El objetivo fue relacionar los hallazgos de las IRM y de la artroscopia, y valorar la morfología de los meniscos con rupturas. En el estudio se incluyeron 39 pacientes, de ambos sexos, cuyo rango de edad fue de 13 a 74 años (media: 42,6 años), con y sin antecedentes de trauma, a quienes se le indicó estudio de IRM y artroscopia de la rodilla, debido a clínica de lesión articular. Las IRM fueron analizadas por dos especialistas de manera independiente antes de la artroscopia. Las mediciones de los meniscos medial y lateral se realizaron en cada cuerno meniscal en las imágenes sagitales, en densidad protónica y en supresión grasa. Las IRM detectaron 8 casos de ruptura del menisco lateral de los 11 catalogados por artroscopia y 11 casos de ruptura del medial de los 13 precisados artroscópicamente. La sensibilidad y especificidad de las IRM para las rupturas meniscales laterales fueron 72% y 100%, y para las mediales, 85% y 89%. Las rupturas meniscales afectaron mayormente al cuerno posterior. Las dimensiones del cuerno posterior del menisco lateral fueron mayores en meniscos rotos (altura, 7,1 ± 1,3 mm vs 6,1 ± 0,7 mm, p < 0,05; ancho, 10, 2 ± 1,6 mm vs 8,8 ± 1,3 mm, p < 0,05). La ruptura meniscal condiciona cambios morfológicos particularmente en el cuerno posterior del menisco lateral. La resonancia magnética constituye la técnica de imágenes de elección para el diagnóstico de las lesiones meniscales.]]></p></abstract>

Aceptado: 19-06-2008.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>INTRODUCCI&#211;N&nbsp;</B> </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Las lesiones meniscales constituyen una causa frecuente de dolor e inestabilidad  de la rodilla (1). El sexo masculino es el m&#225;s afectado en lo que se refiere  a dichas lesiones, con un pico de incidencia entre los 30 y 40 a&#241;os (2).  Desde los primeros reportes que sugirieron el uso de las im&#225;genes de resonancia  magn&#233;tica (IRM) para la valoraci&#243;n de los meniscos (3-6), por m&#225;s de 20  a&#241;os esta t&#233;cnica se ha convertido en el m&#233;todo diagn&#243;stico de elecci&#243;n  en pacientes con sospecha de lesiones meniscales. El uso de las IRM ha  permitido una mejor selecci&#243;n de los pacientes que se van a someter a un  procedimiento quir&#250;rgico (7, 8), por ser una t&#233;cnica no invasiva, sin riesgo  para el paciente y con una alta precisi&#243;n diagn&#243;stica. Se ha descrito una  sensibilidad del 95% y una especificidad del 91% de las IRM para las rupturas  (9). La exactitud diagn&#243;stica se ubica alrededor del 90% en algunos estudios  (10, 11). La sensibilidad de las IRM para lesiones de la rodilla es alta;  sin embargo, hay grandes variaciones en sus valores en los estudios publicados  (12); por lo tanto, se deben continuar los esfuerzos para evitar los falsos  negativos y mejorar la exactitud diagn&#243;stica. La sensibilidad para rupturas  meniscales es menor para el menisco lateral que para el menisco medial,  aunque la especificidad es discretamente mayor para las lesiones del lateral  (13, 14). No obstante, suelen existir discrepancias entre el informe imagenol&#243;gico  y el estudio artrosc&#243;pico (15). Se han descrito los factores que condicionan  errores diagn&#243;sticos en las IRM, particularmente en las lesiones del cuerno  posterior del menisco lateral (16), el cual es m&#225;s susceptible de errores  debido a factores anat&#243;micos, como la presencia de la bursa del tend&#243;n  popl&#237;teo y a factores t&#233;cnicos, como los artefactos producidos por las  pulsaciones de la arteria popl&#237;tea y el campo magn&#233;tico. Tambi&#233;n la fibrosis  posterior a intervenciones quir&#250;rgicas dificulta la interpretaci&#243;n.