Caso de una lactante mayor femenina de 23 meses de edad, quien ingresa al Hospital “J M de los Ríos” con enfermedad de 18 meses de evolución caracterizada por lesión nodular de 5 cm de diámetro en un tercio proximal de antebrazo derecho de consistencia blanda, no dolorosa y de crecimiento progresivo por lo cual es referida a nuestro centro donde se realizan estudios, y biopsia que reporta: hallazgos histológicos e inmunohistoquímicos compatibles con un rabdomiosarcoma embrionario con diferenciación ganglioneuromatosa (Ectomesenquimoma maligno). Inicia protocolo de quimioterapia, para sarcoma de Ewing/tumor neuroectodérmico primitivo óseo y extra óseo no metastásico y metastásico, cumple tres ciclos con mejoría clínica de la lesión tumoral.]]></p></abstract>

The mesenchymal component is most often rhabdomyosarcoma. The present study goes directed to discussion of case of a feminine greater suckling baby of 23 months of age, that enters Hospital “J M of Rios” with disease of 18 months of evolution characterized by injury to nodular of 5 cm of diameter in a proximal third of right forearm of soft, no painful consistency and of progressive growth thus is referred our center wherestudies are made biopsy that reports: Compatible histological and immunohistochemestry findings with rabdomiosarcoma embryonic with ganglioneuromatosa differentiation(malignant ectomesenquimoma). It initiates protocol of chemotherapy for sarcoma of Ewing /tumor nonmetastasic and metastatic bony and extra bony neuroectodermic primitive, it fulfills three cycles with clinical improvement of the tumor like injury.]]></p></abstract>

<body><![CDATA[<p align="justify">INTRODUCCIÓN</p> </b>     <p ALIGN="justify">La palabra <i>ectomesenchyme </i>se refiere a las células  mesenquimales que se derivan de la cresta neural <i>Ectomesenchymomas </i>(EMCH)  son los tumores malignos que se deriven de este tejido. Se <SPAN style="color:#000000" >caracterizan por ambas diferenciaciones componentes neuro-ectodérmicosy componentes mesenquimales<Sup>(1)</Sup>. Estos tumores son tradicionalmente compuestos por células bien diferenciadas o neuroblásticas (neuroblastoma, ganglioneuroblastoma, ganglioneuroma) periféricos, tumores neuroectodérmicos primitivos (PNET) y uno o más elementos mesenquimales malignos, por lo general de tipo rabdomiosarcoma. Es considerada como una entidad clínico-patológica especial <Sup>(2)</Sup></SPAN ><SPAN style="color:#003F80" >. </SPAN ><SPAN style="color:#000000" >Este raro tumor se ha descrito en el sistema nervioso central y en diversos tejidos blandos. En este trabajo se describe la enfermedad en una paciente, lactante mayor femenina de 23 meses de edad que presentó un ectomesenquimoma maligno en antebrazo izquierdo, se describe el tratamiento efectuado.</SPAN ></font></p>     <p ALIGN="justify"> <SPAN style="font-family:'Verdana'; font-weight:bold; color:#000000; font-style:normal" ><font size="2">CASO CLÍNICO </font> </SPAN ></p>     <p ALIGN="justify"> <SPAN style="font-family:'Verdana'; color:#000000; font-style:normal; font-weight:normal" ><font size="2">Se trata de una lactante mayor femenina de 23 meses edad, natural y procedente del estado Barinas, madre refiere inicio de enfermedad actual los 5 meses de edad, presentando lesión nodular de aproximadamente 5 cm de diámetro en 1/3 proximal de antebrazo derecho, de consistencia blanda, no dolorosa y crecimiento progresivo; en diciembre 2007presenta exacerbación del patrónde crecimiento y signos de flogosis, acude al hospital de la localidad donde es referido a este centro, ingresando el 1 de febrero de 2008.  </font> </SPAN ></p>     <p ALIGN="justify"> <SPAN style="font-family:'Verdana'; color:#000000; font-style:normal; font-weight:normal" ><font size="2">Paciente en condiciones clínicas estables, hidratada, sin fiebre al tacto, peso y talla acorde a edad, adenopatías de 0,5 cm de diámetro, móviles,no dolorosas, de consistencia dura, adherida a planos profundos a nivel cervical bilateral, adenopatías de 1 cm de diámetro de las mismas características anteriores a nivel supraclavicular y axilar ipsilateral. Aumento de volumen de consistencia dura de 20 cm de diámetro aproximadamente adherida a planos profundos de apariencia nodular y red venosa colateral, con buen llenado capilar, pulso radial presente, en cara lateral y 1/3 proximal de antebrazo derecho, sin limitación funcional y signos de flogosis. Diámetro de antebrazo contra lateral 14 cm. Cardiopulmonar sin hallazgos positivos, abdomen blando, sin vísceromegalias. </font> </SPAN ></p>     <p ALIGN="justify"> <SPAN style="font-family:'Verdana'; color:#000000; font-style:normal; font-weight:normal" ><font size="2">Los exámenes de laboratorio reportaron una hemoglobina en 12,1 g/dL, glóbulos blancos 15 500 mm, plaquetas 944 000 mm, LDH 746 U/L, alfa fetoproteína 4,58 ng/mL, antígeno. carcinoembrionario 1,8 ng/mL, ácido vanilmandélico 5,5 mg/L </font> </SPAN ></p>     <p ALIGN="justify"> <SPAN style="font-family:'Verdana'; color:#000000; font-style:normal; font-weight:normal" ><font size="2">En el ecosonograma de partes blandas se aprecia en región externa del antebrazo derecho un aumento de volumen con engrosamiento de tejido muscular de la zona, de características heterogéneas bien circunscrita que llega al tejido óseo que ocupa desde 1/3 interior de codo hasta 5 cm por encima de la muñeca de 7 cm de longitud con 25 cm x 32 cm de grosor. </font> </SPAN ></p>     <p ALIGN="justify"> <SPAN style="font-family:'Verdana'; color:#000000; font-style:normal; font-weight:normal" ><font size="2">Eco Doppler en sistema arterial derecho: tumoración sólida vascularizada con áreas de necrosis central y de calcificación con flujo de arteria radial y arteria cubital con flujo disminuido por compresión.