<self-uri xlink:href="http://ve.scielo.org/scielo.php?script=sci_arttext&amp;pid=S0798-22592007000100005&amp;lng=en&amp;nrm=iso"></self-uri><self-uri xlink:href="http://ve.scielo.org/scielo.php?script=sci_abstract&amp;pid=S0798-22592007000100005&amp;lng=en&amp;nrm=iso"></self-uri><self-uri xlink:href="http://ve.scielo.org/scielo.php?script=sci_pdf&amp;pid=S0798-22592007000100005&amp;lng=en&amp;nrm=iso"></self-uri><abstract abstract-type="short" xml:lang="es"><p><![CDATA[Con el fin de conocer el patrón de susceptibilidad antimicrobial en la población equina activa, se extrajeron treinta y ocho muestras de lavados traqueo-bronquiales trans-endoscópico de equinos alojados en el hipódromo Nacional de Santa Rita ubicado en municipio Santa Rita del estado Zulia, Venezuela; de ambos sexos, adultos, sin lesiones y sin evidencia clínica de ninguna enfermedad respiratoria, seleccionados al azar; dichas secreciones obtenidas fueron remitidas al laboratorio bacteriológico para su cultivo y antibiograma, donde se probaron 25 antimicrobianos diferentes, para pruebas de susceptibilidad antimicrobiana. En todos los ejemplares muestreados se aisló al menos una bacteria aeróbica. Hubo aislamientos mixtos de dos o tres bacterias, lo más común fueron los aislamientos dobles (68,42%); de las especies bacterianas aisladas con mayor frecuencia la Pseudomona aeruginosa 28 veces (73,68%), Escherichia coli 13 veces (32,21%) y Klebsiella pneumoniae 10 veces (26,32%). Se realizaron tablas de frecuencias para la proporción de susceptibilidad de las bacterias respiratorias a los agentes antimicrobiales, con el procedimiento FREQ para el cálculo de frecuencias del SAS. Se determinó que la P. aeruginosa y la E. coli fueron sensibles a 11 y resistentes a 14 antimicrobianos de los 25 antimicrobianos probados in vitro; la K. pneumoniae fue sensible a 19 antimicrobianos y solo resistente a 6 de los 25 antimicrobianos probados in vitro. La P. aeruginosa y la E. coli fueron sensibles a dos de los tres aminoglicósidos probados, en cambio la K. pneumoniae fue sensible a los tres aminoglicósidos. La mayoría de las cepas de estas bacterias aeróbicas conseguidas en este hipódromo, fueron resistentes a los b-lactámicos, combinaciones de ellos o cefalosporinas (de primera y segunda generación)]]></p></abstract>

In all the units samples at least one aerobic bacterium was isolated. <body><![CDATA[<p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2"><B><FONT COLOR="000000">Key words: </FONT></B><FONT COLOR="000000" SIZE="2">Horses, infection, susceptibility, antimicrobial, bal washing, bacterial  isolation. </FONT></font>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font size="2"><b><font COLOR="000000" face="Verdana">Recibido: </font></b><font COLOR="000000" face="Verdana">16 / 06 / 2005.&nbsp; <b>Aceptado:</b> 11 / 11 / 2006.</font></font><font face="Verdana"> </font>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><B><FONT COLOR="000000" size="2" face="Verdana"> INTRODUCCIÓN </FONT></B></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> En la práctica rutinaria equina, a nivel de los hipódromos, el veterinario  se enfrenta con una innumerable cantidad de infecciones sub-clínicas en  el tracto respiratorio de caballos estabulados, aparentemente sanos, y  que al ser sometidos al estrés del transporte o ejercicio, es cuando se  manifiestan clínicamente este tipo de infecciones. En las condiciones de  estos establecimientos, dichas infecciones son comúnmente provocadas por  <I>Enterobacterias</I> contaminantes del ambiente de los establos y caballerizas,  que logran acceder al tracto respiratorio debido al hacinamiento, mal manejo  y mala higiene [18]. El único tratamiento profiláctico y/o curativo es  el tratamiento con antimicrobianos, pero la gran mayoría de las veces estas  infecciones