&nbsp; </FONT></P> </MULTICOL> <MULTICOL GUTTER="31" COLS="2">     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> En las lesiones meniscales existe alteraci&#243;n de la morfolog&#237;a, descrita  como irregularidad del menisco, particularmente en las rupturas completas  (17); sin embargo, no se tienen trabajos en los cuales se describan y establezcan  diferencias en cuanto a la morfolog&#237;a meniscal de una manera cuantitativa  entre los meniscos afectados por ruptura y aquellos indemnes. El an&#225;lisis  de los distintos hallazgos o signos de manera conjunta, podr&#237;a evitar que  lesiones menos evidentes pasen inadvertidas. Por lo cual, se establecieron  como objetivos, relacionar los hallazgos de las IRM con los obtenidos en  el procedimiento artrosc&#243;pico, y evaluar la morfolog&#237;a meniscal en los  meniscos rotos y compararlos con aquellos indemnes.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>PACIENTES Y M&#201;TODOS&nbsp;</B> </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Se realiz&#243; un estudio descriptivo-anal&#237;tico, transversal, que se desarroll&#243;  entre junio de 2004 y septiembre de 2005, e incluy&#243; pacientes de ambos  sexos, 26 del sexo femenino (67%) y 13 del sexo masculino (33%), con edades  comprendidas entre 13 y 74 a&#241;os (media, 42,6 a&#241;os), a quienes se les realiz&#243;  procedimiento artrosc&#243;pico de la rodilla y se les practic&#243;, previo a la  intervenci&#243;n artrosc&#243;pica, estudio de resonancia magn&#233;tica de la rodilla.  Los pacientes seleccionados acudieron a la consulta por dolor, tumefacci&#243;n,  limitaci&#243;n funcional y/o bloqueo articular en la rodilla. En el estudio  se incluyeron aquellos individuos cuyo estudio de resonancia magn&#233;tica  se hab&#237;a realizado en el lapso de seis meses previos a la artroscopia.  Se excluyeron aquellos pacientes con antecedentes de cirug&#237;a en la rodilla.  Cada paciente fue informado sobre el protocolo de estudio y se obtuvo su  consentimiento para participar en el mismo.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> A todos los pacientes se les pr&#225;ctico un estudio de IRM con un equipo de  1,5 Tesla (Gyroscan ACS-NT, Philips), sigui&#233;ndose el mismo protocolo en  todos los pacientes obteni&#233;ndose: Im&#225;genes sagitales en las secuencias  en Densidad Prot&#243;nica (DP), aplicando una secuencia DUAL, Turbo-Esp&#237;n-Eco,  con tiempo de repetici&#243;n (TR) de 3540 milisegundos (ms) y tiempo de eco  (TE) de 17 ms, y campo de visi&#243;n de 200 mm, matriz de imagen de 212 x 256,  espesor del corte de 3 mm, y un intervalo &#237;nterseccional de 0,3 mm; im&#225;genes  sagitales y axiales en secuencias de DP con supresi&#243;n grasa (SPIR), con  TR/TE de 2512/17 ms, campo de visi&#243;n de 180 a 200 mm, matriz de imagen  de 179/256, espesor de corte de 4 mm e intervalo de 0,4 mm e im&#225;genes coronales  en secuencias potenciadas T2 con supresi&#243;n grasa, con TR/TE de 3638/70  ms, campo de visi&#243;n de 200 mm, matriz de im&#225;genes de 189/256, y espesor  de corte de 4 mm e intervalo de 0,4 mm. En todos los casos se realizaron  al menos 20 cortes.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>Evaluaci&#243;n de los meniscos&nbsp;</B> </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Los meniscos evaluados mediante IRM, se consideraban normales, si conservaban  su forma e intensidad de se&#241;al. Se catalog&#243; como degeneraci&#243;n intrasustancia,  si la alteraci&#243;n en la intensidad de se&#241;al estaba confinada al interior  del menisco sin afectar la superficie. Se diagnostic&#243; ruptura, cuando hab&#237;a  alteraci&#243;n de la forma y/o una banda de intensidad de se&#241;al anormal que  surcaba todo el espesor meniscal contactando su superficie, y con la identificaci&#243;n  de un fragmento meniscal desplazado. (18,19). Mediante artroscopia se registr&#243;  si hab&#237;a ruptura o no. En ambos casos, se registr&#243; la localizaci&#243;n de la  lesi&#243;n, es decir, si se afect&#243; el menisco medial y/o lateral, y el comprometimiento  de los cuernos anterior y posterior.