Se trataron en forma prospectiva, no aleatoria, 30 pacientes con SKE con el régimen ABV. Se obtuvo beneficio clínico entre el 60% y 100% de los enfermos con 1 a 4 ciclos de ABV. Se puede concluir que el régimen ABV produce un buen porcentaje de beneficio clínico con lo cual se disminuye la morbilidad y se mejora la calidad de vida de los pacientes con SKE, por lo tanto se puede usar en pacientes quienes no pueden cubrir los costos de la DL.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[At present, the treatment of choice for patients with advanced epidemic Kaposi‘s sarcoma (EKS) is pegylated-liposomal doxorubicin (PLD); nevertheless, with doxorubicin (20 mg/m2)-bleomycin (10 mg/m2)-vincristine (1.4 mg/m2, maximun 2 mg) (ABV) regimen, every three weeks, clinical benefit can be obtained with low cost. Thirty, non-randomized patients, with advanced EKS, were treated with ABV. Between 60% to 100% of patients, obtained clinical benefits with 1 to 4 ABV cycles. Finally, patients with EKS obtain clinical benefits with ABV, and therefore it decreases morbidity and improves quality of life. Also, it is an acceptable alternative for patients with low financial resources who can not pay for PLD.]]></p></abstract>

Also, it is an acceptable alternative for patients with low financial resources who can not pay for PLD.</P> <B>    <P ALIGN="JUSTIFY">Key Words: </B>Advanced Epidemic Kaposi’s Sarcoma, Trectment, ABV, Liposomar Doxorubicin (PLD)</P>     <P>__________________</P>     <P>&nbsp;</P>     <P>&nbsp;</P> <B>    <P ALIGN="JUSTIFY">Introducci&oacute;n</P> </B>    <P ALIGN="JUSTIFY">El Sarcoma de Kaposi Epid&eacute;mico (SKE) es la neoplasia mas frecuente que afecta a los pacientes infectados por el Virus de Inmunodeficiencia Humana (VIH)(1), y en algunas cohortes de homosexuales masculinos con el S&iacute;ndrome de Inmunodeficiencia Adquirida (SIDA), el riesgo de estos individuos de desarrollar SKE durante la vida se aproxima a un 50%(2). El tratamiento de elecci&oacute;n para pacientes con lesiones cut&aacute;neas extensas, progresivas y SKE visceral es la poliquimioterapia. El r&eacute;gimen ABV (doxorubicina-bleomicina-vincristina) ha sido por varios a&ntilde;os el tratamiento standard para el SKE avanzado, y se obtiene un 25% a 35% de respuestas parciales(3). Recientemente, la doxorubicina liposomal (DL) se ha convertido en el tratamiento de elecci&oacute;n para el SKE avanzado ya que se puede obtener un 58% de respuestas objetivas con una toxicidad m&iacute;nima(4). El factor limitante de la DL es el costo ya que su precio es 10 veces mayor que el r&eacute;gimen ABV, por lo tanto es muy dif&iacute;cil cubrir estos gastos en pacientes con SKE provenientes de pa&iacute;ses en v&iacute;as de desarrollo. Adem&aacute;s, es bien conocida la dificultad que existe en definir una respuesta parcial en pacientes con SKE avanzado(5), por esta raz&oacute;n creemos que la evaluaci&oacute;n del beneficio cl&iacute;nico tiene mas relevancia para los pacientes cuando se utiliza un tratamiento cuyo objetivo fundamental es reducir la morbilidad producida por el SKE.</P>     <P ALIGN="JUSTIFY">Debido a las razones mencionadas, nosotros decidimos realizar un estudio prospectivo no aleatorio con el r&eacute;gimen ABV en pacientes con SKE avanzado con la intenci&oacute;n de evaluar: 1) Beneficio cl&iacute;nico obtenido con el tratamiento, 2) Si el r&eacute;gimen ABV puede ser una alternativa aceptable para el tratamiento del SKE avanzado en pacientes con escasos recursos econ&oacute;micos.