El objetivo del estudio fue analizar los cambios del estado nutricional y algunos de sus condicionantes en América Latina en estos últimos años. Se realizó una búsqueda electrónica y manual de artículos publicados entre 1995 y 2005, seleccionándose los que abordaron la situación alimentario nutricional de países latinoamericanos. Adicionalmente, se obtuvieron datos de encuestas nacionales y reportes internacionales incluyendo fundamentalmente información a partir de 1990. Los países han evolucionado hacia diferentes estadios de transición, observándose un incremento de exceso de peso, particularmente en mujeres adultas, en que se supera el 30% y en algunos países llega al 70%. Igualmente preocupante es la situación infantil, donde cinco países superan el 6% de obesidad. Hay una tendencia a disminuir el déficit de peso, siete de nueve países presentaron una reducción de mujeres con bajo peso y el déficit de crecimiento disminuyó generalizadamente. Se observó un aumento de la disponibilidad calórica en 17 de 20 países. La población que vive bajo la línea de pobreza ha disminuido en 9 de 13 países, aunque hay algunos que superan el 60%. Los indicadores de sedentarismo han aumentado, el parque automotriz ha subido en todos los países y algo similar ocurre con televisores y computadores. La evolución del estado nutricional refleja una acción multifactorial donde destacan: aumento de la ingesta calórica, incremento de actividades sedentarias en un entorno más urbanizado promotor de tecnologías que limitan la actividad física y disminución de la inseguridad alimentaria producto del descenso de la pobreza aunque desigualdades sociales persisten.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[The nutrition situation has evolved in different ways in Latin-American countries. The aim of the study was to analyze the nutritional change and some conditions within Latin America in the last years. An electronic and manual search of articles published between 1995 and 2005 was made, selecting those that included the nutritional situation of Latin American countries. Additionally, data was collected from national surveys and international reports including information since 1990. The countries have evolved through different transition stages. For example, an increase of excess of weight, particularly in adult women, which exceeded 30% in some countries also reached up to 70% in others. Equally worrisome was the over 6% obesity level in children in five of the countries surveyed. There is a tendency to diminish the deficit of weight with seven of nine countries displaying a reduction of women with low weight while stunting diminished globally. An increase of the caloric availability in 17 of 20 countries was also observed. The total population that lives under the poverty line has diminished in 9 of 13 countries although some exceeded 60%. Some indicators of sedentary lifestyle have increased, the number of cars has been increasing in all countries and similar figures are recorded with respect to television and personnel computers. The evolution of the nutritional status is reflected through the action of set factors, the most important of which being the increase in caloric ingestion and sedentary activities in an urbanized environment, the promotion of the use of technologies that limit physical activity. En este sentido, la transición nutricional se ha desarrollado de manera concomitante o precedida por cambios demográficos y epidemiológicos (5) y en vista de que los países en vías de desarrollo no son homogéneos - en términos de patrones nutricionales - dados variados factores políticos, socioeconómicos y culturales, la magnitud y el tipo de problemas también es diferente.</span> </p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>Las sociedades urbanas han incorporado estilos de vida sedentarios y un consumo de alimentos ricos en lípidos, azúcar, pobres en fibras y micronutrientes. En este sentido, Latinoamérica, ha cambiado de una condición de alta prevalencia de bajo peso y déficit de crecimiento hacia un escenario marcado por un incremento de la obesidad que acompaña a enfermedades crónicas como las cardiovasculares, diabetes y cáncer (6-9).</span></p>      ]]></body>
