Hay una prevalencia de 1,2%, demostrada en estudios aislados, 2,5% en una serie realizada en población pediátrica y 1,92% en trabajadores de salud. Se desconoce la prevalencia y características epidemiológicas de VHC en el estado Aragua. Se presenta la experiencia de 5 años del Comité de Hepatitis del Estado Aragua (CHA). Objetivos: Caracterizar epidemiológicamente la población infectada por VHC en el Estado Aragua, tomando en cuenta género, edad, estado civil, actividad laboral, antecedente de exposición ocupacional, cirugías previas, transfusiones sanguíneas, hemoderivados, procedencia, genotipo predominante, coinfección por VHB, VIH, VEB, Parvovirus B-19. Materiales y métodos: Encuesta epidemiológica a 101 pacientes con diagnóstico serológico de VHC; rango de edad, 18 - 80 años. Resultados: 60% género masculino, edad promedio 43 años ambos géneros (31,3%), 67,7% casados, provenientes en mayor porcentaje del Municipio Girardot(27,4%). En su mayoría sin profesión definida (80,2%), predominando comerciantes y obreros. De los trabajadores del sector salud (8%), 42% con antecedente de exposición ocupacional. 81,1% asintomáticos en el momento del diagnóstico y 31,8% presentó síntomas inespecíficos. 60,2% refirió cirugías previas; 29,8% transfusiones sanguíneas y, en este grupo, 49% las recibió antes de 1992. Genotipo 1 fue predominante (33,3%); sólo 8% de la población presentó coinfección por VHB, VIH, VEB o Parvovirus B-19. Conclusiones: Predominó el genero masculino, edad promedio 43 años, casados, no profesionales, 60,2% refirió cirugías previas y, en menor porcentaje (29,8%), transfusiones sanguíneas, predominó genotipo 1 y en su mayoría fueron asintomáticos.]]></p></abstract>

The prevalence and epidemiologic characteristics of the HCV infection in the Aragua State are unknown. Here, it is reported the experience of the Hepatitis Committee in the Aragua State for 5 years. Objetives: Carry out the epidemiologic characterization of the HCV infected population of the Aragua State, regarding gender, age, civil status, address, job, history of occupational exposure, previous surgeries, blood transfusion, genotype of the virus, and coinfection with HBV, HIV, EBV or Parvovirus B-19. Materials and methods: Epidemiologic interview applied to 101 patients with serologic diagnosis of HCV, between 18 and 80 years old. Results: The 60 % of infected patients were males, the average age to both genders was of 43 years old (31.3 %), 67.7 % were married, mainly from the Giradot District (27.4 %). Most of this infected patients didnÊt have a specific profession (80.2 %), and consisted predominantly of traders and workers. Of the health care professionals (8 %), 42 % had history of occupational exposition. The 81.1 % of infected patients were asymptomatic at the time of the diagnosis, and 31.8 % showed unspecific symptoms. The 60.2 % had been previously subjected to surgery; 29.8 % to blood transfusion, and 49 % of the patients in this group received the transfusion before 1992. En el mundo  existen más de 170 millones de personas infectadas; de ellas 1,8 millones en  Estados Unidos de Norteamérica<sup>(9</sup>). En estudios aislados realizados en nuestro  país, se estima una prevalencia de aproximadamente 1,2% en la población  general<sup>(2)</sup>; en una serie realizada en pacientes pediátricos hospitalizados se  encontró 2,5 % de los pacientes infectados por VHC <sup>(6)</sup> y en otras series se  estimó que el 1,92 % de los trabajadores de la salud poseen anticuerpos contra  este virus<sup>(2)</sup>.</font></p>     <p align="justify"><font face="Verdana" size="2">Los factores  de riesgo asociados con esta infección son, el uso de drogas endovenosas, uso de  cocaína intranasal, promiscuidad sexual, uso de piercings o tatuajes,  transmisión vertical (materno-fetal), prácticas médicas no seguras y receptores  de transfusiones sanguíneas, ésta última con mayor prevalencia antes de  1990-1992, años en los que comenzó a implementarse las técnicas de screening  para VHC en los bancos de sangre. Así mismo, en un alto porcentaje de casos no  ha podido identificarse el factor de riesgo asociado a esta infección<sup>(12)</sup>.</font></p>     <p align="justify"><font face="Verdana" size="2">Las  investigaciones epidemiológicas hasta los actuales momentos en el país y en el  estado Aragua son muy escasas, existiendo subregistro de datos y desconociéndose  sí nuestra población presenta el mismo comportamiento, en cuanto a factores de  riesgo se refiere, que la población mundial. El conocimiento de estos factores  podrá contribuir a fórmulas encaminadas a disminuir la incidencia de esta  infección en nuestra población, surgiendo así el interés en caracterizar  epidemiológicamente a la población infectada en el estado Aragua, tomando en  cuenta género, edad, estado civil, actividad laboral, antecedente de exposición  ocupacional, cirugías previas, transfusiones sanguíneas o de hemoderivados,  procedencia, genotipo predominante y coinfección por otros virus (VHB, VIH, VEB,  Parvovirus B-19).</font></p>     <p align="justify"><b><font face="Verdana" size="2">MATERIALES  Y MÉTODOS:</font></b></p>     <p align="justify"><font face="Verdana" size="2">Se realizó un  estudio descriptivo de corte transversal aplicando una encuesta epidemiológica a  101 pacientes con diagnóstico serológico de VHC con rango de edad de 18 - 80  años, en un período de tiempo de 5 años (2002 a 2007).</font></p>     ]]></body>
