La resistencia bacteriana a los antimicrobianos tiene distribución mundial, ha motivado la introducción de nuevos antibióticos y actualmente hay pocos nuevos en proceso de evaluación clínica. Los ß-lactámicos fueron los de primera aplicación y siguen siendo los de mayor uso tanto para infecciones nosocomiales como las de la comunidad y el mecanismo de resistencia más frecuente es la producción de ß-lactamasas mediante plásmidos o de origen cromosomal. Presentamos los determinantes genéticos de resistencia bacteriana para los ß-lactámicos y los mecanismos de resistencia y así tenemos: 1) Alteración del receptor, las proteínas fijadoras de penicilina (PFP o PBP en inglés). 2) Mecanismos que disminuyen la concentración del antimicrobiano en el interior celular bacteriano. 3) Producción de enzimas inactivantes (ß-lactamasas). Mecanismo presente tanto en bacterias grampositivas como gramnegativas, siendo más frecuente en las últimas. Estas enzimas se han clasificado con diferentes criterios y la más actualizada es la funcional de Bush-Jacoby-Medeiros. Para combatir este tipo de resistencia se han elaborado inhibidores como: ácido clavulánico, sulbactam y tazobactam, los cuales no son totalmente efectivos en la inactivación de todos los tipos de ß-lactamasas.]]></p></abstract>

S.aureus was the first to show resistance to penicillin very soon after the beginning of its use. Bacterial resistance has worldwide distribution and motivated the introduction of new antibiotics. At present, there are few new antimicrobial agents in process of clinical evaluation, b-lactams were the first used antibiotics and they remain the most frequently used for the treatment of infections in hospitals and the community and the most frequent mechanism of resistance is through chromosomal or plasmid transmission b-lactamase production. We discuss the bacterial genetic determinants of resistance and their mechanisms as follow: 1) Changes in the receptor binding capacity: penicillin binding proteins (PBP). 2) Alterations of the mechanisms of entry to the bacterial cell of the antimicrobial agent. 3) Production of inactivating enzymes (b-lactamases). This mechanism is present in both grampositive and gramnegative bacteria, but is most common in the latter. b-lactamases have been classified according to different criteria and the most updated is the functional based named Bush-Jacoby-Medeiros. In order to overcome this type of resistance there have been produced inhibitors such as clavulanic acid, sulbactam and tazobactam. These inhibitors are not effective in all cases because they are unable to inhibit all types of bacterial b-lactamases.]]></p></abstract>

A los pocos a&ntilde;os de su introducci&oacute;n, aparecieron cepas de <I>Staphylococcus aureus</I> resistentes a la penicilina debido a la producci&oacute;n de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasa</FONT><FONT SIZE=3>; &eacute;stas comenzaron a proliferar en los hospitales, y a producir infecciones nosocomiales graves<SUP>(2)</SUP>. Esto condujo pronto a la s&iacute;ntesis de penicilinas resistentes a penicilinasas (meticilina y posteriormente las isoxazolil-penicilinas).</P>     <P ALIGN="JUSTIFY">En 1960, en Europa y en EE.UU.<SUP>(2)</SUP>, poco despu&eacute;s de la s&iacute;ntesis de penicilinas penicilinasa-resistentes, aparecieron cepas resistentes de <I>Staphylococcus aureus</I> a las mismas. Eran cepas de <I>S. aureus</I> meticilino-resistentes (MRSA) que sufrieron cambios estructurales de las proteinas fijadoras de penicilina<SUB>2-a</SUB> (PBP<SUB>2-a</SUB>)<SUP>(3)</SUP>. Hasta mediados de 1960, el <I>Staphylococcus aureus</I> MR fue el pat&oacute;geno resistente m&aacute;s importante<SUP>(4)</SUP>.</P>     <P ALIGN="JUSTIFY">Desde entonces, hay una relaci&oacute;n muy estrecha entre la aparici&oacute;n de resistencia a los </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos y el desarrollo de nuevos antimicrobianos.</P>     <P ALIGN="JUSTIFY">Para ese momento, la infecci&oacute;n por microorganismos gramnegativos como <I>Pseudomonas aeruginosa</I> no exist&iacute;a. En esos a&ntilde;os <I>Escherichia coli</I> era el germen responsable de infecciones por gramnegativos, y se comenzaba a hablar de resistencia en especies como <I>Proteus y Klebsiella<SUP>(4)</SUP>.</P> </I>    <P ALIGN="JUSTIFY">Progresivamente fueron introducidas las penicilinas semisint&eacute;ticas con actividad contra g&eacute;rmenes gramnegativos: la ampicilina (1963), la carbenicilina (1970) y tambi&eacute;n la primera cefalosporina (1964). Posteriormente, aparecieron otras cefalosporinas y penicilinas de espectro expandido. Estos antimicrobianos se constituyeron en f&aacute;rmacos de primera l&iacute;nea por m&aacute;s de una d&eacute;cada hasta el momento de la aparici&oacute;n de bacilos gramnegativos resistentes, debido a la producci&oacute;n de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;</FONT><B><FONT SIZE=3> -lactamasas</B>. La primera </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;</FONT><B><FONT SIZE=3> -lactamasa</B> observada en gramnegativos fue la TEM-1 descrita por primera vez en 1963<SUP>(5)</SUP>.