Con el objeto de examinar los componentes antropométricos, hematológicos y dietéticos para identificar el perfil de riesgo nutricional en embarazadas adolescentes de bajo nivel socioeconómico, se estudiaron 215 jóvenes (16,9 ± 0,7 años), (rango 13 -18 años) en su primer trimestre de gestación, en la Maternidad del Sur de Valencia, Venezuela. Se obtuvieron datos socioeconómicos, antropométricos: peso pregestacional, peso actual, talla actual, circunferencia de brazo (CB), pliegue tricipital (PT), área grasa (AG), área muscular (AM) e índice de masa corporal pregestacional (IMCP); bioquímicos: hemoglobina y ferritina; y dietéticos (2 recordatorios de 24 horas). Según la edad se clasificaron en 2 grupos: grupo 1 (13 a 15) y grupo 2 (16 a 18 años). Para evaluar el perfíl de riesgo nutricional se consideró: Edad ginecológica < 4 años, IMCP <19,8 kg/m², talla < percentil 10 de la referencia, Hb < 11 g/dl y ferritina < 12 my g/l. El 96,3% clasificó en pobreza. 83,3% estaban solteras, un 83,3% cursaba educación básica previo al embarazo y un 84,1% se desempeñaba en oficios del hogar. Según grupos de edad, hubo diferencias significativas (p<0,001) para edad ginecológica y edad de menarquia. Los valores promedios de variables antropométricas, bioquímicas y dietéticas no mostraron diferencias estadísticamente significativas según la edad. En riesgo nutricional se observa que el 35,3 % tenía edad ginecológica menor de 4 años, el 23,3 % presentó talla < percentil 10, el 36,3% mostró un IMCP <19,8 kg/m², el 26,5% tuvo un CB < del percentil 10, el 13,7% mostró Hb < 11 g/dl y el 18,4% de ferritina < 12 my g/l. En el 87,3% se observó un aporte de energía por debajo de la recomendación y la vitamina A y C, el calcio y el cinc se encontraron deficientes en el 36,3%, 25,9%, 88,7% y 73,5% respectivamente. Se observó en el grupo la presencia de factores de alta vulnerabilidad (edad, pobreza, bajo peso, anemia y déficit de energía y nutrientes). <abstract abstract-type="short" xml:lang="en"><p><![CDATA[SUMMARY. Nutritional risk in pregnant adolescents. A sample of 215 adolescents (16.9± 0.7 y; 13-18 y) from a low socioeconomic level of Valencia, Venezuela, at their first trimester of pregnancy were studied in order to identify nutritional risk. Socioeconomic, anthropometrical, hematological and dietary characteristic were assessed. Pregestational weight, actual weight, height, arm circumference (AC), triceps skin fold (TS), fat and muscle area (FA/MA) and pregestational body mass index (PBMI) were determined. Hemoglobin (Hb) and ferritin were measured by colorimetric and ELISA methods. Two 24 hour recalls were obtained. According to age, two groups were created. Pregnant adolescents were grouped by age: Group 1, 13 to15 years old and Group 2, 16 to 18 years old. Nutritional risk was defined as: Gynecological age lower than 4 years since menarche, PBMI < 19.8 kg/m², height below 10th Percentile of reference, Hb below 11 g/dL and ferritin < 12 mu g/L. Almost all the adolescents (96.3%) were in poverty. 83.3% were single, 83.3% were at elementary school before pregnancy but 84.1% were attending house chores at the time of exam. There were significant differences (p<0.001) for gynecological age and menarche age , but not for anthropometrical, hematological and dietary variables when Group 1 was compared to group 2. 35.3% of the adolescents had a gynecological age lower than 4 years, 23.3% were below 10th percentile for height, 36,3% had a PBMI lower than 19.8 kg/m², 26.5% were below 10th percentile for arm circunference, anemia was present in 13.7% and low levels of ferritin in 18.4% of the adolescents. Calorie intake was below recommendation in 87,3% and adequacy for vitamin A and C, calcium and zinc were below 2/3 of recommendation in 36.3%, 25.9%, 88.7% and 73.5% of the adolescents respectively. Studied adolescents had a high prevalence of risk factors for nutritional deficiencies (short age, poverty, low weight, anemia and deficient intake). Early assessment of this conditions allows to identify the risk, to establish interventions and to monitor pregnancy evolution.]]></p></abstract>

