El grupo de estudio estuvo constituido por 62 embarazadas eutróficas, sanas, con edades entre 16 y 37 años, evaluadas en el Centro de Atención Nutricional Infantil "Antímano", en el lapso comprendido entre octubre de 1.998 y agosto de 2.000, a quienes se les practicó una evaluación bioquímica trimestral que incluyó: hematología completa, hierro sérico, ferritina, porcentaje de saturación de transferrina, capacidad de unión del hierro a la transferrina, glucosa, nitrógeno ureico, creatinina, ácido úrico, calcio, fósforo, fosfatasa alcalina, magnesio, triglicéridos, colesterol, proteínas totales, albúmina y relación albúmina/globulina. Se calcularon los descriptivos (promedio, mediana y desviación estándar) y los cambios intertrimestrales y totales de dichas variables. Se aplicó "t" de Student (p-valor<0.05). Los cambios intertrimestrales de los parámetros hematológicos y bioquímicos resultaron en una disminución global significativa excepto para glóbulos blancos, volumen corpuscular medio, capacidad de unión del hierro a la ferritina, ácido úrico, triglicéridos, colesterol y fosfatasa alcalina, los cuales incrementaron sus valores. En la mujer gestante con estado nutricional adecuado se producen variaciones fisiológicas de los parámetros hematológicos y bioquímicos, de allí la necesidad de utilizar valores específicos en este grupo vulnerable en la práctica clínica diaria.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[The object was to determine the serum levels of the hematological and biochemical variables in the there trimesters of gestation and to analize the changes of these variables during gestation. The study group was formed by 62 pregnant women, well nourished, healthy and between the ages of 16 and 37, evaluated at the Centro de Atención Nutricional Infantil Antímano (CANIA), between October 1998 and August 2000. A hematological profile, serum iron, ferritin, transferrin saturation (%), iron-binding capacity, glucose, ureic nitrogen, creatinine, uric acid, calcium, phosphorus, alkaline phosphatase, magnesium, tryglicerides, cholesterol, total protein, albumin, and the protein/albumin rate were obtained a from each one of the pregnant women in each pregnancy trimester. The descriptive statistics (mean, median and standard deviation) of the biochemical variables of the three trimesters of gestation, the intertrimestrial and total changes were obtaineda. "t" Student was aplied (p-value < 0.05). The intertrimestrial changes of the hematological and biochemical profiles resulted in a significant global decrease except for the white blood cells, mean corpuscular volume, total iron-binding capacity, uric acid, trygliceride, cholesterol and phosphatase alkaline whose values increased. <body><![CDATA[<p ALIGN="JUSTIFY">En la mujer gestante se producen importantes mecanismos de adaptación metabólicos y fisiológicos poco tiempo después de la concepción que continúan durante todo el embarazo (1,2). Los cambios fisiológicos sanguíneo se producen como adecuación del organismo de la mujer a las mayores exigencias derivadas del desarrollo fetal y uterino, que incluyen la formación de un nuevo órgano esencialmente vascular, la placenta, que consume gran parte del gasto sanguíneo (3,4). Las modificaciones sanguíneas son de tipo físico y químico. Las de tipo físico incluyen un aumento del volumen sanguíneo en un 40%-45%, con ganancia absoluta de plasma, de volumen celular y de hemoglobina (5). Sin embargo, el incremento de estos dos últimos elementos ocurre a menor velocidad que el del plasma por lo que se origina una oligocitemia relativa. La hipervolemia inducida por el embarazo es<i> </i>necesaria para cubrir las demandas del útero gravídico, para proteger a la madre y al feto de los efectos deletéreos de la disminución del retorno en posición supina y erecta y para evitar que la madre padezca los efectos adversos de la pérdida de sangre durante el parto (1). Entre las alteraciones químicas se encuentran los cambios en diversos componentes del sistema de coagulación, modificaciones en las proteínas totales y fraccionadas, en los lípidos plasmáticos, hierro sérico y los componentes del metabolismo del calcio. A pesar de la importancia de conocer los cambios fisiológicos de las variables bioquímicas