</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">En poco tiempo predominaron las infecciones nosocomiales producidas por bacilos gramnegativos. Fue necesaria la s&iacute;ntesis de nuevas clases de antibi&oacute;ticos </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos resistentes a estas nuevas </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3>. Los g&eacute;rmenes gramnegativos productores de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3> fueron responsables de epidemias en todo el mundo<SUP>(6)</SUP>. A partir de 1978, se introdujeron nuevas clases de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos como las penicilinas anti-pseudomonas y las cefalosporinas de segunda y tercera generaci&oacute;n (1981), los inhibidores de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasa</FONT><FONT SIZE=3> (1984), los monobact&aacute;micos y los carbapenemos (1985). Las cefalosporinas de tercera generaci&oacute;n y los carbapenemos surgieron como una necesidad ante la presencia de bacilos gramnegativos productores de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3> tanto cromosomales como plasm&iacute;dicas<SUP>(6)</SUP>, capaces de inactivar a las cefalosporinas de segunda generaci&oacute;n y a las penicilinas activas contra gramnegativos.</P>     <P ALIGN="JUSTIFY">Las </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3> aparecieron gradualmente, pero es a partir de los a&ntilde;os 80 que se identificaron en forma alarmante<SUP>(7,8,9)</SUP>. Actualmente se describen unas doscientas </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3> entre las cuales se encuentra un gran n&uacute;mero de espectro expandido<SUP>(10)</SUP>, capaces de inactivar los nuevos grupos de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos. Las &uacute;ltimas descritas son producidas por un gran n&uacute;mero de bacterias y son activas contra los carbapenemos; entre las mismas se incluyen las metalo-</FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><SUP><FONT SIZE=3>(11)</SUP>.</P>     <P ALIGN="JUSTIFY">Tambi&eacute;n fueron desarrollados los inhibidores de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3>, los cuales bloquean la actividad de dichas enzimas y representan el mecanismo m&aacute;s espec&iacute;fico desarrollado para evadir la resistencia a los </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos. Esto constituy&oacute; un importante logro pues con estos f&aacute;rmacos se recupera la actividad de los </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos cl&aacute;sicos como ampicilina, amoxicilina, piperacilina, ticarcilina, entre otros. Lamentablemente su eficacia cl&iacute;nica es limitada pues s&oacute;lo son capaces de inactivar algunas </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3> [clase mol. A, grupo 2 de Bush]<SUP>(12)</SUP>.</P>     <P ALIGN="JUSTIFY">Luego sigui&oacute; un periodo caracterizado por una disminuci&oacute;n importante de la s&iacute;ntesis e introducci&oacute;n de nuevos agentes antimicrobianos, acompa&ntilde;ado de un aumento alarmante de la resistencia bacteriana a los antimicrobianos existentes, lo cual arroj&oacute; como resultado, la aparici&oacute;n de serios problemas en la salud p&uacute;blica mundial.</P>     <P ALIGN="JUSTIFY">Los mecanismos de resistencia a los </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos por parte de las bacterias son diversos, tambi&eacute;n lo son sus mecanismos de transmisi&oacute;n<SUP>(13,14)</SUP>; por tanto, (despu&eacute;s de esta visi&oacute;n general de los acontecimientos sobre la resistencia bacteriana a </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos) es pertinente referirnos en forma sucinta a los determinantes gen&eacute;ticos y a los mecanismos de resistencia de los que se valen las bacterias para enfrentarse a los antimicrobianos.</P> <B>     <P ALIGN="JUSTIFY">Determinantes gen&eacute;ticos</P> </B>     <P ALIGN="JUSTIFY">Las bacterias tienen una gran capacidad de resistencia a cualquier f&aacute;rmaco antimicrobiano. Esa resistencia puede ser natural o puede desarrollarse con el uso del antimicrobiano. La variaci&oacute;n gen&eacute;tica es esencial para que ocurra la evoluci&oacute;n bacteriana y los agentes antimicrobianos ejercen una fuerte presi&oacute;n de selecci&oacute;n sobre las poblaciones bacterianas, favoreciendo aquellos organismos que son capaces de resistir al antibi&oacute;tico<SUP>(14,15)</SUP>.