Tanto la deficiencia como el exceso nutricional afectan la respuesta inmune, actuando sobre mecanismos similares o diferentes. Se evaluaron 104 preescolares aparentemente sanos pertenecientes a un instituto privado de Valencia, Venezuela, con el objetivo de evaluar las subpoblaciones linfocitarias y su relación con el estado nutricional antropométrico y el zinc sérico. El estado nutricional se determinó utilizando el indicador peso para la talla (desnutrición actual), las subpoblaciones linfocitarias por citometría de flujo y el zinc sérico por espectrofotometría de absorción atómica. Se encontró que un alto porcentaje de la población (87,5%) pertenecía al estrato socioeconómico II (clase media alta). El 9,5% de los niños presentaron déficit nutricional y la malnutrición por exceso afectó al 16,2% de la población. Los valores de las subpoblaciones linfocitarias y zinc sérico se encontraron dentro de los rangos de referencia y no presentaron diferencias significativas con relación a sexo, edad y estado nutricional. Utilizando como punto de corte 73 my g/dL se encontró un 6,7% de hipozincemia. No se encontraron diferencias significativas para las subpoblaciones evaluadas entre los niños hipozincemicos y los normozincemicos, aún cuando se observó para la población de linfocitos T una tendencia a valores inferiores en los niños clasificados como hipozincémicos. Los resultados encontrados en este estudio pudieran servir de referencia para futuras investigaciones las cuales son necesarias para avanzar en el campo de la inmunonutrición.]]></p></abstract>

<body><![CDATA[<P ALIGN="JUSTIFY">El reconocimiento de que la dieta es un determinante importante en la resistencia a infecciones ha estimulado a aumentar el n&uacute;mero de estudios en el campo de la nutrici&oacute;n y la inmunolog&iacute;a (1). </P>     <P ALIGN="JUSTIFY">Los efectos de los nutrientes sobre la respuesta inmune suelen presentarse de forma precoz, dependiendo del nutriente implicado y de sus interacciones con otros nutrientes esenciales. La deficiencia de algunos nutrientes, ya sea relativamente leve o severa as&iacute; como el sobrepeso y la obesidad alteran la respuesta inmunol&oacute;gica afectando algunos mecanismos similares y otros diferentes; por lo que es conveniente ajustar la dieta a las demandas fisiol&oacute;gicas para garantizar una respuesta inmune &oacute;ptima, en ausencia de otros da&ntilde;os (1-4). </P>     <P ALIGN="JUSTIFY">La interacci&oacute;n entre nutrici&oacute;n e inmunidad es un fen&oacute;meno complejo; en el cual los alimentos en conjunto y sus componentes en particular ejercen un papel importante en el desarrollo y preservaci&oacute;n del sistema inmune. Las deficiencias marginales, los excesos cr&oacute;nicos &oacute; el desequilibrio entre los nutrientes pueden da&ntilde;arlo. </P>     <P ALIGN="JUSTIFY">Los factores nutricionales que act&uacute;an directamente sobre las c&eacute;lulas del sistema inmune pueden cambiar la capacidad funcional de estas c&eacute;lulas, alterando tanto los componentes intracelulares como los de membrana externa y sus receptores para el reconocimiento de los est&iacute;mulos extra&ntilde;os; y por lo tanto, modificar la respuesta proliferativa de las c&eacute;lulas a varios factores hormonales y de crecimiento (5,6). Adem&aacute;s dichos factores pueden afectar la inmunidad celular produciendo una reducci&oacute;n del n&uacute;mero total de linfocitos perif&eacute;ricos y de c&eacute;lulas T, la proporci&oacute;n del subconjunto de c&eacute;lulas T y la reducci&oacute;n del timo, importante &oacute;rgano de la inmunidad celular (7,8).</P>     <P ALIGN="JUSTIFY">Por otra parte, el sistema inmunol&oacute;gico humoral se encuentra menos afectado por la desnutrici&oacute;n que la respuesta inmunol&oacute;gica mediada por c&eacute;lulas, aunque puede haber disminuci&oacute;n del n&uacute;mero de c&eacute;lulas B. En muchos casos, la producci&oacute;n de anticuerpos por los linfocitos B depende de las c&eacute;lulas T, las cuales se encuentran depletadas en individuos desnutridos; y tambi&eacute;n en los problemas de malnutrici&oacute;n por exceso (9), afectando indirectamente la subpoblaci&oacute;n de linfocitos B.