El objetivo del presente trabajo fue demostrar las variaciones del perfil lipídico posprandial, después de una comida rica en grasa y su relación con el IMC, antecedentes familiares, personales y la enfermedad arterial coronaria en dicha respuesta, en 190 pacientes, a los que se les realizó el perfil lipídico basal (colesterol, triglicéridos, HDL-C, LDL-C) con 14 horas de ayuno. Posteriormente se les suministró una comida con 100 gramos de grasa, y se les realizó una toma de muestra de sangre periférica a las 6 y 8 horas respectivamente. Se encontró que los individuos intolerantes y lipémicos se correlacionan positivamente con IMC > de 25, HTA y antecedentes personales y familiares para enfermedad arterial coronaria. La prueba de tolerancia de grasa es un estudio sencillo, fácil de realizar y un buen método para detectar pacientes de alto riesgo para enfermedad cardiovascular.]]></p></abstract>

<body><![CDATA[<P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3>Key words:</FONT></B> <FONT face="Times New Roman" size=3> Fat tolerance, Cardiovascular disease.</FONT></P>     <P ALIGN="JUSTIFY">&nbsp;</P>     <P ALIGN="JUSTIFY"><B><FONT face="Times New Roman" size=3>INTRODUCCI&Oacute;N</FONT> </B></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Numerosos estudios han se&ntilde;alado la asociaci&oacute;n entre el metabolismo posprandial de las lipoprote&iacute;nas ricas en triglic&eacute;ridos y la presencia de enfermedad arterial coronaria, los triglic&eacute;ridos posprandiales elevados generan mayor cantidad de remanentes de quilomicrones (QM) y VLDL, esto promueve la formaci&oacute;n de LDL-C peque&ntilde;as y densas, as&iacute; como la disminuci&oacute;n en las concentraciones de HDL-C.  En la sangre los triglic&eacute;ridos son transportados en la lipoprote&iacute;nas llamadas QM y VLDL, los triglic&eacute;ridos muestran variaciones diurnas significativas debido a que su concentraci&oacute;n aumenta posterior a una ingesta de comida rica en grasa, y actualmente se conoce que valores posprandiales y no en ayunas son lo m&aacute;s importantes para el pron&oacute;stico y el desarrollo de enfermedad coronaria<sup>(1-5)</sup>.</font>  </P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">La lipemia posprandial se refiere a un incremento exagerado en plasma de las concentraciones de triglic&eacute;ridos consecuencia del aumento en el tiempo de residencia de estas lipoprote&iacute;nas transportadoras, especialmente de triglic&eacute;ridos por efectos de trastornos en su metabolismo, fundamentalmente por alteraci&oacute;n de la LPL, luego de la ingesti&oacute;n de una comida rica en grasa<sup>(3)</sup>.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">La cantidad de grasa sugerida para la prueba es de 60 a 75 gramos por m2 de superficie corporal, equivalente a 80-120 gramos de grasa para un hombre de 70 kg; concentraciones de triglic&eacute;ridos a las 6 y 8 horas posterior a la ingesta de grasa menores de 200 mg/dL, son consideradas normales entre 200 y 270; intolerancia a las grasas y niveles mayores 270 mg/dL lipemia posprandial, el aclaramiento posprandial de las lipoprote&iacute;nas dura aproximadamente 4 a 8 horas; su magnitud y duraci&oacute;n est&aacute; determinada por factores gen&eacute;ticos, fisiol&oacute;gicos como la edad, sexo, ejercicio y nutricionales (en especial contenido de grasa y carbohidratos).</font>  </P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3> El objetivo del presente trabajo fue demostrar las variaciones del perfil lip&iacute;dico (triglic&eacute;ridos y HDL-C) posprandial, despu&eacute;s de una comida rica en grasa y su relaci&oacute;n con el IMC, antecedentes familiares, personales y la enfermedad arterial coronaria en dicha respuesta. </FONT> </P> <FONT FACE="Times" SIZE=2>      <P ALIGN="JUSTIFY"></P> </FONT> <B>    <P><FONT face="Times New Roman" size=3> MATERIALES Y M&Eacute;TODOS </FONT></P> </B><FONT FACE="Times" SIZE=2>     <P ALIGN="JUSTIFY"></P> </FONT>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3> Este estudio fue realizado en un per&iacute;odo de 18 meses con pacientes que acud&iacute;an al Departamento de Endocrinolog&iacute;a y Enfermedades Metab&oacute;licas del Hospital Militar. </FONT>  </P>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3> En dicho trabajo se estudiaron 190 individuos distribuidos 49 % hombres, 51 % mujeres con un promedio de edad 45,7 ± 12,05 a&ntilde;os (19-75 a&ntilde;os).  