O objetivo do trabalho foi avaliar o efeito da ingestão diária de um novo produto de soja, fermentado com Enterococcus faecium e Lactobacillus jugurti sobre os níveis de lípides séricos de indivíduos normocolesterolêmicos, adultos e do sexo masculino. O estudo foi randomizado, duplo cego e controlado por placebo, com duração de 6 semanas. Quarenta e quatro voluntários, com idade entre 40 e 55, anos foram aleatoriamente separados em 2 grupos: um que recebeu diariamente 200 ml do produto fermentado (grupo F) e o outro que recebeu 200 ml de placebo (grupo P). Amostras de sangue foram retiradas no início do estudo e depois de 3 e 6 semanas e analisadas quanto aos níveis de colesterol total, HDL-colesterol e triglicérides. A concentração do LDL-colesterol foi obtida por cálculo. Os resultados mostraram que nenhuma alteração significativa foi observada nos níveis de colesterol total, LDL-colesterol e triglicérides do grupo F, durante o período de 6 semanas, enquanto que o nível do HDL-colesterol se mostrou significativamente aumentado (p<=0,05) ao final do mesmo período. No grupo P, o colesterol total e o LDL-colesterol apresentaram-se com um aumento significativo (p<= 0,05) ao final de 6 semanas, em relação aos valores iniciais, mas sem alterações nos níveis de HDL-colesterol e triglicérides. Em conclusão, a ingestão de 200 ml/dia do produto de soja fermentado foi capaz de manter inalterada a concentração de colesterol total e de LDL-colesterol, e de aumentar em 10% o nível do HDL-colesterol, nas condições de estudo.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[SUMMARY. This study was undertaken to verify the effect of a daily intake of a new fermented soy milk produced with Enterococcus faecium and Lactobacillus jugurti on the serum lipid levels in normocholesterolemic middle-aged men. The study was randomized, double-blind and placebo-controlled and was performed for a period of 6 weeks. Forty-four normocholesterolemic healthy, male volunteers, aged 40-55 years old were randomly separated in two groups: The F-group received 200 ml of the fermented product daily and the P-group received 200 ml of placebo (chemically fermented). The blood samples were drawn initially and after 3 and 6 weeks and serum values for total cholesterol, HDL-cholesterol and triglyceride were determined. The LDL-cholesterol value was estimated. No significant changes in the fermented group (F) were observed for total cholesterol, LDL-cholesterol or triglyceride levels, while the HDL-cholesterol level was significantly higher (p<= 0,05) after 6 weeks. The total cholesterol and LDL-cholesterol levels were significantly higer (p<= 0,05) in the placebo group (P), but no changes were found for the HDL-cholestrol and triglyceride levels during the experimental period. Entretanto, a maioria desses estudos foi realizada com o <I>Lactobacillus acidophilus</I>, n&atilde;o havendo muita informa&ccedil;&atilde;o sobre outras bact&eacute;rias que poderiam apresentar efeitos similares.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Dentro desse contexto, Rossi et al (7) realizaram um estudo com o <I>Enterococcus faecium</I> e demonstraram a capacidade <I>in vitro </I>desse microrganismo de reduzir em aproximadamente 54% o colesterol adicionado ao meio de cultivo. Apoiados nesses resultados, foi desenvolvido um produto de soja fermentado com o <I>E. faecium</I> associado ao <I>Lactobacillus jugurti</I>, o qual apresentou propriedades tecnol&oacute;gicas e sensoriais semelhantes aos fermentados tradicionais (8).  O efeito hipocolesterol&ecirc;mico desse produto foi verificado em coelhos  machos, hipercolesterol&ecirc;micos e  com idade de 8 semanas. Os resultados mostraram que uma dose de 10 ml/dia, administrada durante 30 dias, foi capaz de reduzir o n&iacute;vel de colesterol total em 18,4% e de aumentar a fra&ccedil;&atilde;o HDL em 17,8% (9).  Considerando a import&acirc;ncia de se conhecer o comportamento deste produto em humanos, o presente estudo foi conduzido com o objetivo de avaliar o efeito decorrente da sua ingest&atilde;o di&aacute;ria, sobre os n&iacute;veis de l&iacute;pides s&eacute;ricos de indiv&iacute;duos normocolesterol&ecirc;micos, adultos e do sexo masculino.