Se valoraron 320 ojos de 300 pacientes, entre 40 a 60 años, que acudieron a la consulta de catarata del Hospital Universitario de Caracas, enero - agosto de 2005. El 59,38 % correspondió al sexo masculino. La AV de ingreso preoperatorio fue -0,6 a -0,7 con 62,50 %. El PAM de estos pacientes con 31,25 % fue de -0,50. El 68,75 % mejoraron 3 o más renglones de la cartilla de Snellen. El 62,50 % de los pacientes a los tres meses cursaban con agudeza visual de 0,00, y un 1,56 % presentó una agudeza visual de -0,70. Las pruebas de PAM predicen la agudeza visual posoperatoria en pacientes con grados leves de cataratas.]]></p></abstract>

We evaluated 320 eyes of 300 patients, between 40 - 60 years old that look up to the cataracts consult of Hospital Universitario de Caracas, January - August 2005. The 59.38 % were male. The visual acuty pre-operation was -0.6 a -0.7 with 62.50 %. The PAM of these patients was -0.50 with 31.25 %. The 68.75 % get better in 3 and more lines of the Snellen cart. The 62.50 % of the patient in three mounts had presented a visual acuty in 0.00, and a 1.56% had presented and visual acuty in -0.70. Se mantiene en su posici&oacute;n gracias a una serie de fibras que constituyen las z&oacute;nulas<sup>(1)</sup>.  Su propiedad principal es la plasticidad, que le permite modificar su curvatura y su &iacute;ndice de refracci&oacute;n durante la acomodaci&oacute;n<sup>(1)</sup>.  Es un &oacute;rgano que no tiene nervios ni vasos; todos los intercambios los realiza a trav&eacute;s de la c&aacute;psula.  Esto explica que sus metabolismos sean fr&aacute;giles, de ah&iacute; que exista la posibilidad de opacificaci&oacute;n o catarata<sup>(1)</sup>.</font>  </P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">La opacificaci&oacute;n lenticular, tanto cong&eacute;nita como adquirida, sigue siendo una de las causas m&aacute;s comunes de disminuci&oacute;n de la agudeza visual.  Las cataratas cong&eacute;nitas se encuentran entre las causas principales de ceguera en ni&ntilde;os, y su detecci&oacute;n y tratamiento precoz tiene importancia vital para minimizar la ambliop&iacute;a y mejorar la recuperaci&oacute;n visual<sup>(2)</sup>.  La catarata senil es el tipo de catarata m&aacute;s frecuente en personas mayores y su prevalencia aumenta marcadamente con la edad.  Dado el envejecimiento progresivo de la poblaci&oacute;n, no es sorprendente que est&eacute; aumentando la prevalencia de cataratas, ni que la extracci&oacute;n de cataratas sea una de las intervenciones quir&uacute;rgicas realizadas con mayor frecuencia<sup>(2)</sup>.</font>  </P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">En ausencia de medidas preventivas, el &uacute;nico recurso para su tratamiento es la cirug&iacute;a.  La cirug&iacute;a de cataratas es una intervenci&oacute;n altamente efectiva que en general devuelve al paciente una visi&oacute;n pr&aacute;cticamente normal a un costo relativamente bajo mejorando significativamente la calidad de vida<sup>(3)</sup>.  De todas maneras, el impacto socioecon&oacute;mico asociado con el diagn&oacute;stico y tratamiento de esta patolog&iacute;a es considerable<sup>(3)</sup>.</font>   </P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El &eacute;xito de la cirug&iacute;a es medido por el grado de recuperaci&oacute;n de la capacidad visual.   Debido a la edad de los pacientes con cataratas, no es excepcional la presencia de otras afecciones oculares coexistentes, sobre todo degeneraciones maculares.  De estar presentes estas afecciones, y si no son correctamente evaluadas, los resultados de la cirug&iacute;a, aunque t&eacute;cnicamente perfecta, ser&aacute;n inferiores a los esperados<sup>(3-6)</sup>.