El objetivo de este trabajo es revisar la presencia de anormalidades en los electro-encefalogramas (EEG) de un grupo de 23 pacientes psicóticos no epilépticos y evaluar las implicaciones en el tratamiento en aquellos casos en que se demuestra su existencia. El 65% de ellos con pobre respuesta al tratamiento previo. A todos se les realizó EEG y se indicó nuevos esquemas de tratamiento durante seis meses. Resultados: 69.6% con EEG anormal, 73.9% recibió anticonvulsivantes (solos o en combinación), el 70.6% de los pacientes estaba asintomático después de seis meses. Existe una asociación significativa de la respuesta terapéutica con EEG anormal y tratamiento con anticonvulsivantes. Esto plantea la posibilidad de que las anormalidades eléctricas cerebrales son un factor etiológico de la pobre respuesta terapéutica en un subgrupo de pacientes con trastornos psicóticos.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Previous studies have shown a significant percentage of electroencephalographic abnormalities in groups of psychiatric non-epileptic patients. The objective of this research was to revise the presence of abnormalities in the electro-encephalograms (EEG) of a group of 23 psychotic non-epileptic patients and to evaluate the implications in the treatment in those cases with abnormal EEG. 65% Of these patients were non-respondent to the previous treatment. EEG was carried out to all of them and a new treatment was indicated during 6 months. Results: 69.6% of patients had abnormal EEG, 73.9% received anticonvulsive therapy alone or in combination, 70.6% of the patients were asymptomatic after six months. A significant association between the therapeutic response, the abnormal EEG and the treatment with anticonvulsive drug was observed. <body><![CDATA[<P><B><font size="3" face="Times New Roman">INTRODUCCI&Oacute;N</font></P> </B>     <P align="justify"><font size="3" face="Times New Roman">Existe en la literatura informaci&oacute;n sobre hallazgos de anormalidades electroencefalogr&aacute;ficas asociadas a los trastornos psiqui&aacute;tricos, que van desde irregularidades inespec&iacute;ficas hasta las mas frecuentes anormalidades focales, parox&iacute;sticas y epileptiformes. Se discute el significado de los hallazgos electroencefalogr&aacute;ficos en los trastornos psiqui&aacute;tricos de pacientes no epil&eacute;pticos. Hay trabajos que plantean la comorbilidad o papel etiol&oacute;gico de dichas irregularidades, las cuales ser&iacute;an interpretadas como expresi&oacute;n de un factor org&aacute;nico vinculado a la cl&iacute;nica de los distintos trastornos pero esto no es sustentado por la mayor&iacute;a de los trabajos. Joyce Small(1), realiz&oacute; una extensa revisi&oacute;n de trabajos cl&iacute;nicos que se&ntilde;alan una asociaci&oacute;n frecuente entre anormales electroencefalogramas y los distintos diagn&oacute;sticos psiqui&aacute;tricos, hasta 50% de los electroencefalogramas (EEG) anormales. Boutros y colaboradores(2) encontraron una significativa frecuencia de anormalidades electroencefalogr&aacute;ficas en pacientes con trastornos psic&oacute;ticos y afectivos. Se describe hasta un 67% de las anormalidades electroencefalogr&aacute;ficas en pacientes psiqui&aacute;tricos en general(3).</font></P>     <P align="justify"><font size="3" face="Times New Roman">Algunos trabajos le asignan un papel de comorbilidad a las anormalidades electroencefalogr&aacute;ficas, lo que establecer&iacute;a la existencia de subgrupos dentro de los diferentes diagn&oacute;sticos, en algunos casos se&ntilde;alan la posible funci&oacute;n etiol&oacute;gica a las actividades el&eacute;ctricas anormales registrada en pacientes con s&iacute;ntomas psic&oacute;ticos y otras manifestaciones psiqui&aacute;tricas. En muchos de esos pacientes los s&iacute;ntomas desaparecieron al recibir tratamiento con anticonvulsivantes(4,5,6,7,8).