Anticonceptivos orales</span></b></span></p>     <p align="center"><span class="font0" style="font-weight:700"> <font face="Verdana" size="2">Dr Manuel Cabrera, Dra. Indira Centeno Asistente: Dr. Nilfran Nottola</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">El hiperandrogenismo, una de las características cardinales del síndrome de ovario poliquístico(SOP), es directamente responsable de muchos síntomas clínicos y alteraciones metabólicas. Existe evidencia de un circulo vicioso entre insulino resistencia y exceso de andrógenos, que puede perpetuar las anormalidades metabólicas, reproductivas y endocrinas de estas pacientes<sup>(1,2)</sup>.</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">Por lo anterior, la atenuación del hiperandrogenismo, es de importancia fundamental en la terapia de esta condición; no sólo para controlar las manifestaciones clínicas, sino también para intervenir en la probable etiopatogenia del síndrome. De allí, que una de las opciones terapéuticas especificas sea el uso de los anticonceptivos orales (ACOs), porque estos juegan un papel crucial en el manejo de la anovulación crónica y sus posibles complicaciones (trastornos menstruales, hiperplasia endometrial, carcinoma endometrial, etc.), así como las consecuencias clínicas del hiperandrogenismo (hirsutismo, acné, seborrea y alopecia).</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">Los ACOs actúan de varias formas en el control del hiperandrogenismo.</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">El componente estrogénico estimula la producción hepática de globulina transportadora de esteroides sexuales (SHBG), lo cual conlleva a una disminución de la testosterona libre</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">Las progestinas disminuyen la liberación</font></span><span class="font2" style="font-weight:bold;"><font face="Verdana" size="2"> </font> </span><span class="font2"><font face="Verdana" size="2">hipotalámica de hormona liberadora de gonado-tropinas (GnRH) y la secreción hiperreactiva de LH, lo que da lugar a una disminución de la síntesis ovárica de andrógenos<sup>(3)</sup>. Adicional-mente, ciertas progestinas con actividad anti-androgénica demostrada, actúan directamente en la célula blanco (en mayor o menor grado de acuerdo a la potencia intrínseca), y se unen de forma competitiva a los receptores androgé-nicos<sup>(4,5)</sup> inhibiendo la 5 alfa reductasa en piel<sup>(6)</sup>.</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">Finalmente, la combinación de estrógenos y progestágenos en los ACOs, suprime la secreción de gonadotropinas hipofisarias, lo que da lugar a una reducción de la producción de andrógenos. De igual manera, se ha observado una reducción en la secreción de andró-genos adrenales, producto de una disminución plasmática de ACTH<sup>(7)</sup>.</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">Los ACOs constituyen el tratamiento de primera línea para el hirsutismo. La elección del ACO es importante, debido a que muchas progestinas poseen efectos androgénicos variables. Las 4 progestinas con propiedades antiandrogénicas son, en orden de mayor a menor potencia: acetato de ciproterona (ACP), dienogest (DNG), drospirenona (DRSP) y acetato de clormadinona (ACM)<sup>(5,8)</sup>. Éstas ejercen su efecto, por varios mecanismos: • Se unen al receptor de dihidrotetosterona (DHT) en el citoplasma del folículo piloso y así impiden su llegada al núcleo.</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">•&nbsp;Inhiben la 5 alfa reductasa, con reducción de la producción de DHT.</font></span></p>     ]]></body>
