Pacientes - Métodos: De 666 pacientes se seleccionaron 62 con lesiones mesentéricas. 42 se diagnosticaron en las primeras horas, en 20 fue tardío. Se evaluó: morbilidad, mortalidad, estancia hospitalaria. Los resultados se presentan en distribución de frecuencias. La prueba "t" para comparar los dos grupos. Chi-cuadrado para variables categóricas, P = 0,01. Resultados: 74,19 % por accidentes viales. 58 con lesión del meso del intestino delgado y 4 en mesocolon. Mediante el lavado peritoneal se diagnosticaron 34 pacientes grupo A. 14 en el B. Las complicaciones fueron menores en A, comparado con el B. (p = 0,0001). La estancia en el A 11 ± 1,2 días - 23 ± 2,3, en B ( P = 0,004). Conclusiones: El diagnóstico tardío esta asociado con alta morbilidad y duración de la hospitalización. Los traumas abdominales cerrados, deben ser explorados exhaustivamente para establecer el diagnóstico precoz.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Objective: To present our experience of mesenteric injuries after blunt abdominal trauma. Patients and Methods: 62 patients with mesenteric injuries out of 666, 42 were diagnosed within 6 hours and in 20 the diagnosis was delayed. Results: The diagnosis was confirmed by diagnostic peritoneal lavage in 34 patients in the group "A" whereas 14 in the group "B" Most of the injuries (n = 46) were caused by road traffic accidents. 58 patients had injured the small bowel mesentery and 4 the large bowel mesentery. There were lower complications in the A group, compared in the B group (p < 0, 0001). Median hospital stay in the A group was 11±1.2 days, compared with 23 ± 2.3 days1) in the B group (P = 0.004). Conclusion: Because delay in diagnosis is significantly associated with morbidity and duration of the hospital stay were recommend that all patients admitted with blunt abdominal trauma should have a diagnostic peritoneal lavage and laparoscopic diagnostic as soon as possible.]]></p></abstract>

<body><![CDATA[<P ALIGN="JUSTIFY">&nbsp;</P> </FONT> <B>    <P ALIGN="justify"><font face="Times New Roman" size="3">INTRODUCCI&Oacute;N</font></P>  </B>    <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Las lesiones localizadas en el mesenterio que se producen luego del trauma abdominal cerrado son raras y de dif&iacute;cil diagn&oacute;stico<sup>(1)</sup>.  El mecanismo usual de su producci&oacute;n es la compresi&oacute;n de las asas intestinales contra la columna vertebral<sup>(2)</sup>.  Tambi&eacute;n, puede ser el resultado de fuerzas ejercidas en el abdomen particularmente en los puntos de inserci&oacute;n del meso, y m&aacute;s raramente por un incremento brusco de la presi&oacute;n intraabdominal<sup>(3)</sup>. Hoy en d&iacute;a los accidentes de tr&aacute;nsito son su raz&oacute;n principal, involucrando a menudo el uso del cintur&oacute;n de seguridad <sup>(4-6)</sup>.  Su grado de severidad va desde la contusi&oacute;n simple a la completa transecci&oacute;n del meso, produciendo isquemia, perforaci&oacute;n con derrame del contenido s&eacute;ptico, seguido de peritonitis, absceso o f&iacute;stula.  Tambi&eacute;n puede tener lugar la estenosis o estrangulaci&oacute;n, <sup>(7)</sup> que se manifiestan horas, d&iacute;as o semanas despu&eacute;s de producirse el trauma.  La incidencia exacta de este tipo de lesiones es desconocida, aunque, se ha se&ntilde;alado entre el 10 % y 13 % en pacientes sometidos a laparotom&iacute;a de emergencia<sup>(8,9)</sup>.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El animo del presente estudio es analizar 62 pacientes con lesiones mesent&eacute;ricas luego del trauma abdominal cerrado, en un intento de encontrar cu&aacute;l es el m&eacute;todo m&aacute;s confiable a fin de establecer el diagn&oacute;stico y tratamiento quir&uacute;rgico temprano en este subgrupo de pacientes, que presentan cl&iacute;nica confusa.