</P>     <P ALIGN="JUSTIFY">Las variaciones gen&eacute;ticas pueden ocurrir por diversos mecanismos. Unos puntos de mutaci&oacute;n pueden ocurrir en pares de bases de nucle&oacute;tidos, proceso referido como cambio microevulocionario. Estos &quot;puntos de mutaci&oacute;n&quot; pueden alterar el sitio activo del receptor para un agente antimicrobiano. Un segundo nivel de variabilidad gen&oacute;mica en la bacteria es referido como cambio macroevolucionario y resulta de rearreglos de largos segmentos de ADN; tales rearreglos incluyen: inversiones, duplicaciones, inserciones o transposiciones de larga secuencias de ADN, de un sitio a otro del cromosoma bacteriano. Estos rearreglos, en gran escala y de grandes segmentos de cromosomas, son creados frecuentemente por elementos gen&eacute;ticos especializados conocidos como transposones o secuencias de inserci&oacute;n, que tienen la capacidad de moverse independientemente del resto del cromosoma bacteriano.</P>     <P ALIGN="JUSTIFY">Un tercer nivel de variaci&oacute;n gen&eacute;tica en bacterias es creado por la adquisici&oacute;n de ADN de otra bacteria, siendo transportados por pl&aacute;smidos o transposones<SUP>(13,14)</SUP>.</P>     <P ALIGN="JUSTIFY">Estos mecanismos favorecen a las bacterias y contribuyen con la habilidad de los microorganismos de ganar a la presi&oacute;n de selecci&oacute;n impuesta por los agentes antimicrobianos.</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Esto ayuda a las bacterias a desarrollar resistencia ante cualquier agente antimicrobiano. Una vez que aparece un gen con resistencia a un antimicrobiano, esta resistencia puede esparcirse a otras bacterias por: transformaci&oacute;n, transducci&oacute;n, conjugaci&oacute;n o transposici&oacute;n. De esta forma, los clones favorecidos de bacterias, pueden proliferar en la flora de pacientes expuestos a antibi&oacute;ticos.</P>     <P ALIGN="JUSTIFY">La introducci&oacute;n de los antibi&oacute;ticos en las pasadas cinco d&eacute;cadas, ha provocado la diseminaci&oacute;n de genes de resistencia a los antibi&oacute;ticos por v&iacute;a de elementos gen&eacute;ticos extra-cromosomales y m&oacute;viles: pl&aacute;smidos, transposones y los casets de genes e integrones [los cuales tienen informaci&oacute;n de resistencia para muchos antimicrobianos a la vez]<SUP>(13,14)</SUP>. De igual manera se favorece la resistencia cromosomal. El r&aacute;pido aumento y expansi&oacute;n de la resistencia a antimicrobianos, dentro de una especie y entre especies diferentes<SUP>(16)</SUP> es debida a todos estos mecanismos<SUP>(17)</SUP>.</P> <B>     <P ALIGN="JUSTIFY">Mecanismos de resistencia a </B></FONT><FONT FACE="Times New Roman" SIZE=3>&#946;</FONT><B><FONT SIZE=3> -lact&aacute;micos</P> </B>     <P ALIGN="JUSTIFY">Los antimicrobianos m&aacute;s usados son los </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos; existen varias razones que lo explican: su espectro de actividad antimicrobiana, su seguridad, su eficacia y su baja toxicidad<SUP>(18)</SUP>. Sin embargo, por ser los primeros antibi&oacute;ticos introducidos en cl&iacute;nica, la resistencia bacteriana ante estos f&aacute;rmacos se ha constituido en un problema por m&aacute;s de 40 a&ntilde;os. El desarrollo de resistencia ante los </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos es complejo e incluye algunas adaptaciones bacterianas manifestadas por un limitado n&uacute;mero de mecanismos de resistencia: </P><DIR>      <P ALIGN="JUSTIFY">1)&#9; Alteraci&oacute;n del sitio receptor (PBPs) </P>     <P ALIGN="JUSTIFY">2) Enzimas inactivantes (</FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3>) </P>     <P ALIGN="JUSTIFY">3)&#9;Disminuci&oacute;n de la concentraci&oacute;n del antimicrobiano en el interior de la bacteria.</P> </DIR>      <P ALIGN="JUSTIFY">Se sabe que el mecanismo m&aacute;s importante de resistencia producida ante estos f&aacute;rmacos, es la s&iacute;ntesis de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3>, cuya esencia es la ruptura del anillo </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>mico y como consecuencia su inactivaci&oacute;n.</P>     <P ALIGN="JUSTIFY">El problema de resistencia de microorganismos gramnegativos resistentes a antimicrobianos </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>mico se describe a partir de 1960, cuando aparecieron cepas de <I>Escherichia coli</I> productoras de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><SUP><FONT SIZE=3>(5)</SUP>. Posteriormente tambi&eacute;n se describieron <I>Klebsiella pneumoniae, Haemophilus influenzae</I>, <I>Neisseria gonorrhoeae</I>, resistentes a </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos por producci&oacute;n de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><SUP><FONT SIZE=3>(4)</SUP>.</P>     <P ALIGN="JUSTIFY">Pasaremos a discutir con m&aacute;s detalles los diferentes mecanismos de resistencia ante los </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos. </FONT></P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><FONT SIZE=3><B>Cambios estructurales de las PBPs</P> </B>    <P ALIGN="JUSTIFY">Las PBPs (protein binding penicillin) son prote&iacute;nas fijadoras de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos, las cuales se encuentran en la membrana citoplasm&aacute;tica de las bacterias. Se han descrito numerosas PBPs, de las cuales 6 son las m&aacute;s conocidas, y de ellas son cuatro: 1a, 1b, 2 y 3 las que aparentemente son responsables del mecanismo de acci&oacute;n antimicrobiano de los </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos<SUP>(19)</SUP>.