<body><![CDATA[<p align="justify"><span class="font2"><font face="Verdana" size="2">•&nbsp;Inhiben la producción de gonadotropinas y reducen la esteroidogénesis.</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">ACP solo y en combinación con EE ha sido considerado por más de tres décadas como tratamiento de primera línea en hiperandrogenismo<sup>(9)</sup>.</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">Clormadinona desarrollado como progestina en la década de los 60, ha sido utilizada en los últimos años en combinación con EE como anticonceptivo; al evaluar su efecto antiandro-génico en comparación con la combinación EE/ LNG no se encontraron cambios mayores en los niveles de andrógenos, pero se reportó una diferencia significativa en la mejoría clínica del acné<sup>(10)</sup>.</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">Dienogest progestina híbrida desarrollada en la década de los 90, exhibe actividad antiandrogénica inferior a ACP, pero superior al resto de las progestinas antiandrogénicas conocidas. Su combinación con EE sólo ha sido utilizada en Europa, no existe mayor experiencia publicada<sup>(11)</sup>.</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">Drospirenona la más recientemente desarrollada, es un derivado de la 17 </font> </span><span class="font6"><font face="Verdana">&#945;</font></span><span class="font2"><font face="Verdana" size="2"> espironolactona, además de su acción antiandrogénica, es la única con actividad antimineralocorticoide a dosis clínica<sup>(12)</sup>.</font></span></p>     <p align="justify"><span class="font2"><font size="2" face="Verdana">Los ACOs de baja dosis (menos de 50 &#8734;g de EE) son tan efectivos en la disminución de la testosterona libre, como los de altas dosis</font><font size="2"><sup><font face="Verdana">(13,14)</font></sup></font><font size="2" face="Verdana"><sub> </sub>y producen una respuesta clínica similar; incluso los que contienen levonorgestrel (LNG), derivado de la 19 nor-testosterona, con ciertas propiedades androgénicas<sup>(14,15)</sup>, pero a baja dosis.</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">Formulaciones de ACOs con desogestrel, gestodeno y norgestimato, están asociadas a un incremento mayor en SHBG y una disminución significativa en la testosterona libre; por lo que. teóricamente, esos productos, deberían ser más efectivos en el tratamiento del hirsu-tismo. Sin embargo, un estudio comparativo</font></span><span class="font2" style="font-weight:bold;"><font face="Verdana" size="2"> </font> </span><span class="font2"><font face="Verdana" size="2">entre dos ACOs de segunda y tercera generación (levonorgestrel vs. desogestrel) no evidenció diferencias clínicas en cuanto a la gradación de hirsutismo entre ellos<sup>(16)</sup>.</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">La combinación EE/DRSP demostró, en un estudio piloto, beneficios en los signos clínicos y bioquímicos del hiperandrogenismo gracias a su actividad antiandrogénica, sin evidencia de deterioro significativo en los parámetros meta-bólicos, lo que pudo deberse a su actividad antimineralocorticoide<sup>(17)</sup>.</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">Todos los ACOs. a través de su mecanismo de acción, conllevan a una respuesta clínica similar en el tiempo<sup>(3)</sup>. A pesar de que los ACOs. no han sido aprobados por la FDA para el tratamiento del hirsutismo. un gran número de estudios observacionales, no aleatorizados, han reportado una mejoría del mismo<sup>(18)</sup>.</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">Por otro lado, acné y seborrea representan otras de las manifestaciones clínicas comunes del hiperandrogenismo, pues la producción de sebo está bajo control hormonal de los andró-genos. sea cual sea su origen. La utilización de ACOs que contienen progestina tipo norgestimato, desogestrel, gestodeno, acetato de ciproterona, acetato de clormadinona y drospirenona han demostrado en múltiples trabajos su efecto positivo sobre el desarrollo y la respuesta clínica al tratamiento con estos<sup>(19</sup>-<sup>23)</sup>. La primera combinación eficaz se obtuvo cuando se asoció un estrógeno y un proges-tágeno con efecto antiandrogénico (etinil estra-diol con acetato de ciproterona)<sup>(24)</sup>.</font></span></p>     ]]></body>