Early assessment of this conditions allows to identify the risk, to establish interventions and to monitor pregnancy evolution.</P> <B>    <P ALIGN="JUSTIFY">Key words:</B> Adolescence, pregnancy, nutritional risk.</P>     <P ALIGN="JUSTIFY"><B>INTRODUCCION</P> </B>     <P ALIGN="JUSTIFY">La adolescencia es un per&iacute;odo significativo en el ser humano, caracterizado por r&aacute;pidos y profundos cambios en el crecimiento f&iacute;sico, la maduraci&oacute;n y el desarrollo psicosocial, lo que necesariamente requiere de ajustes y adaptaciones que permitan al adolescente integrarse arm&oacute;nicamente al medio ambiente (1). La adolescencia comienza con la pubescencia que es el signo m&aacute;s temprano del desarrollo de las caracter&iacute;sticas sexuales secundarias y contin&uacute;a con los cambios morfol&oacute;gicos y fisiol&oacute;gicos hasta aproximarse a la edad adulta, usualmente cerca del final de la segunda d&eacute;cada de la vida (2). Es una etapa que se&ntilde;ala la entrada del individuo joven al mundo de los adultos  y entre los retos que debe enfrentar la adolescente, est&aacute; el despertar a la sexualidad, su susceptibilidad al embarazo y la maternidad,  con las consecuencias negativas tanto en el aspecto biol&oacute;gico como en el social. Las estad&iacute;sticas muestran que el embarazo en las adolescentes se ha convertido en uno de los principales problemas de salud p&uacute;blica en muchos pa&iacute;ses de la regi&oacute;n (3). </P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Venezuela ha visto incrementar el n&uacute;mero de adolescentes embarazadas a m&aacute;s temprana edad, lo que aumenta su  riesgo obst&eacute;trico. Para 1994, seg&uacute;n la Oficina Central de Estad&iacute;stica e Inform&aacute;tica, (OCEI) (4), el 19,6% de los nacimientos vivos proven&iacute;an de madres menores de 19 a&ntilde;os, cifra que para el estado Carabobo fue de 19,1%. Para los  a&ntilde;os 1998 y 1999, el 11% y 12%  de las embarazadas que fueron atendidas por parto normal en la Maternidad del Sur &quot;Dr. Armando Arcay&quot;, de la ciudad de Valencia, correspondi&oacute; a menores de 18 a&ntilde;os.</P>     <P ALIGN="JUSTIFY">Datos obtenidos en la Ciudad Hospitalaria &quot;Dr. Enrique Tejera&quot; de Valencia, muestran que para el mismo a&ntilde;o el 23% de los nacimientos correspondi&oacute; a adolescentes menores de 19 a&ntilde;os y de ellas el 13% ten&iacute;an edades comprendidas en los 12 y 15 a&ntilde;os.</P>     <P ALIGN="JUSTIFY">Aquellas adolescentes que se embarazan antes de alcanzar la madurez fisiol&oacute;gica, es decir, en los cuatro a&ntilde;os  siguientes a la menarquia, est&aacute;n en situaci&oacute;n de alto riesgo nutricional, ya que deben satisfacer las necesidades de energ&iacute;a y nutrientes de su crecimiento y adem&aacute;s, cubrir las demandas propias del embarazo. Adicionalmente  una edad ginecol&oacute;gica menor de dos a&ntilde;os las ubica  en muy alto riesgo para problemas asociados con el embarazo (5). Estudios indican que entre las adolescentes embarazadas puede haber competencia con el feto por los nutrientes, y muestran que es m&aacute;s frecuente encontrar productos de embarazo con bajo peso, alta mortalidad y morbilidad entre adolescentes que en mujeres adultas (6).