antiinflamatorios no esteroideos, que inhiben la s&iacute;ntesis de prostaglandinas, aten&uacute;a el efecto antihipertensivo de los beta-bloqueantes.</P>        <P ALIGN="JUSTIFY">4) Reducci&oacute;n de la s&iacute;ntesis y liberaci&oacute;n de endotelina-1 en c&eacute;lulas endoteliales humanas: La endotelina-1 es un potente p&eacute;ptido vasoconstrictor producido y liberado por c&eacute;lulas endoteliales; numerosos estudios relacionan los niveles plasm&aacute;ticos de endotelina-1 con la severidad de varias patolog&iacute;as cardiovasculares. El efecto de los beta-bloqueante sobre la producci&oacute;n de endotelina-1 puede explicar parcialmente la eficacia de los beta-bloqueantes en el tratamiento de patolog&iacute;as como son la falla cardiaca avanzada, hipertensi&oacute;n esencial y los s&iacute;ndromes coronarios agudos<SUP>(13)</SUP>. La disfunci&oacute;n endotelial se encuentra implicada en la patog&eacute;nesis de las enfermedades cardiovasculares<SUP>(14)</SUP>. Beta-bloqueantes como nebivolol y carvedilol pueden mejorar la funci&oacute;n endotelial en pacientes con hipertensi&oacute;n<SUP>(15)</SUP>.</P>        <P ALIGN="JUSTIFY">5) Efecto sobre el sistema nervioso central: Se ha postulado que los beta-bloqueantes podr&iacute;an ejercer su efecto hipotensor a nivel del SNC, ya que en animales de experimentaci&oacute;n se ha demostrado reducci&oacute;n del tono simp&aacute;tico y descenso de la presi&oacute;n arterial tras su infusi&oacute;n intraventricular. Sin embargo, los beta-bloqueantes hidrof&iacute;licos, que atraviesan escasamente la barrera hematoencef&aacute;lica, tienen un efecto hipotensor similar a los liposolubles.</P>        <P ALIGN="JUSTIFY">6) Disminuci&oacute;n de los niveles de calcio citos&oacute;lico: La HTA se caracteriza por un aumento de la concentraci&oacute;n intracelular de calcio en la fibra muscular lisa, lo que favorece la vasoconstricci&oacute;n y el aumento de las resistencias vasculares. Se ha demostrado que la reducci&oacute;n de la PA obtenida con beta-bloqueantes se acompa&ntilde;a de una disminuci&oacute;n de los niveles de calcio citos&oacute;lico en hemat&iacute;es y plaquetas.</P>        <P ALIGN="JUSTIFY">7) Bloqueo de los receptores beta-2 presin&aacute;pticos: Estos receptores facilitan la liberaci&oacute;n de noradrenalina por la terminal nerviosa simp&aacute;tica, por lo que su bloqueo reducir&iacute;a el tono alfa-adren&eacute;rgico vascular favoreciendo la vasodilataci&oacute;n. Este mecanismo explicar&iacute;a el descenso de las resistencias vasculares que se produce al cabo de algunos d&iacute;as de tratamiento con beta-bloqueantes.</P></DIR>            <P ALIGN="JUSTIFY">Actualmente se acepta que ninguno de los mecanismos expuestos explicar&iacute;a en forma aislada el efecto hipotensor de los beta-bloqueantes, por lo que se considera que hay varios mecanismos implicados teniendo mayor o menor importancia seg&uacute;n el tipo de beta-bloqueante.</P>        <P ALIGN="JUSTIFY">Los efectos que producen estos f&aacute;rmacos en el organismo los podemos clasificar en:</P><DIR>            <P ALIGN="JUSTIFY">1) Efectos Cardiovasculares: Reducen la frecuencia cardiaca al actuar sobre el nodo sinusal. Disminuyen la contractilidad del miocardio, la velocidad de expulsi&oacute;n sist&oacute;lica y la velocidad de elevaci&oacute;n de la presi&oacute;n intraventricular. Por todo ello reducen el gasto cardiaco con independencia de la disminuci&oacute;n de la frecuencia cardiaca. La reducci&oacute;n de la frecuencia y de la contractilidad mioc&aacute;rdica contribuyen a disminuir el trabajo cardiaco y el consumo de ox&iacute;geno mioc&aacute;rdico. El bloqueo beta supone, adem&aacute;s una reducci&oacute;n en la velocidad de conducci&oacute;n en el nodo AV, por lo que aumenta el espacio PR del EKG. Elevan la resistencia vascular perif&eacute;rica inicialmente, logr&aacute;ndose su disminuci&oacute;n tras la administraci&oacute;n cr&oacute;nica de estos f&aacute;rmacos.