&nbsp; </FONT></P> </MULTICOL> <MULTICOL GUTTER="31" COLS="2">     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Para la morfolog&#237;a meniscal, se midieron la altura y el di&#225;metro anteroposterior,  expresada en mm, en la porci&#243;n externa de ambos cuernos, tom&#225;ndose los  cortes sagitales m&#225;s centrales y contiguos de cada menisco. Se midi&#243; el  &#225;ngulo formado entre las superficies de cada cuerno meniscal, trazando  una l&#237;nea paralela a la superficie superior y otra a la superficie inferior.  Se determin&#243; el &#237;ndice de la altura sobre ancho meniscal. Tambi&#233;n se evalu&#243;  la presencia de lesiones de los ligamentos cruzados, colaterales y el cart&#237;lago  patelar. Los estudios de IRM fueron interpretados de manera independiente  por dos especialistas en radiolog&#237;a, antes de la realizaci&#243;n de la artroscopia  y sin saber la impresi&#243;n diagn&#243;stica el uno del otro.&nbsp; </FONT></P>     ]]></body>
<body><![CDATA[<P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> El estudio artrosc&#243;pico se realiz&#243; en todos los casos por el mismo equipo  de traumat&#243;logos. Los resultados de la artroscopia constituyeron el est&#225;ndar  de comparaci&#243;n de los hallazgos de la resonancia magn&#233;tica. En las lesiones  meniscales se calcularon los indicadores de la eficacia diagn&#243;stica de  las IRM: sensibilidad, especificidad, valores predictivos y poder diagn&#243;stico  total. Se estableci&#243; la asociaci&#243;n de las lesiones meniscales con el sexo,  antecedente traum&#225;tico y la localizaci&#243;n de la lesi&#243;n.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>An&#225;lisis estad&#237;stico</B>&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Los datos se expresaron como n&#250;meros y proporciones para las variables  categ&#243;ricas. Las variables continuas se expresaron como media &#177; desviaci&#243;n  est&#225;ndar. Se hizo un an&#225;lisis de la data, para determinar el tipo de distribuci&#243;n  y varianza. En caso de distribuci&#243;n normal se aplic&#243; la prueba &#147;<I>t de Student</I>&#148;  para comparar las medias de las variables continuas. Se aplicaron pruebas  no param&#233;tricas (Prueba de Mann-Whitney), cuando los datos no se distribuyeron  normalmente. Se realiz&#243; la prueba chi-cuadrado (</FONT><FONT COLOR="#1f1a17" SIZE="2" FACE="Symbol">c</FONT><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"><SUP>2</SUP>) para establecer las  diferencias entre las variables categ&#243;ricas. Para las variables diagn&#243;sticas  se estimaron los intervalos de confianza con un 95%. Se consider&#243; un valor  de p menor a 0,05 estad&#237;sticamente significativo.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>RESULTADOS&nbsp;</B> </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Se evaluaron 40 articulaciones de la rodilla con IRM y procedimiento artrosc&#243;pico  en los 39 pacientes seleccionados, en un caso la evaluaci&#243;n fue bilateral.  La mayor&#237;a no refiri&#243; antecedente traum&#225;tico de importancia (65%). Todos  los pacientes refirieron dolor en la articulaci&#243;n. La tumefacci&#243;n (21 casos),  la limitaci&#243;n funcional (19 casos) y el bloqueo articular (14 casos) fueron  las manifestaciones m&#225;s frecuentes. Solo dos pacientes refirieron chasquidos.  Las rodillas evaluadas se dividieron por igual en cuanto al lado afectado:  20 rodillas derechas y 20 izquierdas.