<body><![CDATA[<p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>El presente artículo tiene como objetivo analizar la evolución del estado nutricional en la población latinoamericana enfocándose en niños y mujeres, de manera de establecer como se ha desarrollado la transición nutricional. Analiza la variación y prevalencia de condiciones como bajo peso, sobrepeso y obesidad y algunos de sus factores condicionantes.&nbsp;</span></p>      <p align="justify"><b><span style='font-size:10.0pt;font-family:Verdana'>Consideraciones metodológicas&nbsp;</span> </b></p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>Para establecer la transición nutricional en países de Latinoamérica, se seleccionaron áreas temáticas que permitieran visualizar la evolución del problema en los últimos años, particularmente a partir de los noventa. Para esto fueron seleccionados los siguientes tópicos: <i>transición nutricional y estado nutricional </i>empleándose para la búsqueda de información las bases de datos Medline (Pubmed), Lilacs y Scielo<i>. </i>Se utilizó para<i> </i>la búsqueda en Medline los términos:<i> Nutrition (MeSH), Nutrition Transition</i> (término libre), <i>Nutritional Status (MeSH), Nutrition Assessment (MeSH)</i> y las combinaciones mediante conector &quot;AND&quot;. En Lilacs se usó los términos descriptores <i>Transición, Nutricional, Transición Nutricional, Estado Nutricional, Encuestas Nutricionales, Encuestas de Salud y Nutrición</i> y combinaciones mediante &quot;AND&quot; y &quot;OR&quot; cuando correspondió. En Scielo se empleo método similar.</span></p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>Una vez desarrollada la búsqueda se procedió a la selección de los artículos empleando los siguientes criterios: fecha de publicación entre el 01 de Enero de 1995 y el 30 de Julio de 2005, investigaciones con información de países latinoamericanos, incorporación de datos provenientes de muestras de al menos 1000 personas.</span> </p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>Se realizó además búsqueda manual en revistas del área nutricional de alcance local y revisión de informes técnicos para obtener información nutricional y sociodemográfica publicada en el mismo periodo y con criterio similar para encuestas y estudios. En países en los cuales se evaluó tendencia y que no disponían de información de dos mediciones en el periodo antes descrito, se incorporó como referencia inicial el último año previo a 1990 en que se dispuso de datos.</span> </p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>Fue revisada información de la Organización de las Naciones Unidas para la Agricultura y la Alimentación (FAO), Fondo de las Naciones Unidas para la Infancia (UNICEF), Organización Mundial de la Salud (OMS), Comisión Económica para América Latina y el Caribe (CEPAL) y Banco Mundial. Desde el portal <i>Measure DHS</i> fueron obtenidos directamente datos para distintos países para analizar la condición de bajo peso infantil. Los datos sobre estado nutricional en mujeres adultas se obtuvieron de los indicadores nutricionales de las siguientes encuestas: Bolivia <i>ENDSA 1998 y 2003 </i>(10,11), Chile <i>Encuesta Nacional de Salud 2003 </i>(12), Colombia <i>ENDS 1995 y 2000 </i>(13,14), Haití <i>EMMUS II 1995 y III 2000 </i>(15,16), México <i>Encuesta Nacional de Nutrición 1988 y 1999</i> (17,18), Perú <i>ENDES 1996 - 2000 </i>(19,20), Guatemala <i>ENSMI 1995 - 1999 </i>(21,22), Nicaragua <i>ENDESA 1998 - 2001 </i>(23,24) y República Dominicana <i>ENDESA1991 y 1996</i><sup> </sup>(25,26).</span> </p>      <p align="justify"><b><span style='font-size:10.0pt;font-family:Verdana'>Evolución del estado nutricional</span><o:p></o:p></b></p>      <p align="justify"><i><span style='font-size:10.0pt;font-family:Verdana'>El déficit nutricional.</span><o:p></o:p></i></p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>La tendencia del bajo peso al nacer (BPN) ha mostrado diferente patrón de comportamiento en los quinquenios comprendidos entre 1990 y 2002. Entre 1990 y 1995 se observaron discretas disminuciones en Chile, Costa Rica, Venezuela y Trinidad y Tobago mientras Cuba y Nicaragua permanecieron estables. La reducción del BPN se ha observado para la mayoría de los países en el periodo 1995 - 2002 destacando la mostrada por Colombia y Venezuela mientras aumentó en Costa Rica, Trinidad y Tobago y Nicaragua; los dos primeros habían evidenciado disminución en el periodo anterior (<a href="#fig1">figura 1</a>).</span> </p>      <p align="center"><a name="fig1"><img border=0 width=580 height=387 id="_x0000_i1025" src="/img/fbpe/alan/v56n1/art2fig1.jpg"></a>&nbsp; </p>

<body><![CDATA[<p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>Entre los países que poseen al menos 2 mediciones en el tiempo, se destacan las altas prevalencias de emaciación para Guatemala y Haití y una importante disminución de un 54% mostró Brasil, República Dominicana también mostró una fuerte reducción. De manera global, todos los países analizados presentaron una disminución<sup> </sup>(<a href="#tab1">Tabla 1</a>). El déficit de crecimiento (talla) también disminuyó, siendo República Dominicana y Brasil los que evidenciaron las mayores variaciones. Datos de encuestas nacionales desde 1986 a 1999 publicados en el trabajo de Rivera et al. (9) muestran como la prevalencia de retardo en el crecimiento en menores de 3 años ha disminuido en la mayoría de los países a excepción de El Salvador que muestra un leve incremento (dato no mostrado). Sin embargo, esta condición sigue siendo un problema de Salud Pública en vista de que los últimos datos disponibles muestran prevalencias no inferiores a 8% y llegando a más del 40% en Guatemala.