</P>     <P ALIGN="JUSTIFY">La edad, la estatura, el peso pregestacional, la baja ganancia de peso, la anemia, la paridad y el aporte inadecuado de energ&iacute;a y nutrientes, colocan a las adolescentes embarazadas doblemente en riesgo nutricional, ya que no solo est&aacute; presente el continuo crecimiento materno, sino tambi&eacute;n el crecimiento del feto (7). </P>     <P ALIGN="JUSTIFY">La evidencia sugiere que el estado nutricional materno es un factor modificable que puede afectar seriamente tanto la salud de la madre como la del feto, m&aacute;s a&uacute;n conociendo que los requerimientos maternos est&aacute;n aumentados  por el crecimiento y desarrollo (5).</P>     <P ALIGN="JUSTIFY">Es com&uacute;n observar entre adolescentes patrones de comidas irregulares y deficientes en vitaminas y minerales y a&uacute;n cuando se ha encontrado que las adolescentes consumen dietas que pudieran ser adecuadas en energ&iacute;a, son bajas en  cinc, hierro y otros nutrientes (8).</P>     <P ALIGN="JUSTIFY">Con base en lo antes mencionado y considerando la poca disponibilidad de referencias a nivel nacional tanto de la situaci&oacute;n de embarazo en las adolescentes  como sobre el riesgo nutricional, se decidi&oacute; evaluar el perfil de riesgo de las adolescentes embarazadas durante el primer trimestre de embarazo.</P>     <P ALIGN="JUSTIFY"><B>METODOLOGIA</P>     <P ALIGN="JUSTIFY">Sujetos</P> </B>    <P ALIGN="JUSTIFY">El estudio fue conducido en la Maternidad del Sur &quot;Dr. Armando Arcay&quot;, instituci&oacute;n de servicio p&uacute;blico, ubicada en la zona Sur de la ciudad de Valencia, dependiente de la Fundaci&oacute;n Instituto Carabobe&ntilde;o para la Salud (INSALUD) y cuya caracter&iacute;stica principal es el alojamiento conjunto (madre-hijo). Como parte de un estudio prospectivo longitudinal que se inici&oacute; en 1997, se  realiz&oacute; un corte transversal en el primer trimestre de embarazo. Los datos se presentan para 215 embarazadas  que cumplieron con los criterios de inclusi&oacute;n: embarazo simple, primer trimestre de embarazo y no presentar cuadro cl&iacute;nico asociado. Todas las embarazadas recibieron educaci&oacute;n nutricional como parte de los cuidados prenatales en la maternidad y de acuerdo al protocolo del trabajo. </P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><B>Evaluaci&oacute;n nutricional </P> </B>    <P ALIGN="JUSTIFY">Sociodemogr&aacute;ficos: A cada embarazada le fue evaluado tanto el estrato social seg&uacute;n el m&eacute;todo Graffar modificado por M&eacute;ndez Castellano (9) como las caracter&iacute;sticas personales: estado civil, nivel educativo alcanzado, ocupaci&oacute;n actual, paridad, h&aacute;bitos de alcohol, suplementaci&oacute;n, drogas, cigarrillo y caf&eacute;.</P>     <P ALIGN="JUSTIFY">Cl&iacute;nico-antropom&eacute;trica: edad actual (EA), edad gestacional (EG) calculada en semanas seg&uacute;n la fecha del &uacute;ltimo per&iacute;odo menstrual reportado por la embarazada; edad de menarquia (EM) y edad ginecol&oacute;gica (EGin) calculada como el tiempo de intervalo entre la edad de menarquia y la fecha de la concepci&oacute;n (5). </P>     <P ALIGN="JUSTIFY">Las variables antropom&eacute;tricas de dimensi&oacute;n corporal evaluadas fueron: peso pregestacional (PP), peso actual (PA), talla actual (TA), circunferencia de brazo izquierdo (CBI) y pliegue tricipital (PT). En composici&oacute;n corporal fueron: &aacute;rea muscular (AM), &aacute;rea grasa (AG) y el &iacute;ndice de masa corporal pregestacional (IMCP).</P>     <P ALIGN="JUSTIFY">Las mediciones fueron realizadas por dos investigadores debidamente entrenados y estandarizados, utilizando las t&eacute;cnicas recomendadas (10). Cada una de las j&oacute;venes fue pesada y medida sin zapatos y con el  m&iacute;nimo de ropa. Para el peso se utiliz&oacute; una balanza con escala graduada, marca DETECTO, registrandose &eacute;ste al 0,1Kg m&aacute;s cercano.  