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>Hallazgos de las IRM&nbsp;</B> </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Las IRM mostraron 8 casos de ruptura del menisco lateral, 9 casos de degeneraci&#243;n  intrasustancia y 23 sin lesiones. La mayor&#237;a de los casos de ruptura (5  de 8) afectaron al cuerno posterior. Los casos de degeneraci&#243;n intrasustancia  afectaron mayormente al cuerno anterior del menisco lateral. Hubo dos casos  de degeneraci&#243;n intrasustancia que se asociaron a ruptura meniscal y en  tres casos la afectaci&#243;n fue de ambos cuernos. En el menisco medial hubo  14 casos de ruptura por IRM, todos los casos interesaban al cuerno posterior.  Hubo 12 casos de degeneraci&#243;n intrasustancia. Se afect&#243; m&#225;s la rodilla derecha,  con 14 casos, que la rodilla izquierda donde hubo 8 casos de ruptura meniscal  (</FONT><FONT COLOR="#1f1a17" SIZE="2" FACE="Symbol">c</FONT><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"><SUP>2</SUP> = 3,6; p = 0,06). Solo 10 pacientes de los 22 (45%) refirieron antecedente  de traumatismo importante.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> En las 40 rodillas evaluadas se precisaron, por artroscopia, 11 casos de  ruptura del menisco lateral y 13 casos del menisco medial. En dos casos  la ruptura afect&#243; ambos meniscos. La ruptura del menisco lateral se relacion&#243;  a sexo, rodilla evaluada y antecedente traum&#225;tico. Hubo 6 (54,5%) casos  del sexo masculino (</FONT><FONT COLOR="#1f1a17" SIZE="2" FACE="Symbol">c</FONT><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"><SUP>2</SUP> = 3,4; p = 0,07). En la rodilla derecha hubo 5 casos  y en la izquierda 6. Se hall&#243; antecedente de traumatismo en 6 casos. No  hubo asociaci&#243;n estad&#237;stica entre la ruptura meniscal lateral y el antecedente  traum&#225;tico.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> De igual forma, la ruptura del menisco medial se distribuy&#243; por sexo, rodilla  evaluada y antecedente traum&#225;tico. Hubo 3 casos del sexo masculino y 10  del sexo femenino. La m&#225;s afectada fue la rodilla derecha, con 10 casos  (76,9%), las diferencias fueron estad&#237;sticamente significativas (</FONT><FONT COLOR="#1f1a17" SIZE="2" FACE="Symbol">c</FONT><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"><SUP>2</SUP> = 5,6;  p &lt; 0,05). En la rodilla izquierda solo se encontraron 3 casos (23,1%).  Solo en 5 (38,5%) casos hab&#237;a antecedente de traumatismo.&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Las IRM detectaron 8 de los 11 casos de ruptura del menisco lateral (<a href="#fig1">Fig.  1</a>). Hubo 3 falsos negativos, en los cuales se encontr&#243; una lesi&#243;n asociada  del menisco medial. En 2 casos de los falsos negativos la lesi&#243;n interes&#243;  al cuerno posterior y en un caso hubo lesi&#243;n asociada del ligamento cruzado  anterior. En el menisco medial las IRM detect&#243; 11 de los 13 casos de ruptura  (<a href="#fig2">Fig. 2</a>). Hubo 2 falsos negativos que fueron catalogados, de acuerdo al  estudio de IRM, como degeneraci&#243;n intrasustancia. Hubo 3 falsos positivos,  en los cuales la lesi&#243;n se localiz&#243; en el cuerno posterior (<a href="#fig3">Fig. 3</a>) y hubo  lesiones asociadas del menisco contralateral o del ligamento cruzado anterior. Sin embargo, los valores no fueron estad&#237;sticamente significativos  en la mayor&#237;a de las variables. Hubo diferencias significativas al comparar  los promedios de los &#237;ndices de altura/anchura del cuerno posterior del  menisco medial (p &lt; 0,05).&nbsp; </FONT></P>     ]]></body>
