Método: Estudio retrospectivo de las complicaciones retinianas presentadas en el posoperatorio de 6 876 pacientes con indicación de cirugía fotorrefractiva, (137 sometidos a PRK y 6,739 a LASIK) entre 1997-2002 en el Servicio de Córnea y Cirugía Refractiva de la APEC. Resultados: Edades entre 20 y 51 años, refracciones preoperatorias (Esfera -3,25 a -18,50 y cilindro -0,25 a -4,00). A 8 (5,9 %) se les realizó PRK y presentaron DR (5,9 %) en un período de 3 meses y 4 años posterior a la cirugía. Discusión: El uso de microquerátomos en la cirugía refractiva puede no ser un factor predisponente en la aparición de complicaciones retinianas sino la miopía y los cambios fondoscópicos retinianos consecuentes a ésta.]]></p></abstract>

<body><![CDATA[<P ALIGN="JUSTIFY">&nbsp;</P></DIR>  <B>    <P ALIGN="justify"><FONT face="Times New Roman" size=3>INTRODUCCI&Oacute;N</FONT></P> </B>    <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Los ojos mi&oacute;picos tienden a desarrollar degeneraci&oacute;n de Lattice, agujeros retinianos y desprendimiento de retina. Existe una relaci&oacute;n lineal entre el grado de miop&iacute;a y el desprendimiento de retina<sup>(1)</sup>. La prevalencia aceptada en la poblaci&oacute;n en general es aproximadamente de 0,3 %, en las personas con miop&iacute;a alta es del 5 % y un 2 % en individuos af&aacute;quicos<sup>(2)</sup>.</font><FONT FACE="Times" SIZE=1> </P> </FONT>    <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">La edad m&aacute;s com&uacute;n de ocurrencia del desprendimiento de retina, est&aacute; entre los 40 y 70 a&ntilde;os, y algunos estudios refieren una creciente incidencia sobre la vida del paciente, siendo &eacute;sta una causa de p&eacute;rdida visual en algunas personas<sup>(2,3)</sup>.</font></P>     <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">En cambio, el desprendimiento de retina en ojos pseudof&aacute;quicos, var&iacute;a mucho m&aacute;s entre el 0,3 % y 9,2 %<sup>(4,5)</sup>. Esta amplia variaci&oacute;n pudiera ser ocasionada por las diferencias entre las t&eacute;cnicas quir&uacute;rgicas, tipos de complicaciones y el uso de lentes intraoculares. Aunque las causas m&aacute;s significativas de desprendimiento de retina en pacientes mi&oacute;picos son el grado de miop&iacute;a y la licuefacci&oacute;n del v&iacute;treo<sup>(1, 3,6)</sup> y un factor adicional a esto es la gran longitud axial en ciertos casos <sup> (7)</sup>.</font></P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>El presente trabajo es con la finalidad de determinar a trav&eacute;s de un estudio retrospectivo las distintas complicaciones retinianas presentadas en el posoperatorio de pacientes tratados con queratectom&iacute;a fotorrefractiva (PRK), entre 1997-2002 en el Servicio de C&oacute;rnea y Cirug&iacute;a Refractiva de la APEC. </FONT></P> <B>    <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>MATERIALES Y M&Eacute;TODOS </FONT></P> </B>    <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>Se realiz&oacute; un estudio retrospectivo, de las historias m&eacute;dicas de los pacientes con diagn&oacute;stico de miop&iacute;a alta con o sin astigmatismo y los cuales fueron sometidos a queratectom&iacute;a fotorrefractiva en el Servicio de C&oacute;rnea y Cirug&iacute;a Refractiva entre el per&iacute;odo de 1997-2002 del Hospital Dr. Luis S&aacute;nchez Bulnes. Asociaci&oacute;n Para Evitar la Ceguera en M&eacute;xico (APEC). </FONT> </P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>Correspondiendo en estos seis a&ntilde;os, un total: de 6 876 ojos operados (137 ojos sometidos a PRK y 6 739 ojos a LASIK). De los cuales se revisaron 480 expedientes (960 ojos) de pacientes operados que antes y/o despu&eacute;s de la cirug&iacute;a