La talla se midi&oacute; con una cinta m&eacute;trica no extensible (pl&aacute;stico) fijada a una pared, se&ntilde;alando el tope con una escuadra en &aacute;ngulo recto, con la persona con los hombros en posici&oacute;n relajada y los brazos colgantes. Los valores de peso y talla se expresaron en kilogramos y cent&iacute;metros respectivamente. La circunferencia  de brazo se tom&oacute;  con una cinta m&eacute;trica  flexible, marcando un punto medio entre el borde inferior del acr&oacute;mion y la ap&oacute;fisis del ol&eacute;cranon. El pliegue tricipital se midi&oacute; a nivel del punto medio, en la cara posterior del brazo, mediante el uso de un calibrador Lange y durante tres oportunidades, obteni&eacute;ndose un valor promedio (10).</P>     <P ALIGN="JUSTIFY">El &aacute;rea grasa y el &aacute;rea muscular se estimaron de acuerdo a las referencias y f&oacute;rmulas de Frisancho (11) y el &iacute;ndice de  masa corporal pregestacional fue calculado seg&uacute;n la f&oacute;rmula: peso pregestacional/talla<SUP>2</SUP>, consider&aacute;ndose como punto de corte: Bajo &lt; 19,8 kg/m<SUP>2</SUP>, Normal de 19,8 a 26,9 kg/m<SUP>2</SUP> y Exceso ( 27,0 kg/m<SUP>2</SUP> (12). El peso pregestacional  fue obtenido por recordatorio en el 90% de las embarazadas y correspondi&oacute; al peso que la adolescente hab&iacute;a tenido antes del embarazo. Para el 10% restante se consider&oacute; el peso de la primera visita en el primer trimestre, como lo refiri&oacute; Neuman  que si la madre desconoce el peso pregestacional, el peso tomado en las primeras 12 semanas pudiese ser tomado como de inicio (13). </P>     <P ALIGN="JUSTIFY"><B>Laboratorio </P> </B>    <P ALIGN="JUSTIFY">Se extrajeron 3 ml de sangre  venosa, despu&eacute;s de un ayuno de 12 horas, los cuales se recolectaron en tubos nuevos libres de elementos traza para la  determinaci&oacute;n de hemoglobina, utilizando un contador semi-automatizado (Symex) con anticoagulante EDTA y la ferritina por el m&eacute;todo de enzimo inmunoensayo (14). Se consider&oacute; anemia en el primer trimestre de embarazo, cuando las embarazadas adolescentes presentaron hemoglobina menor a 11 mg/dl (15); deficiencia de hierro cuando la ferritina era menor a 12 <FONT FACE=Symbol>&#109;</FONT> g/l y anemia por deficiencia de hierro (Hemoglobina menor a 11 mg/dl y ferritina menor a 12<FONT FACE=Symbol>&#109;</FONT> g/l) (16).</P>     <P ALIGN="JUSTIFY"><B>Diet&eacute;tica</P> </B>    <P ALIGN="JUSTIFY">El aporte diet&eacute;tico fue evaluado utilizando el m&eacute;todo de recordatorio de 24 horas durante dos d&iacute;as no consecutivos. Para el an&aacute;lisis de energ&iacute;a, macro y micronutrientes se utiliz&oacute; un  programa computarizado donde adem&aacute;s de los nutrientes contenidos en la Tabla de Composici&oacute;n de Alimentos de Venezuela (17), se han incorporado los valores para otros elementos de la tabla de composici&oacute;n de alimentos de M&eacute;xico (18). Los valores individuales de energ&iacute;a y nutrientes fueron revisados bas&aacute;ndose en recomendaciones nacionales (19)  e internacionales (20), a&uacute;n cuando las comparaciones se realizaron contra las nacionales. Se uso para energ&iacute;a, la ecuaci&oacute;n para el grupo de adolescentes (femenino) de 10 a 18 a&ntilde;os, que determina el gasto energ&eacute;tico en reposo  (GER), definiendo para este grupo la actividad &quot;ligera&quot; con un factor de actividad de 1,5. Como punto de corte de riesgo de deficiencia para vitaminas y minerales se us&oacute; el criterio de 2/3 de la recomendaci&oacute;n diet&eacute;tica (21) aplicado sobre las recomendaciones nacionales. Cabe destacar que para establecer la adecuaci&oacute;n de energ&iacute;a y nutrientes se parti&oacute; de la recomendaci&oacute;n b&aacute;sica para el grupo etario, sin modificaci&oacute;n por embarazo ya que estaban en el primer trimestre.