a lo largo del embarazo para su adecuada interpretación, son pocas las investigaciones publicadas sobre este tópico a nivel internacional; entre ellas se encuentran las siguientes: Knight et al. realizó un estudio de tipo transversal (la muestra varió entre 13 y 143 casos), en gestantes suplementadas con polivitamínicos, a las que se les determinó en cada trimestre de la gestación el valor promedio de hematología completa, ferritina, proteínas totales y fraccionadas, creatinina, nitrógeno ureico (BUN), ácido úrico y calcio (5). Padrón et al. estudiaron 54 mujeres gestantes, sanas, suplementadas con hierro con el objetivo de conocer los cambios del estado nutricional del hierro durante la gestación (6). Tedesco et al. realizaron un estudio longitudinal en gestantes sanas para determinar el comportamiento de la ferritina a lo largo del embarazo (7). Mathias et al. efectuaron un estudio longitudinal de niveles séricos de ácido úrico en 52 gestantes sanas (8). </p>      <p ALIGN="JUSTIFY">Por otra parte, en nuestro país, no existen valores de referencia para las variables hematológicas y bioquímicas durante el embarazo, por lo que los especialistas del área se ven obligados a utilizar los valores de referencia para mujeres no gestantes, teniendo presente los cambios fisiológicos durante la gestación para su interpretación, de allí que se propone una investigación para determinar los valores séricos de las variables hematológicas y bioquímicas en los tres trimestres de la gestación, y analizar sus cambios durante el embarazo en mujeres eutróficas.</p> <b>      <p ALIGN="CENTER">Materiales y métodos</p> </b>      <p ALIGN="JUSTIFY">El grupo de estudio estuvo constituido por 62 embarazadas eutróficas, sanas, con edades entre 16 (todas las adolescentes tenían edad ginecológica mayor de 4 años) y 37 años, evaluadas entre octubre de 1.998 y agosto de 2.000 en el Centro de Atención Nutricional Infantil &quot;Antímano&quot;. Las gestantes tenían control nutricional mensual y fueron suplementadas con 5 mg interdiarios de ácido fólico (menor concentración encontrada en el país) y 30 mg diarios de hierro elemental bajo la forma de sulfato ferroso desde el primer trimestre hasta el final del embarazo y una cápsula de polivitamínico a partir del cuarto mes de la gestación, esquema de suplementación recomendado en gestantes en condiciones nutricionales adecuadas.</p>      <p ALIGN="JUSTIFY">Los criterios de inclusión fueron los siguientes: edad gestacional £ 13 semanas, certeza de la edad gestacional calculada por fecha de última regla validada, embarazos simples y una evaluación hematológica y bioquímica en cada trimestre.</p>      <p ALIGN="JUSTIFY">Las variables hematológicas y bioquímicas y las pruebas de laboratorio utilizadas para su determinación fueron: contaje celular por impedancia, hemoglobina (cianometahemoglo-bina), hierro sérico y capacidad de unión del hierro no saturado a la transferrina (ferrocina), creatinina (picrato alcalino), calcio (o-cresolftaleína), fósforo (molibdato de amonio), magnesio [1azo-2hidroxi-3-(2,4-dimetilcarboxanilido)-naftaleno-1-(2-hidroxibenceno-5-sulfonato)], proteínas totales (iones cúpricos) y albúmina (bromocresol verde aniónico) por métodos colorimétricos; glicemia (hexoquinasa/glucosa 6 fosfato deshidrogenasa), nitrógeno ureico (ureasa glutamato deshidrogenasa) ácido úrico (uricasa/peroxidasa), triglicéridos (L-a-glicerol fosfato oxidasa), colesterol (esterasa/oxidasa/peroxidasa) y fosfatasa alcalina (fosfato p-nitrofenil) por métodos enzimáticos. El equipo utilizado para la hematología fué el Coulter® y para la química el Express Plus®, con control de calidad bianual.