son refractarias a ellos, debido a la resistencia desarrollada  por los microorganismos, debiéndose recurrir a la aspiración de las secreciones  traqueo-bronquiales, su cultivo y antibiograma, para determinar los patrones  de sensibilidad <I>in vitro</I> de las bacterias aisladas en ellas, y aplicar  los medicamentos antimicrobianos más certeros, para así garantizar el éxito  de la terapia, con las menores complicaciones y periodo de convalecencia  [18]. </FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> Las razones para tratar lo más tempranamente posible estas infecciones  sub-clínicas, es la de evitar peores secuelas, que pueden causar, desde  intolerancia al ejercicio hasta pleuroneumonías, y además, las importantes  perdidas económicas que dichas infecciones acarrean en la industria equina,  por costos en medicamentos y cuidados veterinarios [2,18,19]. </FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> La falla clínica en la terapia antimicrobiana tiene numerosas causas y  puede no estar relacionado a la resistencia a los antibióticos; sin embargo,  la resistencia es una de las razones de esta falla y puede ser controlada.  Los clínicos pueden implementar medidas para reducir el potencial de resistencia  por selección, cuando diseñan y planifican el tratamiento, y por ende minimizan  la presión de selección que creará y perpetuará la resistencia. Sin embargo,  el clínico debe reconocer que la remoción de la presión de selección puede  no dar resultado en la disminución de la resistencia. En el antibiograma,  las zonas de inhibición son medidas de comparación de los microgramos de  medicamento que son necesarios para inhibir el crecimiento de un microorganismo  [27]. </FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> Las bacterias aeróbicas más comúnmente relacionadas con la pleuroneumonía  equina serian el <I>Estreptococo spp </I>  </FONT> <I> <FONT COLOR="000000" size="2" face="Verdana"> b-hemolítico, Pasteurella spp, Actinobacillus  spp, Escherichia coli y Klebsiella pneumoniae</FONT></I><FONT COLOR="000000" size="2" face="Verdana">. Se han aislado también en  secreciones obtenidas por lavado traqueo-bronquial trans-endoscópico de  caballos asintomáticos: <I>Pseudomona aeruginosa (7,68%); Escherichia coli  (32,31%) y Klebsiella pneumoniae (26,32%),</I> de las cuales en su mayoría  fueron infecciones dobles (68,42%) [18]. La mayoría de los caballos tienen cultivos mixtos, con bacteria aeróbicas y anaeróbicas. Los anaerobios más  comunes son <I>Bacteroides fragilis y Clostridium spp</I>., aunque puede existir  una amplia variedad de otros anaeróbicos en caballos [21, 25,26].</FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> Los aminoglicósidos (gentamicina, kanamicina, amikacina) son particularmente  indicados para el tratamiento de infecciones sistémicas causadas por bacilos  Gram negativos aeróbicos, los cuales incluyen a las <I>Enterobacterias</I> [2]. </FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> Muchos organismos Gram negativos, incluyendo las <I>Pseudomonas</I>, son  intrínsicamente resistentes a los macrólidos y ciertos antimicrobianos b-lactámicos porque las  drogas son muy hidrofóbicas para difundir a través de la membrana bacterial  externa. Mientras que las drogas típicamente no causan mutación genética, su  presencia crea una presión de selección en los mutantes. La actual forma de  resistencia adquirida ha sido identificada en cada gen patógeno bacterial como  también en numerosos tipos de flora comensal [12].</FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> A pesar de los diferentes tipos de mecanismos por los cuales los microorganismos  adquieren resistencia antimicrobiana, solo hay unas pocas categorías de  mecanismos básicos de resistencia. El primer mecanismo por el cual la resistencia  puede ocurrir es por la exclusión de los antimicrobianos del ambiente interno  de las bacterias. El blanco de un antimicrobiano puede permanecer inactivo  si este no alcanza las concentraciones apropiadas dentro del microorganismo.  