Catorce pacientes con diagnósticos clínico y radiológico sugestivos de tumor cerebral maligno fueron seleccionados de acuerdo a criterios específicos, lo que incluyó la evaluación según el esquema de Karnofsky, el cual determina el estado funcional independiente del paciente. Se realizó la resección quirúrgica, se efectuó el estudio histopatológico y se tomó una muestra del tumor para cultivo en monocapas. La confluencia celular se alcanzó entre 15 y 45 días y los cultivos fueron expuestos a los citostáticos (1,3-bis(2-cloroetil)-1-nitrosouréa, cisplatino o vincristina por una hora. Se determinó la integridad de la membrana mediante la exclusión del colorante azul de Tripán. Se encontraron diferencias estadísticamente significativas entre las drogas utilizadas y entre los tumores, incluso con el mismo diagnóstico histopatológico, lo cual indica la existencia de resistencia diferencial de las células tumorales a los citostáticos utilizados. Este estudio constituye un ensayo que sustenta la realización de evaluaciones a largo plazo con el fin de emplear una quimioterapia específica en pacientes en los que probablemente se combine ésta con la radioterapia.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Malignant glioma cell cultures have been done for studying the biology of the tumors, the efficiency of antineoplasic drugs, and for determining the differential sensitivity to cytostatics. Fourteen patients with clinical and radiological diagnostics of malignant brain tumor were selected according to specific criteria, including the evaluation by Karnofsky scale, which determines the functional independence of the patient. After surgical resection of tumors, histopathological study was done and a sample was taken for monolayer cultures. Confluence was reached 15 to 45 days after plating and the cultures were exposed to (1,3-bis(2-chloroethyl)-1-nitrosourea, cisplatin or vincristine for one hour. Integrity of cell membranes was evaluated by the exclusion of Tripan blue. There were statistical significant differences between drugs and between the tumors, including those with the same histophatological diagnostic, indicating a differential resistance of the tumoral cells to the cytostatics used. The results of this study support the viability for further evaluations in order to suggest specific antineoplasic treatments probably in combination with radiotherapy for patients having malignant brain tumors.]]></p></abstract>

Estos tumores est&aacute;n compuestos por poblaciones malignas y variadas de c&eacute;lulas, entre ellas c&eacute;lulas no malignas<SUP>(1,2)</SUP>. La interacci&oacute;n con el portador y entre las c&eacute;lulas del tumor no puede ser completamente reproducida en estudios en cultivo<SUP>(3)</SUP>. Sin embargo, se han utilizado tres t&eacute;cnicas principales para el estudio de gliomas <I>in vitro</I>: cultivo de &oacute;rganos, que consiste en la implantaci&oacute;n de fragmentos del tumor en una matriz inerte<SUP>(4,5)</SUP>; cultivos tridimensionales en forma de esferoides<SUP>(6,7)</SUP>; y cultivos en monocapas que han sido usados en varios estudios<SUP>(8,9)</SUP>.</P>     <P ALIGN="JUSTIFY">La resistencia a la administraci&oacute;n de la quimioterapia es uno de los mayores problemas en el tratamiento de los gliomas<SUP>(10)</SUP>. Con el objeto de determinar la resistencia diferencial a las drogas se han dise&ntilde;ado t&eacute;cnicas de ensayos <I>in vitro</I> para evaluar la sensibilidad espec&iacute;fica cuantitativa de c&eacute;lulas en cultivo. Estas determinaciones han sido sugeridas como indicadoras de la respuesta <I>in vivo</I> y posiblemente como una directriz en la escogencia de la quimioterapia selectiva<SUP>(3,11)</SUP>. Existen estudios que se&ntilde;alan que este tipo de tumores responde a la radioterapia m&aacute;s que a la quimioterapia<SUP>(12)</SUP>, sin embargo, se plantea una terapia combinada con radio y quimioterapia. De manera que ser&iacute;a deseable contar con una orientaci&oacute;n en relaci&oacute;n a la sensibilidad espec&iacute;fica de un tumor particular a ciertas drogas.