<body><![CDATA[<p align="justify"><b><font face="Verdana" size="2">RESULTADOS:</font></b></p>     <p align="justify"><font face="Verdana" size="2">Al analizar  las características generales de los 101 pacientes con diagnóstico serológico de  VHC (<a href="#tab1">Tabla 1</a>), se encontró que predomina en individuos de género masculino  (60%); estado civil casado (31,3%); (<a href="#tab2">Tabla 2</a>) con edad promedio de 43 años  (67,7%) (<a href="#tab3">Tabla 3</a>).</font></p>     <p align="center"><a name="tab1"> <img border="0" src="/img/fbpe/gen/v63n4/art04tab1.gif" width="508" height="116"></a></p>

<p align="justify"><font face="Verdana" size="2">Del grupo de  pacientes profesionales existe un 8% perteneciente al sector salud y de ellos  42% refirió como antecedente exposición ocupacional. El 81,1% se encontró  asintomático en el momento del diagnóstico y, entre los que refirieron síntomas  en su mayoría fueron inespecíficos (31,8%), seguidos por ictericia (27,2%).</font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="Verdana" size="2">Al indagar  sobre algunos de los factores de riesgo asociados a esta enfermedad se encontró  que (<a href="#tab6">Tabla 6</a>), 60,2% de los pacientes refirió cirugías previas y 29,2 %  antecedente de transfusión de hemoderivados (<a href="#tab7">tabla 7</a>); de ellos 49% los recibió  antes de 1992. <p align="justify"><font face="Verdana" size="2">El genotipo  predominante es el 1 y sólo el 8% de los pacientes presentó coinfección por  otros virus (VIH, VHB, Parvovirus B-19, VEB). Es importante mencionar que en el  100% de los pacientes, sus parejas fueron discordantes en cuanto a infección por  VHC, en el momento del estudio.</font></p>     <p align="justify"><b><font face="Verdana" size="2">DISCUSIÓN:</font></b></p>     <p align="justify"><font face="Verdana" size="2">De acuerdo a  los resultados obtenidos en este estudio podemos decir que en el estado Aragua  la prevalencia de VHC es mayor en hombres que en mujeres, con un promedio de  edad de 43 años, de estado civil casado, probablemente asociado al grupo etario  más predominante. El Municipio Girardot es el área geográfica más afectada por  este virus, estando posiblemente en relación a la densidad poblacional, que es  mayor en esta región, sin embargo, sería importante diseñar un estudio  comparativo entre las distintas poblaciones a fin de confirmar o rechazar esta  observación, así como determinar la causa por la que los comerciantes y los  obreros son la población más afectada. Hasta los actuales momentos no se ha  podido definir -de acuerdo a la bibliografía revisada y apoyado por los  resultados de este estudio-, cual ha sido el factor de riesgo directamente  implicado en la infección por este virus.</font></p>     <p align="justify"><font face="Verdana" size="2">Tal como lo  describen estudios internacionales<sup>(7, 8, 9)</sup> el mayor número de pacientes se  encuentra asintomático en el momento del diagnóstico o refieren síntomas  inespecíficos, reportándose como signo clínico predominante en la población  estudiada la ictericia.</font></p>     <p align="justify"><font face="Verdana" size="2">Algo  importante que se encontró en nuestra población, tal como lo reportaron Alter MJ  y col. en 1995, en cuanto a las transfusiones sanguíneas y de hemoderivados, la  población estudiada se ha comportado tal como la población mundial; ha  disminuido el porcentaje de infectados después de 1992, fecha en que comenzó a  realizarse screening en los bancos de sangre para detectar el virus.</font></p>     <p align="justify"><font face="Verdana" size="2">Un resultado  llamativo es que más del 50% de los pacientes refirió antecedente de cirugías  previas, a pesar de que este no es un factor de riesgo para VHC descrito hasta  ahora. Una proporción de estos pacientes (14,9%) fué transfundido durante la  cirugía. Sin embargo, es poco probable que este haya sido el origen de la  infección en dicho grupo, puesto que no se observó el descenso esperado en el  número de pacientes infectados a partir 1992 tal como en el grupo de pacientes  transfundidos. A pesar de que el género femenino no fue el predominante en la  muestra estudiada (40%), la cesárea fue la intervención quirúrgica más común  (14,4%) entre las intervenciones reportadas; siendo necesario complementar estas  observaciones con estudios posteriores.</font></p>     <p align="justify"><font face="Verdana" size="2">En cuanto al  genotipo del virus presente en la población de pacientes infectados, el genotipo  1 fue el predominante, comportamiento semejante al descrito en la  bibliografía<sup>(9,13, 24)</sup>. Se documentó un porcentaje de coinfección por otros  virus de 8%, hallándose al Parvovirus B-19 en 25% de los casos documentados.</font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="Verdana" size="2">No se puede  descartar otras fuentes o factores de riesgo para adquirir el virus en los  actuales momentos, ya que la biología del mismo no ha sido completamente  dilucidada.</font></p>     <p align="justify"><b><font face="Verdana" size="2"> CONCLUSIONES:</font></b></p>     <p align="justify"><font face="Verdana" size="2">La infección  por VHC predominó en el género masculino, con edad promedio de 43 años, estado  civil casado, no profesionales, 60,2% refirió cirugías previas, fenómeno que  debe ser complementado con estudios posteriores y en menor porcentaje  transfusiones sanguíneas (29,8%).