</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><B>Perf&iacute;l de riesgo</P> </B>    <P ALIGN="JUSTIFY">A los fines del estudio,  se considera  riesgo la presencia de al menos una de las siguientes variables: Edad ginecol&oacute;gica &lt; 4 a&ntilde;os (8), talla &lt; del percentil 10 de Frisancho para el grupo de 13 a 18 a&ntilde;os (11), IMCP &lt;19,8 kg/m<SUP>2</SUP> (12, 22), Hb &lt; 11 g/dL y ferritina &lt; 12 <FONT FACE=Symbol>&#109;</FONT> g/L (15,16, 23). Con el fin de comparar los resultados, los datos se presentan clasificando las adolescentes en dos grupos de edad: Grupo 1 (13 a 15 a&ntilde;os) y Grupo 2 (16 a 18 a&ntilde;os).</P>     <P ALIGN="JUSTIFY"><B>Pruebas estad&iacute;sticas</P> </B>    <P ALIGN="JUSTIFY">Las estad&iacute;sticas descriptivas fueron generadas usando un programa SPSS (versi&oacute;n 8.0 para Windows), valores que se expresan como media y desviaci&oacute;n est&aacute;ndar seg&uacute;n grupos. Se utiliz&oacute; la prueba de t de Student para muestras independientes, an&aacute;lisis de varianza y distribuci&oacute;n percentilar. La normalidad de las variables energ&iacute;a y nutrientes fue revisada con la prueba de Kormogorov- Smirnov y se us&oacute; la prueba de Mann- Whitney para datos no param&eacute;tricos(24). En todos los casos el nivel de significancia estad&iacute;stica se fij&oacute; con una p &lt;0,05.</P>     <P ALIGN="JUSTIFY"><B>RESULTADOS</P> </B>     <P ALIGN="JUSTIFY">La selecci&oacute;n de las adolescentes se realiz&oacute; en el primer trimestre de embarazo, con una edad gestacional promedio de 12,1<FONT FACE=Symbol>&#177;</FONT>  2,7 semanas. La edad promedio de las adolescentes seg&uacute;n grupos fue de 14,5<FONT FACE=Symbol>&#177;</FONT>  0,6 a&ntilde;os en el grupo 1, en una proporci&oacute;n de un 21,9% (n= 47), mientras que en el grupo 2 fue de 16,3<FONT FACE=Symbol>&#177;</FONT>  1,2 a&ntilde;os, 78,1%  (n=168). La <a href="#tab1"> Tabla 1</a> muestra los datos sociodemogr&aacute;ficos  seg&uacute;n edad, donde resalta un elevado porcentaje de adolescentes que se ubican en pobreza (96,3%), con un 83,3% de adolescentes solteras, de las cuales el 83,3% (<FONT FACE=Symbol>&#99;</FONT> <SUP>2</SUP>= 0,009) solo alcanzaron la educaci&oacute;n b&aacute;sica y que al salir embarazadas abandonaron la escuela y se desempe&ntilde;an actualmente en oficios del hogar (84,1%). Seg&uacute;n la paridad, 84,7% son primiparas y el 15,3% son mult&iacute;paras, lo que las ubica en mayor riesgo nutricional, dados los embarazos previos. El 36,3%  (<FONT FACE=Symbol>&#99;</FONT> <SUP>2</SUP>= 0,017) refiri&oacute; estar usando suplementos. El 22,8% de las  adolescentes report&oacute; consumir alcohol y el 69,2% caf&eacute;. <body><![CDATA[<P ALIGN="JUSTIFY"><font size="3">Requerimiento energ&eacute;tico promedio calculado para este grupo (2074 Kcal/d)</font></P> <SUP>    <P ALIGN="JUSTIFY"><font size="3">a</font></SUP> <font size="3"> Percentiles (25-75) del grupo estudiado &quot;t&quot;  Student  (no significativa).</font></P>     <P ALIGN="JUSTIFY">Al evaluar el perfil de riesgo presente en este grupo <a href="#graf1"> (Gr&aacute;fico 1)</a> se observa que el 35,3 % de las adolescentes ten&iacute;a una edad ginecol&oacute;gica promedio menor de 4 a&ntilde;os, siendo las del grupo 1 (44,7%) las m&aacute;s vulnerables de riesgo. El 23,3 % de las j&oacute;venes tuvo una talla promedio inferior al percentil 10 de la referencia (11). El 36,3% inici&oacute; su embarazo con un IMCP &lt; 19,8 kg/m<SUP>2</SUP>. El 26,5% mostr&oacute; un CB &lt; del percentil 10. El 13,7% de las embarazadas presentaba anemia y el 18,4% tuvieron