</span> </p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>Para los países con información disponible de bajo peso en mujeres adultas (IMC &#8804; 18,5) se observó para la mayoría (7 de 9) una reducción de la prevalencia mientras Perú y Bolivia mostraron leves incrementos. Haití mostró la reducción más significativa evidenciando una variación de 36% en el periodo (<a href="#fig2">figura 2</a>).</span></p>      <p align="center"><i><a name="tab1"><img border=0 width=480 height=650 id="_x0000_i1026" src="/img/fbpe/alan/v56n1/art2tab1.jpg"></a><o:p></o:p></i></p>

<p align="justify"><i><span style='font-size:10.0pt;font-family:Verdana'>Exceso de Peso</span></i></p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>En las mujeres mayores de 15 años se observaron altas prevalencias de sobrepeso y obesidad. Las informaciones analizadas de 8 países muestran que existe una alta prevalencia de exceso de peso en mujeres en edad fértil superando el 30% en los países analizados, llamando la atención que en Paraguay éste supera el 70%. En este país, Chile, y México sólo la condición de obesidad supera el 20% (12)(17)(31).</span> </p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>En los casos en que se contó con al menos dos mediciones se observó una tendencia al aumento en la mayoría de los países (<a href="#fig3">Figura 3</a>). En México la obesidad se duplicó en 10 años mientras en Chile, hay una tendencia ascendente pasando desde un 14% a un 27,3% aunque el incremento en la última década ha sido menor (12)(32)(33).</span></p>      <p align="center"><a name="fig3"><img border=0 width=576 height=500 id="_x0000_i1028" src="/img/fbpe/alan/v56n1/art2fig3.jpg"></a>&nbsp;</p>      
<p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>Respecto a la población infantil, los datos dan cuenta de que Argentina, Chile, Perú, República Dominicana y Uruguay superan el 6% de obesidad. Los países que disponían de dos mediciones en el tiempo mostraron un aumento en las prevalencias de obesidad salvo Uruguay que permaneció estable (<a href="#fig4">figura 4</a>).</span> </p>      ]]></body>

<p align="justify"><b><span style='font-size:10.0pt;font-family:Verdana'>Condicionantes del Estado Nutricional.</span><o:p></o:p></b></p>      <p align="justify"><i><span style='font-size:10.0pt;font-family:Verdana'>Disponibilidad de calorías.</span><o:p></o:p></i></p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>Basado en la información publicada por la FAO (37) para los periodos 1990 -1992 al 2000 - 2002 se observó un aumento de la disponibilidad de calorías. Las mayores alzas ocurrieron en Perú, Haití y Cuba con un 30, 17 y 10% respectivamente. Argentina, Brasil y México por su parte superan las 3000 Kcal/día/persona. Venezuela y Guatemala mostraron reducción (<a href="#tab2">tabla 2</a>). Hasta 1986 la disponibilidad de calorías basada en hidratos de carbono era variable para los distintos países. Sin embargo a partir de este año y hasta 1998 la tendencia fue decreciente para la mayoría de los países exceptuando Argentina, Venezuela y Perú, este último quien presenta la mayor disponibilidad de carbohidratos. Al contrario, el caso de la disponibilidad de calorías por grasas aumentó en la mayoría de los países salvo discretos descensos en Venezuela y Brasil. <p align="justify"><i><span style='font-size:10.0pt;font-family:Verdana'>Contexto socioeconómico y demográfico</span><o:p></o:p></i></p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>Indicadores como el ingreso económico per capita, nivel de pobreza e indigencia, urbanización y tasa de fecundidad, entre otros, son considerados reflejo de la condición de un país en el contexto de los determinantes de salud, nutrición y estilos de vida. El último informe del Banco Mundial (39) indica que los países mejor posicionados en el contexto latinoamericano corresponden a México, Chile, Venezuela, Argentina y Uruguay que se encuentran en el rango de ingreso medio alto, es decir entre U$3.256 y U$10.065. El resto de países se encuentran en el rango de ingreso medio bajo (U$826 - U$3.255) siendo los de menor ingreso en este grupo Colombia, Bolivia y Paraguay. Haití por su parte corresponde a la economía de menor ingreso en la región siendo este inferior a U$ 826. La variación de la población bajo línea de pobreza y de indigencia se muestra en la <a href="#tab3"> Tabla 3</a> para aquellos países que disponían de al menos dos mediciones en el tiempo. La población en condición de pobreza supera el 60% en Honduras, Nicaragua y Guatemala, siendo los dos primeros quienes presentan también el mayor porcentaje de indigencia. En Sudamérica, Perú y Colombia superan el 50% de población pobre. La reducción más importante a pesar de mantener más de un 30% de indigencia se observó en Guatemala. Los menores niveles se presentan en Chile y Costa Rica.</span></p>      ]]></body>