</font>   </P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Existen dos tipos b&aacute;sicos de pruebas de potencial de visi&oacute;n como: subjetivas y objetivas.   Las subjetivas requieren la colaboraci&oacute;n del paciente para responder acerca de diversos est&iacute;mulos visuales que le son presentados<sup>(7,8)</sup>.  El PAM test (Potential Acuity Meter), el test de Lotmar y diversos interfer&oacute;metros se encuentran entre los m&aacute;s utilizados para pruebas subjetivas.  Los tests objetivos son pruebas electrofisiol&oacute;gicas en las cuales la respuesta a los est&iacute;mulos visuales es medida electr&oacute;-nicamente, como los electrorretinogramas y los potenciales evocados visuales<sup>(7,8)</sup>.</font>    </P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El medidor de agudeza potencial (PAM) es un instrumento que se adapta a la l&aacute;mpara de hendidura.  Funciona como un agujero virtual que proyecta una cartilla de agudeza visual de Snellen a trav&eacute;s de un peque&ntilde;&iacute;simo agujero a&eacute;reo aproximadamente de 0,1 mm de di&aacute;metro<sup>(9-11)</sup>.</font>    </P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>Con el prop&oacute;sito del presente trabajo, investigar la utilidad de las pruebas de potencial de agudeza visual (PAM) en la evaluaci&oacute;n preoperatoria de pacientes con cataratas, mediante el an&aacute;lisis de la predicci&oacute;n correcta de agudeza visual posoperatoria. </FONT>  </P> <FONT FACE="Times" SIZE=2>     <P ALIGN="JUSTIFY"></P> </FONT> <B>    <P ALIGN="justify"><FONT face="Times New Roman" size=3>POBLACI&Oacute;N Y M&Eacute;TODOS </FONT></P> </B><FONT FACE="Times" SIZE=2>    <P ALIGN="JUSTIFY"></P> </FONT>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>Se estudi&oacute; la poblaci&oacute;n de pacientes en edades comprendidas de 40 a 60 a&ntilde;os, que consultaron a la consulta de catarata, con diagn&oacute;stico de cataratas inmaduras, uni-bilateral (independientemente de la etiolog&iacute;a) para cirug&iacute;a en el Hospital Universitario De Caracas desde enero de 2005 hasta agosto de 2005.  La muestra estaba constituida por un total de 320 ojos de 300 pacientes, los cuales fueron seleccionados por azar simple, se realiz&oacute; facoemulsificaci&oacute;n a pacientes con AV mejor o igual a 20/400. </FONT></P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>Se realiz&oacute; el examen oftalmol&oacute;gico antes de la cirug&iacute;a, que incluy&oacute; agudeza visual con y sin correcci&oacute;n, refracci&oacute;n computarizada con cicloplej&iacute;a, fondo de ojo con lente + 90D y PAM (por sus siglas en ingl&eacute;s Potential Acuity Meter) realizado con el medidor modelo &quot;Guyton Minkowski&quot;, marca Mentor, adaptado a la l&aacute;mpara de hendidura Haab-Strait 900 marca Topcon, sin evidenciarse ninguna alteraci&oacute;n oftalmol&oacute;gica que pudiera ocasionar la exclusi&oacute;n del paciente. </FONT>  </P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>Para poder realizar la exploraci&oacute;n, es necesario que el paciente conserve al menos una agudeza visual (AV) de cero cinco d&eacute;cima (0,050).  La prueba se realiza bajo m&aacute;xima dilataci&oacute;n pupilar.  