Con la técnica de cromatografía líquida de alta presión de intercambio catiónico (HPLC-CE) y utilizando el programa beta-thalassemia Short ProgramR del equipo Variant* Bio Rad, se estudió la frecuencia de hemoglobinopatías en un total de 4000 muestras de pacientes referidos al Laboratorio de Investigación de Hemoglobinas Anormales del Hospital Universitario de Caracas, por presentar problemas de anemia. Se encontró que el 26% de los pacientes con diagnóstico de anemia, obedece a la presencia de hemoglobinopatías. Entre las detectadas, la Hb S es la variante más frecuente, seguida de las variantes C y D. Además, se observó la presencia de Beta Talasemia y su asociación a las hemoglobinas S y C. Debido a la cuantificación del porcentaje de Hb A por el método de HPLC-CE fue posible clasificar a los pacientes doble heterocigotos Hb S-Beta Talasemia: en Hb S-beta+ Tal Tipo 1, Hb S-beta+ Tal Tipo 2, Hb S-beta0 Talasemia. De la misma manera, fueron clasificados los pacientes doble heterocigotos con Hb C-Beta Talasemia. La técnica de HPLC-CE es sensible y precisa en el estudio de un gran número de muestras, lo cual se logra en muy corto tiempo, tiene un alto poder de resolución y reproducibilidad de los resultados. En todo paciente con anemia hemolítica congénita, la detección de estas patologías es de suma importancia para lograr un adecuado monitoreo, establecer un tratamiento precoz, consejo genético y un manejo terapéutico multidisciplinario.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[The hemoglobinopathies are a very heterogeneous group of congenital hemolytic anemias, which includes hemoglobin (Hb) variants, thalassemia and hereditary persistence of fetal hemoglobin (HPFH). The aim of this study was to determine the frequency of hemoglobinopathies using the High Performance Liquid Chromatography (HPLC-CE) technique with the beta-thalassemia Short ProgramR of Variant* Bio Rad. Four thousand blood samples from anemic patients from the Laboratorio de Investigación de Hemoglobinas Anormales, Hospital Universitario de Caracas were studied. Twenty six percent of the anemia patients had hemoglobinopathies. The Hb S was the most frequent variant found, followed by the Hb C and Hb D. Also we observed the association of beta thalassemia with Hb S and Hb C. Aceptado: 17-06-2004.&nbsp;</P> <B>    <P>INTRODUCCI&Oacute;N&nbsp; </P> </B>    <P ALIGN="JUSTIFY">El estudio del gen beta globina ha proporcionado grandes hallazgos para el conocimiento de des&oacute;rdenes gen&eacute;ticos hereditarios, tales como las hemoglobinopat&iacute;as, las cuales comprenden: las variantes de hemoglobina, las talasemias y la persistencia hereditaria de hemoglobina fetal.&nbsp; </P>     <P ALIGN="JUSTIFY">Las variantes de hemoglobina son alteraciones de tipo cualitativo, producidas por mutaciones puntuales o peque&ntilde;as deleciones o inserciones de nucle&oacute;tidos en diferentes regiones de los genes globinas (1). Las talasemias y la persistencia hereditaria de hemoglobina fetal (HPFH) son alteraciones de tipo cuantitativo las cuales afectan la tasa de expresi&oacute;n de los genes beta y gamma globinas, respectivamente. La beta talasemia se ha observado asociada a las variantes de hemoglobinas S, C y a niveles elevados de hemoglobina fetal, conduciendo &eacute;sto a una alta variabilidad cl&iacute;nica en cada grupo de pacientes. Embury y col. en 1994 (2) propusieron clasificar los pacientes doble heterocigotos Hb S-<FONT FACE=Symbol>b</FONT> talasemia de acuerdo al porcentaje de Hb A que presenten: pacientes Hb S-<FONT FACE=Symbol>b</FONT><SUP>+</SUP> talasemia tipo 1, cuyos valores de Hb A oscilan entre 5-15%, pacientes Hb S-<FONT FACE=Symbol>b</FONT><SUP>+</SUP> talasemia tipo 2, cuyos valores de Hb A var&iacute;an entre 20-25% y el grupo de pacientes Hb S-<FONT FACE=Symbol>b</FONT>º talasemia donde no hay producci&oacute;n de Hb A. Este criterio tambi&eacute;n puede ser aplicado en los casos doble heterocigotos Hb C-<FONT FACE=Symbol>b</FONT> Talasemia.