</P>     <P ALIGN="JUSTIFY">De all&iacute; que una adecuada ingesta de nutrientes tenga un rol fundamental para el buen funcionamiento del sistema inmune y que la severidad del deterioro inmunol&oacute;gico depender&aacute; de cuan grave sea la deficiencia de nutrientes, de las interacciones con otros nutrientes esenciales, de la edad del individuo y de la presencia de infecciones concomitantes.</P>     <P ALIGN="JUSTIFY">A pesar de esto son muy pocos los micronutrientes estudiados con relaci&oacute;n a su papel dentro del funcionamiento del sistema inmune (8-11). Uno de los m&aacute;s estudiados por ser un micronutriente esencial para el crecimiento, divisi&oacute;n y diferenciaci&oacute;n celular, adem&aacute;s de intervenir en procesos inmunes es el zinc (12,13), cuya deficiencia severa es poco frecuente pero la forma moderada de &eacute;sta parece ser relativamente com&uacute;n en el mundo. </P>     <P ALIGN="JUSTIFY">El zinc es un elemento traza esencial para el sistema inmune, y otros sistemas org&aacute;nicos, su deficiencia est&aacute; acompa&ntilde;ada de un estado inmunol&oacute;gico deficiente y trae como resultado un aumento en el numero de infecciones. El funcionamiento del sistema inmune est&aacute; delicadamente regulada por el zinc, por lo que tanto el incremento como la disminuci&oacute;n de sus niveles alteran la funci&oacute;n inmune (14-15). Cabe resaltar que la ingesta excesiva de nutrientes podr&iacute;a disminuir la respuesta inmune y que la presencia de inmunodeficiencia asociada al d&eacute;ficit nutricional puede ser corregida con una adecuada suplementaci&oacute;n de zinc en la dieta (16). </P>     <P ALIGN="JUSTIFY">La evaluaci&oacute;n de los cambios en el sistema inmune cuando hay alteraciones nutricionales y el aporte de datos que pueden servir de referencia a nivel nacional en ni&ntilde;os preescolares constituyeron los objetivos del estudio.</P> <B>    <P ALIGN="left">SUJETOS Y METODOS</P> </B>    ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Se realiz&oacute; un estudio descriptivo de corte transversal, durante el mes de Mayo el a&ntilde;o 2000, en una poblaci&oacute;n de ni&ntilde;os aparentemente sanos pertenecientes a un preescolar privado de Valencia (capital del estado Carabobo) una de las principales ciudades industriales de Venezuela, ubicada en la parte centro norte del pa&iacute;s. Con base en la aceptaci&oacute;n a participar en la investigaci&oacute;n, a partir de una poblaci&oacute;n de 240 preescolares en edades comprendidas entre 3 y 6 a&ntilde;os de edad, se seleccionaron 104 ni&ntilde;os (43,3% de la poblaci&oacute;n) aplicando un dise&ntilde;o de muestreo probabil&iacute;stico. Antes de empezar el estudio se les explicaron a los representantes los objetivos, beneficios y posibles riesgos inherentes a la participaci&oacute;n de los ni&ntilde;os en la evaluaci&oacute;n y se obtuvo su consentimiento por escrito y su cooperaci&oacute;n durante el estudio. </P>     <P ALIGN="JUSTIFY">Los ni&ntilde;os seleccionados llenaron los criterios de inclusi&oacute;n: aparentemente sanos, que no tuvieran signos y/o s&iacute;ntomas de procesos virales o bacterianos, ni procesos al&eacute;rgicos para el momento de la extracci&oacute;n de la sangre. A los representantes se les aplic&oacute; un cuestionario el cual permiti&oacute; conocer los datos personales y sociodemogr&aacute;ficos de los ni&ntilde;os en estudio y se determin&oacute; el nivel socioecon&oacute;mico utilizando el M&eacute;todo de Graffar modificado por M&eacute;ndez –Castellano, el cual es un m&eacute;todo que consta de cuatro variables (profesi&oacute;n jefe de la familia, nivel de instrucci&oacute;n de la madre, principal fuente de ingreso de la familia, condiciones de la vivienda) cada una de ellas conformadas por cinco items. Cada item corresponde a una ponderaci&oacute;n decreciente del 1-5. La suma de los items determina el estrato social al que pertenece la familia investigada de acuerdo a una escala previamente establecida cuya clasificaci&oacute;n es la siguiente, estrato I (clase alta), estrato II (clase media alta), estrato III (clase media baja), estrato IV (pobreza relativa), estrato V (pobreza cr&iacute;tica) (17).