</P>     <P ALIGN="JUSTIFY">Estas PBPs son transpeptidasas responsables del entrecruzamiento, carboxipeptidasas responsables de la elongaci&oacute;n, y endopeptidasas (terminaci&oacute;n), las cuales intervienen en la s&iacute;ntesis y estructuraci&oacute;n de la pared bacteriana. El efecto antibacteriano preciso de un </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>mico depende de la PBP a la cual se una<SUP>(19)</SUP>. As&iacute;, dos </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos diferentes pueden ejercer sus efectos a trav&eacute;s de su afinidad por dos diferentes PBPs y el efecto farmacol&oacute;gico final diferir&aacute;. Los cambios en la estructura de las PBPs, resultan en una sensibilidad disminuida a los antibi&oacute;ticos </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos<SUP>(20)</SUP>. Este mecanismo parece ser com&uacute;nmente el responsable de la resistencia en bacterias grampositivas y especialmente en <I>Staphylococcus aureus</I> resistentes a la meticilina, las cuales producen PBP estructuralmente alteradas con baja afinidad por este antibi&oacute;tico. Se incluyen especies de <I>Streptococcus, Enterococcus</I> y diferentes especies de<I> Staphylococcus </I>con este mecanismo de resistencia.</P>     <P ALIGN="JUSTIFY">La disminuci&oacute;n de afinidad de las PBP media la resistencia en gramnegativos, aunque su influencia es de menor cuant&iacute;a. Entre estos &uacute;ltimos, est&aacute; bien documentada en caso de<I> Neisseria gonorrhoeae y Haemophilus Influenzae</I><SUP>(21)</SUP>.</P>     <P ALIGN="JUSTIFY">Existen reportes de PBP alterada en otros bacilos gramnegativos. El mecanismo est&aacute; relacionado a la emergencia de resistencia de <I>Pseudomonas aeruginosa </I>durante el tratamiento ya que la PBP-3 pierde la afinidad para unirse a penicilinas marcadas<SUP>(22)</SUP>. Tambi&eacute;n, recientemente se describi&oacute; <I>C</I>En esta clasificaci&oacute;n podemos observar la existencia de cepas de <I>Proteus vulgaris, Pseudomonas</I> <I>aeruginosa, Enterobacter cloacae, K. pneumoniae y Citrobacter sp</I>., capaces de producir </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3> mediadas por cromosoma o pl&aacute;smidos, que inactivan las cefalosporinas de espectro expandido, lo que representa un grave problema terap&eacute;utico en pacientes con infecciones nosocomiales. Seg&uacute;n esta clasificaci&oacute;n existen cuatro grupos principales de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3>. Est&aacute;n descritas en el grupo 1 y 2, cefalosporinasas que hidrolizan las cefalosporinas de espectro ampliado y dependiendo de su capacidad de ser inhibidas o no por el &aacute;cido clavul&aacute;nico, pertenecen a diferentes subgrupos. Sobre todo, constituyen un problema aquellas pertenecientes a los grupos 1,3 y 4, las cuales no son inhibidas por los conocidos inhibidores de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3>.</P>     <P ALIGN="JUSTIFY">Recientemente apareci&oacute; una clasificaci&oacute;n denominada funcional, de Bush-Jacoby-Medeiros<SUP>(28)</SUP> (<a href="#tab1">tabla 1</a>), la cual se basa en la clasificaci&oacute;n original de Bush. Algunas de las diferencias son:</P>     <P ALIGN="JUSTIFY">Grupo 1: incluye 32 cefalosporinasas, pobremente inhibidas por el clavulanato, pero el tazobactam tiene mayor actividad inhibitoria<SUP>(12)</SUP>. Su resistencia es cromos&oacute;mica, aunque recientemente se incluyeron algunas mediadas por pl&aacute;smidos y pertenecientes a cepas de <I>K. pneumoniae</I>;<I> s</I>on clase molecular C.</P>     <P ALIGN="JUSTIFY">Grupo 2: es la categor&iacute;a m&aacute;s grande, incluye 138 </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3>. Son de la clase molecular A, a excepci&oacute;n de cloxacilinasas que son clase D (2d). Se incluyeron carbapenemasas cromosomales.</P>     <P ALIGN="JUSTIFY">Grupo 3: son clase molecular B, metalo-</FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3> que hidrolizan un amplio grupo de substratos, incluyendo carbapenemos.</P>     <P ALIGN="JUSTIFY">Grupo 4: est&aacute; formado por un peque&ntilde;o grupo de penicilinasas, no inhibidas por clavulanato y son cromos&oacute;micas.</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">La clasificaci&oacute;n de las </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3> en clases moleculares representa un esquema de clasificaci&oacute;n obvio y de los primeros en aparecer<SUP>(9)</SUP>. Cuatro clases moleculares de acuerdo a su estructura han sido propuestas: <I>Clase A</I>, serina-penicilinasa; <I>clase B</I>, metaloenzima, estas dos identificadas por Ambler; <I>clase C</I>, serina-cefalosporinasa, seg&uacute;n Jaurin y Grundstrom; <I>clase D</I>, serina-</FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3> que hidrolizan la oxacilina, la &uacute;ltima en ser propuesta. Lamentablemente no todas las </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3> descritas tienen identificada su clase molecular.