</font></P> <B>     <P ALIGN="left"><font size="3" face="Times New Roman">MATERIAL E M&Eacute;TODOS</font></P>     <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Material</font></B></P>     <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Constitu&iacute;do por amostras de "iogurte" de soja fermentado com <I>Enterococcus faecium</I> e <I>Lactobacillus jugurti</I> e de placebo (produto n&atilde;o fermentado), ambos produzidos na Unidade de Produ&ccedil;&atilde;o e Desenvolvimento de Derivados de Soja -UNISOJA da FCF-UNESP-Araraquara.</font></P> <B>    ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Grupo de estudo</font></P> </B>    <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Constitu&iacute;do por 44 pessoas volunt&aacute;rias do sexo masculino, com idade entre 40 e 55 anos, que apresentavam n&iacute;veis normais de colesterol total, HDL-colesterol, triglic&eacute;rides, glicose, alanina-aminotransferase (ALT), aspartato-aminotransferase (AST) e creatininina. Todos apresentavam press&atilde;o arterial abaixo de 130/85 mm Hg, n&atilde;o possu&iacute;am hist&oacute;ricos de doen&ccedil;as cardiovasculares, cerebrovasculares ou metab&oacute;licas. Os volunt&aacute;rios foram devidamente instru&iacute;dos quanto ao tipo de estudo, objetivos, conduta individual, medidas de seguran&ccedil;a e considera&ccedil;&otilde;es sobre riscos<SUP>*</SUP>.</font></P> <B>    <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Planejamento experimental</font></P> </B>    <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">O estudo foi duplo cego, randomizado, controlado por placebo e teve uma dura&ccedil;&atilde;o de 6 semanas.</font></P>     <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Os indiv&iacute;duos foram aleatoriamente divididos em 2 grupos, cada um com 22 pessoas. Um grupo consumiu diariamente 200 ml do produto fermentado (grupo F), enquanto o outro consumiu diariamente 200 ml de placebo (grupo P). Todos foram orientados para que mantivesse constante o hor&aacute;rio de ingest&atilde;o do produto ao longo do experimento.</font></P>     <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Amostras de sangue foram coletadas no in&iacute;cio do estudo (determina&ccedil;&atilde;o inicial) e no final da 3<SUP>a</SUP> e 6<SUP>a</SUP> semanas, sempre no per&iacute;odo da manh&atilde;, ap&oacute;s um jejum de no m&iacute;nimo 12 horas.</font></P> <B>    <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Obten&ccedil;&atilde;o do produto fermentado</font></P> </B>    <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">O produto foi processado na Unidade de Desenvolvimento e Produ&ccedil;&atilde;o de Derivados de soja -UNISOJA do Depto. de Alimentos e Nutri&ccedil;&atilde;o da FCF/UNESP, a cada 7 dias, de acordo com a metodologia descrita por Rossi et al (10), tendo a seguinte constitui&ccedil;&atilde;o b&aacute;sica: extrato aquoso de soja (&quot;leite&quot; de soja) acrescido de 8% de sacarose; 1,0% de &oacute;leo de soja; 1% de lactose e 0,5% de gelatina. Como in&oacute;culo foi utilizado 1,5% (v/v) de cultivo<I> E. faecium</I> CRL 183 e 1,5% (v/v) de cultivo de <I>L. jugurti</I> 416. O produto depois de fermentado apresentou a seguinte composi&ccedil;&atilde;o qu&iacute;mica: 3,39% de prote&iacute;na; 2,74% de l&iacute;pides; 12,05% de carboidratos; 0,47% de cinzas e 81,32% de umidade, fornecendo 864,2 Kcal/litro.</font></P>     <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Cada lote de produto foi submetido a uma quantifica&ccedil;&atilde;o de c&eacute;lulas vi&aacute;veis, empregando-se meios de cultivos espec&iacute;ficos: M17 &aacute;gar e MRS &aacute;gar. As col&ocirc;nias formadas foram contadas e tiveram suas caracter&iacute;sticas morfol&oacute;gicas observadas.</font></P>     <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">O placebo, de constitui&ccedil;&atilde;o id&ecirc;ntica ao produto fermentado, apenas sem conter os microrganismos em estudo, foi acidificado pela adi&ccedil;&atilde;o direta de &aacute;cido l&aacute;tico de grau aliment&iacute;cio, at&eacute; que fosse alcan&ccedil;ado o pH id&ecirc;ntico ao do produto fermentado.