</p>      <p ALIGN="JUSTIFY">Las variables bioquímicas fueron categorizadas como normales o alteradas tanto en déficit como en exceso durante la gestación, aplicando los puntos de corte del Instituto de Medicina de Estados Unidos para la hemoglobina y el hematocrito (9), los valores de referencia para mujeres gestantes de Sauberlich para la proteína, la albúmina, el calcio, el fósforo, la fosfatasa alcalina y el magnesio (10), los de Knight para los glóbulos rojos y blancos, los índices hematimétricos, la ferritina, la creatinina y el nitrógeno ureico (BUN) (5) y los de Mathias para el ácido úrico (8). El resto de los parámetros hematológicos (neutrófilos, linfocitos, monocitos, eosinófilos, basófilos y plaquetas) y bioquímicos (hierro sérico, % de transferrina, globulina, glucosa, triglicéridos y colesterol) se compararon con valores de referencia para mujeres no gestantes por no disponerse de valores de referencia publicados para este grupo vulnerable tanto en la literatura nacional como internacional (11). </p>      <p ALIGN="JUSTIFY">A todas las mujeres se les aplicaron pruebas intradérmicas y serológicas, para descartar infección reciente por Toxoplasma, en el Instituto de Medicina Tropical de la Universidad Central de Venezuela y pruebas serológicas para Rubeola y Citomegalovirus en el Instituto Nacional de Higiene &quot;Rafael Rangel&quot;, con el fin de clarificar el estado de salud de las gestantes.</p>      <p ALIGN="JUSTIFY">La evaluación del estado nutricional de la gestante se hizo en base a indicadores clínicos y antropométricos. La clasificación antropométrica del estado nutricional en el primer trimestre se hizo aplicando un software diseñado para tal fin, que utilizó la metodología sistematizada en Venezuela para la evaluación nutricional de la mujer (12). En ese lapso la interpretación de los indicadores tradicionales y de composición corporal se hizo utilizando los estándares antropométricos derivados por Frisancho AR. a partir de los datos del NHANES I y II (13), ya que los nacionales solo incluyen datos hasta los 19 años. En los siguientes trimestres de la gestación para la categorización del estado nutricional en ambos grupos de edad, sólo se consideró el índice de masa corporal (IMC) de acuerdo a las semanas de gestación, utilizando los valores de referencia validados de la gráfica de Atalah et al. (14). Ésto debido a que es el único indicador antropométrico del que se tienen datos para su interpretación a lo largo del embarazo. </p>      <p ALIGN="JUSTIFY">La categoría eutrófica resultó de las siguientes combinaciones:</p>     ]]></body>

(14)</p> </font>      <p ALIGN="JUSTIFY">Para el análisis estadístico se calcularon los descriptivos (promedio, mediana y desviación estándar) trimestrales de las variables y los de sus cambios trimestrales. Se aplicó &quot;t&quot; de Student (p-valor&lt;0.05) para determinar si los cambios trimestrales son significativos. El procesamiento de los datos se realizó con el programa SPSS (Versión 9,0).</p> <b>      <p ALIGN="CENTER">Resultados</p> </b>      <p ALIGN="JUSTIFY">El promedio y la desviación estándar de las variables hematológicas para cada uno de los trimestres del embarazo se observan en el <a HREF="#Cua1"><b>Cuadro 1</b></a>. El valor promedio de hemoglobina (Hb) y hematocrito (Hto) en los tres trimestres de la gestación se encontró dentro de los rangos de normalidad para una mujer gestante. Es de destacar los valores bajos de la concentración de hemoglobina corpuscular media (CHCM) y de la hemoglobina corpuscular media (HCM) a lo largo de todo el embarazo respecto a mujeres gestantes (5). En relación a las variaciones trimestrales de los parámetros hematológicos, los cambios significativos fueron tanto por disminución como por incremento entre el primero y segundo trimestre y entre el segundo y tercero, con un resultado global de disminución significativo para GR, plaquetas, Hb, hematocrito y CHCM, y de incremento para glóbulos blancos (GB) y volumen corpuscular medio (VCM) <a HREF="/img/fbpe/avn/v15n1/art3cu2.htm"><b>(Cuadro 2)</b></a>. </p> <b>

<p ALIGN="JUSTIFY">El promedio y desviación estándar de la química sanguínea para cada uno de los trimestres del embarazo se observa en el<a HREF="#Cua3"><b> Cuadro 3</b></a>. Es de señalar los valores séricos bajos de creatinina, nitrógeno ureico y calcio en los tres trimestres de la gestación en relación a los valores de referencia de una mujer gestante (5,10). El resto de los parámetros bioquímicos que se compararon con dichos valores de referencia, se encontraron dentro de los rangos de normalidad en los tres trimestres analizados (10). Los siguientes valores estuvieron por encima de la normalidad para una mujer no embarazada: colesterol en el segundo y tercero y triglicéridos, sólo en el último. Respecto a las variaciones trimestrales de las variables bioquímicas, la mayoría de los cambios fueron por disminución significativa entre el primer y segundo trimestre y por incremento entre el segundo y tercero, con un resultado global de disminución para todas las variables excepto TIBC, ácido úrico, triglicéridos, colesterol y fosfatasa alcalina que incrementaron sus valores <a HREF="/img/fbpe/avn/v15n1/art3cu4.htm"><b>(Cuadro 4)</b></a>.</p> <b>