</P>     <P ALIGN="JUSTIFY">En cuanto a la forma como es estimulada la s&iacute;ntesis de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3> se conocen dos tipos: 1) constitutiva 2) inducible, al ser afectada por la exposici&oacute;n al antimicrobiano. La constitutiva mantiene un nivel estable y basal, es independiente del estimulo externo (las </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3> mediadas por pl&aacute;smidos en gramnegativos son com&uacute;nmente constitutivas).</P>     <P ALIGN="JUSTIFY">La </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasa</FONT><FONT SIZE=3> inducible se produce en gran cantidad despu&eacute;s de la exposici&oacute;n a un determinado </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>mico inductor. La inducci&oacute;n por </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos puede aumentar la producci&oacute;n de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasa</FONT><FONT SIZE=3> tanto como mil veces. Las </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3> cromos&oacute;micas de bacterias gramnegativas pueden ser altamente inducibles en presencia de un </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>mico particular. Algunas cefalosporinas de segunda y tercera generaci&oacute;n (cefoxitin, moxalactam, cefotaxima) son resistentes a la hidr&oacute;lisis ante algunas </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3>, pero han demostrado habilidad para inducir la producci&oacute;n de estas enzimas en algunos microorganismos.</P>     <P ALIGN="JUSTIFY">De esta manera, existe un potencial para el antagonismo entre dos </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos, si est&aacute; presente uno que sea fuerte inductor de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasa</FONT><FONT SIZE=3>.</P>     <P ALIGN="JUSTIFY">Otro punto &aacute;lgido en el estudio de las </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamas</FONT><FONT SIZE=3>as es la forma como ocurre la producci&oacute;n de la inducci&oacute;n en la bacteria<SUP>(29)</SUP>. En grampositivos como el <I>Staphylococcus sp.,</I> la </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasa</FONT><FONT SIZE=3> es predominantemente codificada por pl&aacute;smidos y se produce extracelularmente. Aparentemente, el paso al interior de la bacteria del antimicrobiano </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>mico no es necesario para estimular la inducci&oacute;n enzim&aacute;tica; su presencia es registrada por sensores de la cara externa de la membrana citoplasm&aacute;tica y esta informaci&oacute;n llega a los componentes reguladores intracelulares que controlan la expresi&oacute;n de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><SUP><FONT SIZE=3>(29)</SUP>.</P>     <P ALIGN="JUSTIFY">En gramnegativos, la perturbaci&oacute;n de la s&iacute;ntesis de peptidoglicanos de la pared lleva a la acumulaci&oacute;n de sus precursores en el espacio peripl&aacute;smico, produciendo &eacute;stos la se&ntilde;al para la inducci&oacute;n enzim&aacute;tica<SUP>(29)</SUP>. La </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasa</FONT><FONT SIZE=3> es producida en mucha menor cantidad que en grampositivos.</P>     <P ALIGN="JUSTIFY">Es importante se&ntilde;alar que aunque el mecanismo m&aacute;s importante de resistencia a los </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos es la producci&oacute;n de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3>, cualquier microorganismo puede desarrollar m&aacute;s de un mecanismo de resistencia a la vez, pudiendo uno de ellos ser el origen m&aacute;s importante de la expresi&oacute;n de resistencia o ser solo un factor contribuyente que ayuda a la eficacia de la expresi&oacute;n de la misma<SUP>(23)</SUP>.</P>     <P ALIGN="JUSTIFY">La gran y frecuente producci&oacute;n de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasa</FONT><FONT SIZE=3>s como mecanismo de resistencia, condujo a la s&iacute;ntesis y purificaci&oacute;n de sustancias que inhiben su actividad.</FONT></P>     <P ALIGN="center"><B><I><a name="tab1"></a><font size="3">Tabla 1: Esquema de clasificaci&oacute;n funcional para b-lactamasas de Bush-Jacoby-Medeiros*</font></P> </I></B>     <div align="center">       ]]></body>