</font></SPAN ></p>     <p ALIGN="justify"> <SPAN style="font-family:'Verdana'; color:#000000; font-style:normal; font-weight:normal" ><font size="2">Resonancia magnética nuclear del brazo derecho con contraste: se aprecia imagende lesiónde ocupación de espacio (LOE) de partes blandas aexpensas predominante de elementos musculares extensores del antebrazo de comportamiento sólido con áreas de necrosis en su interior, ademásdel crecimiento mencionado existente extensión del LOE hacia la porción anterior del brazo a través del músculo braquial.</font></SPAN ></p>     <p ALIGN="justify"> <SPAN style="font-family:'Verdana'; color:#000000; font-style:normal; font-weight:normal" ><font size="2">Se realiza biopsia que reporta: los hallazgos histológicos e inmunohistoquímicos son compatibles con un rabdomiosarcoma embrionario con diferenciación ganglioneuromatosa (Ectomesenquimoma maligno). Inicia protocolode quimioterapia, para sarcoma de Ewing y tumor neuroectodérmico primitivo óseo y extra óseo no metastásico y metastásico, cumple tres ciclos con mejoría clínica de la lesión tumoral.</font></SPAN ></p>     ]]></body>
<body><![CDATA[<p ALIGN="justify"> <SPAN style="font-family:'Verdana'; font-weight:bold; color:#000000; font-style:normal" ><font size="2">DISCUSIÓN</font></SPAN ></p>     <p ALIGN="justify"> <SPAN style="font-family:'Verdana'; color:#000000; font-style:normal; font-weight:normal" ><font size="2">Los ectomesenquimomas malignos son tumores raros, compuesto de neuroblastos y / o células ganglionares y componentes malignos mesenquimales de tejidos de diversos tipos, generalmente rabdomiosarcoma</font><Sup><font size="2">(3,4)</font></Sup><font size="2">.</font></SPAN ></p>     <p ALIGN="justify"> <SPAN style="font-family:'Verdana'; color:#000000; font-style:normal; font-weight:normal" ><font size="2">La teoría más ampliamente aceptada sugiere que este tumor deriva de los restos de células migratorias de la cresta neural y, por tanto, desde el mesénquima</font><Sup><font size="2">(2-5)</font></Sup><font size="2">.</font></SPAN ></p>     <p ALIGN="justify"> <SPAN style="font-family:'Verdana'; font-style:normal; font-weight:normal; color:#000000" ><font size="2">A finales del siglo XIX, se descubrió que el ectodermo dorsal de la cabeza ha contribuido a la formación de células mesenquimalesen el cartílago de los arcos viscerales y ladentina. Holimon y Rosenblum propusieron el término “</font></SPAN ><SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#000000" ><font size="2">gangliorhabdomyosarcoma” </font> </SPAN > <SPAN style="font-family:'Verdana'; font-style:normal; font-weight:normal; color:#000000" ><font size="2">paradescribir un tumor en el ángulo cerebeloso, es decir un ganglioneuroma que contiene elementos </font> </SPAN > <SPAN style="font-family:'Verdana'; font-style:italic; font-weight:normal; color:#000000" ><font size="2">rhabdomyosarcomatosos </font> </SPAN > <SPAN style="font-family:'Verdana'; font-style:normal; font-weight:normal; color:#000000" ><Sup><font size="2">(4)</font></Sup></SPAN ><font size="2"><SPAN style="font-family:'Verdana'; color:#000000; font-style:normal; font-weight:normal" >. Más tarde, Naka y col </SPAN ></font>  <SPAN style="font-family:'Verdana'; font-style:normal; font-weight:normal; color:#000000" ><Sup><font size="2">(7)</font></Sup></SPAN ><font size="2"><SPAN style="font-family:'Verdana'; color:#000000; font-style:normal; font-weight:normal" >, propusieron el término EMCH después de encontrar una variedad de elementos mesenquimales malignos en un tumor retroperitoneal mixto con ganglioneuroblastoma.</SPAN ></font></SPAN ></p>     <p ALIGN="justify"> <SPAN style="font-family:'Verdana'; color:#000000; font-style:normal; font-weight:normal" ><font size="2">Clínicamente significativas los síntomasson poco comunes y generalmente han sido relacionados con la presión local del tumor. Exámenes de laboratorio en general, no pueden ayudar salvo en los casos en que los componentes de los neuroblastos estén presentes, los datos de laboratorio muestran elevación de ácido vanilmandélico en orina</font><Sup><font size="2">(8,9)</font></Sup></SPAN ><SPAN style="font-family:'Verdana'; color:#003F80; font-style:normal; font-weight:normal" ><font size="2">.</font></SPAN ></p>     <p ALIGN="justify"> <SPAN style="font-family:'Verdana'; color:#000000; font-style:normal; font-weight:normal" ><font size="2">Ectomesenquimomas con frecuencia se confunde con rabdomiosarcomas debido al hecho que sus componentes neuronales sonfácilmente pasados por alto.