</font></P>     <P align="justify"><font size="3" face="Times New Roman">Por otro lado, hay trabajos cuyos resultados sustentan la utilidad del EEG como instrumento para identificar pacientes con trastornos psic&oacute;ticos, depresi&oacute;n, demencia y para establecer subgrupos cl&iacute;nicos(9,10). As&iacute; mismo, se ha establecido las modificaciones del EEG que ocurren una vez iniciado el tratamiento.</font></P>     <P align="justify"><font size="3" face="Times New Roman">El objetivo de este trabajo es revisar la presencia de anormalidades electroencefalogr&aacute;ficas en un grupo de pacientes psic&oacute;ticos no epil&eacute;pticos y evaluar las implicaciones en el tratamiento en aquellos casos en que se demuestra su existencia.</font></P> <B>    <P><font size="3" face="Times New Roman">POBLACI&Oacute;N Y M&Eacute;TODO</font></P> </B>     <P align="justify"><font size="3" face="Times New Roman">Se presentan los resultados de 23 pacientes con diagn&oacute;sticos de trastornos psic&oacute;ticos, que formaron parte de un trabajo que se realiz&oacute; con un grupo de 143 pacientes psiqui&aacute;tricos no epil&eacute;pticos y en el cual se incluyeron los pacientes mayores de 16 a&ntilde;os, que sin importar sexo o diagn&oacute;stico psiqui&aacute;trico cumplieran con los siguientes criterios:</font></P>     <P align="justify"><font size="3" face="Times New Roman">1) Pacientes que persist&iacute;an con s&iacute;ntomas psiqui&aacute;tricos a pesar de estar recibiendo tratamiento adecuado al diagn&oacute;stico respectivo.</font></P>     <P align="justify"><font size="3" face="Times New Roman">2) Pacientes que reportaron en su historia antecedentes perinatales que son asociados a posibles secuelas org&aacute;nicas o con antecedentes de convulsiones febriles en la infancia.</font></P>     <P align="justify"><font size="3" face="Times New Roman">3) Pacientes con antecedentes de conducta que pueden ser interpretados como expresi&oacute;n de una disfunci&oacute;n org&aacute;nica (impulsividad, conducta explosiva y agresiva, irritabilidad, conducta Disocial o Disruptiva en la infancia o adolescencia) que no ten&iacute;a relaci&oacute;n con el motivo de consulta.</font></P>     ]]></body>
<body><![CDATA[<P align="justify"><font size="3" face="Times New Roman">Para el estudio se excluyeron aquellos pacientes que ten&iacute;an diagn&oacute;stico de cualquier tipo de epilepsia o trastornos neurol&oacute;gicos.</font></P>     <P align="justify"><font size="3" face="Times New Roman">Los diagn&oacute;sticos se realizaron seg&uacute;n los Criterios Diagn&oacute;sticos de la Clasificaci&oacute;n Internacional de las Enfermedades Mentales de la OMS (CIE-10)(11). Los informes electroencefalogr&aacute;ficos se obtuvieron de varios neur&oacute;logos independientes y los registros de EEG se realizaron con los electrodos colocados en todas las derivaciones del sistema internacional 10-20.</font></P>     <P align="justify"><font size="3" face="Times New Roman">Los electroencefalogramas (EEGs) fueron clasificados como: Difusos, Focales y Paroxisticos difusos.</font></P>     <P align="justify"><font size="3" face="Times New Roman">La respuesta al tratamiento indicado posterior al informe electroencefalogr&aacute;fico fue considerada como positiva cuando los pacientes reportaban ausencia de s&iacute;ntomas despu&eacute;s de seis meses de tratamiento. Si la remisi&oacute;n de los s&iacute;ntomas era parcial se consider&oacute; como respuesta negativa.</font></P>     <P align="justify"><font size="3" face="Times New Roman">Para procesar los datos se utiliz&oacute; la "t" de Student, la Chi cuadrada (X2), Coeficiente de Contingencia, Prueba exacta de Fisher y la Prueba de McNemar para el significado de los cambios. Como nivel de significaci&oacute;n estad&iacute;stica se tom&oacute; una probabilidad igual o menor a 0,05.