En ambiente escot&oacute;pico se le proyecta la luz con la l&aacute;mpara de hendidura con un campo de 3,5 grados y un diafragma de 0,5 mm en el centro de la pupila y comenzando con unos par&aacute;metros en el tambor de una agudeza visual de 0,05 un diafragma de 0,15 y filtro verde. </FONT>  </P> <FONT FACE="Times" SIZE=2>     <P ALIGN="JUSTIFY"></P> </FONT> <B>    <P ALIGN="justify"><FONT face="Times New Roman" size=3>Criterios de exclusi&oacute;n </FONT></P><DIR>  </B>    <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>1. Pacientes con patolog&iacute;as retinianas y maculares </FONT></P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>2. Pacientes con agudeza visual de mayor a -1,30 (Logmar). </FONT></P> <FONT FACE="Times" SIZE=2>     <P ALIGN="JUSTIFY"></P></DIR>  </FONT>      <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>Se presentan los resultados de las mediciones de cada una de las variables en t&eacute;rminos de distribuciones de frecuencias.  Se utilizaron t&eacute;cnicas estad&iacute;sticas descriptivas con medidas porcentuales y frecuencia bruta de los datos para describir la incidencia de las variables </FONT></P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>Los controles posoperatorios se realizaron a las 24 horas, 72 horas, 1 semana, 1 mes, 3 meses. </FONT></P> <FONT FACE="Times" SIZE=2>     <P ALIGN="JUSTIFY"></P> </FONT> <B>    <P ALIGN="justify"><FONT face="Times New Roman" size=3>RESULTADOS </FONT> </P> </B><FONT FACE="Times" SIZE=2>    ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"></P> </FONT>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>Durante el per&iacute;odo de enero - agosto de 2005, en el Servicio de Oftalmolog&iacute;a del Hospital Universitario de Caracas se diagnosticaron un total de 320 ojos de 300 pacientes con diagn&oacute;stico de cataratas, de los cuales el 59,38 % correspond&iacute;a al sexo masculino siendo la edad m&aacute;s frecuente entre 56 – 60 a&ntilde;os con 40,53 % y el 40,63 % al sexo femenino ocupando la misma edades en frecuencia con un 38,46 %.  <B>(<a href="#Cuadro 1">Cuadro 1</a>)</B>. </FONT></P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>Al evaluar la agudeza visual de ingreso pre-operatorio de los pacientes en la consulta de catarata se observ&oacute; que el mayor grupo cursaba con una AV c/c (Logmar) de -0,6 a -0,7 con un 62,50 %.  <B>(<a href="#Cuadro 2">Cuadro 2</a> – Figura 1)</B>. </FONT></P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>La realizaci&oacute;n del PAM en estos pacientes presentaron su mejor agudeza visual en -0,50 con un 31,25 %.  <B>(<a href="#Cuadro 3">Cuadro 3</a>).</B>  Observamos que existe una mejor&iacute;a de la agudeza visual de nuestros pacientes en el momento de realizar el PAM preoperatorio lo que nos hablar&iacute;a del buen pron&oacute;stico visual posoperatorio. Las predicciones fueron m&aacute;s precisas en pacientes con buena agudeza visual preoperatoria, grados leves de cataratas.  Pero es tambi&eacute;n en este grupo donde la predicci&oacute;n mediante la historia cl&iacute;nica y el examen f&iacute;sico es m&aacute;s confiable y son menos necesarios test diagn&oacute;sticos adicionales(12).  El examen oftalmol&oacute;gico en nuestros pacientes era el que nos daba una informaci&oacute;n m&aacute;s exacta sobre el pron&oacute;stico visual, aunque el PAM nos ayudaba a establecer un pron&oacute;stico visual.</font></P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">En la mayor parte de los trabajos se observ&oacute; que la capacidad de las pruebas de visi&oacute;n potencial para predecir visi&oacute;n posoperatoria empeoraba a medida que aumentaba la severidad de las cataratas.  En casi todos los casos, tendieron a subestimar la capacidad posoperatoria del paciente(13,14).  Nosotros consideramos de gran ayuda el uso de esta prueba por el tipo de cataratas incipientes, reflejado as&iacute; en nuestros pacientes.