valores de ferritina &lt; 12 <FONT FACE=Symbol>&#109;</FONT> g/L. El 87,3% mostr&oacute; un aporte de energ&iacute;a por debajo de la recomendaci&oacute;n promedio para el grupo durante el primer trimestre de embarazo y la vitamina A y C, el calcio y el cinc se encontraron en riesgo de deficiencia (&lt; 2/3 de recomendaci&oacute;n) en el 36,3%, 25,9%, 88,7% y 73,5% de las j&oacute;venes estudiadas, respectivamente. No se encontr&oacute; correlaci&oacute;n entre las variables antropometricas, bioqu&iacute;micas y diet&eacute;ticas. <body><![CDATA[<P ALIGN="JUSTIFY">El embarazo en la adolescencia implica una situaci&oacute;n compleja desde el punto de vista ps&iacute;cosocial, biol&oacute;gico y nutricional, lo que habitualmente se traduce en mayores riesgos maternos y perinatales. En Venezuela se ha observado un incremento del n&uacute;mero de adolescentes embarazadas a m&aacute;s temprana edad, lo que aumenta su riesgo obst&eacute;trico. Seg&uacute;n la Oficina Central de Estad&iacute;stica e Inform&aacute;tica (OCEI) (4), el 19,6% de los reci&eacute;n nacidos vivos provienen de madres menores de 19 a&ntilde;os. Dada la poca informaci&oacute;n que a nivel nacional existe fundamentalmente en relaci&oacute;n con las caracter&iacute;sticas biol&oacute;gicas y nutricionales de este grupo et&aacute;rio, es que se plante&oacute; la necesidad de evaluar el perfil de riesgo de las adolescentes  en etapas tempranas del embarazo y as&iacute; prevenir la ocurrencia de efectos adversos tanto para la madre como para el producto. Al examinar las caracter&iacute;sticas sociodemogr&aacute;ficas del grupo estudiado encontramos una mayor&iacute;a de j&oacute;venes de bajo nivel socioecon&oacute;mico y en pobreza (96,3%), donde un alto porcentaje estaban solteras (83,3%) y con bajo nivel de escolaridad (84,9%) solo hab&iacute;an alcanzado hacer 9 a&ntilde;os de escolaridad, lo cual es bajo para la edad. El 84,1% no ten&iacute;a ocupaci&oacute;n definida ya que manifestaron haber abandonado los estudios al conocer que estaban embarazadas. Como ha sido mostrado en diferentes estudios, cada uno de estos aspectos  tiene sus efectos importantes sobre la madre y en especial si es adolescente, aumentando los riesgos durante el embarazo; as&iacute; como sobre el peso del producto y sus posibilidades de desarrollo futuro (26-28). </P>     <P ALIGN="JUSTIFY">En este estudio, a&uacute;n cuando los valores promedios de edad de menarquia y edad ginecol&oacute;gica mostraron diferencias significativas respecto a los grupos de edad, la edad de menarquia fue ligeramente mas baja que la edad promedio nacional (12,6 a&ntilde;os) (29) y a las de adolescentes de EEUU y Jap&oacute;n de 12,1 a 13,0 a&ntilde;os (8, 30). El valor promedio para la edad ginecol&oacute;gica fue tambi&eacute;n  menor (4,0<FONT FACE=Symbol>&#177;</FONT> 1,5 a&ntilde;os) a los reportados por otros autores (7, 31). Un 35,3% de las adolescentes estudiadas ten&iacute;an menos de cuatro a&ntilde;os de edad ginecol&oacute;gica, hallazgo similar a otros estudios que sugieren que las adolescentes embarazadas en estas circunstancias est&aacute;n en alto riesgo nutricional, ya que son m&aacute;s susceptibles de presentar complicaciones obst&eacute;tricas y nacimientos de bajo peso, entre otros porque no han alcanzado su  madurez fisiol&oacute;gica y tienen que satisfacer las necesidades para su crecimiento y las demandas de energ&iacute;a y nutrientes por el embarazo (8, 32). </P>     <P ALIGN="JUSTIFY">Las madres adolescentes son m&aacute;s peque&ntilde;as y de menos peso a la concepci&oacute;n que las madres adultas y  la estatura baja en estas madres pudiera ser un reflejo de maduraci&oacute;n temprana, de la continuaci&oacute;n del crecimiento puberal actual o de desnutrici&oacute;n pasada. As&iacute; mismo existen evidencias de diferentes fuentes que indican que las madres de talla baja probablemente presentan mayor riesgo de tener reci&eacute;n nacidos de menor estatura y de bajo peso independientemente de la edad materna (33).