&nbsp; </P>     <P></MULTICOL><MULTICOL GUTTER="31" COLS="2"></P>     <P ALIGN="JUSTIFY">Durante m&aacute;s de cuatro d&eacute;cadas Arends y col. (3-7) y Bravo y col. (8) estudiaron la distribuci&oacute;n y frecuencia de las hemoglobinopat&iacute;as en nuestro pa&iacute;s, demostrando que las variantes de hemoglobina S, C y D y la beta talasemia tanto en su forma homocigota, como en la heterocigota compuesta, son las m&aacute;s frecuentes, representando algunas de ellas un problema de salud p&uacute;blica. Estos estudios fueron realizados utilizando t&eacute;cnicas bioqu&iacute;micas convencionales, tales como la electroforesis sobre acetato de celulosa a pH alcalino y sobre citrato agar a pH &aacute;cido, cromatograf&iacute;a de intercambio i&oacute;nico (9) y m&eacute;todos de desnaturalizaci&oacute;n por &aacute;lcalis (10).&nbsp; </P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Con el desarrollo de m&eacute;todos alternativos para el estudio de hemoglobinopat&iacute;as tales como la t&eacute;cnica de Isoelectroenfoque y la t&eacute;cnica de cromatograf&iacute;a l&iacute;quida de alta presi&oacute;n de intercambio cati&oacute;nico (HPLC-CE), esta &uacute;ltima ha resultado ser un m&eacute;todo sensible, espec&iacute;fico y reproducible, convirti&eacute;ndose en una alternativa &uacute;til para el estudio y cuantificaci&oacute;n de hemoglobinas de significancia cl&iacute;nica (11-13). Por tal motivo, en la presente investigaci&oacute;n se introduce la t&eacute;cnica de cromatograf&iacute;a l&iacute;quida de alta presi&oacute;n de intercambio Cati&oacute;nico (HPLC-CE), desarrollada por Tan y col. en 1993 (14) como m&eacute;todo de diagn&oacute;stico de variantes hemoglob&iacute;nicas, siendo &eacute;sta la primera experiencia en nuestro pa&iacute;s. Cabe recordar que la gran mayor&iacute;a de los casos con hemoglobinopat&iacute;as se presentan bajo la forma de emergencias m&eacute;dicas, por lo tanto su identificaci&oacute;n r&aacute;pida y precisa son esenciales para iniciar un tratamiento adecuado, con el fin de disminuir la morbi-mortalidad de estos pacientes.&nbsp; </P> <B>    <P>MATERIALES Y M&Eacute;TODOS&nbsp; </P> </B>    <P ALIGN="JUSTIFY">Se estudiaron 4000 muestras de pacientes referidos al Laboratorio de Investigaci&oacute;n de Hemoglobinas Anormales del Hospital Universitario de Caracas con problemas de anemia. A cada paciente se le extrajo, previa autorizaci&oacute;n, 10 mL de sangre perif&eacute;rica en un tubo con EDTA como anticoagulante. Este estudio fue aprobado por el Comit&eacute; de Bio&eacute;tica del Hospital Universitario de Caracas, siguiendo los lineamientos de la declaraci&oacute;n de Helsinki y del C&oacute;digo de Bio&eacute;tica y Bioseguridad del FONACIT.&nbsp; </P>     <P ALIGN="JUSTIFY">La presencia de las distintas hemoglobinas: Hb A, Hb F, Hb A<SUB>2</SUB>, Hb S y Hb C y su cuantificaci&oacute;n fue determinada utilizando la t&eacute;cnica de Cromatograf&iacute;a l&iacute;quida de alta presi&oacute;n, de intercambio cati&oacute;nico (HPLC-CE), de acuerdo a los procedimientos descritos por Tan y col. en 1993 (14) y utilizando el <FONT FACE=Symbol>b</FONT><I> Tal Short Program<B><SUP>R</B></SUP>* </I>de BioRad. Este sistema consta de componentes anal&iacute;ticos individuales, entre los cuales se encuentran el sistema de bomba (2 bombas de doble pist&oacute;n), el dispensador autom&aacute;tico de muestras y el detector de longitud de ondas, 3 reservorios de reactivos y un tanque de desechos.