<body><![CDATA[<P ALIGN="JUSTIFY">B.  cepacia</FONT></TD> </TR> </TABLE>    </center> </div>  <I><FONT SIZE=2>    <P ALIGN="center">*Seg&uacute;n referencia 28</P> </FONT> </I><FONT SIZE=3> <B>    <P ALIGN="JUSTIFY">Inhibidores de </B></FONT><FONT FACE="Times New Roman" SIZE=3>&#946;</FONT><B><FONT SIZE=3>-lactamasas</P> </B>     <P ALIGN="JUSTIFY">El camino m&aacute;s obvio para acabar con la resistencia producida por las </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3> es desarrollar un compuesto que las inactive. Como mencionamos anteriormente, la idea de que un </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>mico pueda inhibir una </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasa</FONT><FONT SIZE=3>, se describi&oacute; con el uso de la meticilina<SUP>(30)</SUP>. Luego algunos otros compuestos han sido estudiados. La penicilina anti-estafiloc&oacute;ccica cloxacilina no es hidrolizada por las </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3> producidas por <I>S.aureus</I>; sin embargo, por su estrecho espectro de inhibici&oacute;n, su uso es restringido. El nuevo concepto de inhibidores espec&iacute;ficos de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3> comenz&oacute; con el reporte del &aacute;cido clavul&aacute;nico en 1976<SUP>(31)</SUP>, siendo &eacute;ste una sustancia aislada del <I>Streptomyces clavuligerus</I>, en el cual el &aacute;tomo de azufre de la penicilina es reemplazado por un &aacute;tomo de ox&iacute;geno. Fue el primero en ser efectivo in vitro e in vivo. Tiene una pobre actividad antibacteriana y su uso es posible al ser combinado con penicilinas y cefalosporinas susceptibles de ser inactivadas por </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3>. Se ha combinado para su uso con amoxicilina y ticarcilina.</P>     <P ALIGN="JUSTIFY">Luego fue sintetizado el sulbactam, mol&eacute;cula semisint&eacute;tica, a partir del &aacute;cido 6-amino penicil&aacute;nico. Tiene actividad antibacteriana, es activo contra <I>N. gonorrhoeae</I> y <I>Acinetobacter sp.</I> Al igual que el &aacute;cido clavul&aacute;nico, tiene utilidad terap&eacute;utica al combinarlo con otro </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lact</FONT><FONT SIZE=3>ámico que sea sensible a la inactivaci&oacute;n por </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3>. El sulbactam es combinado con ampicilina y cefoperazona. El sulbactam y el acido clavul&aacute;nico son excelentes inhibidores de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3>, pertenecientes a los grupos II, III y IV, de Richmond y Sykes o el grupo 2 de Bush. Sulbactam es buen inhibidor de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3> clase C, mostrando mayor actividad que el &aacute;cido clavul&aacute;nico en algunos casos<SUP>(32)</SUP> ante algunas enzimas, pero este &uacute;ltimo tiene mayor permeabilidad y alcanza mayores concentraciones en el espacio peripl&aacute;smico; de esto resulta que los dos son similares en su efecto de inhibici&oacute;n de algunas </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3>. Sin embargo actualmente se conoce que el &aacute;cido clavul&aacute;nico es mayor inductor de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3>.</P>     <P ALIGN="JUSTIFY">En relaci&oacute;n al mecanismo de inhibici&oacute;n de ellos, es a trav&eacute;s de la formaci&oacute;n de un complejo estable, irreversible (</FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasa</FONT><FONT SIZE=3>-Inhibidor de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasa</FONT><FONT SIZE=3>) siendo posteriormente destruida la mol&eacute;cula, tanto la del &aacute;cido clavul&aacute;nico como el sulbactam; por esta raz&oacute;n se denominan inhibidores suicidas<SUP>(31)</SUP>. Se describe un doble mecanismo de acci&oacute;n: reversible e irreversible.</P>     <P ALIGN="JUSTIFY">Se ha descrito la uni&oacute;n del sulbactam a PBPs de gramnegativos, espec&iacute;ficamente a la PBP-1a y esto confiere actividad a bacterias como <I>Escherichia coli, Klebsiella sp, Proteus vulgaris, Acinetobacter sp. (especialmente) y Serratia sp.</P> </I>    <P ALIGN="JUSTIFY">Existe otra mol&eacute;cula similar a estas (clavulanato y sulbactam) en cuanto a su mecanismo de acci&oacute;n, el tazobactam. Dicha mol&eacute;cula tiene mayor actividad que sus antecesores ante </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3> cromosomales (grupo 1, 2a, 2b, 2b’), las cuales hidrolizan a cefalosporinas de tercera generaci&oacute;n, por lo que su espectro es mayor<SUP>(28)</SUP>. Existen evidencias que muestran que se producen reacciones reversibles con cada enzima antes de que ocurra la completa inactivaci&oacute;n<SUP>(31)</SUP>.</P>     <P ALIGN="JUSTIFY">Por otra parte, el clavulanato es inductor de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3> del grupo I, al igual que el sulbactam y el tazobactam que parecen tener menor capacidad de inducci&oacute;n<SUP>(33)</SUP>. Para todas las enzimas clase A, el tazobactam es el m&aacute;s activo. Sin embargo, el clavulanato fue m&aacute;s efectivo inhibiendo las b-LEE, derivadas de las TEM. En relaci&oacute;n con los inhibidores de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><FONT SIZE=3>, es importante conocer el efecto de su uso sobre la evoluci&oacute;n de la resistencia a </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos por parte de los bacilos gramnegativos aer&oacute;bicos; en este sentido, hemos publicado algunos resultados<SUP>(34,35)</SUP>.