Cuando ocurre, particularmente en la región anterior, el tratamiento rehabilitador tiene la función de recuperar la función y la estética de los dientes que fueron dañados por el trauma. El recubrimiento del fragmento, cuando es posible, crea una respuesta emocional positiva en el paciente y simplifica el mantenimiento de la oclusión original. En este caso, los autores exponen el tratamiento de una fractura oblicua de corona-raíz en el incisivo lateral superior con invasión del espacio biológico. Tres años después del tratamiento, el diente mostró función normal, estética favorable y una buena salud periodontal. Podemos concluir que la técnica de recubrimiento del fragmento ofrece la posibilidad de una rehabilitación estética y funcional con características más semejantes a la dentición natural, teniendo siempre que ser considerada como una de las alternativas de tratamiento para dientes fracturados, donde se tenga el fragmento dental]]></p></abstract>

Lo ideal sería que las restauraciones&nbsp; sean tan resistentes como los dientes  naturales e imperceptibles a los&nbsp;&nbsp; pacientes más exigentes.<sup>1, 2</sup></span></font></p>     <p align="justify" style="word-spacing: 0; line-height: 100%"> <font size="2" face="Verdana"><span lang="ES-VE"> En las&nbsp; actividades clínicas diarias se verifica&nbsp; que,  difícilmente, el&nbsp; tratamiento puede ser ejecutado de forma separada. Para una  atención multidisciplinaria, se hace&nbsp; necesario que los profesionales realicen  una rigorosa anamnesis, partiendo del motivo de la consulta y de la&nbsp; historia  del paciente hasta el examen clínico y radiográfico de los tejidos blandos y  duros, identificando situaciones que muchas veces cambian y pasan&nbsp; sin ser  percibidas durante los procedimientos convencionales del examen.</span></font></p>     <p align="justify" style="word-spacing: 0; line-height: 100%"> <font size="2" face="Verdana"><span lang="ES-VE"> Las fracturas dentarias en cualquier región, ya sean estas  coronarias, radicular&nbsp; o mixtas&nbsp; ocurren por varias razones, y son situaciones  comunes en la&nbsp; clínica. <sup>3-6 </sup>Identificarlas exactamente es  extremamente importante para la planificación terapéutica.</span></font></p>     <p align="justify" style="word-spacing: 0; line-height: 100%"> <font size="2" face="Verdana"><span lang="ES-VE"> En el siguiente caso clínico se muestra&nbsp; la recuperación del  espacio&nbsp; biológico perdido, mediante la cirugía de aumento de corona clínica,  como también el recubrimiento del fragmento dental, recuperando de esta manera  la estética y&nbsp; la función&nbsp; dental.</span></font></p>     ]]></body>
<body><![CDATA[<p align="justify" style="word-spacing: 0; line-height: 100%"><b> <font size="2" face="Verdana"><span lang="ES-VE"> INFORMES DE CASOS</span></font></b></p>     <p align="justify" style="word-spacing: 0; line-height: 100%"> <font size="2" face="Verdana"><span lang="ES-VE"> Un paciente, de sexo femenino, con 17 años de edad, fue derivado  por un odontólogo a la Clínica del Curso de Especialización&nbsp; en Periodoncia de  la Facultad de Odontología de Pernambuco, para una evaluación del&nbsp; incisivo  lateral superior izquierdo. Al examen clínico, el diente presentaba alteración  del color, sintomatología dolorosa, presencia&nbsp; de inflamación&nbsp; gingival,  movilidad y una restauración temporaria compatible con la abertura coronaria (<a href="#fig1">Figs.  1</a> e <a href="#fig2">2</a>). El&nbsp; examen radiográfico permitió detectar la  ausencia de tratamiento endodóntico&nbsp; y una fractura mixta (corona / raíz) a  nivel cervical (<a href="#fig3">Fig. 3</a>). Después del sondaje, se constató  la&nbsp; invasión del espacio&nbsp; biológico. De acuerdo con el relato