&nbsp; </P>     <P ALIGN="JUSTIFY">Las muestras fueron inicialmente sometidas a un hemolisado r&aacute;pido, por parte del operador e incorporadas al flujo anal&iacute;tico mediante inyecci&oacute;n autom&aacute;tica, pasando a trav&eacute;s de un cartucho que contiene una resina de intercambio cati&oacute;nico. Entre cada inyecci&oacute;n de las muestras, el dispensador autom&aacute;tico fue lavado con una soluci&oacute;n acuosa a fin de minimizar la posibilidad de mezclar las muestras entre s&iacute;. La elusi&oacute;n y separaci&oacute;n de los diferentes componentes se realiz&oacute; utilizando dos tampones fosfato de sodio, los cuales forman un gradiente de 4 g/L (tamp&oacute;n A) a 14 g/L (tamp&oacute;n B) a pH 6.4. La separaci&oacute;n de las hemoglobinas presentes se obtiene es tan s&oacute;lo 6.5 minutos por muestra y los componentes separados pasan a trav&eacute;s del detector de doble longitud de onda a 415 nm y 690 nm. Los datos de absorbancia son transmitidos desde una unidad de procesamiento central (CPU) y mostrados como un Cromatograma de tiempo real (gr&aacute;fico de tiempo vs absorbancia).&nbsp; </P>     <P></MULTICOL><MULTICOL GUTTER="31" COLS="2"></P> <B>    <P>RESULTADOS&nbsp; </P> </B>    <P ALIGN="JUSTIFY">Por el m&eacute;todo HPLC-CE se encontr&oacute; que un 26% de los pacientes con diagn&oacute;stico de anemia estudiados, obedece a la presencia de hemoglobinopat&iacute;as (<a href="#fig1">Fig. 1</a>). Los resultados obtenidos demuestran, una vez m&aacute;s, que la Hb S es la variante m&aacute;s frecuente en nuestro pa&iacute;s, seguida de las variantes C y la D (<a href="#tab1">Tabla I</a>). La variante Hb S eluye entre la hemoglobina A y la hemoglobina A<font face="Times New Roman" size="3"><sub>2</sub></font><FONT FACE="Caslon224 Bk BT" SIZE=1> </FONT>(<a href="#fig2">Fig. 2</a>). Adem&aacute;s, se observ&oacute; la presencia de Beta Talasemia y su asociaci&oacute;n a las hemoglobinas S y C. Por otro lado, por medio de la cuantificaci&oacute;n del porcentaje de Hb A fue posible clasificar a los pacientes doble heterocigotos Hb S-Beta Talasemia: en Hb S-b<SUP>+</SUP> Tal Tipo 1, Hb S-b<SUP>+</SUP> Tal Tipo 2, Hb S-b<SUP>0</SUP> Talasemia. De la misma manera fueron clasificados los pacientes doble heterocigotos con Hb C-Beta Talasemia (<a href="#tab2">Tabla II</a>).&nbsp;</P>     <P ALIGN="CENTER"><a name="fig1"><IMG SRC="/img/fbpe/ic/v45n4/art03fig1.jpg" WIDTH=288 HEIGHT=163></a></P> <B>

Esta condici&oacute;n present&oacute; niveles variables de Hb F, a pesar de que fue producida por una misma mutaci&oacute;n, como pudo observarse en el estudio de los miembros de cuatro generaciones de una familia de ind&iacute;genas Warao, provenientes de la poblaci&oacute;n de Winiquina del Delta del Orinoco, cuyos valores de HbF cuantificados por HPLC-CE oscilaron entre 3,7-7,7%.