El segundo mecanismo general de resistencia a los antimicrobianos involucra  la inactivación del componente antimicrobiano por el mismo. El tercer mecanismo<B>  </B>de resistencia involucra la alteración del sitio de acción del antimicrobiano,  lo cual a menudo es a partir de los mecanismos de replicación celular.  No todos los mecanismos de resistencia pueden alcanzar una significancia  clínica o resistencia detectable [12]. </FONT></P>      ]]></body>
<body><![CDATA[<P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> Otro método para impedir la alta concentración de un antimicrobiano intracelularmente,  es removiendo el antimicrobiano a una tasa que excede su captación. El  mejor estudio que muestra este ejemplo es la bomba de eflujo de las tetraciclinas.  Los genes de plásmidos codifican las proteínas de la membrana que actúa  como un transportador dependiente de energía<B> </B>[10, 22]. El mecanismo exacto  por el cual estas proteínas bombean las tetraciclinas hacia afuera de la  célula no es conocido. Se ha reportado también un mecanismo activo de eflujo  que bombea hacia afuera de las bacterias Gram negativas los antimicrobianos  hidrofílicos y las fluoroquinolonas<B> </B>[1]. </FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> Otro mecanismo de resistencia usado por las bacterias, es aquel que involucra la  inactivación de sustancias antibióticas similares. Se han sido identificados los  plasmidos que producen enzimas modificadoras de los antimicrobianos y que  confieren resistencia a los aminoglicósidos y al cloranfenicol. Esas enzimas  hacen a los antimicrobianos inactivos por acetilación o fosforilización  selectiva respectivamente. La gama de antimicrobianos más ampliamente  inactivados son los b-lactámicos, por un tipo de  enzima que hidroliza el núcleo de los antimicrobianos, las b-lactamasas, tales como las penicilinas y las  cefalosporinas<B> </B>[16]. Las b-lactamasas son producidas  por numerosas bacterias Gram negativas, algunas Gram positivas y algunos  anaerobios como los bacteroides<B>  </B>[11,20]. Los genes de la gran mayoría de las b-lactamasas son codificados  por los plasmidos. Los plasmidos productores de b-lactamasas fueron  identificados por primera vez en los años sesenta y ahora son considerados  propios de las bacterias Gram negativas [16].</FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> Otro ejemplo de alteración en el sitio de acción de los antimicrobianos,  incluye las mutaciones en el ADN girasa, el cual es el sitio de acción  primario de las fluoroquinolonas. Han sido desarrollados otros mecanismos  de protección ribosomal para conferir resistencia a las tetraciclinas y  macrólidos, los cuales dependen de la conjugación a los ribosomas de las  bacterias, para inhibir la síntesis proteica<B> </B>[20, 22]. </FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> Una cefalosporina como el ceftiofur sódico demostró tener una alta actividad  contra los patógenos comunes bacterianos del tracto respiratorio del equino;  la dosis utilizada fue la de 2,2 mgrs/kgrs una vez al día [8, 9, 10]. </FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> El uso de la enrofloxacina en potros se ha desestimado primariamente por  las artropatías que ella induce y que ha sido demostrado en animales inmaduros  tales como ratas, perros, conejos, y cerdos de guinea; sin embargo, no  hay reportes de tales artropatías en potros [2]. </FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> La enrofloxacina es un antimicrobiano que inhibe la ADN girasa, la enzima  que es necesaria para formar la