</font></P> <B>    ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Dosagem dos l&iacute;pides s&eacute;ricos</font></P> </B>    <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">As dosagens do colesterol total, HDL-colesterol e triglic&eacute;rides foram realizadas em soros obtidos das amostras de sangue, por meio de "kits" enzim&aacute;ticos espec&iacute;ficos:</font></P>     <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">-Para o colesterol total foi empregado o m&eacute;todo do &quot;colesterol fast-color" (11).</font></P>      <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">-Para o HDL-colesterol, foi necess&aacute;rio primeiramente realizar uma precipita&ccedil;&atilde;o seletiva de lipoprote&iacute;nas, segundo a metodologia de Bergmeyer (12) e, posteriormente, determin&aacute;-lo pelo m&eacute;todo utilizado para o colesterol total.</font></P>      <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">-Os triglic&eacute;rides foram determinados pelo m&eacute;todo do "triglic&eacute;rides fast-color" (13).</font></P>      <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Quanto ao LDL-colesterol, as concentra&ccedil;&otilde;es foram calculadas por meio da equa&ccedil;&atilde;o de Friedwald (14).</font></P> <B>    <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">An&aacute;lise estat&iacute;stica</font></P> </B>    <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Os dados foram submetidos &agrave; an&aacute;lise de vari&acirc;ncia (ANOVA). As compara&ccedil;&otilde;es, intra-grupos, das m&eacute;dias foram feitas pelo teste de Newmaan-Kewls, por meio do programa GB-STAT para MS windows, vers&atilde;o 5.3.</font></P> <B>     <P ALIGN="left"><font size="3" face="Times New Roman">RESULTADOS E DISCUSS&Atilde;O</font></P> </B>     <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Os dados cl&iacute;nicos dos indiv&iacute;duos volunt&aacute;rios, observados no in&iacute;cio do estudo, mostraram que todos apresentavam-se sem defici&ecirc;ncias renais ou hep&aacute;ticas, sem diabetes e n&atilde;o hipertensos, isto &eacute;:  press&atilde;o arterial &#60;  130/85 mm Hg (<a href="#TABELA 1">Tabela 1</a>).</font> </P>     ]]></body>

Esse resultado &eacute; devido, em parte, ao aumento do colesterol total observado para o grupo P.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Para os triglic&eacute;rides, os n&iacute;veis permaneceram sem altera&ccedil;&otilde;es durante as 6 semanas de estudo, tanto para o grupo F como para o grupo P, demonstrando que o produto fermentado n&atilde;o exerceu nenhum efeito sobre esse l&iacute;pide s&eacute;rico.</font></P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Os valores das fra&ccedil;&otilde;es HDL e LDL quando relacionados com o colesterol total ou entre si, permitem o estabelecimento do grau de risco coronariano (14). Com base nos valores de refer&ecirc;ncia foi poss&iacute;vel calcular o grau de risco coronariano para os dois grupos em estudo. Pode-se notar na <a href="#FIGURA 1"> Figura 1</a> que para o grupo F, no in&iacute;cio do estudo, 32% dos indiv&iacute;duos apresentavam um grau de risco moderado (RM) e ao final do estudo esse percentual caiu para 14%. Consequentemente o percentual de indiv&iacute;duos com risco padr&atilde;o (RP) ou baixo (RB) aumentou de 68% para 86%. Sem d&uacute;vida, o fato da grande maioria dos indiv&iacute;duos do grupo F terem, ao final do estudo, se caracterizado como indiv&iacute;duos de risco coronariano padr&atilde;o ou baixo se deve ao aumento dos n&iacute;veis da fra&ccedil;&atilde;o HDL e a manuten&ccedil;&atilde;o dos n&iacute;veis de LDL e de colesterol total.</font></P>     <P ALIGN="JUSTIFY"></P>     <P ALIGN="CENTER"><font face="Times New Roman" size="3"><a name="FIGURA 1">FIGURA 1</a></font></P>     <P ALIGN="CENTER"><font face="Times New Roman" size="3">Distribui&ccedil;&atilde;o percentual dos indiv&iacute;duos em estudo em fun&ccedil;&atilde;o do grau de risco coronariano e do tempo de experimenta&ccedil;&atilde;o</font></P>     <P ALIGN="JUSTIFY"></P>     <P ALIGN="center"><img border="0" src="/img/fbpe/alan/v53n1/art07img01.jpg" width="434" height="365"></P>