<p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>La urbanización ha sido un factor que se ha vinculado a la transformación de hábitos y estilos de vida. La proporción de población que vive en grandes centros urbanos ha aumentado en todos los países de la región. Estimaciones para el año 2010 muestran que países como Argentina, Chile, Uruguay y Venezuela superarían el 85% de población urbana. La mayor variación relativa desde 1990 hasta la estimación a 2010 la evidencian Haití, Honduras, Paraguay y Bolivia (<a href="#tab4">Tabla 4</a>).</span> </p>      <p align="center"><a name="tab4"><img border=0 width=490 height=865 id="_x0000_i1033" src="/img/fbpe/alan/v56n1/art2tab4.jpg"></a></p>      
<p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>La tasa global de fecundidad (TGF) aporta información relevante por sus repercusiones en el tamaño de la familia y en los espacios intergenésicos entre los hijos (a través de ello podría haber un potencial efecto sobre el estado nutricional). En este sentido, se observó una reducción de la TGF en todos los países pero con ritmo diferente para los quinquenios comprendidos entre 1990 y 2005 (40). La TGF más baja para el quinquenio 2000 - 2005 la presentan Cuba y Trinidad y Tobago (1.6), Chile (2.0), Brasil y Uruguay (2.3). Por el contrario Bolivia, Paraguay y Guatemala superan una TGF de 3.5. Las reducciones más significativas para el periodo 1990 - 2005 la presentan Nicaragua (-1.3), Guatemala (-0.9), Bolivia, Paraguay y Perú (-0.8).</span></p>      <p align="justify"><i><span style='font-size:10.0pt;font-family:Verdana'>Variación en el gasto energético</span><o:p></o:p></i></p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>Dado que no se dispone de información directa del nivel de sedentarismo, se analizó la información referida a condiciones que favorecen la disminución del gasto energético. El análisis se basó en la información sobre uso de automóvil a través de datos de parque automotriz, televisores y computadores.</span> </p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>Para los países que contaban con información sobre parque automotriz se observó un incremento en todos ellos, destacándose que éste supera el 50% en Bolivia, Colombia y Perú (41).</span> </p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>El número de televisores para el periodo 1996 - 2003 también aumentó, solo un país disminuyó el número de ellos y en Chile se evidenció un alza relativa de 87%. Además, Brasil, Colombia, Jamaica y Trinidad y Tobago superan los 300 televisores por mil personas. Todos los países aumentaron el uso de computadores personales, siendo Chile y Trinidad y Tobago quienes presentan el mayor número de estos equipos con 119 y 79,5 computadores por mil personas respectivamente (<a href="#fig6">Figura 6</a>).</span></p>      <p align="center"><a name="fig6"><img border=0 width=443 height=430 id="_x0000_i1034" src="/img/fbpe/alan/v56n1/art2fig6.jpg"></a></p>      
<p align="justify">&nbsp;<b><span style='font-size:10.0pt;font-family:Verdana'>Discusión</span> <o:p></o:p></b></p>      ]]></body>
<body><![CDATA[<p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>Desde el punto de vista nutricional, los países latinoamericanos han evolucionado hacia diferentes estadios de transición expresándose en diferencias en la prevalencia de trastornos nutricionales. La información analizada demuestra un incremento del exceso de peso en todos los grupos y países, particularmente en mujeres adultas. Igualmente preocupante es la situación de niños en los que si bien, no de manera tan marcada, también ha aumentado. Coherentemente con lo anterior hay una tendencia a disminuir las prevalencias de déficit de peso y estatura. Lo anterior no sería reflejo de un factor de manera aislada, sino de un conjunto de ellos, destacándose:</span> </p>      <p class=MsoNormal align="justify"><span style='font-size:10.0pt;font-family:Verdana'>- Un aumento de la ingesta calórica &quot;aparente&quot; que se refleja en todos los análisis de disponibilidad de alimentos por países.</span>&nbsp; </p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>- Un aumento de la proporción de grasas en el total del consumo energético.&nbsp;</span></p>      <p class=MsoNormal align="justify">- <span style='font-size:10.0pt;font-family:Verdana'>Un incremento generalizado del sedentarismo por el uso de tecnologías que lo favorecen.&nbsp;</span> </p>      <p align="justify">- <span style='font-size:10.0pt;font-family:Verdana'>Una disminución de la inseguridad alimentaria debido a un descenso de la población que vive en condiciones de pobreza y especialmente de indigencia, aunque las desigualdades sociales persisten en muchos lugares.