del&nbsp; paciente esta  condición&nbsp; ocurrió durante el&nbsp; proceso masticatorio, después de la&nbsp; abertura del<span style="color: red"> </span>canal radicular por el&nbsp; endodoncista. Después de que fueron  proporcionadas todas estas informaciones y de llegar al&nbsp; diagnóstico de fractura  radicular mixta con compromiso pulpar, partimos para la planificación del&nbsp;  tratamiento. Como, normalmente, en odontología tenemos más de una solución para  una determinada condición clínica, debemos tomar la decisión final en conjunto  con el paciente, escuchando sus necesidades y exigencias, así como también  debemos aclarar las ventajas, desventajas y el pronóstico de cada tratamiento,  siempre teniendo en consideración la situación financiera del paciente. En el&nbsp;  referido caso, optamos por el recubrimiento autógeno llevando en consideración  la edad del&nbsp; paciente, la perfecta adaptación del&nbsp; fragmento, como también el&nbsp;  factor emocional positivo de la permanencia del diente natural.</span></font></p>     <p align="center" style="word-spacing: 0; line-height: 100%"><a name="fig1"> <font face="Verdana" size="2"> <img border="0" src="/img/fbpe/aov/v48n1/art24fig1.gif" width="429" height="346"></font></a></p>

<p align="justify" style="word-spacing: 0; line-height: 100%"><b> <font size="2" face="Verdana"><span lang="ES-VE"> ENFOQUE TERAPÉUTICO MULTIDISCIPLINARIO</span></font></b></p>     <p align="justify" style="word-spacing: 0; line-height: 100%"> <font size="2" face="Verdana"><span lang="ES-VE"> Para proceder al recubrimiento del&nbsp; fragmento fue&nbsp; planificada la  incisión de un colgajo muco-perióstico, para la perfecta&nbsp; visualización del&nbsp;  límite de la fractura, y así permitir la reconstitución de las distancias&nbsp;  biológicas, y de la forma transquirúrgica realizar el&nbsp; recubrimiento autógeno  del fragmento y la apertura de la cámara permitiendo al&nbsp; endodoncista la  realización del&nbsp; tratamiento endodóntico.</span></font></p>     <p align="justify" style="word-spacing: 0; line-height: 100%"> <font size="2" face="Verdana"><span lang="ES-VE"> Inicialmente, fue realizado un colgajo conservador con una  incisión&nbsp; intrasurcular (<a href="#fig4">Fig. 4</a>). Después del  desprendimiento del&nbsp; colgajo muco-perióstico y remoción del&nbsp; fragmento dental se  observó el límite de la fractura dental (<a href="#fig5">Figs. 5</a> e <a href="#fig6">6</a>). Posteriormente, fue realizada&nbsp; una osteotomía y  osteoplastia con fresas quirúrgicas, para la &nbsp;recuperación del espacio&nbsp;  biológico (<a href="#fig7">Figs. 7 e 8</a>), se irrigó constantemente &nbsp;con suero  fisiológico durante el&nbsp; acto quirúrgico. El restablecimiento del espacio  biológico fue mantenido con una distancia&nbsp; de 3 mm entre el fin de la  preparación y el margen&nbsp; óseo remanente (<a href="#fig9">Fig. 9</a>).  Concomitantemente al acto quirúrgico, el&nbsp; profesional especialista en  odontología restauradora preparó el fragmento, manteniéndolo&nbsp; hidratado y  removió el material restaurador temporario, así como también los restos  necróticos del órgano pulpar, que estaban promoviendo la alteración del color  del diente. En esta&nbsp; etapa se tuvo&nbsp; el&nbsp; cuidado de no alterar la adaptación  marginal del fragmento (<a href="#fig10">Figs. 10</a>, <a href="#fig11">11 e 12</a>).&nbsp;  A seguir fue realizado el&nbsp; acondicionamiento ácido y la&nbsp; aplicación del &nbsp;sistema  adhesivo simplificado (Adper Single Bond; 3M / ESPE), tanto en el<span style="color: red">&nbsp; </span>fragmento como en el remanente dental, teniendo el cuidado de<span style="color: red"> </span>no foto- activar el&nbsp; agente de unión&nbsp; con el objetivo de no&nbsp; alterar la  adaptación marginal (<a href="#fig13">Figs. 13</a> a <a href="#fig16">16</a>).  