<body><![CDATA[<P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Em trabalho semelhante realizado por Agerbaek et al (16), os autores, utilizando tamb&eacute;m <I>E.faecium</I> e com um planejamento experimental de 6 semanas, encontraram que o produto fermentado levou a uma redu&ccedil;&atilde;o nos n&iacute;veis de colesterol total, enquanto que o placebo fez com que esses valores permanecessem inalterados. Entretanto, &eacute; v&aacute;lido salientar que esses autores utilizaram o <I>S.thermophilus</I> como microrganismo associado ao <I>E. faecium </I>e o leite de vaca como mat&eacute;ria-prima, que segundo alguns pesquisadores, apresenta por si s&oacute; um &quot;fator de redu&ccedil;&atilde;o do colesterol&quot; (17-19). Esse estudo foi refeito por outros pesquisadores, apenas aumentando o tempo de observa&ccedil;&atilde;o de 6 semanas para 6 meses. Os autores conclu&iacute;ram que a redu&ccedil;&atilde;o da fra&ccedil;&atilde;o LDL no grupo que consumia o produto fermentado, s&oacute; era verificada at&eacute; o primeiro m&ecirc;s de estudo. A partir disso, os n&iacute;veis dessa fra&ccedil;&atilde;o passaram a apresentar-se sem diferen&ccedil;a significativa para os dois grupos em estudo (20).</font></P>     <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Desde a publica&ccedil;&atilde;o do trabalho pioneiro de Mann &amp; Spoerry (21), demonstrando o efeito hipocolesterol&ecirc;mico de produtos fermentados com <I>L. acidophilus</I>, muitos outros trabalhos nesta mesma linha t&ecirc;m sido apresentados e com eles um grande n&uacute;mero de mecanismos foram sugeridos, sem, entretanto, nenhum ter sido comprovado at&eacute; o presente momento.</font> </P>     <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Sendo assim, torna-se muito dif&iacute;cil tentar propor alguma explica&ccedil;&atilde;o vi&aacute;vel para os efeitos observados nesse estudo, principalmente pelo fato do efeito hipocolesterol&ecirc;mico n&atilde;o ter sido explicitamente evidenciado, mas sim de uma forma indireta (impedindo o aumento do colesterol total no grupo F em rela&ccedil;&atilde;o ao grupo P). Por outro lado, o efeito positivo do aumento da fra&ccedil;&atilde;o HDL permite supor uma prov&aacute;vel a&ccedil;&atilde;o do produto fermentado ao n&iacute;vel da s&iacute;ntese do colesterol, n&atilde;o se aplicando neste caso em particular, nenhum dos mecanismos baseados na desconjuga&ccedil;&atilde;o de sais biliares pelos microrganismos do produto, condi&ccedil;&atilde;o que ocasionaria uma diminui&ccedil;&atilde;o na absor&ccedil;&atilde;o do colesterol ex&oacute;geno (3, 22, 5). Entretanto, os resultados desse estudo, permitem afirmar que os efeitos observados para o grupo F s&atilde;o, sem d&uacute;vida, dependentes da presen&ccedil;a dos microrganismos no produto fermentado, cuja a&ccedil;&atilde;o poderia ser uma decorr&ecirc;ncia direta dessa presen&ccedil;a ou ent&atilde;o devido a algum metab&oacute;lito produzido durante o processo fermentativo.</font></P>     <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Por outro lado, &eacute; sabido que a soja apresenta componentes isoflav&ocirc;nicos  que desempenham papel importante na redu&ccedil;&atilde;o do colesterol (23, 24). Entretanto, n&atilde;o se pode atribuir a elas, nem mesmo de forma parcial, os efeitos verificados no grupo F, uma vez que os mesmos efeitos n&atilde;o foram observados no grupo P. Isso pode ser explicado provavelmente pelo fato das isoflavonas terem sido eliminadas, em grande parte, no decorrer do processamento do "leite" (25) que se constitui  na mat&eacute;ria-prima b&aacute;sica do produto fermentado em estudo.</font></P> <B>    <P ALIGN="left"><font size="3" face="Times New Roman">CONCLUS&Otilde;ES</font></P> </B>    <P ALIGN="JUSTIFY"><font size="3" face="Times New Roman">Em conclus&atilde;o, o consumo de 200 ml/dia do produto fermentado com <I>E. faecium </I>e <I>L. jugurt</I>i, por um per&iacute;odo de 6 semanas, mostrou-se capaz de impedir um aumento do colesterol total e da fra&ccedil;&atilde;o LDL e de propiciar uma eleva&ccedil;&atilde;o em cerca de 10% na fra&ccedil;&atilde;o HDL. Essa situa&ccedil;&atilde;o permitiu que 86% dos indiv&iacute;duos apresentassem grau de risco coronariano padr&atilde;o ou baixo ao final do estudo, evidenciando que o produto pode conferir um &quot;fator de prote&ccedil;&atilde;o&quot; a doen&ccedil;as ateroscler&oacute;ticas.