fotorrefractiva, consultaron el Servicio de C&oacute;rnea y Retina de la APEC. </FONT> </P> <B>    <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>RESULTADOS </FONT></P> </B>    ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>Se evaluaron un total de 960 ojos operados con cirug&iacute;a fotorrefractiva que antes y/o despu&eacute;s de la cirug&iacute;a fotorrefractiva, consultaron el servicio de c&oacute;rnea y retina de la APEC y de los cuales; se encontraron 8 casos de PRK (16 ojos) y 472 casos de LASIK (944 ojos) de pacientes sometidos a procedimiento quir&uacute;rgico fotorrefractivo. </FONT></P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>De esta muestra se encontraron 16 ojos que presentaron complicaciones retinianas, de los cuales, 8 ojos fueron operados con PRK y 8 ojos operados de LASIK. </FONT></P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>En los casos posoperados de PRK, las edades de los pacientes eran entre un rango de 20 y 51 a&ntilde;os, con una media de 34,68 ± 11,76 a&ntilde;os. Encontr&aacute;ndose este procedimiento refractivo m&aacute;s frecuente en el sexo femenino con un 62,5 % y masculino de un 37,5 %. </FONT></P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>Presentaron diagn&oacute;stico de miop&iacute;a y astigmatismo entre un rango de esfera –2,25 a 18,50 y un cilindro de 0,25 a 4,00; con un equivalente esf&eacute;rico (EE) preoperatorio de 12,49 ± 5,59 dioptr&iacute;as. Y el equivalente esf&eacute;rico preoperatorio de los 16 ojos con complicaciones retinianas era de -11,59 ± 4,51 dioptr&iacute;as, siendo el EE preoperatorio de los 8 casos sometidos a PRK con complicaciones retinianas de 9,00 a 18,00 dioptr&iacute;as. </FONT></P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>En el <B><a href="#Cuadro 1">Cuadro 1</a></B>, se muestran las agudezas visuales (LogMar) de los pacientes operados de PRK con su correcci&oacute;n a&eacute;rea preoperatoria, agudeza visual (LogMar) sin correcci&oacute;n pos-PRK y posterior a la complicaci&oacute;n retiniana. </FONT></P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>Las complicaciones retinianas encontradas en el posoperatorio de los ojos sometidos a cirug&iacute;a fotorrefractiva, como la queratectom&iacute;a fotorrefractiva (PRK), fueron el desprendimiento de retina (DR) en los 8 casos de un total de 137 casos operados, de los cuales 3 con afectaci&oacute;n del &aacute;rea macular y 5 sin compromiso macular, 3 ojos tambi&eacute;n presentaron hemorragia v&iacute;trea. <body><![CDATA[<P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">La PRK es un procedimiento en el cual se realiza una ablaci&oacute;n directa sobre el epitelio corneal; a diferencia del LASIK, en el cual, el tejido es removido creando un flap corneal con un microqueratomo, preservando la membrana de Bowman y produciendo la ablaci&oacute;n directamente sobre el estroma corneal(9).</font></P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>El procedimiento fotorefractivo (LASIK, PRK) puede producir una serie de complicaciones, tales como: subcorreci&oacute;n o sobrecorrecci&oacute;n, crecimiento epitelial, queratitis, agujeros retinianos, desprendimiento de retina, astigmatismo irregular, complicaciones inherentes al flap corneal en el caso del uso de microqueratomos (LASIK), entre otras.</FONT></P>     <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">En 1990, se ha propuesto la t&eacute;cnica del flap corneal para la queratomileusis in situ asistida con l&aacute;ser (LASIK) como una alternativa a la queratectom&iacute;a fotorrefractiva (PRK) en el tratamiento de las miop&iacute;as moderadas y altas. As&iacute; como tambi&eacute;n, ciertos trabajos han mostrado que el PRK no es efectivo, seguro en las miop&iacute;as mayores de 6,00 dioptr&iacute;as (D)(8-10). El LASIK aparenta ser ahora tan seguro como el PRK en la correcci&oacute;n de las miop&iacute;as bajas pero superior en seguridad que el PRK en la correcci&oacute;n de las miop&iacute;as moderadas y altas(13).