<body><![CDATA[<p ALIGN="JUSTIFY">Los hallazgos en los parámetros hematológicos: disminución global significativa de los glóbulos rojos y de la CHCM, se explica por el aumento del volumen plasmático durante el embarazo (15) aunado a los cambios fisiológicos de la serie roja (16) y a las limitaciones en la disponibilidad de los depósitos de hierro por los altos requerimientos asociados con la gestación (17). </p>      <p ALIGN="JUSTIFY">La disminución significativa de hemoglobina y hematocrito del primero al segundo trimestre y su incremento significativo del segundo al tercero coincide con lo reportado por Scholl et al. (17) y el Comité de Prevención, Detención y Manejo de la Anemia por Deficiencia de Hierro en la Mujer Embarazada (9), lo cual es reflejo de los cambios fisiológicos de estas variables en un embarazo normal (16). </p>      <p ALIGN="JUSTIFY">El descenso significativo del hierro sérico, ferritina y porcentaje de saturación de transferrina del primero al segundo trimestre, se debe a la expansión del volumen plasmático, lo que provoca dilución de la ferritina plasmática total sin cambios reales del hierro de depósito (18). La disminución progresiva del hierro sérico coincide con lo reportado por Padrón et al. (6) y la de la ferritina con Tedesco et al. (7).</p>      <p ALIGN="JUSTIFY">Los valores promedio de glóbulos blancos y rojos en los tres trimestres en esta investigación, fueron superiores a los reportados en mujeres gestantes descendientes de afro-americanos de una población urbana de los Estados Unidos (5). Las variaciones significativas en el contaje total de glóbulos blancos y la fórmula leucocitaria coincide con lo señalado por Cunningham (1) y se explica por la leucocitosis característica del embarazo normal, a expensas de los neutrófilos con disminución de los linfocitos y eosinófilos (3). Las variaciones observadas en el número de plaquetas entre el primero y segundo trimestre del embarazo fueron similares a lo señalado por otros autores (19,20). Las mismas se deben a diferentes factores como son un aumento del consumo de plaquetas durante el embarazo normal (20), una vida media más corta y una disminución de su tamaño (1).</p>      <p ALIGN="JUSTIFY">El valor promedio de creatinina y nitrógeno ureico en los tres trimestres de la gestación fue inferior al reportado en el estudio de mujeres gestantes afro-americanas (5), lo cual pudiera ser explicado por diferencias en el diseño de ambas investigaciones (longitudinal versus transversal) y al desconocimiento del estado nutricional de las embarazadas del otro grupo de estudio. La disminución significativa de los niveles séricos para ambas variables bioquímicas, desde el inicio de la gestación, producto del aumento del índice de filtración glomerular (1,5), coincide con lo señalado por Kalhan (4). Esta respuesta adaptativa del metabolismo del nitrógeno durante la gestación pudiera ser consecuencia de los depósitos tisulares incrementados por parte de la madre en las etapas iniciales y de ésta y el recién nacido en las finales (4).</p>      <p ALIGN="JUSTIFY">El incremento progresivo de los niveles de ácido úrico en esta investigación, coincide con lo reportado por Mathias et al. (8) y Knight et al. (5) en otros grupos de gestantes, aunque en ambas investigaciones los valores promedio fueron superiores en los tres trimestres.</p>      <p ALIGN="JUSTIFY">El descenso significativo de la glicemia encontrado en esta investigación era de esperarse, ya que la embarazada sana presenta un ligero grado de hipoglicemia; sin embargo, el descenso del primero al tercer trimestre de la gestación (3,97 mg/dL), fue bastante menor que el reportado por Butte N. (10 - 15 mg/dL) (21).