De ellos, 213 (80,98%) presentaban incontinencia urinaria y en 22 (10,33%) fue necesario recurrir a la cirugía por fracasar el tratamiento conservador. En 14 casos realizamos una cistoplastia de aumento combinada con implante de esfínter artificial en 7 y con reimplante ureteral en 4. El esfínter artificial aislado se colocó en 2 pacientes y un tercero se trató con una cincha aponeurótica. En 2 pacientes se realizó una plastia de cuello vesical y finalmente en 3 procedimos a una derivación continente tipo Mitrofanoff. Uno de los esfínteres no ha funcionado adecuadamente. Todos los demás pacientes están secos al menos por 3 horas aunque una de las derivaciones tiene pérdidas ocasionales.]]></p></abstract>

En este trabajo queremos presentar nuestra experiencia en el tratamiento quirúrgico de la incontinencia neurogénica en pediatría.</p> <b>      ]]></body>
<body><![CDATA[<p ALIGN="JUSTIFY">&nbsp;</p>      <p ALIGN="JUSTIFY">MATERIALES Y MÉTODOS</p> </b>      <p ALIGN="JUSTIFY">Hemos revisado por el método de hoja única las historias de los pacientes del Servicio de Cirugía Infantil del Hospital San Juan de Dios de Caracas operados por incontinencia neurógena, analizando las indicaciones de la cirugía, procedimientos empleados y la evolución</p>      <p ALIGN="JUSTIFY">&nbsp;</p> <b>      <p ALIGN="JUSTIFY">RESULTADOS</p> </b>      <p ALIGN="JUSTIFY">Entre 1984 y 1996 se estudiaron 263 pacientes con edades entre 3 meses y 18 años (<i><b>Gráfico 1</b></i>) portadores de vejiga neurogénica, en la mayoría de origen congénito (91,81%). De ellos 213 (80,98%) presentaban incontinencia y en 22 (10,33%) realizamos uno o más procedimientos quirúrgicos para tratar la incontinencia</p>      <p ALIGN="JUSTIFY">Las indicaciones de la cirugía fueron (<i><b>Tabla 1</b></i>): el fracaso del tratamiento médico en 86,36% en tanto que al resto se le indicó cirugía como tratamiento primario, dada la imposibilidad de tratamiento conservador.</p> <b><i>      <p ALIGN="center"><a href="#graf1">Grafico</a> 1: Distribución según Edad.</p>      <p ALIGN="center"><a name="graf1"></a></p> </i>      <p ALIGN="center"><img src="/img/fbpe/rfm/v23n2/11incontinencia1.jpg" alt="11 incontinencia 1.jpg (29870 bytes)" width="410" height="331"></p>

<body><![CDATA[<p ALIGN="JUSTIFY">3</font></td>   </tr> </table>   </center> </div> <font SIZE="3"><b>      <p ALIGN="JUSTIFY">&nbsp;</p> </b>      <p ALIGN="JUSTIFY">La <i><b>Tabla 2</b></i> resume las intervenciones practicadas. Es de notar que en 5 casos la cistoplastia de aumento se combinó con reimplante ureteral, bilateral en 2, y en 7 con implante de esfínter artificial dos de ellos también con reimplante ureteral.</p>      <p ALIGN="JUSTIFY">La indicación de cistoplastia de aumento se basó en hiperreflexia del detrusor severa sin respuesta al tratamiento médico, con presión mayor de 40 cm H<sub>2</sub>0 a la mitad o menos de la capacidad calculada Todas las cistoplastias se realizaron con intestino destubularizado utilizado como parche salvo en un caso con vejiga muy pequeña y de gruesas paredes que se resecó conservando sólo el trígono y el cuello. En 12 casos utilizamos colon sigmoides y en el resto ileon.</p>      <p ALIGN="JUSTIFY">La edad promedio de estos pacientes fue 8,5 años con rango 4 a 18 años. Todos se realizan cateterismo al menos 2 veces al día y todos se mantienen secos por 3 a 4 horas salvo un varón con muy ocasionales pérdidas. El tiempo de control va de 7 a 2 años, la mayoría tiene más de 3 años.</p>      <p ALIGN="JUSTIFY">La indicación de implantar un esfínter artificial se hizo cuando encontramos una resistencia uretral menor a 30 cm H<sub>2</sub>0 a pesar del tratamiento farmacológico.</p>      <p ALIGN="JUSTIFY">En todos los casos utilizamos el modelo AS-800 con reservorio de presión media y manguito implantado en el cuello vesícal. Unas vez colocado el esfínter lo dejamos inactivado por 3-6 semanas.</p>      <p ALIGN="JUSTIFY">La edad promedio de estos pacientes fue 9,37 años con rango 7 a 18 años. Sólo dos son hembras.</p>      <p ALIGN="JUSTIFY">Ningún paciente tiene más de 4 años de control. Se mantienen secos por 4 horas o más 8 de ellos, pero un esfínter no ha funcionado apropiadamente a pesar de una revisión quirúrgica y cambio de reservorio.</p>      <p ALIGN="JUSTIFY">En los 7 casos que el esfínter se combinó con cistoplastia insistimos en el cateterismo intermitente aunque 4 de ellos orinan espontáneamente pero con residuo.</p>      ]]></body>
