<body><![CDATA[<p align="justify"><font face="Verdana" size="2"><b>INTRODUCCIÓN</b></font></p>     <p align="justify"><font face="Verdana" size="2">La hernia de diafragmática  congénita (HDC) es una malformación estructural que se deriva de una fusión  inadecuada de estructuras en el período embrionario y que posee una elevada  mortalidad, cuya frecuencia de aparición es de aproximadamente 1/3 000  nacimientos. El porcentaje de supervivencia para recién nacidos con hernias del  foramen de Bochdalek varía de institución a institución; pero en general, ha  quedado en aproximadamente 50 % por casi medio siglo, a pesar de los avances en  las unidades de cuidados intensivos neonatal, anestesia, y cirugía.</font></p>     <p align="justify"><font face="Verdana" size="2"><b>HERNIA DIAFRAGMÁTICA  CONGÉNITA</b></font></p>     <p align="justify"><font face="Verdana" size="2">En Grecia antigua Hipócrates  (460 - 370 a.c) escribió “Las heridas penetrantes de la vejiga, el cerebro, el  corazón, septum transversum, intestino delgado, estómago o hígado son  fatales”(1).</font></p>     <p align="justify"><font face="Verdana" size="2">El diafragma muscular se forma  embriológicamente por una compleja fusión de cuatro estructuras embrionarias:  septum transverso, membranas pleuroperitoneales, mesenterio dorsal del esófago y  pared corporal(2). La hernia diafragmática congénita (HDC) se deriva de la  fusión inadecuada de estas estructuras que origina un orificio del diafragma que  permite que parte del contenido abdominal se traslade al tórax por diferencias  de presiones(3). El defecto es izquierdo en un 75 % a 90 % de los casos por lo  cual el hallazgo usual es la herniación del estómago hacia el tórax con  desplazamiento del corazón al lado derecho(4). También se puede encontrar en el  tórax asas intestinales e inclusive hígado. Esta malformación estructural se  presenta en 1:2 000 a 5 000 nacimientos(5,6) y la mortalidad neonatal supera al  70 % en los centros más altamente especializados que cuentan con ventilación de  alta frecuencia y oxigenación de membrana extracorpórea. En centros que no  poseen estas tecnologías la mortalidad es cercana al 100 %, principalmente en  los casos de aparición temprana en el segundo trimestre(7).</font></p>     <p align="justify"><font face="Verdana" size="2">La etiología de la HDC aun no  está muy clara, la mayoría son esporádicas y sólo el 2 % tiene una asociación  familiar (8). Aunque la mayoría de los casos de HDC son idiopáticos, ha sido  reportada una rara asociación con teratógenos como phenometrazina y talidomida  (9). Esta patología se encuentra asociada a alteraciones cromosómicas que  incluyen trisomías 13 y 18, y a otras alteraciones estructurales por lo cual se  sugiere incluir estudio citogenético y examen ecográfico detallado en el  protocolo de manejo; otros defectos asociados incluyen defectos cardíacos  congénitos agenesia o hidronefrosis renal, atresia intestinal, secuestro  pulmonar extralobar y defectos neurológicos (10,11).</font></p>     <p align="justify"><font face="Verdana" size="2">La sobreviva de la HDC aislada  severa es muy pobre, llegando a ser la mortalidad del 93 % aproximadamente en  infantes con otras anormalidades acompañantes. La sobrevida de esta patología  aislada varía de institución a institución, encontrándose aproximadamente en el  50 % en aquellas de tercer nivel(12). La alta mortalidad de esta patología  diagnosticada en el período prenatal ha sido confirmada por varios  autores(13-16).</font></p>     <p align="justify"><font face="Verdana" size="2">El primer reporte de hernia  diafragmática congénita (HDC) lo realizó Lazarus Riverius (1589 - 1655) en 1679  en un hallazgo incidental en un examen post mortem de un joven de 24 años de  edad. La primera descripción pediátrica de HDC en la literatura fue reportada en  1701 por Sir Charles Holt, en su artículo describe las conclusiones clínicas y  post mortem de un recién nacido con HDC. Ya Laënnec postuló en 1800 la  reparación quirúrgica por vía abdominal de la HDC. La primera reparación exitosa  de HDC fue informada en la literatura germana en 1905. En 1925 Hedbolm reportó  un 75 % de mortalidad en infantes si el defecto no era reparado antes del primer  mes de vida, sugiriendo así que la cirugía temprana podría mejorar la sobrevida  de estos pacientes, llevando así al concepto de que la HDC era una “emergencia  quirúrgica”(17). La primera reparación exitosa de HDC en un niño fue reportada  en el <i>Journal of the American Medical Association </i>por Bettman y Hess en  1929(18). La introducción del modelo de cordero fetal por Lorimier y sus  compañeros de trabajo en 1967 suministró unos medios confiables para el estudio  de la fisiopatología de la HDC. La viabilidad y el beneficio de la reparación  quirúrgica del diafragma in útero fueron a su vez estudiados en HDC de cordero  fetal y aplicado a un número limitado de pacientes(19). La oxigenación con  membranas extracorpóreas (ECMO) ha sido también introducida para el tratamiento  de las fallas respiratorias del neonato. Ciertas evidencias soportan que el ECMO  en la HDC sea aun conflictivo. En 2002 Cochrane Review(20) mostró que todos los  recién nacidos con insuficiencia respiratoria en quienes se utilizaron ECMO como  terapia coadyuvante, mejoraban la supervivencia de forma importante, sin  embargo, la ventaja era mucho menos para los pacientes con HDC. A pesar de los  avances en el cuidado neonatal, 50 % de los recién nacidos afectados de hernia  diafragmática congénita (HDC) aislada, grave, mueren por insuficiencia  respiratoria secundaria a hipoplasia pulmonar(21,22). La supervivencia está  relacionada con la morbilidad importante a largo plazo, principalmente por  enfermedad pulmonar, neurológica, y gastrointestinal(23,24).</font></p>     <p align="justify"><font face="Verdana" size="2">Durante el último trimestre de  la gestación en seres humanos y ovejas, el desarrollo pulmonar está  caracterizado por la expansión de las vías aéreas principales, el intersticio se  reduce y así también se reduce el grosor de la barrera de intercambio,  incrementando de esa forma el área de superficie para intercambio gaseoso (25),  produciéndose también la subdivisión de los sacos terminales y moldeamiento de  los alvéolos. Todos estos cambios del desarrollo pulmonar fetal están en función  del líquido que se produce en este órgano en volúmenes suficientes, generados  por un proceso de secreción activa que varía entre 2,7 y 3,7 mL /kg/h en el  último trimestre(26).</font></p>     <p align="justify"><font face="Verdana" size="2">Es bien conocido que el  crecimiento del pulmón puede ser modificado por manipulación del volumen del  líquido pulmonar. Así fue que la oclusión traqueal ha sido valorada tras crear  quirúrgicamente la HDC in útero en fetos de corderos. Por lo tanto, sabemos que  el drenaje del fluido pulmonar fetal retrasa el crecimiento del pulmón,  resultando así en hipoplasia pulmonar (27,28), mientras que con la oclusión de  la tráquea resulta en hiperplasia pulmonar(27,29).</font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="Verdana" size="2">Aunque este procedimiento  amplía el pulmón, aumenta también la masa pulmonar, y temporalmente el  intercambio de gas mejora(30). Los estudios adicionales han revisado los cambios  generados de la oclusión traqueal antes del parto con resultados  variables(31,32). Este cierre de tráquea en forma prenatal también ha sido  aplicado a fetos humanos(33); con resultados iniciales pocos alentadores(34).</font></p>     <p align="justify"><font face="Verdana" size="2">Las reducciones del volumen de  líquido intraluminal son relacionadas con hipoplasia pulmonar(27,35); a la  inversa, la oclusión traqueal obstaculiza el egreso de líquido del pulmón  acelerando el crecimiento de pulmón fetal(27,36-39).</font></p>     <p align="justify"><font face="Verdana" size="2">El diagnóstico ecográfico es  sencillo y cada vez más reportado y consiste en observar en una ecografía de  rutina aproximadamente a las 20 semanas de gestación la cámara gástrica ubicada  en el tórax en el corte de 4 cámaras, lo cual se corrobora posteriormente al  hacer el corte longitudinal corporal fetal(40-42). La dextroposición cardíaca se  evidencia al observar el corazón en el hemitórax derecho pero sin desviación del  eje cardíaco. A pesar de esto la HDC aislada sólo es diagnosticada en el 50 % de  los casos durante el período prenatal. Avances en resonancia magnética (RM) han  proveído imágenes de alta calidad de los órganos intratorácicos, definiendo las  relaciones espaciales de los pulmones fetales y la medición del volumen  pulmonar(43,44).</font></p>     <p align="justify"><font face="Verdana" size="2">Mahieu - Caputo y col.(44)  calculó las mediciones de MRI del volumen de pulmón fetal, calculando el volumen  de pulmón esperado como una función de la edad gestacional. Descubrió que la  relación de lo observado por lo esperado de volumen de pulmón fetal, el  porcentaje era significativamente más bajo en HDC que en controles, postulando  que en HDC aislada la medición de volumen de pulmón fetal por MR es un indicador  potencial de hipoplasia pulmonar y resultado posnatal.