<body><![CDATA[<p align="justify"><span class="font2"><font face="Verdana" size="2">Cuando se comparan diferentes ACOs contra placebo en el control del acné, existen diferencias importantes en cuanto a su eficacia a favor de los ACOs<sup>(25)</sup>. De las progestinas antes mencionadas, la única que ha sido comparada con ACP en relación con la mejoría del acné fue drospirenona, y se demostró efecto similar e incluso superior a la anterior, en pacientes con acné leve y moderado<sup>(26)</sup>.</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">Con base en lo anteriormente expuesto, son claros los beneficios que nos ofrecen los anticonceptivos orales para el manejo de la anovulación crónica y sus consecuencias, asi como para el hlperandrogenismo y sus expresiones clínicas.</font></span></p>     <p align="justify"><font face="Verdana" size="2">Los ACOs de altas dosis<span class="font2"> ( &#8805;50 </span> <span class="font7">&#8734;</span></font><span class="font2"><font face="Verdana" size="2">g</font></span><span class="font7"><font face="Verdana" size="2">)  </font> </span><span class="font2"><font face="Verdana" size="2">incrementan el riesgo de insulino resistencia, por lo que su uso debe evitarse en mujeres anovulatorias con sobrepeso. Sin embargo, ACOs de baja dosis tienen poco efecto sobre el metabolismo do carbohidratos (insulina y glucosa), por lo cual la mayoría de las pacientes hiperinsulinémicas, hiperandrogénicas, responden de manera satisfactoria a estos<sup>(27)</sup>. Estudios de larga data no han demostrado que el uso de ACOs en la población general incremente el riesgo de diabetes mellitus tipo 2, tanto en usuarias anteriores ni actuales<sup>(28,29)</sup>.De Igual manera, no se ha evidenciado un incremento del riesgo de enfermedades cardiovasculares en antiguas usuarias do ACOs<sup>(30,31)</sup>.</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">Se ha evaluado el uso de ACOs de baja dosis en pacientes con diagnóstico reciente de diabetes gestacional, sin demostrarse impacto adverso; en aquellas mujeres con diabetes insulino dependiente, el uso de ACOs de baja dosis no condujo a deterioro del perfil lipídico, de marcadores bioquímicos para enfermedades cardiovasculares, y no se evidenció incremento en retinopatía ni nefropatía<sup>(32,33)</sup>.</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">En pacientes obesas con insulinorresistencia, un estudio prospectivo de 10 años de duración, documentó un deterioro de la tolerancia a la glucosa e hiperinsulinemia en el grupo de pacientes quienes no usaron ACOs, en contraste con una discreta mejoría de dichos parámetros en las usuarias de ACOs<sup>(34)</sup>. Sin embargo, un trabajo de pequeña muestra reportó deterioro discreto de la tolerancia glucosada en este grupo do pacientes<sup>(35)</sup>.</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">En pacientes con hiperinsulinemia y SOP, la respuesta a metformin + flutamida, ha sido mejor en comparación con el uso de ACOs solos. Sin embargo, estudios recientes han sugerido que la respuesta a niveles de adipo-nectina, hiperresistencia vascular ovárica, y adiposidad central en las pacientes usuarias de metformin, y flutamida, no desmejora en usuarias de anticonceptivos que contengan drospirenona<sup>(36-38)</sup>. Más aún, en pacientes con elevación adicional de cifras de aldosterona, la utilización de ACO con efecto antimineralo-corticoide debería considerarse<sup>(39)</sup>.</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">Adicionalmente, un estudio piloto reportó mejoría en pacientes con SOP usuarias de ACO con la adición de simvastatina, tanto en los parámetros clínicos como metabólicos, así como una disminución significativa, en cuanto a factores de riesgo cardiovascular<sup>(40)</sup>.</font></span></p>     <p align="justify"><span class="font3" style="font-weight:bold;"> <font face="Verdana" size="2">CONCLUSIONES</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">•&nbsp;La mejor terapia de mantenimiento para regularizar el ciclo menstrual y evitar las consecuencias do la anovulación crónica, son los ACOs.</font></span></p>     <p align="justify"><span class="font2"><font face="Verdana" size="2">•&nbsp;Los ACOs están indicados en pacientes con hirsutismo, acné moderado y/o severo, seborrea.</font></span></p>     ]]></body>