</P>     <P ALIGN="JUSTIFY">Para la evaluaci&oacute;n inmunol&oacute;gica y de zinc s&eacute;rico se realiz&oacute; extracci&oacute;n de sangre venosa de una vena anticubital (3 ml), previo ayuno de por lo menos ocho horas. Una alicuota de 2 ml de sangre completa se utiliz&oacute; para las determinaciones de las subpoblaciones linfocitarias y 1 ml de suero para la determinaci&oacute;n s&eacute;rica de zinc. Las muestras fueron identificadas y transportadas inmediatamente al laboratorio para su almacenamiento y an&aacute;lisis.</P>     <P ALIGN="JUSTIFY">Las determinaciones de las subpoblaciones linfocitarias se procesaron el mismo d&iacute;a y las de zinc posteriormente, manteni&eacute;ndose la muestra congelada a –70ºC.</P>     <P ALIGN="JUSTIFY">Las subpoblaciones linfocitarias se determinaron por citometr&iacute;a de flujo mediante doble marcaje con anticuerpos monoclonales directamente conjugados con fluorocromos y mediante t&eacute;cnica descrita seg&uacute;n protocolo internacional (18,19), utilizando el siguiente panel de las designaciones de CD: control CD3<SUP>+</SUP> FITC<SUP> </SUP>para linfocitos T totales, CD3<SUP>+</SUP> FITC/ CD4<SUP>+</SUP>PE para linfocitos T ayudadores-inductores, CD3<SUP>+</SUP>FITC/CD8<SUP>+</SUP>PE para linfocitos T supresores-citot&oacute;xicos y CD20<SUP>+</SUP>PE para linfocitos B, donde FITC es isotiocianato de fluoresce&iacute;na y PE es ficoeritrina. La relaci&oacute;n linfocitos T cooperadores(CD4+)/linfocitos T citot&oacute;xicos (CD8+) fue calculada como el porcentaje de linfocitos T CD4+ dividido entre el porcentaje de linfocitos T CD8+, adem&aacute;s se utilizo un control isot&iacute;pico. Se realiz&oacute; la optimizaci&oacute;n diaria del equipo para el an&aacute;lisis de dos colores utilizando las part&iacute;culas pl&aacute;sticas (Calibrite) mediante el software Autocomp. Para la adquisici&oacute;n y an&aacute;lisis de las diferentes subpoblaciones estudiadas se utiliz&oacute; el software LYSYS II versi&oacute;n 2.0. </P>     <P ALIGN="JUSTIFY">Los puntos de corte para definir niveles deficientes de las variables inmunol&oacute;gicas, luego de haber realizado la distribuci&oacute;n percentilar fueron para CD3: &lt;62% para CD4: &lt;30% para CD8: &lt;25% para la relaci&oacute;n CD4/CD8: &lt;1,0 y para CD20: &lt;21%.</P>     <P ALIGN="JUSTIFY">La determinaci&oacute;n de zinc s&eacute;rico se realiz&oacute; por espectrofotometr&iacute;a de absorci&oacute;n at&oacute;mica. Para ello se emple&oacute; un espectrofot&oacute;metro marca Perkin-Elmer modelo 3100 (20,21). Se consideraron como valores normales de zinc s&eacute;rico, concentraciones de 70 a 120 <FONT FACE=Symbol>m</FONT> g/dL y como punto de corte a los fines de este estudio para definir hipozincemia se utiliz&oacute; el percentil 10 de la distribuci&oacute;n del grupo de ni&ntilde;os, correspondiendo a la concentraci&oacute;n de 73 <FONT FACE=Symbol>m</FONT> g/dL.