súper-helicoide del ADN bacterial. Su espectro  de actividad lo hace un antimicrobial potencialmente útil para los potros,  debido a su actividad bactericida contra varios aeróbicos Gram negativos  (Enterobacterias<I>, Actinobacillus spp, Pasteurella spp, y Pseudomona aeruginosa</I>)  y su moderada actividad contra varios aerobios Gram positivos, (<I>Staphylococcus  spp, Rhodococcus equi y Micobacterium spp</I>) Adicionalmente alcanza grandes  concentraciones intracelulares y tisulares [3]. </FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> El uso de la enrofloxacina en caballos no ha sido aprobado. Reportes de  la farmacocinética de la enrofloxacina (2,5-5 mgrs/ kgrs oral o intravenoso  cada 12 horas), han mostrado altas concentraciones en el plasma, liquido  sinovial, orina, endometrio; y bajas concentraciones en el peritoneo y  liquido cerebro-espinal [8]. </FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> El objetivo de este estudio fue el de conocer el patrón de susceptibilidad  de los microorganismos aislados de las secreciones respiratorias de los  animales asintomáticos (sin evidencia clínica de infección respiratorias,  es decir, sin moco, fiebre o secreciones a la auscultación), activos y  adultos de la población de caballos de este hipódromo. </FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><B><FONT COLOR="000000" size="2" face="Verdana"> MATERIALES Y MÉTODOS </FONT></B></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> El presente trabajo se realizó con caballos pura sangre de carreras (P.S.C.)  alojados en el Hipódromo Nacional de Santa Rita, ubicado en el municipio  Santa Rita, al noreste del estado Zulia, Venezuela. Esta zona geográfica  es una planicie correspondiente al bosque seco tropical a nivel del mar,  con una pluviosidad anual de 844 mm, temperatura promedio anual de 27,9°C  y una humedad relativa de 76%<SUP>1</SUP>. </FONT></P>      ]]></body>
<body><![CDATA[<P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> En dicho hipódromo existían para el momento del estudio una población total  de aproximadamente 515<SUP>2</SUP> caballos, con edades comprendidas entre 18 meses  y 7 años de edad, de los cuales para el 21 de marzo de 2002, aproximadamente  35 estaban enfermos, 128 eran potros y 108 estaban lesionados, quedando  una población restante de 244 que fue la población total de interés, dado  que cumplió con los criterios de este estudio, es decir eran adultos, activos  y sin evidencia clínica de enfermedad respiratoria. Los caballos se encontraban  alojados en compartimientos individuales, de aproximadamente 12 metros  cuadrados de superficie, fueron alimentados con concentrados y heno dos  veces al día, se les administró agua “<I>ad-libitum</I>”; fueron entrenados todos  los días durante las mañanas entre 06:00 y 10:00 A.M.; las carreras se  realizan una vez por semana en la noche. </FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> Dado a que la naturaleza de este trabajo fue descriptiva, se decidió tomar  una muestra de cuarenta caballos. De los cuales dos fueron descartados,  uno posterior al examen clínico por presentar signos de enfermedad y otro,  por no mostrar ningún tipo de crecimiento bacteriano en el laboratorio,  por lo que la muestra definitiva fue de treinta ocho (38) equinos, de ambos  sexos y con edades comprendidas entre los 3 y los 7 años, se seleccionaron  al azar, 21 machos (55,26%) y 17 hembras (44,74%), practicándosele una  cuidadosa evaluación clínica a cada uno de ellos según lo indicado por  Beech y Cook [4,5]. </FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> La toma de la muestra de las secreciones traqueo-bronqueales se realizó  según Dixon [6], utilizando un Endoscopio marca Olympus, modelo GIF- XP20  (Japón), y catéteres marca Olympus modelo PW-2L-1 (Japón); debidamente  