<body><![CDATA[<P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> <B>DISCUSI&#211;N&nbsp;</B> </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Los hallazgos del presente trabajo con respecto a la sensibilidad y especificidad  de las IRM para las rupturas meniscales, concuerdan con lo hallado por  De Smet y col. (13), los cuales encontraron una sensibilidad menor para  la detecci&#243;n de la ruptura del menisco lateral y una especificidad discretamente  mayor. En el presente estudio la sensibilidad para la detecci&#243;n de ruptura  meniscal lateral fue de 72%, mientras que para la lesi&#243;n del menisco medial  fue de 85%. Los falsos negativos para la ruptura del menisco lateral se  localizaron mayormente en el cuerno posterior mediante artroscopia, lo  cual se corresponde con un estudio reciente que concluye que uno de los  factores que lleva a la no detecci&#243;n de una ruptura meniscal lateral es  su ubicaci&#243;n en el cuerno posterior (20). Por otra parte, se ha descrito  que cuando existen lesiones asociadas, la sensibilidad para la detecci&#243;n  de lesiones meniscales es menor, particularmente para la ruptura del menisco  medial (21). La especificidad para la ruptura del menisco lateral fue de  100% y para la medial de 89%. Contrastan estos resultados con los trabajos  que indican que el menisco lateral, espec&#237;ficamente el cuerno posterior,  es m&#225;s susceptible a errores diagn&#243;stico, que llevan a resultados falsos  positivos (16). En el presente estudio no hubo falsos positivos para la  lesi&#243;n del menisco lateral, solo para la lesi&#243;n del medial. En estos casos  la ruptura meniscal medial se localiz&#243; en el cuerno posterior por IRM y  hubo lesiones asociadas.&nbsp; </FONT></P> </MULTICOL> <MULTICOL GUTTER="31" COLS="2">     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Aunque la artroscopia es considerada el est&#225;ndar de comparaci&#243;n, algunas  rupturas ubicadas en el cuerno posterior pudieran no ser vistas en el estudio  artrosc&#243;pico, debido a su abordaje anterior y la incapacidad de visualizar  directamente toda la superficie meniscal, es decir, hay que considerar  los posibles falsos negativos por artroscopia (22, 23). De cualquier manera  se continua buscando mejorar la sensibilidad y especificidad de la resonancia  magn&#233;tica y en este sentido, el uso de un campo magn&#233;tico mayor, de 3,0  Tesla, mejora la sensibilidad para la detecci&#243;n de lesiones meniscales  (24).&nbsp; </FONT></P> </MULTICOL> <MULTICOL GUTTER="31" COLS="2">     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> Con relaci&#243;n a la morfolog&#237;a meniscal, se ha descrito que el tama&#241;o normal  de los meniscos oscila de 3 a 5 mm de altura (18). En el presente trabajo  se pudo establecer que los valores de altura y anchura de los meniscos  lesionados tienden a ser mayores que en los meniscos sin rupturas. Sin  embargo, esto no se logr&#243; demostrar estad&#237;sticamente en gran parte de las  mediciones. Se pudo destacar que la altura y anchura del cuerno posterior  del menisco lateral fue de mayor magnitud en los meniscos lesionados, aunque  no todas las lesiones fueron de dicho cuerno, la mayor&#237;a de las rupturas  se ubicaron en el cuerno posterior. En el caso del menisco medial no hubo  diferencias en cuanto a altura y anchura del cuerno posterior, donde se  ubicaron los casos de ruptura, pero si hubo diferencias entre los grados  de angulaci&#243;n de las superficies meniscales del cuerno posterior, el cual  fue asiento de todos los casos de ruptura. El aumento de las dimensiones  de los meniscos rotos se debe a la separaci&#243;n de los fragmentos, interposici&#243;n  de l&#237;quido sinovial y proliferaci&#243;n vascular en el &#225;rea perif&#233;rica (6,  17).&nbsp; </FONT></P>     <P ALIGN="justify"><FONT COLOR="#1f1a17" SIZE="2" FACE="Verdana"> La investigaci&#243;n realizada demuestra que la resonancia magn&#233;tica, es la  t&#233;cnica no invasiva m&#225;s &#250;til para el diagn&#243;stico de lesiones meniscales,  con una alta sensibilidad y especificidad. Las rupturas meniscales afectaron  m&#225;s al menisco medial y a los cuernos posteriores de ambos meniscos. El  antecedente de trauma no se asoci&#243; de manera significativa a las lesiones.