</P> <B>    <P>Algunas Ideas sobre epidemiolog&iacute;a de la Resistencia</P> </B>    ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">A la hora de examinar las infecciones producidas por bacterias resistentes, es conveniente considerar al hospital y a la comunidad como ecosistemas separados. Esta divisi&oacute;n refleja diferentes poblaciones, presiones de selecci&oacute;n, reservorios y otros factores que son importantes en la aparici&oacute;n, persistencia y transmisi&oacute;n de organismos resistentes a antimicrobianos.</P>     <P ALIGN="JUSTIFY">Es en el ecosistema hospitalario donde tiene relevancia la resistencia de bacilos gramnegativos aer&oacute;bicos. En la actualidad, la responsabilidad de los bacilos gramnegativos productores de infecciones nosocomiales es alta, tanto en EE.UU., como en el resto del mundo<SUP>(36)</SUP>.</P>     <P ALIGN="JUSTIFY">De cuarenta millones de hospitalizados en los Estados Unidos, dos millones padecen la infecci&oacute;n nosocomial y de ellos, del 50% al 60% desarrollan resistencia a antimicrobianos; la mortalidad entre ellos alcanza la cifra de 60.000 a 70.000.</P>     <P ALIGN="JUSTIFY">Los bacilos gramnegativos han aumentado su resistencia, no solo a antimicrobianos </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos, si no tambi&eacute;n a las otras opciones de tratamiento como los aminoglic&oacute;sidos y las quinolonas y las cepas multirresistentes son cada vez m&aacute;s frecuentes<SUP>(13,15,37)</SUP>.</P>     <P ALIGN="JUSTIFY">Adem&aacute;s de la resistencia de los bacilos gramnegativos, existen serios problemas de resistencia por parte de otras bacterias de importancia cl&iacute;nica y responsables de infecciones comunitarias como el <I>Streptococcus pneumoniae</I> y la <I>Neisseria gonorrhoeae</I> frente a la penicilina; y los <I>Staphylococus</I> resistentes a la meticilina, los <I>Enterococus</I> resistentes a la Vancomicina y los <I>Mycobacterium sp. </I>multirresistentes responsables de infecciones nosocomiales<SUP>(38)</SUP>. En pa&iacute;ses del tercer mundo ambos tipos de resistencia son alarmantes; en los pa&iacute;ses desarrollados, el problema se concentra mayormente en la resistencia nosocomial, en caso de enfermos cr&oacute;nicos e inmunosuprimidos.</P>     <P ALIGN="JUSTIFY">Todos estos tipos de resistencia responsables de epidemias nosocomiales o comunitarias y diseminados a trav&eacute;s de todo el mundo<SUP>(13)</SUP>, han llevado a una serie de expertos de diferentes especialidades, a dar la voz de alarma &quot;estamos entrando en la era post-antibi&oacute;tica&quot;<SUP>(37)</SUP>. La r&aacute;pida aparici&oacute;n y diseminaci&oacute;n de microorganismos resistentes a los antimicrobianos constituye un grave problema de salud p&uacute;blica. Esto conduce a prolongar el tiempo de hospitalizaci&oacute;n y al aumento de costos para el paciente y las instituciones de salud<SUP>(36,39,40)</SUP>, adem&aacute;s de aumentar la morbilidad y mortalidad, ya que la resistencia bacteriana incrementa el riesgo de selecci&oacute;n inadecuada de antimicrobianos.</P>     <P ALIGN="JUSTIFY">Esta alarmante situaci&oacute;n ha llevado a elaborar programas de vigilancia de la resistencia bacteriana, regionales y locales, en pa&iacute;ses de la Comunidad Europea<SUP>(41)</SUP> y en Am&eacute;rica<SUP>(36,40,42)</SUP>, donde desde hace algunos a&ntilde;os existen programas con este prop&oacute;sito. Uno de ellos involucra a la Organizaci&oacute;n Mundial de la Salud, llamado «Programa de Vigilancia de la Resistencia Antimicrobiana» y conocido como &quot;WHONET&quot;<SUP>(43)</SUP>. En Venezuela existe un «Programa nacional de Vigilancia de la Resistencia Antimicrobiana» (GVRB) que funciona desde 1988<SUP>(44,45)</SUP>.</P>     <P ALIGN="JUSTIFY">Existen diferencias importantes en los valores de resistencia de g&eacute;rmenes gramnegativos aer&oacute;bicos ante </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos, entre los pa&iacute;ses de la Comunidad Europea. Por otra parte los estudios de resistencia a </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos nacionales<SUP>(46)</SUP> tienen algunas similitudes con los de pa&iacute;ses como Espa&ntilde;a, Portugal y Francia que difieren en forma importante con los resultados de Suecia [los porcentajes de resistencia mas bajos del planeta]<SUP>(47)</SUP>. La relaci&oacute;n hecha<SUP>(48)</SUP> con un estudio de resistencia bacteriana proveniente de instituciones francesas<SUP>(49)</SUP> nos demuestra la necesidad que tenemos de estudios locales; no podemos predecir el grado de resistencia local, basados en resultados de estudios de otros pa&iacute;ses.</P> <B>     <P ALIGN="JUSTIFY">CONCLUSIONES</P> </B>     <P ALIGN="JUSTIFY">La informaci&oacute;n epidemiol&oacute;gica de la resistencia antimicrobiana puede provenir de diferentes or&iacute;genes: vigilancia, investigaci&oacute;n de las epidemias y estudios prospectivos. Los datos de resistencia bacteriana a antimicrobianos obtenidos a trav&eacute;s de un programa de vigilancia, tiene limitaciones. Esta informaci&oacute;n ayuda a identificar las tendencias en la frecuencia de resistencia. Sin embargo, es limitada en lo que se refiere a dar respuestas a factores asociados con la aparici&oacute;n, la persistencia y la transmisi&oacute;n de esa resistencia por las bacterias.