estériles. Se procedió como sigue: </FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> La parte termino-distal del catéter trans-endoscopio fue obstruida con  agar estéril de un 1cm de profundidad. </FONT></P>      <blockquote>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2"><FONT COLOR="000000"> 1) </FONT><FONT COLOR="000000" SIZE="2"> En algunos caballos se requirió un método de sujeción (axial labial). </FONT></font></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2"><FONT COLOR="000000"> 2) </FONT><FONT COLOR="000000" SIZE="2"> El catéter se mantuvo dentro de la parte distal del endoscopio. </FONT></font></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2"><FONT COLOR="000000"> 3) </FONT><FONT COLOR="000000" SIZE="2"> El endoscopio fue introducido a través del tracto respiratorio anterior  y una vez alcanzada la región proximal de la traquea, el catéter se hizo  avanzar a través del endoscopio hasta protruir y utilizando una jeringa  estéril llena de aire, fue expelido el tapón de agar, quedando este en  la traquea. </FONT></font></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2"><FONT COLOR="000000"> 4) </FONT><FONT COLOR="000000" SIZE="2"> El endoscopio fue introducido distalmente dentro de la traquea irrigando  las paredes con 60 ml. de solución fisiológica, los cuales posteriormente  son aspirados con jeringa estéril. </FONT></font></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2"><FONT COLOR="000000"> 5) </FONT><FONT COLOR="000000" SIZE="2"> Una vez colectada la mayor cantidad posible de solución fisiológica irrigada,  la muestra se comparte: una muestra para citología en un tubo vacutainer  estéril con EDTA y la otra muestra para cultivo y antibiograma en un medio  de transporte. </FONT></font></P>  </blockquote>      ]]></body>
<body><![CDATA[<P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana">  Las muestras de secreciones para cultivo fueron remitidas el mismo día  en que se tomaron al laboratorio bacteriológico Tierra Negra<SUP>3</SUP>; donde fueron  sembradas usando los métodos rutinarios de bacteriología<SUP>4</SUP>, en los medios  agar Sangre Humana, agar Sangre de Cordero, agar Base GC (con suplemento  vitamínico isovitalex<SUP>5</SUP>), agar McConkey, y agar Sabouraud; las placas sembradas  fueron leídas a las 24; 48, y 72 horas hasta que hubo crecimiento. Casi  todas crecieron en 48 horas, donde se leyeron para estimar el crecimiento  en Unidades Formadoras de Colonias (U.F.C /ml). <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> Se utilizó el método reglamentado y actualmente recomendado por el Subcomité  de Pruebas de Susceptibilidad Antimicrobiana, estandarizadas por el National  Committee Clinical Laboratory Standards<SUP>6</SUP> (NCCLS) que se basa en el método  original descrito por Bauer y col<SUP>7</SUP>. El procedimiento, siguiendo los estándares  de la NCCLS fue el siguiente: de un cultivo de agar sangre humana en placa,  se seleccionaron al menos de tres a cinco colonias, bien aisladas, las  cuales a través de un asa estéril se transfirieron a un tubo con caldo  tripiticasa soya, el cual fue incubado a 35°C hasta alcanzar el patrón  de turbidez 0,5 de McFarland<SUP>8</SUP>, correspondiente a 1 a 2 por 10 unidades  formadoras de colonias por ml (UFC/ml), luego fue sembrado en agar Mueller-Hinton,  donde se colocaron los discos de los antimicrobianos en las placas. </FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> Fueron elaboradas Tablas de frecuencias para la proporción de susceptibilidad  de las bacterias respiratorias a los agentes antimicrobiales, con el procedimiento  FREQ para el cálculo de frecuencias del SAS [23]. </FONT></P>     <p ALIGN="justify" style="word-spacing: 0; line-height: 100%"><font size="2"><B><FONT COLOR="000000" face="Verdana"> RESULTADOS </FONT></B><FONT COLOR="000000" face="Verdana"> <B> Y DISCUSIÓN</B> </FONT></font>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> De las 38 muestras obtenidas a través de los lavados traqueo-bronquiales  trans-endoscópicos, de los caballos con infecciones subclínicas, se determinó  que la bacteria con mayor frecuencia fue la <I>Pseudomona aeruginosa</I> con 28  aislamientos (73,68%), seguida de la <I>Escherichia coli</I> con 13 (32,31%) y  por ultimo la <I>Klebsiella pneumoniae</I> con 10 (26,32%) (<a href="#tab1">TABLA I</a> y <a href="#tab2">II</a>). </FONT></P>      <p ALIGN="left" style="word-spacing: 0; line-height: 100%"><font face="Verdana"><a name="tab2"></a> </font></p>      <p ALIGN="center" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2"><b><font COLOR="000000">TABLA II. </font></b><font COLOR="000000">BACTERIAS AISLADAS CON MAYOR FRECUENCIA DE LOS LAVADOS TRAQUEO-BRONQUIALES EN CABALLOS PURA SANGRE DE CARRERA DEL HIPÓDROMO NACIONAL DE SANTA RITA DE ACUERDO A SU PARTICIPACIÓN DE MANERA COMBINADA O NO CON LAS OTRAS BACTERIAS / THE MOST FREQUENCY BACTERIAL ISOLATED FROM TRAQUEO-BRONCHIAL WASHINGS IN THOROUGHBRED HORSES FROM SANTA RITA NATIONAL RACE TRACK, ACCORDING ITS PARTICIPATING MIXED OR NOT WITH OTHERS BACTERIAS</font></font></p>     ]]></body>

Para las penicilinas-b-lactámicos, las combinaciones  de inhibidores b-lactamasas/b lactámicos y cefalosporinas mostró: ceftriaxona,  cefotaxime (96,43%), piperacilina (92,86%), piperacilina-tazobactan (85,71%),  cefoperazona (71,42%). Se observó que dos de los tres aminoglicósidos  probados fueron 100% efectivos contra la <I>P.aeruginosa</I> y que el otro restante,  tobramicina, tuvo un 85,71% de sensibilidad. Se halló una tendencia de  sensibilidad a las cefalosporinas de tercera generación como lo fueron:  ceftriaxona y cefotaxime (96,43%), cefoperazona (71,42%); penicilinas y b-lactámicos como también: piperacilina (92,86%) y piperacilina/ tazobactan  (85,71%), y para otros antimicrobianos como polimixina B (100%), colimicina  (96,43%), imipenem (85,71%). Los resultados de resistencia fueron: ampicilina,  trimetoprim-sulfa y tetraciclina (100%), ampicilina/sulbactan (96,43%),  amoxicilina /ácido clavulánico (78,57%), cefalotina y cefamandole (89,29%),  estas dos últimas, cefalosporina de segunda generación, no trabajaron bien <I>in vitro</I> contra la <I>P. aeruginosa </I>(<a href="#tab3">TABLA III</a>; <a href="#fig1">FIG. 1</a>)<I>;</I> coincidiendo con  la resistencia encontrada por Lavoie y col. [15], de 96% a la sulfa-trimetoprin;  ampicilina y cefalotina (92%), cloranfenicol (87%) y tetraciclina (83,34%)  confirmando esta tendencia de resistencia a otros antimicrobianos distintos  de los aminoglicósidos y las cefalosporinas de tercera generación (<a href="#tab6">TABLA&nbsp;VI</a>, <a href="#fig2">  FIG.2</a>). Estos resultados coinciden con los encontrados por Ensink y col. [7] donde la <I>Pseudomona</I> apareció resistente a muchos antimicrobianos, y  que sólo 6 de 7 aislamientos fueron susceptibles a la gentamicina. El hallazgo de que la <I>P aeruginosa</I> es resistente a la combinación ampicilina/sulbactan  es coincidente con los resultados de Hoffman y col. [13], Labia y col. [13], quienes también  encontraron que sus aislamientos fueron sensibles un 87% para la gentamicina,  73% para la kanamicina, 62% para la neomicina, 61% para la ampicilina;  con Lavoie y col. [15]<B> </B>quienes encontraron 84% de susceptibilidad para  la gentamicina; Sweeney y col. [24] que encontraron frecuencias de susceptibilidad  del 100% para la amikacina, 76,9% para la gentamicina, frecuencias de resistencia  de 84,6% para la ampicilina y cefalotina, y de 85,7% para la penicilina.  