</p>      <p ALIGN="JUSTIFY">El valor promedio de triglicéridos y colesterol en el tercer trimestre del embarazo por encima del límite superior de la normalidad para una mujer no gestante son aceptables para una mujer embarazada (15). Durante la gestación se desarrolla una hiperlipidemia fisiológica, caracterizada por un incremento de todos los componentes lipídicos del suero, siendo la fracción de triglicéridos la que muestra el mayor incremento proporcional: 300% según King (2), 200% según Garza et al. (22) y 50% según Russel et al. (23) respecto a los valores séricos normales para una mujer no embarazada. El aumento de las concentraciones de colesterol es sólo del 25% (2,23). El porcentaje de incremento de triglicéridos en este estudio (247%) estuvo entre los rangos de King (2) y Garza (22), pero el de colesterol se encontró muy por encima (84%) de los señalados por Russel (23). La hipertrigliceridemia pareciera ser producto de las concentraciones elevadas de estrógenos y de la resistencia a la insulina (21). </p>      <p ALIGN="JUSTIFY">El incremento de la fosfatasa alcalina en el último trimestre del embarazo, se debe en gran parte a la elevación de isoenzimas de ésta a nivel de la placenta (1,23). Los valores promedio de calcio en los tres trimestres de la gestación se encontraron por debajo de los señalados por Knight et al. en mujeres gestantes (5). Este resultado unido a los niveles bajos de calcio según los valores de referencia de Sauberlich, hace pensar en un estado deficiente de este nutriente en este grupo de gestante a pesar de tratarse de mujeres eutróficas desde el punto de vista clínico y antropométrico, por lo que se deberían realizar investigaciones que incluyan variables de consumo e indicadores del metabolsimo del calcio para validar estos resultados. Es de hacer notar que el descenso significativo del calcio difiere de lo reportado por Cross et al. (24) y coincide con lo reportado por Arikan et al (25), quienes encontraron un descenso significativo de los niveles séricos de este mineral a partir de la semana 18 de la gestación, lo cual está condicionado por los mecanismos que explican la homeostasis de este mineral durante el embarazo (15,24,26-28). Las variaciones en el fósforo sérico coinciden con lo reportado por Cunningham (1).</p>      <p ALIGN="JUSTIFY">En relación a los cambios del magnesio observados en este estudio fueron similares a las reportadas por Arikan et al. (25) y difieren de lo señalado por el Instituto de Medicina de los Estados Unidos, Subcomité del Estado Nutricional y Ganancia de Peso de la Embarazada quienes encontraron elevación discreta en etapas tempranas del embarazo, retornando a niveles basales al final del mismo (29). </p>      ]]></body>
<body><![CDATA[<p ALIGN="JUSTIFY">Los valores promedio de proteína en el primer y segundo trimestre de la gestación fueron similares a los reportados por Knight et al (5) en su grupo de gestantes; sin embargo, el del tercero fue inferior al reportado por dicho autor. En relación al metabolismo de las proteínas, en las embarazadas se ha observado que desde etapas tempranas hasta el momento del parto se produce una disminución progresiva de la concentración de proteínas (4) que se acompaña de un descenso de la albúmina y un incremento de las globulinas, con una menor relación albúmina/globulina (1,3,23). Los cambios significativos encontrados en las proteínas totales, albúmina y relación albúmina/globulina en este estudio, son producto de las adaptaciones fisiológicas en el metabolismo proteico durante el embarazo (4). La albúmina es una proteína transportadora de muchos nutrientes, por lo que la marcada disminución de los niveles séricos desde el inicio de la gestación pudiera explicarse por el descenso de las concentraciones de nutrientes en la circulación (2).</p>      <p ALIGN="JUSTIFY">Estos resultados permiten concluir que en la mujer gestante con estado nutricional adecuado se producen variaciones fisiológicas de los parámetros hematológicos y bioquímicos que deben ser tomados en cuenta para la interpretación de los mismos, siendo en general una práctica inadecuada compararlos con los valores normales de mujeres no embarazadas. Adicionalmente, la diferencia entre los resultados de esta investigación y los valores internacionales de embarazadas señalados antes, amerita la validación de estos datos (los cuales constituyen el primer esfuerzo nacional para establecer dichos parámetros en gestantes).