A todos se les realiz&oacute; una encuesta tomando datos del paciente, antecedentes personales, familiares, h&aacute;bitos relevantes (alimentaci&oacute;n, actividad f&iacute;sica, tabaquismo) e IMC, a todos los pacientes se les realizaron ex&aacute;menes de laboratorio que inclu&iacute;an, perfil lip&iacute;dico basal.  (Colesterol, triglic&eacute;ridos, HDL-C, LDL-C) con 14 horas de ayuno, posteriormente se les suministr&oacute; una comida con 100 gramos de grasa (tocineta, jam&oacute;n, 2 huevos fritos, mantequilla, t&eacute;) la toma de muestra se realiz&oacute; a las 6 y 8 horas respectivamente. </FONT></P> <FONT FACE="Times" SIZE=2>      <P ALIGN="JUSTIFY">  </P> </FONT> <B>    <P><FONT face="Times New Roman" size=3> Metodolog&iacute;a del Laboratorio </FONT></P> </B><FONT FACE="Times" SIZE=2>     <P ALIGN="JUSTIFY"></P> </FONT>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3> Se tom&oacute; sangre en ayunas de 14 horas y fueron centrifugadas a 1000 g por 20 min, para determinar colesterol, triglic&eacute;ridos, HDL, y LDL-colesterol.  Los m&eacute;todos de laboratorio utilizados fueron los siguientes: glicemia, colesterol, triglic&eacute;ridos y HDL-colesterol por kit comercial, enzim&aacute;tico colorim&eacute;trico de Roche Diagn&oacute;stico.  LDL-colesterol determinado matem&aacute;ticamente utilizando la f&oacute;rmula de Friedewald. </FONT>  </P> <FONT FACE="Times" SIZE=2>      <P ALIGN="JUSTIFY"></P> </FONT> <B>    <P><FONT face="Times New Roman" size=3> RESULTADOS </FONT></P> </B><FONT FACE="Times" SIZE=2>     <P ALIGN="JUSTIFY"></P> </FONT>     <P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3> De los 190 pacientes estudiados, el 68 % presentaban colesterol  total  elevado; el 48 % una LDL elevada, el 57 % triglic&eacute;ridos elevados y el 56 % una HDL baja.  Correlacionando perfil lip&iacute;dico con obesidad, el 65% ten&iacute;an obesidad y colesterol total elevado, el 67 % eran obesos con HDL bajo y un 36 % IMC &gt; de 25 con triglic&eacute;ridos elevados.  El 62 % de los individuos ten&iacute;an triglic&eacute;ridos elevados y HDL bajo.  Posterior a la comida rica en grasa, del total de individuos estudiados el 57,59 % ten&iacute;an a las 6 y 8 horas valores normales (triglic&eacute;ridos &lt; 200mg/dL), 18,33 % intolerancia a las grasas (triglic&eacute;ridos entre 201-270 mg/dL) y el 24,08 % eran lip&eacute;micos, como se puede observar el 42,41 % presentaron alteraci&oacute;n en el perfil de triglic&eacute;ridos, en cuanto a lipemia e IMC, el 27 % eran lip&eacute;micos obesos, 7 % obesos con intolerancia a las grasas. <body><![CDATA[<P ALIGN="JUSTIFY"><FONT face="Times New Roman" size=3> Asimismo se puede observar que del porcentaje de lip&eacute;micos, el 67 % presentaban colesterol normal y 33 % colesterol elevado.  Comparando resultados de lipemia con antecedentes personales y familiares, se pudo observar que los lip&eacute;micos ten&iacute;an mayor incidencia de IMC, HTA y dislipidemia entre los antecedentes personales, siendo el IMC y la HTA el mayor porcentaje entre los antecedentes familiares. Especialmente asociados con valores bajos de HDL, los triglic&eacute;ridos elevados y HDL bajos es un patr&oacute;n definitivamente aceptado como factor de riesgo independiente para EAC.  Es importante hacer destacar que con la tolerancia de grasa se identifica un grupo denominado por nosotros intolerante con valores de triglic&eacute;ridos a las 6 u 8 horas entre 200 y 270 mg/dL que son igualmente de alto riesgo y que requieren tratamiento.  Se pudo apreciar adem&aacute;s que los individuos que resultaron intolerantes y lip&eacute;micos se correlacionan positivamente con IMC &gt; de 25, HTA y antecedentes personales y familiares positivos, para enfermedad arterial coronaria.  </FONT></P> <FONT FACE="Times" SIZE=2>     <P ALIGN="JUSTIFY"></P>  </FONT> <B>    ]]></body>