Coincidiendo estos dos últimos autores con las frecuencias de susceptibilidad  encontradas en el presente estudio para la <I>P. aeruginosa</I>. En un estudio  realizado en la Universidad de Montreal entre los años 1986-1988 [15],  las bacterias aisladas de varios fluidos corporales mostraron sensibilidad  a la gentamicina que estuvo en el rango del 45 al 95,6%, siendo para la  <I>Pseudomona spp</I> del 84%, lo cual coincide con detectado en este estudio,  y ratifica que la gentamicina aún es efectiva contra esta bacteria. </FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> La <I>E. coli </I>solo fue sensible<I> </I>a 11 de los 25 discos de diferentes antimicrobianos.  Esta bacteria mostró un patrón de sensibilidad muy parecido al de la <I>Pseudomona  aeruginosa</I>, ya que fue también susceptible<I> </I>en un 100% a dos de los tres  aminoglicósidos probados, amikacina y gentamicina, y 61,53% a la tobramicina <a href="#tab4">  (TABLA IV</a>, <a href="#fig3"> FIG. 3</a>), coincidiendo con Prescott y Yielding [19]<B> </B> quienes encontraron frecuencias de susceptibilidad del 98% para la gentamicina,  y con Lavoie y col. [15], quienes encontraron también a su vez susceptibilidad  del 87,23%, para la gentamicina. De igual modo fue susceptible a cuatro  antimicrobianos del grupo de las penicilinas-b-lactámicos,  combinaciones de inhibidores b-lactamasas/b lactámicos y cefalosporinas, como lo fueron: cefalotina,  ceftriaxona (cefalosporina de tercera generación), piperacilina/tazobactan  en 100% (en 9 casos probados de 13 casos aislados de <I>E.coli</I>), que coincide  con el 77% de susceptibilidad encontrado para la cefalotina por Prescott  y Yielding [19]. Fue susceptible a la tetraciclina (69,92%), trimetoprim-sulfa  (76,92%) y cloranfenicol (100%); que coinciden por lo reportado por Lavoie  y col. [15] quienes encontraron 83% para el trimetoprim-sulfa y 70% para  el cloranfenicol. Esta bacteria fue resistente a la ampicilina y carbenicilina en un 46,51% (<a href="#tab7">TABLA VII</a>, <a href="#fig4"> FIG.4</a>), consistente con el porcentaje de resistencia  encontrado del 68% por Lavoie y col. <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"> <font face="Verdana" size="2" COLOR="000000">En el caso de la <I>K. pneumoniae</I>, esta bacteria mostró la mayores frecuencias  de susceptibilidad, a 19 de los 25 discos de antimicrobianos probados en  las tres bacterias aisladas, con respecto a los aminoglicósidos fue 100%  susceptible a los tres aminoglicósidos probados (amikacina, gentamicina  y tobramicina) (<a href="#tab5">TABLA V</a>, <a href="#fig5"> FIG. 5</a>), que coinciden con lo reportado por Lavoie  y col. [15], Prescott y Yelding [19] 100% para la amikacina y 100% para  la gentamicina respectivamente. Con respecto al grupo de las penicilinas-b-lactámicos,  combinaciones de inhibidores b-lactamasas/b lactámicos y cefalosporinas  fue susceptible en 100% a 11 de los 13 antimicrobianos de este grupo probados,  y en 90% para la cefoperazona y de 80% para la combinación de piperacilina/tazobactan.  Otros como imipenem (100%), tetraciclina (80%), trimetoprim- sulfa (100%),  cloranfenicol (100%) y colimicina (100%) (<a href="#tab8">TABLA VIII</a>,  <a href="#fig6">FIG. 6</a>), que coinciden con Prescott y Yielding [19], 100%  para el trimetoprim-sulfa y 88% para el </font> <font face="Verdana" size="2">cloranfenicol. fueron 100% resistente a la ampicilina y carbenicilina. Sweeney y col. [13], la combinación ampicilina/sulbactan tiene un  efecto sinérgico <I>in vivo</I>, para reducir la extensión y la severidad de las  infecciones de bacterias Gram negativas, inducidas experimentalmente en  potros, que coincide con nuestros hallazgos <I>in vitro</I>, de 100% de sensibilidad  de la <I>K. pneumoniae </I> a la combinación de ampicilina/sulbactan, ya que la  <I>K. pneumoniae</I> ante la ampicilina sola mostró 100% de resistencia, pero  fue 100% sensible ante la combinación ampicilina/sulbactan. Hoffman y col.  [13] consideraron que la <I>k. pneumoniae</I> es patogénica del tracto respiratorio  de los potros y de caballos adultos, la cual es considerada resistente  a la ampicilina, ya que los aislamientos clínicos realizados frecuentemente  mantienen plásmidos que codifican la proliferación de cepas que producen b-lactamasas, pero ellos han observado que la combinación ampilcilina/sulbactan  es bactericida para bacterias Gram positivas y Gram negativas, coincidiendo  en esto último, solo en que <I>K. pneumoniae </I>fue 100% sensible <I>in vitro</I>, pero  esta combinación fue 100% de resistente ante la <I>P. aeruginosa y la E. coli </I>[13].</FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> Para Foreman [8], Magdesian y col. [17] la mayoría de las bacterias aeróbicas  en el caballo son sensibles <I>in vitro</I> e <I>in vivo</I>, a uno o más de los aminoglicósidos.  La gentamicina a la dosis de 6,6 mgrs/ kg IV o IM cada 24 horas, es la  dosis más usada. Recientes trabajos [8] han mostrado que el poder bactericida  de la gentamicina sigue siendo el mismo, debido a su efecto post-antimicrobiano,  y que su toxicidad ha sido minimizada al reemplazar la concentración plasmática  crónica, pero tóxica detectable, con la dosis única diaria, en vez de la  dosis triple diaria anteriormente recomendada para ella, lo cual coincide  con la tendencia de susceptibilidad <I>in vitro</I> ante este antimicrobiano,  encontrada en este estudio, en los microorganismos aislados de los lavados  traqueo-bronquéales de estos caballos. </FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><B><FONT COLOR="000000" size="2" face="Verdana"> CONCLUSIONES </FONT></B></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> Del total de los aislamientos y antibiogramas realizados de las secreciones  obtenidas por lavados traqueo-bronquiales trans-endoscópicos de 38 caballos  P.S.C., en el Hipódromo Nacional de Santa Rita, se detectó que 28 de ellos  desarrollaron una infección sub-clínica por <I>P.&nbsp;aeruginosa</I> (73,68%), seguida  por la <I>E. coli</I> 13 aislamientos (32,31%), y de <I>K. pneumoniae</I> 10 aislamientos  (26,32%). </FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> La <I>P. aeruginosa, la E. coli y la K. pneumoniae</I> fueron sensibles a los  tres aminoglicósidos, 100% para la gentamicina y amikacina; y en un 85,71%;  65,53% y 100% a la tobramicina respectivamente. La <I>P. aeruginosa y E. coli</I>  fueron resistentes a 14 antimicrobianos y solo sensibles a 11 de ellos. </FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> La <I>P.&nbsp;aeruginosa y E. coli</I> fueron resistentes: penicilinas originales o  semi-sintéticas (como ampicilina incluso carbenicilina), b-lactámicos,  combinaciones de ellos, cefalosporina, mácrolidos y sulfas Las cepas de  la <I>K. pneumoniae</I> mostraron mayor sensibilidad a los antimicrobianos probados <I>in vitro</I>, ya que fue sensible 19 de los 25 antimicrobianos probados, y  solo resistente en 100% a la ampicilina y carbenicilina, pero cuando la  ampicilina fue combinada con el sulbactan, no hubo resistencia, ya que  fue 100% sensible a esta combinación.</FONT></P>      <P ALIGN="justify" style="word-spacing: 0; line-height: 100%"><FONT COLOR="000000" size="2" face="Verdana"> La <I>P. aeruginosa</I> y la <I>E. coli</I> fueron las bacterias más resistentes, y por  tanto la mas difíciles de tratar; la <I>K. pneumoniae</I> fue la bacteria para  la cual hubo más opciones de antimicrobianos. Las sensibilidades en un  100% de la <I>P. aeruginosa, E. coli y K. pneumoniae </I>ante la gentamicina y  amikacina,<I> </I>demuestran que ellos aún tienen utilidad práctica en la  antibio-terapia de animales que padecen de infecciones clínicas o sub-clínicas,  como es el caso de los caballos de este hipódromo.