</P>        <P ALIGN="JUSTIFY">2) Efectos Respiratorios: Los beta-bloqueantes disminuyen el tono broncodilatador que normalmente equilibra el di&aacute;metro bronquial por bloqueo de los receptores beta-2 bronquiales, por lo que predomina el tono bronco constrictor en especial el mediado por el sistema colin&eacute;rgico, el resultado es un aumento en la resistencia bronquial que puede ser peligroso en pacientes con enfermedad obstructiva reversible de las v&iacute;as a&eacute;reas, tales como asma o enfermedad pulmonar obstructiva cr&oacute;nica con componente obstructivo reversible. De utilizarse en estos individuos deben prescribirse beta-bloqueantes cardioselectivos, estos en pacientes con enfermedad reversible leve a moderada de las v&iacute;as a&eacute;reas no producen cl&iacute;nica significativa, no produciendo efectos respiratorios adversos en el corto tiempo<SUP>(16)</SUP>; sin embargo, debe estrecharse la vigilancia en caso de ser necesario su uso.</P>        <P ALIGN="JUSTIFY">3) Efectos sobre la Funci&oacute;n Renal: Provocan en general una reducci&oacute;n del flujo plasm&aacute;tico renal y de la velocidad de filtraci&oacute;n glomerular, esto se observa durante el uso agudo de estos f&aacute;rmacos. Durante su uso cr&oacute;nico se observan cambios poco importantes de estas funciones renales; aunque cabe se&ntilde;alar que por lo general estos efectos carecen de trascendencia cl&iacute;nica.</P>        ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">4) Efectos Metab&oacute;licos y Hormonales: Se ha demostrado una disminuci&oacute;n de la lip&oacute;lisis mediada por la actividad simp&aacute;tica as&iacute; como una reducci&oacute;n de la tolerancia a la glucosa en los diab&eacute;ticos<SUP>(17)</SUP>. Los beta-bloqueantes deben administrarse con cautela a enfermos con Diabetes Mellitus, dado que pueden bloquear la descarga catecolam&iacute;nica que se produce en las hipoglucemias, agrav&aacute;ndolas y enmascarando su cl&iacute;nica. Adem&aacute;s, en estudios recientes se ha sugerido que en pacientes hipertensos tratados durante largos periodos con beta-bloqueantes existe un mayor riesgo de desarrollar Diabetes Mellitus tipo 2, en contraposici&oacute;n con el resto de f&aacute;rmacos<SUP>(18)</SUP>. A pesar de ello, el tratamiento con beta-bloqueantes es capaz de prevenir las complicaciones micro y macro vasculares de la diabetes de forma similar a los IECA<SUP>(19)</SUP>.</P>        <P ALIGN="JUSTIFY">El uso prolongado de estos medicamentos conlleva a un aumento de los triacilglic&eacute;ridos, aumento del VLDL colesterol y disminuci&oacute;n de las HDL sin alterar el colesterol total; lo que podr&iacute;a empeorar el perfil aterog&eacute;nico de algunos pacientes. Estos cambios en los l&iacute;pidos son menores con los bloqueadores beta-1 selectivos, como el atenolol y el Metoprolol, que con los antagonistas no selectivos como el propanolol, (ver <a href="#tab4">Tabla 4</a>).</P>        <P ALIGN="JUSTIFY">De los beta-bloqueantes podemos considerar al labetalol y Acebutolol como los menos nocivos para el perfil lip&iacute;dico y podr&iacute;an considerarse como los beta-bloqueantes de elecci&oacute;n en el tratamiento de la hipertensi&oacute;n en pacientes con anomal&iacute;as previas de los l&iacute;pidos plasm&aacute;ticos<SUP>(20)</SUP>.