Posteriormente con el auxilio de una resina flow, el&nbsp; fragmento fue adaptado y  se realizó la&nbsp; fotoactivación conjunto resina/agente de unión, reconstituyendo  la integridad dental (<a href="#fig17">Figura 17</a>). En seguida el  recubrimiento del&nbsp; fragmento fue realizado con la&nbsp; abertura coronaria para  facilitar el tratamiento endodóntico posterior (<a href="#fig18">Fig. 18</a>).  La&nbsp; fase transquirúrgica&nbsp; fue concluida con la sutura del colgajo muco-perióstico  (<a href="#fig19">Fig. 19</a>).</span></font></p>     <p align="center" style="word-spacing: 0; line-height: 100%"><a name="fig4"> <font face="Verdana" size="2"> <img border="0" src="/img/fbpe/aov/v48n1/art24fig4.gif" width="415" height="320"></font></a></p>

<p align="justify" style="word-spacing: 0; line-height: 100%"><b> <font size="2" face="Verdana"><span lang="ES-VE"> DISCUSIÓN</span></font></b></p>     <p align="justify" style="word-spacing: 0; line-height: 100%"> <font face="Verdana" size="2"> <span lang="ES-VE" style="font-weight: normal"> El pronóstico de un caso siempre dependerá del correcto  diagnóstico del caso clínico así como también del adecuado plan de tratamiento a  ser ejecutado&nbsp; y, principalmente, de la&nbsp; cooperación del&nbsp; paciente. </span><sup> <span lang="ES-VE" style="font-weight: normal"> 3-6</span></sup></font></p>     <p align="justify" style="word-spacing: 0; line-height: 100%"> <font face="Verdana" size="2"> <span lang="ES-VE" style="font-weight: normal"> El acto de elegir la forma adecuada del tratamiento en  odontología se torna un poco subjetivo, llevando en consideración que &nbsp;cuando  tratamos a un paciente estamos delante&nbsp; de una serie de variables inherentes a  la&nbsp; salud&nbsp; física-emocional del mismo. </span> <span style="font-family: Verdana; font-weight: normal">El&nbsp;  factor emocional se torna una preocupación todavía mayor, cuando el&nbsp; problema  compromete la estética en dientes anteriores. En estos casos, solamente el&nbsp;  restablecimiento&nbsp; de la función&nbsp; dental no satisface las necesidades del  paciente, y esto tiene un factor preponderante en la elección&nbsp; del&nbsp; tratamiento.  Sin embargo, el&nbsp; factor económico puede ser un limitante en esta fase de la  elección. Por eso, normalmente, el tratamiento a </span> <span lang="ES-VE" style="font-family: Verdana; font-weight: normal"> elegir</span><span style="font-family: Verdana; font-weight: normal">  en casos de fractura dental es el recubrimiento del fragmento. </span> <sup><span lang="ES-VE" style="font-weight: normal"> 2-13</span></sup></font></p>     <p align="justify" style="word-spacing: 0; line-height: 100%"> <font face="Verdana" size="2"> <span lang="ES-VE" style="font-weight: normal"> Para tener éxito en la ejecución de la técnica del recubrimiento&nbsp;  primero se debe evaluar la viabilidad del fragmento. </span> <span style="font-family: Verdana; font-weight: normal">Esa  viabilidad va a depender del&nbsp; grado&nbsp; de hidratación y adaptación&nbsp; marginal&nbsp; del  remanente dental. </span><sup> <span lang="ES-VE" style="font-weight: normal"> 5, 7- 8,14- 15</span></sup></font></p>     <p align="justify" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2"><span lang="ES-VE"> Otra preocupación sería evaluar la vitalidad pulpar.