</font></P> <FONT FACE="Times" SIZE=2>     <P ALIGN="JUSTIFY"></P> </FONT> <B>    <P ALIGN="justify"><font face="Times New Roman" size="3">PACIENTE Y M&Eacute;TODOS</font></P> </B><FONT FACE="Times" SIZE=2>    <P ALIGN="JUSTIFY"></P> </FONT>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Entre octubre de 1984, y julio de 2004, ingresaron al <U>S</U>ervicio de Cirug&iacute;a General n&uacute;mero 1 Hospital Universitario Miguel P&eacute;rez Carre&ntilde;o en Caracas (UCV); N = 666 pacientes con traumatismo abdominal que requirieron tratamiento quir&uacute;rgico (penetrantes 519, 78 % - no penetrantes 147 22 %.  De esta poblaci&oacute;n se seleccion&oacute; una muestra a conveniencia de 62 pacientes (9,30 %) con lesiones mesent&eacute;ricas ocasionadas por trauma abdominal contuso.  Los criterios de inclusi&oacute;n fueron: edades entre 15 y 68 a&ntilde;os; trauma cerrado con lesiones mesent&eacute;ricas producidas por cualquier agente.  Fueron excluidos de la investigaci&oacute;n los pacientes con trauma abdominal penetrante.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Los datos fueron recogidos retrospectivamente de las historias cl&iacute;nicas y de las descripciones en las hojas operatorias de los pacientes intervenidos con lesi&oacute;n mesent&eacute;rica.  Se estudiaron las variables: edad, sexo, agente etiol&oacute;gico, sitio, hallazgos cl&iacute;nicos, lesiones asociadas, Tipo de lesi&oacute;n, &oacute;rganos involucrados, m&eacute;todos diagn&oacute;sticos, tipo de reparaci&oacute;n, duraci&oacute;n de la hospitalizaci&oacute;n, morbimortalidad.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Para el an&aacute;lisis e interpretaci&oacute;n se dividieron en dos grupos; de acuerdo con el tiempo transcurrido entre el trauma y la realizaci&oacute;n de la intervenci&oacute;n quir&uacute;rgica.  En el grupo A el diagn&oacute;stico se realiz&oacute; en las primeras 6 horas (X= 158± 20 min), (diagn&oacute;stico precoz) en el B fue tard&iacute;o (X = &#8805; 36 horas) luego de la lesi&oacute;n.  El diagn&oacute;stico se bas&oacute; en los antecedentes de trauma, s&iacute;ntomas, signos cl&iacute;nicos, pruebas de laboratorio, imaginolog&iacute;a (rx simple, contrastada, USG - TAC- RMN), lavado peritoneal continuo y a partir de julio 1993, mediante laparoscopia diagn&oacute;stica simplificada LDS.</font></P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">En vista de que el diagn&oacute;stico de la lesi&oacute;n mesent&eacute;rica fue confirmado durante la laparotom&iacute;a terap&eacute;utica, el lapso de tiempo transcurrido entre el accidente y la realizaci&oacute;n de la intervenci&oacute;n sirvi&oacute; de sustento para subagrupar a los lesionados de acuerdo con el momento en que se realiz&oacute; el tratamiento (precoz o tard&iacute;o).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El LPC fue realizado por la &quot;t&eacute;cnica abierta&quot; en pacientes hemodin&aacute;micamente inestables y fue considerado diagn&oacute;stico cuando se obtuvo l&iacute;quido francamente hemorr&aacute;gico o la cuenta de gl&oacute;bulos rojos fue (CGR &gt; 105 / mL).  Debido a que la toma de la muestra fue realizada en el grupo A sin haberse producido la perforaci&oacute;n de las asas intestinales, no se encontr&oacute; elevaci&oacute;n de: gl&oacute;bulos blancos, concentraci&oacute;n de bilirrubina, amilasa, contenido intestinal.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El ultrasonido fue considerado diagn&oacute;stico cuando se comprob&oacute; la presencia de l&iacute;quido libre en la cavidad peritoneal.  