&nbsp;</font></P>     <P></MULTICOL><MULTICOL GUTTER="31" COLS="2"></P> <B>    <P><font face="Times New Roman" size="3">DISCUSI&Oacute;N&nbsp;</font> </P> </B>    ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El HPLC-CE es un m&eacute;todo r&aacute;pido, sensible y preciso para la detecci&oacute;n de variantes de hemoglobinas y talasemias. Las ventajas de utilizar este m&eacute;todo automatizado, propuesto en este trabajo, sobre los m&eacute;todos convencionales de electroforesis, cromatograf&iacute;a en micro-columna y la t&eacute;cnica de desnaturalizaci&oacute;n por &aacute;lcalis, son: 1)&nbsp;Permite analizar y cuantificar simult&aacute;neamente las Hb A, Hb A<SUB>2</SUB>  y Hb F, as&iacute; como alguna variante de hemoglobina, en una sola preparaci&oacute;n, lo cual se traduce en un menor tiempo y mayor econom&iacute;a. 2) Permite el estudio de numerosas muestras en un corto per&iacute;odo de tiempo manteniendo un alto nivel de reproducibilidad y precisi&oacute;n. 3) Es el &uacute;nico m&eacute;todo que permite diferenciar a la Hb A<SUB>2</SUB> de la Hb C, obteniendo un patr&oacute;n de resoluci&oacute;n completamente diferente, debido a que con el m&eacute;todo convencional de electroforesis a pH alcalino, &eacute;stas migran en la misma posici&oacute;n, lo que dificulta su detecci&oacute;n. La separaci&oacute;n, en un solo paso, de la Hb C y de la Hb A<SUB>2</SUB> es una gran ventaja, especialmente en poblaciones tan mezcladas como la nuestra donde tienen una alta incidencia (4). Este es el primer reporte en Venezuela donde se encontraron cuatro casos no relacionados de doble heterocigoto Hb C-Beta Talasemia, sin haber requerido del estudio familiar.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Los niveles de hemoglobina A<SUB>2</SUB> son variables de acuerdo a la mutaci&oacute;n que ocasione este trastorno. La utilizaci&oacute;n de este m&eacute;todo con medidas tan precisas, como las obtenidas, permite inferir cu&aacute;l es la mutaci&oacute;n presente lo cual facilita el diagn&oacute;stico molecular cuando &eacute;ste puede ser realizado o ayuda para el consejo gen&eacute;tico y asesor&iacute;a al equipo multidisciplinario que maneja estos casos, permitiendo as&iacute; establecer las clasificaciones de acuerdo a la severidad de la mutaci&oacute;n en </font> <FONT FACE=Symbol>b</FONT><font face="Times New Roman" size="3"><SUP>+</SUP> o </font><FONT FACE=Symbol>b</FONT><font face="Times New Roman" size="3">º.&nbsp;</font> </P>     <P></MULTICOL><MULTICOL GUTTER="31" COLS="2"></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">En estudios poblacionales la t&eacute;cnica de HPLC tiene la ventaja de ser m&aacute;s r&aacute;pida y permite el estudio de un mayor n&uacute;mero de muestras, obteniendo resultados en pocos minutos, a&uacute;n as&iacute; no se deben excluir el uso de los m&eacute;todos convencionales de electroforesis de hemoglobina en vista de que algunos casos ameritan estudios complementarios para su identificaci&oacute;n y as&iacute; poder utilizar la vasta experiencia que se tiene en esta metodolog&iacute;a.&nbsp;</font> </P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">En todo paciente con anemia hemol&iacute;tica cong&eacute;nita, el estudio de la beta talasemia y las variantes de hemoglobinas es sumamente importante para lograr un adecuado monitoreo, de manera de establecer un tratamiento precoz, consejo gen&eacute;tico y un manejo multidisciplinario. Adem&aacute;s, este diagn&oacute;stico es indispensable en los hospitales debido a la gran incidencia de estas enfermedades en la poblaci&oacute;n, especialmente en los estratos sociales bajos y ciudades costeras m&aacute;s pobladas, lo cual conlleva a un alto costo hospitalario y humano. El Laboratorio de Investigaci&oacute;n de Hemoglobinas Anormales, HUC es un centro de referencia para todo el pa&iacute;s y es por ello que se requiere una metodolog&iacute;a sencilla, r&aacute;pida y precisa como se plantea en este estudio. En el manejo terap&eacute;utico de los reg&iacute;menes de hipertransfusi&oacute;n en pacientes drepanoc&iacute;ticos, el HPLC-CE ha sido de gran utilidad ya que la cuantificaci&oacute;n de Hb A y Hb S es inmediata, representando &eacute;sto un gran aporte para estos pacientes, pudiendo reemplazarse casi en su totalidad la Hb S presente, tanto para intervenciones quir&uacute;rgicas como para el manejo de las crisis propias de la enfermedad.