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Seg&uacute;n el comit&eacute; de la AHCPR estableci&oacute; que las pruebas de visi&oacute;n potencial pueden predecir relativamente bien la capacidad visual que obtendr&aacute; el paciente luego de la cirug&iacute;a en los casos de grados leves de cataratas pero los resultados no son confiables en casos m&aacute;s avanzados.(15).  En nuestros pacientes siempre hubo mejor&iacute;a visual ya que el tipo de catarata era leve o incipiente lo que permit&iacute;a realizar una mejor prueba.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Cataract Management Guideline Panel (1993) evidenci&oacute; en su trabajo que los pacientes en los que el polo posterior es visible, las pruebas de visi&oacute;n potencial no proveen informaci&oacute;n adicional a la obtenida mediante la historia cl&iacute;nica y el examen f&iacute;sico como para justificar su uso en la evaluaci&oacute;n de pacientes que van a ser sometidos a cirug&iacute;a de cataratas.  En los pacientes en quienes el polo posterior no es visible, las pruebas de visi&oacute;n potencial no son confiables(15).  Coincidimos con esta publicaci&oacute;n ya que en nuestros pacientes que cursaban con una agudeza visual de -1,30 con el Logman eran los que menos mejoraban con el PAM, por lo que no es confiable determinar que &eacute;ste sea un estudio imprescindible en la valoraci&oacute;n preoperatoria de un paciente de catarata para determinar su calidad de agudeza visual posoperatoria.</font></P> <FONT FACE="Times" SIZE=2>     <P ALIGN="JUSTIFY"></P>  </FONT> <B>    <P ALIGN="justify"><FONT face="Times New Roman" size=3>CONCLUSIONES  </FONT></P> </B><FONT FACE="Times" SIZE=2>    <P ALIGN="JUSTIFY"></P>  </FONT>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>Las pruebas de visi&oacute;n potencial predicen razonablemente bien la agudeza visual posoperatoria en pacientes con grados leves de cataratas y relativamente buena visi&oacute;n preoperatoria.  Sin embargo, en estos pacientes no han demostrado aportar informaci&oacute;n adicional a la obtenida mediante la historia cl&iacute;nica y el examen oftalmol&oacute;gico general.  En casos de cataratas m&aacute;s severas y mala agudeza visual preoperatoria, los resultados de las pruebas de visi&oacute;n potencial no son confiables y tienden en general a subestimar la capacidad visual que alcanzar&aacute; el paciente luego de la cirug&iacute;a.  </FONT></P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>Su uso en la evaluaci&oacute;n rutinaria de pacientes que ser&aacute;n sometidos a cirug&iacute;a de cataratas no est&aacute; justificado.  Todav&iacute;a no ha sido demostrado si en ciertos subgrupos la informaci&oacute;n aportada por las pruebas de visi&oacute;n potencial podr&iacute;a ser de utilidad.  </FONT></P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>Las pruebas de visi&oacute;n potencial no deben ser utilizadas en forma rutinaria para evaluar a los pacientes que ser&aacute;n sometidos a cirug&iacute;a de cataratas.  Si bien podr&iacute;a haber un subgrupo de pacientes con medio claro y degeneraci&oacute;n macular en quienes las pruebas de visi&oacute;n potencial podr&iacute;an ser de utilidad, esto a&uacute;n no ha sido demostrado.  </FONT>  </P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>El PAM test es considerado una prueba menor incluida en la visita oftalmol&oacute;gica que no puede ser facturado en forma independiente.  Con respecto a las pruebas de visi&oacute;n potencial se considera que no est&aacute;n indicadas dentro de la evaluaci&oacute;n preoperatoria del paciente con cataratas y de ser realizadas deben asentarse en la historia cl&iacute;nica los motivos por los cuales se consider&oacute; necesaria su utilizaci&oacute;n.  </FONT></P> <FONT FACE="Times" SIZE=2>     ]]></body>