&#9;</P>     <P ALIGN="JUSTIFY">Datos del Centro de Investigaciones en Nutrici&oacute;n (1998) reportaron que madres adultas caracterizadas como talla baja, tuvieron productos significativamente con menor peso, que las de talla normal (34). En nuestro estudio el valor promedio de la talla (156,9<FONT FACE=Symbol>&#177;</FONT> 5,6 cms) corresponde al de la adolescente venezolana no embarazada (29), pero es menor al del percentil 25 de talla para la edad seg&uacute;n la referencia del Centro de Estad&iacute;sticas de Salud de los EEUU (National Center for Health Statistics (NCHS) como lo refiere Frisancho (11). De igual manera, el 23,3% de las adolescentes  presentaron una estatura menor al percentil 10 de la referencia (11), punto de corte considerado de alto riesgo de complicaciones durante el embarazo (35). Esta prevalencia fue menor a la observada por Arcos (36) y superior a la mostrada en adolescentes de Sao Paulo, Brasil (37). </P>     <P ALIGN="JUSTIFY">El peso pregestacional es una variable que esta directamente relacionada con la nutrici&oacute;n materna y con la incidencia de bajo peso al nacer (37), de ah&iacute; que el peso pregestacional, la duraci&oacute;n del embarazo y la estatura materna son significativos predictores de la ganancia de peso. En el presente trabajo se observa que el peso promedio (51,6 <FONT FACE=Symbol>&#177;</FONT>  7,3 kg) de las adolescentes fue superior al percentil 50 de la referencia nacional (29), e inferior a la referencia de Frisancho (11).  El Indice de masa corporal pregestacional para nuestro estudio muestra  un valor promedio menor al encontrado en adolescentes embarazadas de Chile, Brasil y Puerto Rico (36,38), pero sobre el percentil 50 de la referencia (11). No obstante, el estado nutricional pregestacional nos indica que un 36,3% de las adolescentes iniciaron su embarazo con peso bajo, proporci&oacute;n muy superior al encontrado por Arcos (36) y Borah  (39); lo anterior nos estar&iacute;a indicando que estas j&oacute;venes pueden haber sido desnutridas en su ni&ntilde;ez y han permanecido as&iacute; hasta su edad reproductiva, tal como es referido por otros autores (27, 40). </P>     <P ALIGN="JUSTIFY">En Am&eacute;rica Latina, del 10 al 30% de las mujeres en edad reproductiva y por encima del 40-70% de las mujeres embarazadas pueden tener deficiencia de hierro (41). En Venezuela una  proporci&oacute;n significativa de la poblaci&oacute;n sufre de anemia y deficiencia de hierro, especialmente los ni&ntilde;os y las embarazadas (42). </P>     <P ALIGN="JUSTIFY">Durante la adolescencia existe una elevada demanda de hierro por el crecimiento acelerado y las perdidas menstruales y si sumamos a esto la necesidad de hierro por el embarazo, aumentan a&uacute;n m&aacute;s los requerimientos diarios, los cuales dif&iacute;cilmente pueden ser aportados por la dieta, increment&aacute;ndose como consecuencia el riesgo de una deficiencia de hierro (37, 43, 44). En este grupo, el valor promedio para la hemoglobina se encontr&oacute; dentro del rango normal, similar al obtenido en adolescentes venezolanas no embarazadas y en embarazadas adultas (29, 45). Sin embargo, se observ&oacute; una prevalencia de anemia (Hb &lt; 11 g/dL) en el 13,6% de las adolescentes, cifra ligeramente m&aacute;s baja que las reportadas por Rebolledo (3) y Fujimori (38), pero similar a los rangos obtenidos en otros pa&iacute;ses que oscilan entre 9 y 14%  (13, 46,47). </P>     <P ALIGN="JUSTIFY"> La deficiencia de hierro valorada por la concentraci&oacute;n