La TAC fue indicada en pacientes hemodin&aacute;micamente estables despu&eacute;s de haber administrado contraste v&iacute;a oral y endovenosa e interpretada por radi&oacute;logos experimentados ajenos a la investigaci&oacute;n.  Sospech&aacute;ndose la presencia de lesi&oacute;n mesent&eacute;rica cuando se comprob&oacute; la presencia de aire, o sangre sin lesi&oacute;n en los &oacute;rganos s&oacute;lidos, engrosamiento de la pared, extravasaci&oacute;n del material de contraste o hematoma mesent&eacute;rico.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">La laparotom&iacute;a inmediata fue realizada en pacientes hemodin&aacute;micamente inestables; si la USG, el lavado peritoneal o la LDS indicaban lesi&oacute;n; o si el TAC mostraba evidencias de lesiones mesent&eacute;ricas, o de las asas intestinales en pacientes hemodin&aacute;micamente estable.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">La LDS fue indicada en pacientes con trauma abdominal a partir de que estuvo disponible en nuestro medio y se hab&iacute;a adquirido confianza y seguridad con el procedimiento (1993).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Los datos fueron recolectados retrospectivamente de las historias cl&iacute;nicas y de las hojas quir&uacute;rgicas.  Los resultados se presentan en distribuci&oacute;n de frecuencia, y mediante las estad&iacute;sticas descriptivas medidas de tendencia central, dispersi&oacute;n.  El coeficiente de correlaci&oacute;n de Pearson se utiliz&oacute; para analizar la relaci&oacute;n entre dos variables medidas en un nivel por intervalos o de raz&oacute;n, la prueba &quot;t&quot; para verificar si los dos grupos difieren entre s&iacute; en cuanto a sus medias.  El Chi-cuadrado para evaluar acerca de la relaci&oacute;n entre dos variables categor&iacute;as, con un valor de P = 0,01.</font></P> <FONT FACE="Times" SIZE=2>      <P ALIGN="JUSTIFY"></P> </FONT> <B>    <P ALIGN="justify"><font face="Times New Roman" size="3">RESULTADOS</font></P> </B><FONT FACE="Times" SIZE=2>     <P ALIGN="JUSTIFY"></P> </FONT>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El tiempo de duraci&oacute;n de la investigaci&oacute;n, grupos de estudio, sexo, edad, etiolog&iacute;a, diagn&oacute;stico, localizaci&oacute;n de la lesi&oacute;n; en los 62 pacientes, se presentan en el <B><a href="#Cuadro 1">Cuadro 1</a></B>.  Los hallazgos cl&iacute;nicos, los signos, m&eacute;todos diagn&oacute;sticos, sitio de la lesi&oacute;n, se muestran en el <B><a href="#Cuadro 2">Cuadro 2</a></B>.</font></P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Entre las lesiones asociadas las m&aacute;s frecuentes se localizaron en el h&iacute;gado (17,74 %) y las espl&eacute;nicas en el 8,06 % <B>(<a href="#Cuadro 3">Cuadro 3</a>).</B>  El tipo de reparaci&oacute;n m&aacute;s frecuente fue la enterectom&iacute;a + anastomosis termino – terminal (77,41 %), por la severidad de las lesiones que compromet&iacute;an la viabilidad del asa, utilizando las t&eacute;cnicas manuales o las engrapadoras  quir&uacute;rgicas (80,95 % y 70 %) respectivamente<B>  (<a href="#Cuadro 4">Cuadro 4</a>)</B>.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Las complicaciones se presentaron en mayor frecuencia en el grupo B (85 %) <B><a href="#Cuadro 5">Cuadro 5</a></B>.  El grupo A permaneci&oacute; hospitalizado durante un tiempo m&aacute;s corto X= 11±1,2 d&iacute;as- rango 21 (3-24 d&iacute;as), que el B (23 ± 2,3 – rango 51 (10-61) (P = 0,004) y en &eacute;l se presentaron 26,19 % de complicaciones (P = 0,0001). <body><![CDATA[<P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">* % en la relaci&oacute;n con el total del grupo A- ** % en relaci&oacute;n con el total del grupo A</font></B></TD> </TR> </TABLE>    </center> </div>  <B><FONT FACE="Times" SIZE=2>     <P ALIGN="justify">&nbsp;</P> </FONT>     <P ALIGN="justify"><font face="Times New Roman" size="3">DISCUSI&Oacute;N</font></P> </B><FONT FACE="Times" SIZE=2>    <P ALIGN="JUSTIFY"></P> </FONT>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Las lesiones mesent&eacute;ricas luego del trauma abdominal contuso son raras, menos del 1 % en todos los traumatizados (9,30 % en la muestra)(10-11).