</P>     <P ALIGN="JUSTIFY">Para la evaluaci&oacute;n antropom&eacute;trica, los ni&ntilde;os fueron pesados en una balanza de pie marca Health-o-Meter, descalzos y en ropa interior. La balanza fue ajustada a cero antes de cada medici&oacute;n registr&aacute;ndose el peso en kilogramos. La talla de los ni&ntilde;os se determin&oacute; mediante el uso de una cinta m&eacute;trica fijada a la pared. Cada uno de los ni&ntilde;os fue colocado en posici&oacute;n de pie, de espalda a la cinta m&eacute;trica, sin zapatos ni medias con ambos talones juntos y las rodillas estiradas, la cabeza colocada en plano de Frankfort y los hombros relajados. Los gl&uacute;teos y los hombros estuvieron en contacto con la cinta m&eacute;trica. La medida se realiz&oacute; dos veces y se expres&oacute; en metros (22). </P>     <P ALIGN="JUSTIFY">Con la obtenci&oacute;n de las medidas antropom&eacute;tricas ya descritas se construy&oacute; el indicador de dimensi&oacute;n corporal peso para la talla por tratarse de un estudio de tipo transversal, con el objeto de hacer el diagn&oacute;stico nutricional antropom&eacute;trico actual de los ni&ntilde;os y se utilizaron como referencia los puntos de corte de la Organizaci&oacute;n Mundial de la Salud para ese indicador (23), definiendo como d&eacute;ficit valores menores o iguales al percentil 10; como eutr&oacute;ficos valores mayores del percentil10 y menores o iguales al percentil 90 y exceso valores mayores al percentil 90. Los resultados de la evaluaci&oacute;n de los preescolares fueron entregados individualmente a sus representantes y de la globalidad a los directivos del preescolar. Para la ejecuci&oacute;n del estudio se cont&oacute; con un equipo multidisciplinario constituido por m&eacute;dicos, nutricionistas, bioanalistas y bi&oacute;logos debidamente entrenados y estandarizados, miembros del personal del Centro de Investigaciones en Nutrici&oacute;n de la Universidad de Carabobo, y el personal del preescolar. </P>     <P ALIGN="JUSTIFY">Para el an&aacute;lisis de los datos obtenidos se emplearon estad&iacute;sticos descriptivos de tendencia central y de dispersi&oacute;n (media, desviaci&oacute;n est&aacute;ndar y distribuci&oacute;n percentilar) para las variables antropom&eacute;tricas, edad, contaje de leucocitos, las variables inmunol&oacute;gicas y zinc s&eacute;rico. Se determinaron frecuencias absolutas y relativas para el an&aacute;lisis de nivel socioecon&oacute;mico y estado nutricional antropom&eacute;trico. Se aplic&oacute; la prueba de t-student, para comparar las medias de las variables estudiadas y para establecer las diferencias entre las medias de m&aacute;s de dos grupos se emple&oacute; la prueba de Bonferroni. Para todos los an&aacute;lisis se emple&oacute; un nivel de significado estad&iacute;stico de p<FONT FACE=Symbol>&lt;</FONT> 0,05. El paquete estad&iacute;stico utilizado fue The Statistical Package for Social Sciences versi&oacute;n 7.5 (24).</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><B>RESULTADOS </B></P>     <P ALIGN="JUSTIFY">Se evaluaron las subpoblaciones linfocitarias de 104 ni&ntilde;os preescolares de la ciudad de Valencia, cuyas edades estuvieron comprendidas entre 4 y 6 a&ntilde;os, as&iacute; como la relaci&oacute;n con el estado nutricional antropom&eacute;trico y los niveles s&eacute;ricos de zinc. De los ni&ntilde;os estudiados el 55,76% (n=58) pertenec&iacute;a al sexo masculino y el 44,23% (n=46) al sexo femenino. La edad promedio de los ni&ntilde;os fue 4,80<FONT FACE=Symbol>±</FONT> 0,78 a&ntilde;os. La composici&oacute;n del grupo estudiado por edad fue de 42,31% (n=44) en el grupo de 4 a&ntilde;os, el 34,61% (n=36) en el grupo de 5 a&ntilde;os y un 23,08% (n=24) correspondi&oacute; al grupo de 6 a&ntilde;os de edad. De la poblaci&oacute;n estudiada, el 1,92% se ubic&oacute; en el estrato socioecon&oacute;mico I (clase alta), el 87,51% en el estrato socioecon&oacute;mico II (clase media alta) y el 10,57% en el estrato socioecon&oacute;mico III clase media baja), seg&uacute;n el m&eacute;todo de Graffar modificado por M&eacute;ndez-Castellano (17). </P>     <P ALIGN="JUSTIFY">El porcentaje de ni&ntilde;os con un estado nutricional antropom&eacute;trico normal seg&uacute;n el indicador peso para la talla fue de 74,3%, mientras que la malnutrici&oacute;n por exceso se encontr&oacute; en un 16,2% y la malnutrici&oacute;n