de ferritina (&lt; 12 <FONT FACE=Symbol>&#109;</FONT> g/L) estuvo presente en el 18,4% de las adolescentes, cifra inferior a la reportada por Fujimori (38), pero superior a la reportada por otros investigadores (46, 48). No obstante la baja prevalencia de adolescentes an&eacute;micas y deficientes de hierro, evidencia que estando las adolescentes en sus primeras semanas de embarazo, no hab&iacute;a transcurrido el tiempo necesario para que se depletaran las reservas de hierro y por otro lado la expansi&oacute;n del volumen plasm&aacute;tico es menor, si se compara con el segundo y tercer trimestre (15, 23, 44). Por otro lado, las variaciones en torno a las prevalencias en los diferentes trabajos puede tener su origen en el muestreo (no se clasifica por edad) y en muchos casos, no se indica la edad gestacional en que se realiza el ex&aacute;men, lo que pudiera afectar la prevalencia de la anemia. En el presente trabajo el aporte de hierro diet&eacute;tico estuvo dentro de la recomendaci&oacute;n, a&uacute;n cuando este corresponde  fundamentalmente a hierro no hem, de baja biodisponibilidad. </P>     <P ALIGN="JUSTIFY">Aunque el embarazo en adolescentes impone un aumento del riesgo tanto para la madre como para el ni&ntilde;o, no est&aacute; claro si ello es debido a la juventud de la madre o a factores sociales externos entre los cuales estar&iacute;an: inadecuada nutrici&oacute;n, bajo peso pregestacional y una pobre ganancia de peso durante el embarazo. Hay suficientes evidencias que muestran que  las adolescentes embarazadas no difieren de sus contempor&aacute;neas no embarazadas en exhibir patrones diet&eacute;ticos inadecuados (8), de ah&iacute; que, aquellas pertenecientes a grupos de bajo nivel socioecon&oacute;mico, tienen substancialmente consumos m&aacute;s bajos en energ&iacute;a y nutrientes como vitamina A, calcio y cinc, que lo recomendado, especialmente durante el embarazo (22). En este estudio, el valor promedio de energ&iacute;a (1632<FONT FACE=Symbol>&#177;</FONT> 403 kcal), fue similar al registrado por adolescentes mexicanas no embarazadas (49), sin embargo estuvo fuera del rango de 1800 a 2800 kcals, observado por otros investigadores (39, 50), de igual manera se ubica por debajo de la recomendaci&oacute;n en el 87,3% del grupo estudiado, donde hay predominio de adolescentes del grupo de 16 a 18 a&ntilde;os. </P>     <P ALIGN="JUSTIFY">La proporci&oacute;n de calor&iacute;as provenientes de prote&iacute;nas, grasas y carbohidratos (13%, 25% y 61% respectivamente), muestran que la dieta promedio fue equilibrada en composici&oacute;n de macronutrientes.</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">En las adolescentes se han reportado dietas deficientes en cinc y en menor grado calcio, vitaminas A y C (8, 50), siendo similares tanto en embarazadas como en no embarazadas. En el presente estudio se observ&oacute; un aporte por debajo de los 2/3 de la recomendaci&oacute;n para vitaminas A y C, calcio y cinc, lo que coincide con resultados de otros autores (39, 51). Estos hallazgos sugieren que el bajo aporte de energ&iacute;a observado durante esta primera etapa del embarazo en este grupo, sumado a un aporte inadecuado de prote&iacute;nas y micronutrientes pudiera relacionarse no solo con h&aacute;bitos alimentarios inadecuados, sino tambien con la presencia de factores caracter&iacute;sticos en esta etapa del embarazo (n&aacute;useas y v&oacute;mitos) y con un bajo nivel socioecon&oacute;mico.</P>     <P ALIGN="JUSTIFY">En este estudio se observaron caracter&iacute;sticas de riesgo nutricional y reproductivo en la totalidad de las adolescentes, por las deficientes condiciones socioecon&oacute;micas, la edad, el bajo peso materno y el bajo aporte de energ&iacute;a y nutrientes.