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">A causa de su curiosidad y de no presentar s&iacute;ntomas y signos espec&iacute;ficos; su diagn&oacute;stico resulta dif&iacute;cil de establecer.  No obstante, debido a su potencial de morbilidad los cl&iacute;nicos deben sospechar su presencia en todo lesionado con trauma abdominal contuso.  Las consecuencias en el retardo y errores en el diagn&oacute;stico pueden ser fatales, y la hemorragia o peritonitis son motivo de elevada morbilidad(5,3,12).  A menudo, un trauma de menor intensidad puede ocasionar lesiones severas (ca&iacute;das-accidentes deportivos).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Durante el per&iacute;odo de tiempo que comprende esta investigaci&oacute;n (1984-2004), las lesiones mesent&eacute;ricas en el trauma abdominal cerrado se desplegaron con una frecuencia del 9,30 % (raz&oacute;n 0,09 / 1); en pacientes con edad promedio X = 46 ± 1,2 rango 53(15-68); m&aacute;s frecuentes en el sexo masculino (67,74 %) (raz&oacute;n 1,81/1, proporci&oacute;n 0,64) <B>(<a href="#Cuadro 1">Cuadro 1</a>)</B>.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El 74,19 % de las lesiones fue producto de los accidentes de tr&aacute;nsito (46 / 62), las restantes por causas diversas: ca&iacute;das, patadas de animales, accidentes deportivos, o aplastamiento.  El cintur&oacute;n de seguridad causa del s&iacute;ndrome que lleva su nombre se present&oacute; en 7 pacientes (11,29 %).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">En el 20 % de los pacientes del grupo B este tipo de lesiones no fueron reconocidas durante la evaluaci&oacute;n cl&iacute;nica inicial; ya que no se evidenciaron s&iacute;ntomas o signos patognom&oacute;nicos atribuidos a esta variedad de lesiones, no obstante, en m&aacute;s de un paciente se present&oacute; sintomatolog&iacute;a combinada (categor&iacute;as no excluyentes).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">32 pacientes en el grupo A (76,19 %) y 4 del B (20 %) arribaron a la sala de urgencias con cifras tensi&oacute;nales promedio de X= &lt; de 90 mmHg y signos de shock cuando el da&ntilde;o al mesenterio caus&oacute; p&eacute;rdida importante de la volemia <B>(<a href="#Cuadro 2">Cuadro 2</a>)</B>.  Los signos cl&iacute;nicos del hemoperitoneo resultaron vagos, a causa de que no se produjo irritaci&oacute;n peritoneal, circunstancias que enmascaran y retardaron el diagn&oacute;stico.</font></P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Si la perfusi&oacute;n del intestino est&aacute; comprometida por las lesiones vasculares, el dolor abdominal, distensi&oacute;n, contractura, y los ruidos abdominales pueden ponerse en evidencia, si esta cl&iacute;nica permanece por mucho tiempo, el intestino se necrosa resultando en peritonitis y sepsis.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Algunas de estas lesiones (peque&ntilde;as contusiones o laceraciones) pueden ser asintom&aacute;ticas durante las primeras horas luego de la admisi&oacute;n.  Estas lesiones pueden progresar a la isquemia parcial de la pared del asa con ulceraci&oacute;n, inflamaci&oacute;n y fibrosis de la submucosa(3).  Otros autores han reportado la fibrosis de todas las capas(12-18)</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">La cl&iacute;nica no resulta determinante en un alto porcentaje de lesionados para establecer el diagn&oacute;stico preoperatorio.  