por d&eacute;ficit estuvo presente en el 9,5% de los ni&ntilde;os. </P>     <P ALIGN="JUSTIFY">Los par&aacute;metros antropom&eacute;tricos y los valores correspondientes al contaje de leucocitos, contaje de linfocitos, subpoblaciones linfocitarias y de zinc s&eacute;rico se encontraron dentro de los valores considerados normales para este grupo de edad (<a href="#TABLA 1">Tabla 1</a>). </P>     <P ALIGN="CENTER"><a name="TABLA 1">TABLA 1</a></P>     <P ALIGN="CENTER">Caracterizaci&oacute;n de los preescolares estudiados. <body><![CDATA[<P ALIGN="left">DISCUSION</P> </B>    <P ALIGN="JUSTIFY">El sistema inmune requiere de una interacci&oacute;n equilibrada entre c&eacute;lulas efectoras y mol&eacute;culas inmunomoduladoras, cuya s&iacute;ntesis, funci&oacute;n y balance necesita de un aporte igualmente equilibrado de energ&iacute;a, amino&aacute;cidos y dem&aacute;s nutrientes. Por esta raz&oacute;n, cualquier desequilibrio nutricional afectar&aacute; en alguna medida la competencia del sistema inmune y los cambios que se suceden en diferentes &oacute;rganos, c&eacute;lulas y mol&eacute;culas pueden ser variables evaluadas en estudios de inmunonutrici&oacute;n. Este an&aacute;lisis puede llevarse a cabo a trav&eacute;s de diferentes pruebas inmunol&oacute;gicas, que se transforman en herramientas para el diagn&oacute;stico del compromiso nutricional as&iacute; como para el seguimiento y evaluaci&oacute;n del &eacute;xito de las intervenciones nutricionales. Con respecto a la inmunidad celular, el an&aacute;lisis contempla el recuento de leucocitos y el recuento linfocitario total, determinaciones en sangre perif&eacute;rica que se realizan de rutina en todos los centros de salud. La cuantificaci&oacute;n y tipificaci&oacute;n de los linfocitos de importancia para el estudio de inmunonutrici&oacute;n se realiza mediante citometr&iacute;a de flujo (5). El an&aacute;lisis mediante la t&eacute;cnica de citometr&iacute;a de flujo permite la cuantificaci&oacute;n de los linfocitos y sus poblaciones permitiendo de manera objetiva evaluar los cambios en el sistema inmune celular (26), el cual tiene una alta sensibilidad a las modificaciones en el estado nutricional.</P>     <P ALIGN="JUSTIFY">En este estudio se realiz&oacute; la evaluaci&oacute;n de las subpoblaciones de linfocitos T y B en una poblaci&oacute;n de preescolares aparentemente sanos y su relaci&oacute;n con el estado nutricional antropom&eacute;trico y los niveles de zinc s&eacute;rico, como un aporte a la informaci&oacute;n nacional.</P>     <P ALIGN="JUSTIFY">Con relaci&oacute;n al estado nutricional de la poblaci&oacute;n se encontr&oacute; un 9,5% de d&eacute;ficit nutricional y 16,2% de exceso nutricional lo cual muestra asociaci&oacute;n con el estrato socioecon&oacute;mico. Estos valores difieren, como era de esperar, de lo reportado por el Instituto Nacional de Nutrici&oacute;n para el tercer trimestre del a&ntilde;o 2000 (27), en el cual se indica que de la poblaci&oacute;n venezolana entre 2 y 6 a&ntilde;os de edad, el 24,32% lo constituyen ni&ntilde;os en la categor&iacute;a de d&eacute;ficit nutricional y el 10,18% corresponde a aquellos ni&ntilde;os con sobrepeso y obesidad. De igual manera difieren de los valores encontrados en un grupo de preescolares del &aacute;rea marginal de Valencia estudiados por el Centro de Investigaciones en Nutrici&oacute;n de la Universidad de Carabobo, en quienes se report&oacute; que el 14,4% presentaban problemas de malnutrici&oacute;n por d&eacute;ficit y el 21% por exceso (4). Probablemente estas diferencias se deban a que en la poblaci&oacute;n estudiada los ni&ntilde;os proven&iacute;an de hogares de clase media y su riesgo a presentar problemas de malnutrici&oacute;n es menor, debido a que la interacci&oacute;n de factores gen&eacute;ticos y ambientales, en especial aquellos relacionados con la nutrici&oacute;n y con los factores socioecon&oacute;micos, modulan