</P> <B>    ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">M&eacute;todo</P> </B>    <P ALIGN="JUSTIFY">En el estudio se incluyeron 30 pacientes con la infecci&oacute;n por el VIH demostrada por ELISA y confirmada con el Western blot. Veintinueve enfermos eran homo o bisexuales masculinos y una mujer cuya pareja ten&iacute;a SIDA. El rango de edad estuvo comprendido entre 23 y 50 a&ntilde;os (promedio: 34). Todos los pacientes ten&iacute;an biopsia confirmando el diagn&oacute;stico de Sarcoma de Kaposi (SK) y menos de 2<FONT FACE="Times New Roman">00 linfocitos T-CD4/µL. Ocho pacientes tenían una escala de estado general de Karnofsky &lt; 70 y 22 ž 70. Los pacientes fueron estadificados de acuerdo a los criterios del ACTG (AIDS Clinical Trial Group) y todos se clasificaron como SKE avanzado(6). En tod</FONT>os ellos se realiz&oacute; una historia cl&iacute;nica, las medidas de las lesiones, hematolog&iacute;a completa, qu&iacute;mica sangu&iacute;nea y una Rx torax al ingresar al estudio. La gastroscopia y colonoscopia se indicaron en presencia de lesi&oacute;n oral extensa o s&iacute;ntomas digestivos. Diez y siete pacientes ten&iacute;an mas de 25 lesiones cut&aacute;neas y 13 menos de 25, 8 ten&iacute;an lesiones gastrointestinales extensas y los s&iacute;ntomas principales eran disfagia y constipaci&oacute;n, 7 lesiones orales con odinofagia, 3 con lesiones en glande las cuales produc&iacute;an edema intenso y obstrucci&oacute;n uretral, 14 con lesiones plantares dolorosas, 20 con edema intenso en miembros inferiores y 5 con SK pulmonar asociado con tos, disnea e hipoxemia <B><I>(Tabla 1)</B></I>. Los enfermos se trataron con dosis bajas de doxorubicina (20 mg/m2), bleomicina (10 mg/m2) y vincristina (1.4 mg/m2, m&aacute;ximo 2 mg) (ABV) cada 3 semanas(3). El promedio de los ciclos administrados fue 6 (rango: 4-12). Ninguno de ellos hab&iacute;a recibido previamente tratamiento sist&eacute;mico para el SK y solamente 5 estaban bajo tratamiento antiretroviral altamente efectivo a pesar de lo cual las lesiones de SK progresaron. Los criterios para suspender el tratamiento fueron: progresi&oacute;n de la enfermedad, toxicidad elevada o la aparici&oacute;n de una infecci&oacute;n por oportunista durante el tratamiento. La evaluaci&oacute;n de la respuesta se realiz&oacute; predominantemente considerando los beneficios cl&iacute;nicos obtenidos que disminuyen la morbilidad, y secundariamente de acuerdo a los criterios standard: respuesta completa, respuesta parcial, enfermedad estable y progresi&oacute;n, los cuales han sido previamente definidos(5). <body><![CDATA[<P ALIGN="JUSTIFY">Resultados</P> </B>    <P ALIGN="JUSTIFY">Treinta pacientes recibieron el r&eacute;gimen ABV, 9 (30%) de ellos alcanzaron una repuesta parcial, 19 (63%) estabilizaron su enfermedad y 2 (7%) progresaron durante el tratamiento. Referente a los beneficios cl&iacute;nicos <B><I>(Tabla 2)</B></I>, en el 60% de los enfermos la mayor&iacute;a de las lesiones cut&aacute;neas se aplanaron y aclararon entre el segundo y cuarto ciclo de quimioterapia. Todos los pacientes mejoraron sus s&iacute;ntomas gastrointestinales, las lesiones orales junto a la odinofagia, las lesiones en glande junto al edema y la obstrucci&oacute;n uretral, y el dolor incapacitante de las lesiones plantares desde el primer ciclo de tratamiento. En el 80% de los enfermos, el edema en miembros inferiores desapareci&oacute; completamente desde el segundo ciclo de quimioterapia. En el 60% de los pacientes con SK pulmonar la tos y la disnea desaparecieron, la hipoxemia se normaliz&oacute; entre el primer y tercer ciclo de tratamiento, a pesar de observar solamente cambios discretos en la Rx de t&oacute;rax. En general, el beneficio cl&iacute;nico se obtuvo entre el primero y cuarto ciclo de quimioterapia, y no observamos un aumento de este beneficio con m&aacute;s ciclos de tratamiento. <body><![CDATA[<P ALIGN="JUSTIFY">Edema en miembros inferiores</FONT></TD> <TD WIDTH="50%" VALIGN="TOP"> <FONT SIZE=2>    <P ALIGN="JUSTIFY">80</FONT></TD> </TR> <TR><TD WIDTH="50%" VALIGN="TOP"> <FONT SIZE=2>    <P ALIGN="JUSTIFY">Sarcoma de Kaposi pulmonar</FONT></TD> <TD WIDTH="50%" VALIGN="TOP"> <FONT SIZE=2>    <P ALIGN="JUSTIFY">60</FONT></TD> </TR> </TABLE>    </center> </div>      <P ALIGN="JUSTIFY">&nbsp;</P> <B>    <P