El dolor abdominal, la contractura, distensi&oacute;n, equimosis y la disminuci&oacute;n o abolici&oacute;n de los ruidos intestinales junto a los antecedentes del trauma soportaron la presunci&oacute;n diagn&oacute;stica de abdomen agudo quir&uacute;rgico en el 89 %<B>  (<a href="#Cuadro 2">Cuadro 2</a>)</B>.  Si las lesiones (desvascularizaci&oacute;n, sangrado, hematoma, desgarro) permanecen sin ser detectadas o corregidas por largos per&iacute;odos de tiempo, el intestino se necrosa resultando en peritonitis y sepsis.  Los hallazgos cl&iacute;nicos m&aacute;s comunes en esta serie fueron: el dolor, la contractura, disminuci&oacute;n de los ruidos intestinales, equimosis, junto con la taquicardia con pulsaciones d&eacute;biles &gt; 130 por minuto.  3 % de los pacientes ingresaron inconscientes a la sala de emergencia.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">En aquellos pacientes con signos cl&iacute;nicos al ingreso de hipotensi&oacute;n o distensi&oacute;n abdominal el diagn&oacute;stico fue realizado durante la laparoscopia diagn&oacute;stica simplificada o laparotom&iacute;a motivado a que son inespec&iacute;ficos, (hipotensi&oacute;n- shock, distensi&oacute;n) y pueden presentarse en otro tipo de lesiones abdominales, circunstancias que condujeron al retardo en la indicaci&oacute;n quir&uacute;rgica.  En 8 pacientes del grupo &quot;B&quot; (40 %), las lesiones fueron laceraciones o contusiones, y condujeron a isquemia parcial de la pared del intestino, ulceraci&oacute;n de la mucosa, inflamaci&oacute;n de la submucosa y fibrosis, hallazgos comprobados a la exploraci&oacute;n quir&uacute;rgica tard&iacute;a(14,15).</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">La realizaci&oacute;n de la laparotom&iacute;a exploradora es la decisi&oacute;n de elecci&oacute;n en este tipo de pacientes, para as&iacute; lograr detectar con altos porcentajes de &eacute;xitos las lesiones mesent&eacute;ricas.  En la era pre laparosc&oacute;pica, la laparotom&iacute;a exploradora (mini laparotom&iacute;a) fue la herramienta de primera elecci&oacute;n (1984-1992).  En aquellos pacientes en los cuales se sospechaba las probabilidades diagn&oacute;sticas de abdomen agudo quir&uacute;rgico, luego de las distintas exploraciones pre operatorias (radiolog&iacute;a simple, contrastada, TAC,USG, RMN).  Existen estudios que enfocan el valor de LPC, USG y TAC en la evaluaci&oacute;n de tales lesionados(16-20). Recientemente se ha desatado interesante debate sobre la sensibilidad y especificidad del LPC – USG - TAC en el diagn&oacute;stico de las lesiones mesent&eacute;ricas pos traum&aacute;ticas.  En muchos pacientes el TAC puede evitar la realizaci&oacute;n de laparotom&iacute;as innecesarias cuando los hallazgos son inequ&iacute;vocamente claros en pacientes luego del trauma abdominal contuso, pero en las lesiones intestinales y mesent&eacute;ricas el TAC tiene baja sensibilidad (60 % en la nuestra)(8).  Mientras que el LPC y la USG han mostrado elevados porcentajes de sensibilidad pero muy baja especificidad, lo cual los hace presentarse como indicadores poco confiables para la realizaci&oacute;n de la laparotom&iacute;a en pacientes hemodin&aacute;micamente estables.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">A partir de febrero del a&ntilde;o 1993, la LDS pas&oacute; a constituirse en la herramienta diagn&oacute;stica b&aacute;sica en el estudio de los traumatismos cerrados toraco-abdominales porque su sensibilidad y especificidad resultaron m&aacute;s elevadas que las de otras exploraciones (RX- USG-TAC-RMN) (96 % y 98 %) respectivamente.