<body><![CDATA[<p ALIGN="JUSTIFY">La cincha aponeurótica (con aponeurosis del oblicuo mayor), se colocó en un paciente programado para esfínter artificial pero durante la disección hubo desgarro vesicouretral y se consideró inapropiado dejar un material extraño; luego de 3<sup>1/2</sup> meses alcanzó continencia y se mantiene seco por 4 horas luego de 24 meses de control.</p>      <p ALIGN="JUSTIFY">Las plastias de cuello, un Marshall-Marchetti en una hembra de 13 años y un Young- Dees en un varón de 7, se indicaron ante una incontinencia parcialmente controlable con resistencia uretral entre 35 y 40 cm H<sub>2</sub>0. Ambos se mantenían secos en su último control 9 y 7 años después.</p>      <p ALIGN="JUSTIFY">Finalmente, las derivaciones continentes se realizaron según el principio de Mitrofanoff<sup>(3)</sup> usando apéndice en 2 casos e íleon en el último. Las indicaciones: una hembra de 16 años sin control de la cintura pélvica por incapacidad de autocateterizarse y en dos varones por presentar severas escaras perineosacras mantenidas en parte por el constante goteo de orina. La hembra tiene pérdidas ocasionales al igual que un varón.</p> <b>      <p ALIGN="JUSTIFY">&nbsp;</p>      <p ALIGN="JUSTIFY">DISCUSIÓN</p> </b>      <p ALIGN="JUSTIFY">En el niño con incontinencia neurogénica el tratamiento debe planificarse cuidadosamente en particular los procedimientos quirúrgicos dado el efecto que tiene el desarrollo y crecimiento. La modificación del cuadro urinario no es rara<sup>(4)</sup> y la cirugía indicada precozmente podría a la larga mostrarse innecesaria o contraproducente. El tratamiento conservador ofrece buenos resultados sobretodo cuando el problema básico es la hiperreflexia vesical<sup>(5,6)</sup>, pero la hipotonía del esfínter es más difícil de tratar con métodos conservadores <sup>(1,2,7)</sup>.</p>      <p ALIGN="JUSTIFY">La cistoplastia de aumento es un excelente procedimiento cuando el tratamiento médico no logra aumentar la capacidad y acomodación vesical en diversas circunstancias<sup>(8, 9,10)</sup>. Existen diversas técnicas de cistoplastia de aumento más hemos preferido el uso del intestino destubularizado, preferiblemente el sigmoides, pues a pesar del inconveniente del moco que exige lavados vesicales, sus resultados a largo plazo son conocidos<sup>(2,8)</sup>. El uso de estómago, de reciente auge, ha tenido resultados contradictorios<sup>(9,11,12,13)</sup> y tiene poco tiempo de evaluación. Nos preocupa además, el efecto que sobre el crecimiento y desarrollo pueda tener el uso a largo plazo de los bloqueadores H2 o alcalinizantes frecuentemente requeridos.</p>      <p ALIGN="JUSTIFY">La perforación M segmento gastrointestinal es una complicación grave que amerita inmediato diagnóstico y tratamiento<sup>(14, 15)</sup>.</p>      <p ALIGN="JUSTIFY">La medición de la capacidad vesical y la resistencia uretral las consideramos indispensables no sólo para la indicación de cistoplastia, también para decidir cuando actuar sobre el cuello o implantar un esfínter<sup>(2,7,15,16)</sup>. Nuestro número de casos con esfínter es pequeño pero los resultados favorables, aunque deben aceptarse los inconvenientes y complicaciones que todo artefacto puede tener<sup>(1,7,17,18)</sup>. La utilización de una cincha aponeurótica es otra alternativa válida, menos costosa pero de resultado incierto, Cuando encontramos en la uretra cierta resistencia y la incontinencia no es tan severa puede intentarse una plastia de cuello. Uno de los inconvenientes , quizás el mayor, es que si fracasa el posterior implante de un esfínter es más difícil y proclive a complicaciones<sup>(16,18)</sup>, lo que también es válido para la cincha. No hemos utilizado otras técnicas como la de Kroop<sup>(19)</sup> que dificultan la micción pero también el cateterismo.</p>      <p ALIGN="JUSTIFY">La construcción de una derivación continente es así mismo una solución aceptable pero como última alternativa. El principio de Mitrofanoff<sup>(3)</sup> consiste en implantar con técnica antirreflujo el apéndice u otro segmento intestinal que por su otro extremo se aboca a la piel abdominal. Puede hacerse con o sin sección del cuello vesical y el manejo de éste es una de las principales dificultades que explican la incontinencia parcial no tan rara<sup>(20,21,22,23)</sup>.</p>      ]]></body>