ALIGN="JUSTIFY">Discusi&oacute;n</P> </B>    <P ALIGN="JUSTIFY">La evaluaci&oacute;n de las repuestas al tratamiento en los pacientes con SKE es dif&iacute;cil utilizando los criterios oncol&oacute;gicos cl&aacute;sicos de respuesta completa, parcial, enfermedad estable y progresi&oacute;n, ya que estas respuestas dependen del n&uacute;mero de lesiones cut&aacute;neas, nodularidad, edema, color y lesiones viscerales (gastrointestinales y pulmonares). Por lo tanto, es sumamente dif&iacute;cil comparar dos ensayos cl&iacute;nicos ya que los investigadores introducen ciertas variables particulares para definir respuesta(7). Adem&aacute;s, el tratamiento para el SKE es paliativo cuyo objetivo fundamental es reducir morbilidad y mejorar la calidad de vida de los enfermos. Northfelt et al(8), fueron los primeros en incluir algunos par&aacute;metros de beneficio cl&iacute;nico como son: disminuci&oacute;n del dolor y edema, aclaramiento y aplanamiento de las lesiones; elementos fundamentales a evaluar para definir mejor la disminuci&oacute;n de la morbilidad y la mejor&iacute;a en la calidad de vida de los pacientes tratados con DL. En nuestro estudio nosotros incluimos otros elementos de beneficio cl&iacute;nico como son: disfagia, odinofagia, constipaci&oacute;n, obstrucci&oacute;n uretral, tos, disnea e hipoxemia. Al analizar nuestros resultados con el ABV, observamos que entre el 60% y el 100% de nuestros enfermos obtuvieron beneficio cl&iacute;nico con 1 a 4 ciclos de quimioterapia, lo cual trajo como resultado la mejor&iacute;a en la calidad de vida y la disminuci&oacute;n en la morbilidad. Recientemente 2 estudios aleatorios comparando DL con BV (bleomicina-vincristina)(4) y ABV(9) en pacientes con SKE avanzado han sido publicados. Estos ensayos han concluido que la DL se ha convertido en la droga de elecci&oacute;n para el tratamiento del SKE avanzado; sin embargo, estos trabajos hacen mayor &eacute;nfasis en los criterios cl&aacute;sicos de respuesta en lugar de los beneficios cl&iacute;nicos, por lo tanto, es dif&iacute;cil comparar nuestros resultados con los obtenidos con la DL. Al analizar nuestros resultados, siguiendo los criterios cl&aacute;sicos de respuesta, obtuvimos unos porcentajes de respuesta similares a aquellos obtenidos con ABV y publicados por Northfelt et al(9) (30% versus 32%), pero con la ventaja de que ninguno de nuestros enfermos present&oacute; una toxicidad elevada que ameritara interrumpir el tratamiento (0% versus 37%). Esta menor toxicidad se debe fundamentalmente a que los ciclos de ABV en nuestra serie se administraron cada 3 semanas en lugar de cada 2 como en la serie de Northfelt et al(9). En los pa&iacute;ses en v&iacute;as de desarrollo resulta muy costoso para el estado y las compa&ntilde;&iacute;as de seguro cubrir los gastos con DL, ya que un ciclo con este medicamento es 10 veces m&aacute;s alto que un ciclo de ABV. Por lo tanto, en nuestros pa&iacute;ses es mas f&aacute;cil usar el ABV para el SKE avanzado y dejar la DL para pacientes con mejores recursos econ&oacute;micos quienes pueden cubrir estos costos.</P>     <P ALIGN="JUSTIFY">Finalmente, nuestro estudio es el primero reportado en la literatura el cual considera el beneficio cl&iacute;nico obtenido como el objetivo fundamental, ya que en enfermedades como el SIDA donde los pacientes reciben m&uacute;ltiples drogas por la enfermedad de base, la meta del tratamiento del SKE avanzado debe estar dirigida a disminuir la morbilidad y mejorar la calidad de vida. Adem&aacute;s creemos, que con el r&eacute;gimen ABV con bajas dosis de doxorubicina administrado cada 3 semanas, podemos obtener un buen porcentaje de beneficio cl&iacute;nico y resulta una buena alternativa para el tratamiento de enfermos con SKE avanzado y escasos recursos econ&oacute;micos quienes no pueden adquirir la DL.</P>     <P ALIGN="JUSTIFY">&nbsp;</P>     <P ALIGN="JUSTIFY">&nbsp;</P> <B>    ]]></body>