</font></p>     <p align="justify"><font face="Verdana" size="2">La principal causa de la alta  mortalidad que exhiben estos casos es la hipoplasia pulmonar que ocurre por la  compresión pulmonar extrínseca originada de la herniación de las vísceras  abdominales.</font></p>     <p align="justify"><font face="Verdana" size="2">El modelo de HDC en ovejas  abrió camino para las intervenciones fetales y los procedimientos endoscópicos (FETENDO),  todos en un intento de producir el crecimiento del pulmón fetal(45-48).</font></p>     <p align="justify"><font face="Verdana" size="2">Algunos autores han comunicado  que la ligadura de la tráquea en fetos ovinos con hernia diafragmática  iatrogénica impide la hipoplasia pulmonar(49).</font></p>     <p align="justify"><font face="Verdana" size="2">Pero esta terapéutica para el  crecimiento acelerado del pulmón fetal podría tener efectos negativos sobre la  población de neumocitos tipo II (50), lo que ha incitado a algunos  investigadores a estudiar si los esteroides prenatales podrían revertir esos  efectos adversos de la oclusión traqueal(51,52).</font></p>     <p align="justify"><font face="Verdana" size="2">Nuestro Grupo Venezolano de  Investigación en Terapia Fetal ha demostrado el mismo efecto con la técnica de  oclusión traqueal intraluminal fetoscópica que hemos desarrollado en el  Instituto Venezolano de Investigaciones Científicas (IVIC) en modelos  experimentales ovinos. La oclusión traqueal intraluminal durante 3 semanas  resultó en acumulación neta de líquido en el árbol traqueo-bronquial con  expansión pulmonar. El análisis histológico, molecular y funcional de los  pulmones demostró la no detención del desarrollo pulmonar y la proliferación  celular. Basados en estos hallazgos, Harrison y col. han realizado la oclusión  traqueal extraluminal en dos fetos con hernia diafragmática congénita (53) y  Flake y col. en 15 casos por cirugía abierta donde hubo sobrevida de 5 casos (33  %) con serias complicaciones en 4 de ellos por la invasividad de la cirugía(54).</font></p>     <p align="justify"><font face="Verdana" size="2">Quintero y col.(55) han  diseñado un procedimiento endoscópico mínimamente invasivo para ocluir  intraluminalmente en forma fácil y reversible la tráquea fetal en casos de  hernia diafragmática congénita no asociados a cromosomopatías. La técnica  consiste en introducir un fetoscopio diagnóstico con su camisa en la cavidad  oral fetal y avanzarlos hasta la tráquea pasando a través de la glotis. Al  llegar a nivel de la tráquea se mantiene la camisa en posición y se retira el  elemento óptico. A través de la camisa se introduce un dispositivo de oclusión  intratraqueal que es colocado en la luz traqueal al salir por el extremo distal  de la camisa. Este paso es monitorizado por ultrasonido. El dispositivo es capaz  de ocluir en forma reversible la tráquea durante un período de 3 a 4 semanas por  incluir en su estructura doble malla de material reabsorbible. También es capaz  de mantenerse oclusivo a pesar del crecimiento traqueal fetal debido a que es  expansible. Para permitir la ventilación del neonato, el dispositivo puede  retirarse con facilidad al nacer, sin lesionar las vías aéreas superiores al  momento del nacimiento, gracias a una sutura no absorbible que está entrelazada  en la estructura de titanio que permite la retracción del dispositivo al ser  retirado. La edad gestacional óptima para la realización de la cirugía es aún  controversial(56-58).</font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="Verdana" size="2">En nuestro centro se ha  realizado la oclusión traqueal intraluminal fetoscópica en un feto humano con  hernia diafragmática congénita a las 26 semanas de embarazo (<a href="#fig1">Figuras  1</a> y <a href="#fig2">2</a>). Desafortunadamente, la paciente presentó parto  prematuro a las 30 semanas de gestación y el neonato falleció por problemas  inherentes a la prematuridad. No obstante, la autopsia confirmó la ausencia de  hipoplasia pulmonar (<a href="#fig3">Figura 3</a>). El uso de ventilación  oscilatoria de alta frecuencia también ha sido valorado en el tratamiento de  esta patología como técnica de estabilización del paciente tanto intra como  posoperatorio(59-62).</font></p>     <p align="center"><a name="fig1"> <img border="0" src="/img/fbpe/rfm/v31n1/art02fig1.gif" width="474" height="481"></a></p>