</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Existe relaci&oacute;n entre optimizaci&oacute;n sanitaria, higiene, nutrici&oacute;n, nivel y calidad de vida y resistencia bacteriana; adem&aacute;s de las diferencias en cuanto a problemas y practicas de cada pa&iacute;s<SUP>(37,38,42,50,51)</SUP>.</P>     <P ALIGN="JUSTIFY">Es alarmante el aumento de la resistencia a los </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos, hasta 1998<SUP>(44,46,48)</SUP>, ya que son f&aacute;rmacos de primera elecci&oacute;n en el tratamiento de infecciones frecuentes. Los aumentos de resistencia reducen la utilidad de esos antimicrobianos, hasta ahora usados como f&aacute;rmacos de primera l&iacute;nea. Es de hacer notar el hecho de que los bacilos gramnegativos han desarrollado todos los mecanismos de resistencia descritos para los </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactá</FONT><FONT SIZE=3>micos, ante los carbapenemos<SUP>(52)</SUP>, &uacute;ltimo grupo introducido; desde la m&aacute;s sofisticada bomba de eflujo<SUP>(26)</SUP>, cambios en las porinas, cambios en las PBP, hasta la producci&oacute;n de diferentes grupos de </FONT><FONT FACE="Times New Roman" SIZE=3>&#946;-lactamasas</FONT><SUP><FONT SIZE=3>(53)</FONT></SUP><FONT SIZE=3>, incluyendo cromosomales y plasm&iacute;dicas<SUP>(54)</SUP>.</P>     <P ALIGN="JUSTIFY">Existen diferencias de resistencia entre las cepas nosocomiales y las cepas comunitarias<SUP>(48,55)</SUP>. Debemos enfatizar sobre la necesidad de hacer una mayor prevenci&oacute;n en los centros m&eacute;dicos. All&iacute; se concentran una cantidad de variables que no est&aacute;n presentes en la comunidad y que deben ser controladas por el equipo de salud. Esto incluye desde las medidas de asepsia y antisepsia hasta el uso de las dosis y tiempos adecuados del antimicrobiano<SUP>(56,57,58,59,60)</SUP>. Esta es una medida v&aacute;lida en todos los casos, independientemente de su procedencia.</P>     <P ALIGN="JUSTIFY">Aunque es importante mencionar el hecho de que se deben tomar medidas especiales en los hospitales y especialmente en las UCI<SUP>(61)</SUP>, se debe mencionar la mayor resistencia descrita en hospitales grandes (mas de 500 camas), fen&oacute;meno generalmente asociado a hospitales universitarios, donde es relevante la frecuencia de contactos persona a persona<SUP>(62)</SUP>. Este &uacute;ltimo punto explicar&iacute;a en parte, los menores porcentajes de resistencia en infecciones nosocomiales provenientes de cl&iacute;nicas privadas al compararlos con los porcentajes de resistencia de infecciones nosocomiales proveniente de hospitales grandes<SUP>(48,55)</SUP>.</P>     <P ALIGN="JUSTIFY">Por todo lo expuesto es oportuno enfatizar el hecho de que se requiere del esfuerzo de todos los profesionales de la salud involucrados, para controlar este importante problema de salud p&uacute;blica<SUP>(50)</SUP>.</P>     <P ALIGN="JUSTIFY">Teniendo en consideraci&oacute;n que los &uacute;ltimos elementos del problema son los genes de resistencia y sus productos, la aparici&oacute;n de cada gen inicia problemas de resistencia y su diseminaci&oacute;n determina su magnitud<SUP>(51)</SUP>. En este sentido debemos aumentar y divulgar el conocimiento de su presencia y evoluci&oacute;n en nuestras instituciones.</P>     <P ALIGN="JUSTIFY">La resistencia antimicrobiana necesita de monitoreo y de manejo global porque los genes de resistencia y las cepas portadoras viajan entre diferentes pa&iacute;ses. Tambi&eacute;n necesitan de monitoreo y de manejo en cada pa&iacute;s, porque cada pa&iacute;s puede diferir grandemente en sus pr&aacute;cticas y pol&iacute;ticas de control de infecciones; el monitoreo mas importante es el que se hace en cada centro m&eacute;dico.</P>     <P ALIGN="JUSTIFY">Es en cada centro medico donde el equipo de salud debe implementar medidas para prevenir y controlar la aparici&oacute;n y diseminaci&oacute;n de resistencia bacteriana.</P>     <P ALIGN="JUSTIFY">Seg&uacute;n algunos expertos estamos entrando en la era post-antibi&oacute;tica, no solamente por la frecuencia de resistencia de las bacterias ante los diferentes antimicrobianos en uso (mas de doscientos), sino tambi&eacute;n por la ausencia de nuevos antibi&oacute;ticos con nuevos mecanismos de acci&oacute;n. Existen algunos nuevos mecanismos en estudio<SUP>(63,64,65)</SUP> y en fases tempranas de Investigaci&oacute;n<SUP>(66)</SUP>, por lo que tomar&aacute; algunos a&ntilde;os antes de que conozcamos sobre su utilidad terap&eacute;utica.</P>     <P ALIGN="JUSTIFY">La segunda mitad del siglo XX ha sido la edad de oro de los antibi&oacute;ticos, &eacute;stos revolucionaron la terap&eacute;utica de las enfermedades infecciosas y necesitamos que sigan siendo &uacute;tiles en este nuevo siglo; a futuro contaremos con nuevas tecnolog&iacute;as.</P> </FONT> <B><FONT SIZE=3>    ]]></body>