&nbsp;</font></P> <B>    <P><font size="3" face="Times New Roman">RESULTADOS</font></P> </B>     <P align="justify"><font size="3" face="Times New Roman">El grupo de estudio estuvo constituido por 23 pacientes, con edades entre 19 y 51 a&ntilde;os y una media de 30.39 ± 9.4 a&ntilde;os; 13 varones (56.5%) y 10 mujeres (43.5%).</font></P>     <P align="justify"><font size="3" face="Times New Roman">Se obtuvieron 3 diagn&oacute;sticos de trastornos psic&oacute;ticos:</font></P>     <P align="justify"><font size="3" face="Times New Roman">• Psicosis inducida por abuso de coca&iacute;na (CIE-10 F14.5) • S&iacute;ndrome esquizofreniformes (CIE-10 F20.8) • Psicosis aguda transitoria (CIE-10 F23)&nbsp;</font></P>     <P align="justify"><font size="3" face="Times New Roman">El 69.6% de los pacientes result&oacute; con anormalidades en los EEGs no existiendo diferencias significativas por la edad ni sexo (<a href="#tab1">Tabla 1</a>).</font></P>     ]]></body>
<body><![CDATA[<P align="justify"><font size="3" face="Times New Roman">El 56.3% de los EEGs anormales resultaron focales, 37.5% lento difusos y 6.3% parox&iacute;sticos difuso. Casi dos tercios de las anormalidades estaban localizadas en regi&oacute;n frontal y temporal. Todos los EEGs focales estaban ubicados en estas regiones, de manera estad&iacute;sticamente significativa (X2 = 14.29, df 6, P&lt; .02), (<a href="#tab2">Tabla 2</a>).</font></P>     <P align="justify"><font size="3" face="Times New Roman">En cuanto a lateralidad, la mitad resultaron anormalidades del lado izquierdo, todas ellas de tipo focal pero sin significaci&oacute;n estad&iacute;stica (X2 = 5.6, df 2, p&lt; .06).</font></P>     <P align="justify"><font size="3" face="Times New Roman">Para el momento de realizar el EEG, el 65.2% de los pacientes hab&iacute;a recibido tratamiento previo y el 34.8% no hab&iacute;a recibido tratamiento o asist&iacute;a por primera vez a consulta. Hay una asociaci&oacute;n estad&iacute;sticamente significativa entre anormalidad del EEG y los pacientes que fueron incluidos en el trabajo por tener pobre respuesta al tratamiento previo (test exacto de Fisher = p&lt; .026), (<a href="#tab3">Tabla 3</a>).</font></P>     <P align="justify"><font size="3" face="Times New Roman">Dos tercios de los pacientes hab&iacute;an recibido tratamiento con neurol&eacute;pticos y el otro tercio con combinaciones de varios f&aacute;rmacos.</font></P>     <P align="justify"><font size="3" face="Times New Roman">En este subgrupo, la respuesta terap&eacute;utica al tratamiento previo se clasific&oacute; como sigue: el 73,3% como respuesta parcial porque persist&iacute;an con s&iacute;ntomas o reca&iacute;das y 26,7% no hab&iacute;a mostrado ninguna respuesta.</font></P>     <P align="justify"><font size="3" face="Times New Roman">El 30.4% de los pacientes recibi&oacute; tratamiento &uacute;nico con anticonvulsivantes, 43.5% recibi&oacute; anticonvulsivantes en combinaci&oacute;n con otros medicamentos (neurol&eacute;pticos), 26.1% recibi&oacute; tratamiento &uacute;nico con neurol&eacute;pticos, (<a href="#tab4">Tabla 4</a>).</font></P>     <P align="justify"><font size="3" face="Times New Roman">El 93.8% de los pacientes con EEGs anormales recibi&oacute; tratamiento con anticonvulsivantes (sumando porcentajes de los que recibieron anticonvulsivantes solos o en combinaci&oacute;n) Esta diferencia significativa (X2 = 11.5, df = 2, p&lt;0.003) se&ntilde;ala que la selecci&oacute;n del tratamiento estuvo asociada al resultado del EEG, (<a href="#tab4">Tabla 4</a>).</font></P>     <P align="justify"><font size="3" face="Times New Roman">De todos los pacientes que fueron tratados con anticonvulsivantes (monoterapia o en combinaci&oacute;n) 76.5% recibi&oacute; Carbamazepina, 11.8% Acido Valproico y 11.8% Clonazepam.