el crecimiento y desarrollo arm&oacute;nico del ni&ntilde;o (28-29), lo que de alguna manera, marca la diferencia cuando se eval&uacute;an poblaciones de diferentes estratos socioecon&oacute;micos. </P>     <P ALIGN="JUSTIFY">La situaci&oacute;n del problema de malnutrici&oacute;n por exceso en los ni&ntilde;os evaluados refleja la problem&aacute;tica nutricional de la regi&oacute;n, del pa&iacute;s y de otros pa&iacute;ses de Latinoam&eacute;rica, debido a los cambios econ&oacute;micos y culturales. Uauy, R. y col. en el a&ntilde;o 2001 refieren que los problemas de sobrepeso y la obesidad son cada vez m&aacute;s comunes en muchos pa&iacute;ses latinoamericanos (30), donde la publicidad y el poder adquisitivo de los padres, unidos al ritmo de vida que ellos enfrentan favorecen la instalaci&oacute;n de h&aacute;bitos de alimentaci&oacute;n que propician cuadros de sobrepeso y obesidad lo que los hace una poblaci&oacute;n de alto riesgo para enfermedades cr&oacute;nicas no transmisibles, tales como la hipertensi&oacute;n arterial, las enfermedades cardiovasculares y algunos tipos de c&aacute;ncer que afectan negativamente la calidad de vida (31-32). </P>     <P ALIGN="JUSTIFY">Se debe tomar en cuenta que todo esto no hace m&aacute;s que favorecer d&iacute;a a d&iacute;a mayores problemas de salud en los ni&ntilde;os obesos, donde muchos sistemas org&aacute;nicos se van deteriorando y entre ellos se encuentra el sistema inmune. Como se mencion&oacute; anteriormente todos los nutrientes juegan un rol crucial para el mantenimiento &oacute;ptimo de la respuesta inmune, tanto que la deficiencia o el exceso cualquiera de ellos conlleva a consecuencias negativas sobre el funcionamiento del sistema inmune y aumenta la susceptibilidad a una gran variedad de pat&oacute;genos (34).</P>     <P ALIGN="JUSTIFY">Con relaci&oacute;n a las variables bioqu&iacute;micas estudiadas se encontr&oacute; que los valores promedios correspondientes al contaje de leucocitos, a las subpoblaciones linfocitarias y a los niveles s&eacute;ricos de zinc se ubicaron dentro de lo establecido como referencia para este grupo poblacional (21,25). Para las variables inmunol&oacute;gicas luego de realizar la distribuci&oacute;n percentilar se encontr&oacute; que los valores correspondientes al p25 y p75 resultaron ser semejantes a los reportados por Hannet, y col. en 1992, sobre datos que fueron obtenidos de estudios en ni&ntilde;os bien nutridos (25). Una posible explicaci&oacute;n a esta coincidencia ser&iacute;a el hecho de que la poblaci&oacute;n de preescolares estudiados pertenec&iacute;a a un estrato socioecon&oacute;mico alto, aparentemente sanos y en buenas condiciones de salud. </P>     <P ALIGN="JUSTIFY">Los percentiles 25 y 75 fueron para los linfocitos T totales (CD3+) 61 y 69%, para los linfocitos T (CD4+) 30 y 38%, para los linfocitos T (CD8+) 23 y 31% y para los linfocitos B (CD20+) 14 y 20% respectivamente y como se mencion&oacute; anteriormente guardan similitud con los valores encontrados por Hannet (25), lo que nos sugiere que dichos valores podr&iacute;an tomarse como referencia para posteriores investigaciones debido a que no existen valores de referencia nacionales. </P>     <P ALIGN="JUSTIFY">El hecho de no encontrar diferencias significativas para las variables inmunol&oacute;gicas estudiadas seg&uacute;n edad ni estado nutricional, difiere de los reportado por otros autores quienes mencionan cambios en las variables inmunol&oacute;gicas con relaci&oacute;n a la edad; encontrando aumento del porcentaje de c&eacute;lulas T, de la poblaci&oacute;n de linfocitos CD8+, disminuci&oacute;n de la poblaci&oacute;n CD4+, de la relaci&oacute;n CD4/CD8 y de la poblaci&oacute;n de linfocitos B(34), igualmente otros autores reportan diferencias a medida que avanza la edad o cuando existe deterioro del estado nutricional (35-36). Una posible explicaci&oacute;n es que esos estudios se han realizado en poblaciones con un rango de edad m&aacute;s amplio o en poblaciones con algunas patolog&iacute;as asociadas; situaci&oacute;n que difiere de la presente investigaci&oacute;n ya que la poblaci&oacute;n estuvo constituida por ni&ntilde;os entre 4 y 6 a&ntilde;os, es decir al grupo de preescolares y en adecuadas condiciones de salud, ausentes de patolog&iacute;as que pueden afectar el estado nutricional y por ende el sistema inmunol&oacute;gico; debido a la estrecha relaci&oacute;n existente entre ellos.