</font><FONT FACE="Times" SIZE=1> </P> </FONT>    <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Los ojos mi&oacute;picos tienden a desarrollar degeneraci&oacute;n de Lattice, agujeros retinianos y desprendimiento de retina(14). Sin embargo, en contraste con la extracci&oacute;n de cristalinos transparentes(15) o en casos de perforaci&oacute;n del globo ocular, los cuales pueden influenciar en DR, agujeros retinianos o desprendimientos que no siempre est&aacute;n relacionados con LASIK(16), &eacute;stas son parte de la historia natural de los ojos mi&oacute;picos y una complicaci&oacute;n expectante de la cirug&iacute;a refractiva.</font></P>     <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">La degeneraci&oacute;n mi&oacute;pica est&aacute; caracterizada por un aumento de la longitud axial, con degeneraci&oacute;n progresiva del polo posterior. La hemorragia macular en la alta miop&iacute;a, ocurre por la ruptura de la membrana de Bruch o la neovascularizaci&oacute;n coroidea, ellas son una causa t&iacute;pica de la disminusi&oacute;n de la visi&oacute;n(17).</font></P>     <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">En el LASIK, estudios refieren, que lo que ocurre son dos tipos de estr&eacute;s mec&aacute;nico. El primero, es el aumento de la presi&oacute;n intraocular durante la succi&oacute;n ejercida por el microqueratomo y su anillo de succi&oacute;n pudiera ejercer este estr&eacute;s mec&aacute;nico sobre la base del v&iacute;treo y el segundo, es ocasionado por el choque de las ondas ac&uacute;sticas durante la ablaci&oacute;n ejercida por el l&aacute;ser (18).</font><FONT FACE="Times" SIZE=1> </P> </FONT>    <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Se ha reportado una incidencia del 0,08 % de DR, en pacientes operados de PRK, present&aacute;ndose entre un rango de 9 a 48 meses del posoperatorio, con un equivalente esf&eacute;rico (EE) preoperatorio entre un rango de 0 a -3,00 D, con una p&eacute;rdida leve de la visi&oacute;n o sin ninguna disminuci&oacute;n de la visi&oacute;n despu&eacute;s de la cirug&iacute;a de retina (11). As&iacute; como tambi&eacute;n se han descrito pacientes con alta miop&iacute;a (-13,00 a -20,00 D) que presentaron hemorragia macular entre el primer y sexto mes posterior a PRK, y &eacute;stas se resolvieron eventualmente present&aacute;ndose en estos casos una disminuci&oacute;n de la agudeza visual y cambios pigmentarios y atr&oacute;ficos en la m&aacute;cula(12). Aunque en nuestros casos no se encontr&oacute; hemorragia macular, en todos ellos se produjo desprendimiento de retina con o sin afectaci&oacute;n macular y/o hemorragia v&iacute;trea, lo que coincide con los otros estudios que se trataban de pacientes con cambios mi&oacute;picos severos en el fondo de ojo pero sin ninguna lesi&oacute;n predisponente que pudiera contraindicar la cirug&iacute;a.</font> </P>     <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">La fisiopatolog&iacute;a de estos eventos es a&uacute;n incierta. Algunos consideran que los cambios mi&oacute;picos maculares pueden ser causados por el estiramiento mec&aacute;nico de la retina y la coriocapilar dentro del estafiloma posterior(18).</font></P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>Sin embargo, las complicaciones retinianas posteriores a la queratectom&iacute;a fotorrefractiva (PRK) son poco frecuentes, pero al presentarse las patolog&iacute;as v&iacute;treo retinianas deben manejarse r&aacute;pidamente para lograr obtener una buena agudeza visual.