</font></P>     <P align="justify"><font size="3" face="Times New Roman">Un tercio de los pacientes que hab&iacute;an recibido tratamiento previo, se les cambi&oacute; para monoterapia con anticonvulsivantes y algo m&aacute;s de la mitad se le agreg&oacute; anticonvulsivantes al tratamiento que ven&iacute;an cumpliendo.</font></P>     <P align="justify"><font size="3" face="Times New Roman">El 26.1% de los pacientes se excluy&oacute; de la evaluaci&oacute;n de respuesta terap&eacute;utica por que abandonaron la consulta o no cumplieron con el lapso de seis meses, a pesar de tener buena respuesta al tratamiento.</font></P>     ]]></body>
<body><![CDATA[<P align="justify"><font size="3" face="Times New Roman">El grupo qued&oacute; constituido por 17 pacientes que cumplieron seis meses de tratamiento. El 70.6% result&oacute; con respuesta positiva (asintom&aacute;tico) y el 29.4% con respuesta negativa (persistencia de s&iacute;ntomas), (<a href="#tab5">Tabla 5</a>). No hay diferencias por edad ni sexo. Hay una fuerte asociaci&oacute;n entre respuesta terap&eacute;utica positiva y EEG anormales (83.3%), aun cuando no alcanza significaci&oacute;n estad&iacute;stica. Hay una relaci&oacute;n significativa entre la buena respuesta terap&eacute;utica y el tratamiento con anticonvulsivantes (<a href="#tab5">Tabla 5</a>) (Coeficiente de Contingencia = 0.599, p&lt; .009).</font></P>     <P align="justify"><font size="3" face="Times New Roman">Finalmente, de aquellos pacientes con "respuesta parcial" y "ninguna respuesta" al tratamiento anterior (n = 14), el 71.4% mostr&oacute; respuesta positiva con el nuevo tratamiento. Estas cifras se corresponden con las se&ntilde;aladas por otros trabajos.</font></P>     <P align="justify"><font size="3" face="Times New Roman">No se puede establecer con certeza que relaci&oacute;n existe entre las anormalidades del EEG y las manifestaciones psic&oacute;ticas. Sin embargo, llama la atenci&oacute;n que dos tercios de los EEG anormales, en su mayor&iacute;a focales e irritativos, tienen una localizaci&oacute;n frontal y temporal lo que pudiera establecer una relaci&oacute;n con determinados s&iacute;ntomas afectivos, cognitivos y conductuales presentes en los trastornos psic&oacute;ticos. Hay evidencias de que las anormalidades electroencefalogr&aacute;ficas mas frecuentemente asociadas a s&iacute;ntomas afectivo-conductuales involucran a zonas temporo-parietal y frontal(12,13,14).</font></P>     <P align="justify"><font size="3" face="Times New Roman">En el caso de s&iacute;ntomas psic&oacute;ticos, han sido asociados a las anormalidades en l&oacute;bulo temporal izquierdo(14,15).</font></P>     <P align="justify"><font size="3" face="Times New Roman">De esta forma, las actividades el&eacute;ctricas anormales registradas en subgrupos de pacientes podr&iacute;an ser la causa o explicar la evoluci&oacute;n at&iacute;pica del trastorno psiqui&aacute;trico(16,17,18).</font></P>     <P align="justify"><font size="3" face="Times New Roman">En el caso de pacientes con diagn&oacute;stico de Trastornos Psic&oacute;ticos y de S&iacute;ndrome Esquizofreniforme, con EEGs anormales, se ha asociado con la respuesta al tratamiento con anticonvulsivantes(19,20,21).</font></P>     <P align="justify"><font size="3" face="Times New Roman">Tambi&eacute;n se plantea la existencia de un subgrupo o s&iacute;ndrome esquizofreniforme que eventualmente tiene una parcial etiolog&iacute;a org&aacute;nica. Aquellos casos espor&aacute;dicos o &uacute;nicos en una familia (con poca carga gen&eacute;tica) muestran con mas frecuencia alteraciones electroencefalogr&aacute;ficas asociadas a anormalidades estructurales del SNC, signos neurol&oacute;gicos blandos, pobre respuesta a los neurol&eacute;pticos y otros indicadores que sugieren disfunci&oacute;n o da&ntilde;o org&aacute;nico cerebral(3).