</span><b><span lang="ES-VE" style="font-family: Verdana"> </span></b><span lang="ES-VE"> Todo esfuerzo debe ser realizado para mantener la vitalidad  pulpar en los casos de fractura dental, <sup>11</sup> particularmente en  pacientes jóvenes, donde los ápices radiculares no fueron&nbsp; completamente  formados. <sup>12, 16</sup></span></font></p>     <p align="justify" style="word-spacing: 0; line-height: 100%"> <font face="Verdana" size="2"> <span lang="ES-VE" style="font-weight: normal"> Fracturas que involucran el esmalte, dentina, y pulpa representan  del 4% al 16% de todas las molestias traumáticas de la dentición&nbsp; permanente. </span><sup> <span lang="ES-VE" style="font-weight: normal"> 17-20</span></sup></font></p>     <p align="justify" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2"><span lang="ES-VE"> Además de evaluar la vitalidad pulpar, es extremamente importante  observar se hubo invasión del&nbsp; &quot;espacio biológico&quot; por la línea de fractura  dental. Se la línea&nbsp; de fractura es&nbsp; supragingival, el procedimiento de  recubrimiento&nbsp; será directo. </span>No obstante, cuando el local de la fractura es&nbsp; subgingival o  infra-óseo, procedimientos reparadores pueden&nbsp; ser necesarios. <span lang="ES-VE" style="font-family: Verdana">Gargiulo et al <sup>21</sup> determinaron las&nbsp; dimensiones&nbsp; fisiológicas para la unión diente-  encía en humanos. Ellos reportaron los siguientes valores medios: surco  gingival, 0.69mm; epitelio de unión, 1.07mm; y tejido conectivo, 0.97mm. En  este&nbsp;&nbsp; estudio, el concepto de &quot;espacio biológico&quot; fue establecido. Tal et al <sup>22</sup> consideraron el espacio biológico como la suma de la longitud del  tejido epitelial y<b> </b></span> <span lang="ES-VE" style="font-weight: normal"> conectivo. De esta forma la comprensión de los aspectos&nbsp;  histológicos de la unión&nbsp; diente-encía&nbsp; parece ser obligatoria cuando se llevan  a cabo técnicas de recubrimiento son ejecutadas y la fractura se extiende sub  gingivalmente&nbsp; .<sup>10</sup> <sup>&nbsp;</sup>En estos casos, procedimientos como,  elevación del&nbsp; tejido de la encía, gingivectomía para acceso del local de  fractura, cirugía de aumento de corona clínica con remoción del hueso&nbsp; alveolar,  tracción ortodóntica o cirugía de la porción&nbsp; apical son&nbsp; procedimientos que  pueden ser utilizados para viabilizar el&nbsp; éxito del recubrimiento&nbsp; del&nbsp;  fragmento dental.</span></font></p>     <p align="justify" style="word-spacing: 0; line-height: 100%"> <font size="2" face="Verdana"> <span lang="ES-VE" style="font-weight: normal"> Considerando toda esa complejidad para determinar un correcto  plan de tratamiento es de extrema importancia que estos factores sean evaluados  de forma multidisciplinar, ofreciendo al&nbsp; paciente, opciones&nbsp; que conduzcan al  restablecimiento&nbsp; de todas sus&nbsp; funciones.</span></font></p>     ]]></body>
<body><![CDATA[<p align="justify" style="word-spacing: 0; line-height: 100%"> <font size="2" face="Verdana"><span style="font-weight: 700"> CONCLUSIÓN</span></font></p>     <p align="justify" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2"><span style="font-weight: normal"> El procedimiento restaurador transquirúrgico no es un&nbsp;  tratamiento ideal desde el punto de vista periodontal. No obstante, en una  situación extrema, comparado con técnicas alternativas, como la colocación de un  implante dental o prótesis dental, la técnica de recubrimiento del fragmento  ofrece varias ventajas. Permite la rehabilitación estética y funcional inmediata  de los dientes&nbsp; fracturados,&nbsp; la cooperación&nbsp; requerida por parte del paciente  es baja, y el tratamiento es&nbsp; concluido en&nbsp; poco tiempo, de esta forma es&nbsp;  fácilmente aceptada por el paciente. En el caso </span> <span lang="ES-VE" style="font-weight: normal"> reportado, el procedimiento transquirúrgico de recubrimiento del&nbsp;  fragmento fue realizado con éxito, mostrando que es factible alcanzar resultados  favorables&nbsp; a corto plazo con este tipo de procedimiento.