&nbsp;</font> </P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El equipo de HPLC-CE (VARIANT <I>Hemoglobin Testing System<B><SUP>R</SUP></B></I> ) es tan vers&aacute;til que no s&oacute;lo dispone del programa </font> <FONT FACE=Symbol>b</FONT><font face="Times New Roman" size="3"><I> Tal Short Program<B><SUP>R</SUP></B></I>, sino tambi&eacute;n de otros programas, tales como el <I>Sickle Cell Short Program<B><SUP>R</SUP></B></I>, el <I>Globin Chains Program<B><SUP>R</SUP></B></I> y el <I>alfa Tal Short Program<B><SUP>R</SUP></B>, </I>los<I> </I>cuales son utilizados para el diagn&oacute;stico de hemoglobinopat&iacute;as en neonatos, en muestras tomadas en el tal&oacute;n del pie sobre papel de filtro, para el an&aacute;lisis de las cadenas globinas y en el diagn&oacute;stico de alfa talasemia, respectivamente.<I>&nbsp;</I> </font> </P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El equipo de HPLC-CE VARIANT tiene la desventaja de que componentes menores de la Hb S y otras variantes de hemoglobina, pueden co-eluir con la Hb A<SUB>2</SUB> y esto puede resultar en un valor de Hb A<SUB>2</SUB> falsamente elevado.&nbsp;</font> </P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Previo a la utilizaci&oacute;n de la t&eacute;cnica de HPLC-CE, numerosos pacientes doble heterocigotos Hb S-</font><FONT FACE=Symbol>b</FONT><font face="Times New Roman" size="3"> talasemia no eran diagnosticados adecuadamente, especialmente aquellos que carec&iacute;an de estudios familiares, siendo catalogados como Hb S homocigota.&nbsp;</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">En Venezuela, Arends y col. en 1998 (15) demostraron que la incidencia del doble heterocigoto Hb S-</font><FONT FACE=Symbol>b</FONT><font face="Times New Roman" size="3"> talasemia jugaba un papel muy importante como modulador del s&iacute;ndrome drepanoc&iacute;tico y determinaron la presencia de los tres diferentes fenotipos Hb S-</font><FONT FACE=Symbol>b</FONT><font face="Times New Roman" size="3"> talasemia en la poblaci&oacute;n. Por este motivo, se hace indispensable realizar un diagn&oacute;stico diferencial entre pacientes drepanoc&iacute;ticos puros y pacientes con drepanocitosis-</font><FONT FACE=Symbol>b</FONT><font face="Times New Roman" size="3"> talasemia, los cuales habitualmente eran catalogados como &quot;Tara drepanoc&iacute;tica sintom&aacute;tica&quot;, por la presencia de Hb A en estos casos. Con la implementaci&oacute;n de la t&eacute;cnica de HPLC-CE este problema fue resuelto, debido a que la cuantificaci&oacute;n de Hb A<SUB>2 </SUB> se realiza autom&aacute;ticamente.&nbsp;</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">El uso del equipo automatizado de HPLC-CE para el estudio de variantes hemoglob&iacute;nicas, es sumamente &uacute;til en estudios poblacionales, centros de tratamiento de pacientes drepanoc&iacute;ticos y en pesquisa neonatal y los m&eacute;todos convencionales siguen jugando un papel importante especialmente en el diagn&oacute;stico de variantes no end&eacute;micas.&nbsp;</font> </P> <B>    ]]></body>