</span></p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>El aumento del exceso de peso puede ser uno de los resultados de la globalización, probablemente por la importación cada vez mayor de alimentos del mundo industrializado y la caída en los precios. De esta manera la mayor disponibilidad de alimentos no tradicionales han provocado un cambio en los patrones de consumo de los países, demostrándose una mayor oferta de grasas y aceites a bajo costo que han determinado un incremento de su ingesta (43). En consecuencia, la alimentación tradicional basada en cereales y verduras ha cambiado hacia el consumo de alimentos ricos en grasas, azúcar y productos procesados. Por su parte, la mayor disponibilidad de alimentos a precios más bajos ha permitido que grupos de menores ingresos tengan un mayor acceso a alimentos con alto contenido energético. Así, la disposición para cambiar la dieta y la capacidad de adoptar hábitos alimentarios saludables están supeditadas al acceso económico en aquellos que viven en condiciones de restricción. Una relación inversa entre densidad energética de los alimentos y el costo energético (definido como el costo monetario por unidad de energía) se ha observado donde la dieta basada en cereales refinados, azúcar y grasas es más asequible que la basada en el consumo de carnes magras, pescado, frutas y verduras (44). Esto concuerda con el aumento de la disponibilidad de calorías en el continente a partir de grasa evidenciado entre 1991 y 1998.</span> </p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>También el exceso de peso puede estar determinado por una menor actividad física dado la adopción de estilos de vida más sedentarios en un entorno más urbanizado y que promueve el uso de tecnología que limita dicha actividad. Esta es dependiente del tipo de ocupación, actividades domésticas y recreativas, como el deporte y el ejercicio. Actualmente los niños han disminuido su acceso a actividades deportivas y ejercicio físico asociado con un incremento en el acceso a televisión y video juegos que incrementan en gran medida su susceptibilidad al incremento de peso (45-47).</span></p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>El vivir en condiciones de pobreza también ha sido descrito como un factor predisponente de obesidad aunque la relación pobreza y obesidad difiere de una región a otra y puede estar mediada en parte por el bajo costo de alimentos altamente energéticos (48)(49), pero por otro lado, por las preferencias individuales por alimentos que son culturalmente considerados más apetecidos (50). Se ha reportado asociación de inseguridad de alimentos y exceso de peso en mujeres (51)(52), encontrándose correlación positiva dependiendo de la severidad de la inseguridad de alimentos (53), probablemente también asociado al patrón de actividad. No solo el ingreso de los países en términos de producto bruto per cápita se relaciona con la prevalencia de obesidad sino que también su distribución social. El pertenecer a grupos de nivel socioeconómico más bajos en países de bajo ingreso per cápita confiere un factor protector contra obesidad pero un factor de riesgo para la enfermedad en economías con ingreso medio alto (54). Chile y México han mostrado incrementos en la prevalencia de obesidad en mujeres dando cuenta que aquellos países con mayor desarrollo pero con inequidades sociales adoptan estilos de vida inapropiados que conducen a trastornos nutricionales y a veces de manera más drástica que en países de menor desarrollo económico pero más homogéneos.</span> </p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>En contraparte, la reducción en la prevalencia de BPN, déficit ponderal y de crecimiento también obedece a los cambios y condiciones actuales de los países en términos demográficos y socioeconómicos (55)(56). La baja fecundidad se vincula a la disminución del BPN dado que sería reflejo de mejores condiciones maternas como su estado nutricional y espacios intergenésicos. Así se entiende que el BPN tenga menor prevalencia en Chile que tiene la menor TGF y que ha tenido un desarrollo económico importante en estos últimos años mientras hay mayor prevalencia en países con mayor fecundidad y que han presentado menor desarrollo.</span> </p>      <p align="justify"><span style='font-size:10.0pt;font-family:Verdana'>El déficit ponderal y de crecimiento en población infantil del continente, como el enflaquecimiento de la mujer, pueden haber disminuido producto de la mejoría de las condiciones socioeconómicas como la reducción de la pobreza e indigencia, el mayor poder adquisitivo de la población y la consolidación de programas dirigidos a la población más vulnerable. En los países que no han seguido esta tendencia se podría explicar por las desigualdades e inequidades y por limitaciones en el acceso a servicios y oportunidades sociales (55-57).</span></p>      ]]></body>