</P>        <P ALIGN="JUSTIFY">5) Otras Acciones Farmacol&oacute;gicas: Otro efecto importante de los beta-bloqueantes como el Timolol, est&aacute; relacionado con la disminuci&oacute;n de la presi&oacute;n intraocular, siendo &uacute;tiles en el tratamiento del glaucoma debido a que disminuyen la producci&oacute;n del humor acuoso.</P>        <P ALIGN="JUSTIFY">Se ha demostrado que los beta-bloqueantes pueden actuar como anest&eacute;sicos locales, esto se explica por la capacidad de inhibir los canales de sodio en las membranas nerviosas y cardiacas. La aplicaci&oacute;n terap&eacute;utica de este efecto no ha sido bien valorada. Los principales efectos secundarios se pueden clasificar en:</P><DIR>            <P ALIGN="JUSTIFY">1) <B>Cardiacos:</B> Bradicardia, depresi&oacute;n del la conducci&oacute;n AV, y por su efecto inotr&oacute;pico negativo puede descompensar un cuadro de insuficiencia cardiaca al inicio del tratamiento, aunque a la larga el efecto es beneficioso, hipotensi&oacute;n arterial en especial durante el uso intravenoso, extremidades fr&iacute;as debido a la disminuci&oacute;n de la circulaci&oacute;n perif&eacute;rica por vaso constricci&oacute;n y fatiga muscular por disminuci&oacute;n del gasto cardiaco y del riego sangu&iacute;neo a nivel perif&eacute;rico.</P>        <P ALIGN="JUSTIFY">Debido a estos efectos los beta-bloqueantes est&aacute;n contraindicados en el bloqueo aur&iacute;culo- ventricular de 2Âº y 3Âº grado, enfermedades del seno, bradicardia (&lt; 45 lpm) y en forma relativa en las enfermedades vasculares perif&eacute;ricas, como enfermedad de Raynaud o en claudicaci&oacute;n intermitente preexistente.</P>        <P ALIGN="JUSTIFY">2) Pulmonares: Broncoespasmo, m&aacute;s notorio con los no cardioselectivos.</P>        <P ALIGN="JUSTIFY">3) Metabolicos: Hipertrigliceridemia, descenso del colesterol HDL, resistencia a la insulina, enmascaramiento de s&iacute;ntomas de hipoglucemia inducida por insulina en diab&eacute;ticos tipo 1, aumento de peso.</P>        <P ALIGN="JUSTIFY">4) Gastrointestinal: N&aacute;useas, v&oacute;mitos, c&oacute;licos y diarrea.</P>        <P ALIGN="JUSTIFY">5)&#9;Piel: alergias tipo I, eritema, edema y urticaria.</P>        <P ALIGN="JUSTIFY">6) Sistema Nervioso: Se ha atribuido a los beta-bloqueantes liposoluble la capacidad de provocar trastornos del sue&ntilde;o, como pesadillas y alucinaciones, depresi&oacute;n o alteraciones de la esfera sexual; sin embargo, estudios recientes donde se evalu&oacute; el efecto del propanolol en la funci&oacute;n cognitiva se encontraron limitados efectos adversos, no mostrando efectos como depresi&oacute;n o alteraci&oacute;n en la funci&oacute;n sexual<SUP>(22)</SUP>.</P></DIR>            ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Muchos pacientes no toleran las dosis m&aacute;ximas recomendadas de beta-bloqueantes, por lo cual se recomienda su reducci&oacute;n cuando el enfermo presente frecuencia cardiaca menor a 60 lpm, hipotensi&oacute;n sintom&aacute;tica, fatiga excesiva o signos y s&iacute;ntomas progresivos de ICC. Su asociaci&oacute;n con f&aacute;rmacos inductores del metabolismo hep&aacute;tico (citocromo P-450) disminuyen las concentraciones plasm&aacute;ticas de los beta-bloqueantes, mientras que inhibidores del metabolismo hep&aacute;tico como la cimetidina, cloranfenicol y ketoconazol pueden incrementar sus concentraciones plasm&aacute;ticas. <body><![CDATA[<P ALIGN="JUSTIFY">Con el uso de labetalol se ha observado hipotensi&oacute;n ortost&aacute;tica en las primeras semanas de tratamiento, consider&aacute;ndosele muy &uacute;til en el tratamiento de las urgencias hipertensivas.