</P>     <P ALIGN="JUSTIFY">Con relaci&oacute;n al micronutriente evaluado en el estudio se sabe que la deficiencia afecta de alguna manera la respuesta inmune, a&uacute;n cuando muchos de los mecanismos de inmunomodulaci&oacute;n no se han establecido claramente (37), de all&iacute; la necesidad de evaluar en poblaciones aparentemente sanas los minerales encontrados en peque&ntilde;as proporciones pero de indiscutible importancia para el adecuado funcionamiento del sistema inmune como lo es el zinc. La deficiencia de zinc afecta la funci&oacute;n de los leucocitos; entre ellas, la fagocitosis y la respuesta inmune mediada por linfocitos T, disminuye el tama&ntilde;o del timo, y afecta la hipersensibilidad cut&aacute;nea (38). Estudios en animales han podido demostrar que la deficiencia de zinc tiene su efecto sobre todos los componentes del sistema inmune, afect&aacute;ndose la funcionalidad del sistema incrementando la susceptibilidad a infecciones bacterias, virales y parasitarias. En humanos, datos epidemiol&oacute;gicos soportan que el zinc es un factor importante pero oculto de las disfunciones inmunes. El reconocimiento de la importancia del zinc sobre la ontogenia y funcionalidad del sistema inmune a&uacute;n no esta bien establecido (39). </P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Datos como los encontrados en este estudio contribuyen a reforzar la relaci&oacute;n entre el zinc y el sistema inmune en humanos ya que a pesar de que en la poblaci&oacute;n estudiada el zinc s&eacute;rico se encontraba dentro de lo establecido como valores de referencia, los ni&ntilde;os clasificados como hipozincemicos tuvieron una tendencia a valores promedios menores para cada una de las subpoblaciones de linfocitos T en comparaci&oacute;n con los ni&ntilde;os normozincemicos, lo que permite inferir, tal como lo refiere la literatura revisada, que el zinc juega un rol fundamental en la funci&oacute;n inmune. </P>     <P ALIGN="JUSTIFY">En resumen, el presente estudio permiti&oacute; conocer sobre la situaci&oacute;n inmunol&oacute;gica en ni&ntilde;os aparentemente sanos, adem&aacute;s de aportar valores de subpoblaciones linfocitarias que pueden servir de referencia para estudios futuros en el pa&iacute;s, ya que no existen valores nacionales. Igualmente pone en evidencia que el uso de t&eacute;cnicas actualizadas como es la citometr&iacute;a de flujo en la evaluaci&oacute;n nutricional de individuos sanos contribuye a la mejor evaluaci&oacute;n del funcionamiento del sistema inmune as&iacute; como la necesidad de utilizar indicadores m&aacute;s sensibles en la evaluaci&oacute;n de los estados marginales de deficiencia de zinc en poblaciones, siendo un aporte en el campo de la inmunonutrici&oacute;n orientada desde el punto de vista de la medicina preventiva y no de la medicina curativa. </P>     <P ALIGN="JUSTIFY">Se recomienda continuar estudios similares donde se incluyan un n&uacute;mero mayor de ni&ntilde;os con problemas de malnutrici&oacute;n (d&eacute;ficit o exceso) y fomentar la educaci&oacute;n nutricional en los preescolares como factor de cambio en las futuras generaciones de nuestra poblaci&oacute;n. </P> <B>    <P ALIGN="left">AGRADECIMIENTO</P> </B>    <P ALIGN="JUSTIFY">Al Consejo de Desarrollo Cient&iacute;fico, Human&iacute;stico y Tecnol&oacute;gico de la Universidad de Carabobo (CDCH-UC) por su asistencia financiera.