</font></P>     <P align="justify"><font size="3" face="Times New Roman">Todos los datos nos aproximan al concepto de "Dimensi&oacute;n Neurol&oacute;gica de la Psiquiatr&iacute;a", vinculado al tema de la comorbilidad en psiquiatr&iacute;a. Fue introducido por Woods y Short en 1985: "alta incidencia de anormalidades neurol&oacute;gicas asociadas a los trastornos psiqui&aacute;tricos mayores y que no pueden ser atribuidas al trastorno psic&oacute;tico "per se" ni a la medicaci&oacute;n ni a una enfermedad neurol&oacute;gica pero que tiene significativas implicaciones en la prognosis del trastorno psiqui&aacute;trico, diagn&oacute;stico etiol&oacute;gico o en la decisi&oacute;n terap&eacute;utica"(3).En muchos casos surge la posibilidad de que las anormalidades electroencefalogr&aacute;ficas expresen la alteraci&oacute;n de la actividad de peque&ntilde;os circuitos o alteraciones de la electrog&eacute;nesis que no tienen la suficiente intensidad para provocar s&iacute;ntomas neurol&oacute;gicos o convulsivos pero pueden ser la causa de s&iacute;ntomas psiqui&aacute;tricos(4,5,6,7,8).</font></P> <B>    <P><font size="3" face="Times New Roman">CONCLUSIONES</font></P> </B>     <P align="justify"><font size="3" face="Times New Roman">La existencia de un &aacute;rea de corteza cerebral irritable explicar&iacute;a la presencia de s&iacute;ntomas psiqui&aacute;tricos en pacientes con trastornos no atribuidos a abuso de sustancias psicoactivas, trastorno mental org&aacute;nico ni asociados con epilepsia. Esto permitir&iacute;a identificar subgrupos dentro de los diagn&oacute;sticos psiqui&aacute;tricos cuya etiolog&iacute;a ser&iacute;a una disfunci&oacute;n el&eacute;ctrica, lo que deriva en implicaciones terap&eacute;uticas y evolutivas.</font></P>     ]]></body>
<body><![CDATA[<P align="justify"><font size="3" face="Times New Roman">La respuesta parcial, evoluci&oacute;n t&oacute;rpida o ausencia de respuesta a un tratamiento considerado adecuado al diagn&oacute;stico puede ser indicaci&oacute;n para la realizaci&oacute;n de un EEG. La presencia de un EEG con anormalidades aunque no exista cl&iacute;nica neurol&oacute;gica o s&iacute;ntomas epil&eacute;pticos presentes o pasados, ser&iacute;a un criterio para la selecci&oacute;n de tratamiento con medicaci&oacute;n anticonvulsivante.</font></P>     <P align="justify"><font size="3" face="Times New Roman">Los resultados muestran una relaci&oacute;n muy estrecha entre el tratamiento con anticonvulsivantes y la remisi&oacute;n total de la sintomatolog&iacute;a en muchos casos, mas a&uacute;n, la mayor&iacute;a de los pacientes con mala respuesta al tratamiento anterior respondieron satisfactoriamente al tratamiento con anticonvulsivantes.</font></P>     <P align="justify"><font size="3" face="Times New Roman">Todos estos resultados nos llevan a concluir que la presencia de anormalidades electroencefalogr&aacute;ficas (aun cuando sean sutiles) puede ser uno de los factores que explican la evoluci&oacute;n t&oacute;rpida de un grupo de pacientes psiqui&aacute;tricos.</font></P>     <P align="justify"><font size="3" face="Times New Roman">Los resultados de este trabajo apoyan el criterio de otros autores que plantean la existencia de subgrupos dentro de los distintos diagn&oacute;sticos psiqui&aacute;tricos en los cuales el EEG servir&iacute;a para identificarlos y nos permite orientar el tratamiento para mejorar el pron&oacute;stico de los mismos.</font></P>     <P align="justify"><font size="3" face="Times New Roman">En todo caso, es necesario la realizaci&oacute;n de nuevas investigaciones que aporten mayor cantidad de evidencias que consoliden o refuercen estas conclusiones.</font></P> <B>    <P><font size="3" face="Times New Roman">REFERENCIAS BIBLIOGR&Aacute;FICAS</font></P> </B>     <!-- ref --><P align="justify"><font size="3" face="Times New Roman">1.