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">En esta serie, diagn&oacute;stico y la indicaci&oacute;n quir&uacute;rgica fue determinado por el LPC en el 77,41 % de los pacientes, sin falsos negativos o positivos, sin embargo, los hallazgos al TAC retardaron el diagn&oacute;stico y la laparotom&iacute;a en cuatro pacientes del grupo B.  La USG result&oacute; &uacute;til en el 28,75 % de los pacientes del grupo A y 80 % del B.  <B>(<a href="#Cuadro 2">Cuadro 2</a>)</B>.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">En 14 pacientes del grupo B y cuatro de la A la sintomatolog&iacute;a cl&iacute;nica result&oacute; de gran ayuda para orientar el diagn&oacute;stico de abdomen agudo quir&uacute;rgico.  Nuestros datos sugieren que el retardo en el diagn&oacute;stico incrementa la morbi-mortalidad.  Aun cuando, no tenemos mortalidad en esta cohorte, la morbilidad fue m&aacute;s elevada en el grupo B (85 %) que en el A (23,80 %) (P = 0,004).  <B>(<a href="#Cuadro 5">Cuadro 5</a>) </B></font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">La prolongada estancia hospitalaria y el mayor n&uacute;mero de complicaciones posoperatorias en el grupo B fueron consecuencia principalmente del retardo en el diagn&oacute;stico e indicaci&oacute;n quir&uacute;rgica; sumado a la tardanza en el traslado de estos lesionados de otros centros asistenciales; presentando al ingreso lesiones m&aacute;s avanzadas (peritonitis - sepsis) <B>(<a href="#Cuadro 5">Cuadro 5</a>)</B>.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Las lesiones mesent&eacute;ricas a menudo se asociaron en ambos grupos a las localizadas en otras v&iacute;sceras (h&iacute;gado 17,74 %, bazo 8,06 %, fracturas de pelvis 11,29 %) teniendo como consecuencia que las condiciones de los lesionados sea m&aacute;s cr&iacute;tica, con retardo significativo en la indicaci&oacute;n de la exploraci&oacute;n quir&uacute;rgica.</font></P> <FONT FACE="Times" SIZE=2>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"></P>  </FONT> <B>    <P ALIGN="justify"><font face="Times New Roman" size="3">CONCLUSIONES</font></P> </B><FONT FACE="Times" SIZE=2>    <P ALIGN="JUSTIFY"></P>  </FONT>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Se han analizado 62 pacientes con lesiones mesent&eacute;ricas luego del trauma abdominal cerrado, y nuestros resultados confirman lo reportado por otros autores en investigaciones similares.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Esta modalidad de lesi&oacute;n es poco frecuente en nuestro medio y de dif&iacute;cil diagn&oacute;stico; sobre todo en los casos sub agudos o cr&oacute;nicos; com&uacute;nmente se presenta luego de los accidentes de tr&aacute;nsito, donde se utiliza el cintur&oacute;n de seguridad.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Para minimizar el riesgo del diagn&oacute;stico y tratamiento quir&uacute;rgico tard&iacute;o, recomendamos que debe tenerse siempre en mente la posibilidad de su existencia en todos los pacientes con trauma abdominal, aun en aquellos, sin sintomatolog&iacute;a con hallazgos cl&iacute;nicos m&iacute;nimos, sin evidencias de lesi&oacute;n al TAC o USG.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">La LDS resulta en esta &eacute;poca herramientas de primera l&iacute;nea, por su sencillez (alta sensibilidad y especificidad), porque, permiten la visualizaci&oacute;n magnificada de la cavidad abdominal y se logra establecer el diagn&oacute;stico precoz e indicaci&oacute;n quir&uacute;rgica de urgencia, sin que se produzca retardo injustificado para la realizaci&oacute;n del tratamiento quir&uacute;rgico.</font></P> <FONT FACE="Times" SIZE=2>     <P ALIGN="JUSTIFY"></P> <B>    <P ALIGN="justify">&nbsp;</P> </B>  </FONT> <B>     <P ALIGN="justify"><font face="Times New Roman" size="3">REFERENCIAS BIBLIOGR&Aacute;FICAS</font></P> </B><FONT FACE="Times" SIZE=2>    ]]></body>