</P>     <P ALIGN="JUSTIFY">Adem&aacute;s del tratamiento con radioterapia y con quimioterapia, enfoques inmunol&oacute;gicos tambi&eacute;n han sido propuestos para el tratamiento de estos tumores<SUP>(13,15)</SUP>, incluso, una de las aproximaciones terap&eacute;uticas m&aacute;s recientes consiste en el uso de inhibidores de la cinasa proteica C, la cual se encuentra elevada en las l&iacute;neas celulares de gliomas malignos<SUP>(16)</SUP>. Las nuevas alternativas son de especial inter&eacute;s para tumores no abordables de forma quir&uacute;rgica que, debido a su mal pron&oacute;stico, y a la debilitaci&oacute;n de la quimioterapia cl&aacute;sica, ameritan la b&uacute;squeda de tratamientos diferentes.</P>     <P ALIGN="JUSTIFY">Algunos de los estudios realizados predicen una respuesta cl&iacute;nica a agentes quimioterap&eacute;uticos cl&aacute;sicos del orden del 50 al 70%<SUP>(3,17)</SUP>. Sin embargo, las aproximaciones experimentales presentan m&uacute;ltiples problemas tanto t&eacute;cnicos como te&oacute;ricos. Por otro lado, aun conscientes de las limitaciones de los cultivos celulares, estos podr&iacute;an ser de utilidad cl&iacute;nica ya que no existe un estudio prospectivo que verifique a mediano plazo la relaci&oacute;n entre la sensibilidad <I>in vitro</I> y la respuesta <I>in vivo</I>. Por esta raz&oacute;n el objetivo de este trabajo consisti&oacute; en la estandarizaci&oacute;n de t&eacute;cnicas de cultivo en monocapas y la evaluaci&oacute;n de la sensibilidad a citost&aacute;ticos mediante la determinaci&oacute;n de la integridad de la membrana con azul de Trip&aacute;n como abordaje inicial para un estudio a largo plazo que cubra aspectos b&aacute;sicos y cl&iacute;nicos del tratamiento de los gliomas.</P> <B>    <P ALIGN="JUSTIFY">PACIENTES Y M&Eacute;TODOS</P>     <P ALIGN="JUSTIFY">Pacientes y toma de la muestra</P> </B>    <P ALIGN="JUSTIFY">Los pacientes seleccionados cumplieron los siguientes criterios: diagn&oacute;sticos cl&iacute;nico y radiol&oacute;gico altamente sugestivos de tumor cerebral maligno, menores de 60 a&ntilde;os, sin patolog&iacute;a m&eacute;dica incapacitante, sin tratamiento previo, buen estado neurol&oacute;gico evidenciado por tomograf&iacute;a axial computarizada de cr&aacute;neo y en algunos casos de resonancia magn&eacute;tica nuclear, confirmaci&oacute;n histol&oacute;gica mediante biopsia extempor&aacute;nea, con evaluaci&oacute;n m&eacute;dica preoperatoria satisfactoria, y sin alteraciones en las pruebas de rutina de laboratorio. Las evaluaciones se realizaron en el Hospital Miguel P&eacute;rez Carre&ntilde;o. Se utiliz&oacute; el esquema de Karnofsky, el cual eval&uacute;a el estado funcional del paciente, y se consider&oacute; incluir un puntaje no menor de 60, que consiste en s&oacute;lo requerir asistencia ocasional. Se realiz&oacute; la resecci&oacute;n quir&uacute;rgica en forma radical y con la precauci&oacute;n de evitar la producci&oacute;n o el empeoramiento de las deficiencias neurol&oacute;gicas. En la toma de los fragmentos de tejido para cultivo se evitaron zonas de necrosis o edema perif&eacute;rico, luego se lavaron con abundante soluci&oacute;n fisiol&oacute;gica est&eacute;ril y fueron colocados en frascos con soluci&oacute;n Hank libre de calcio y magnesio en presencia de 20 U/ml de penicilina cristalina y mantenidos en una estufa a 37°C hasta la ma&ntilde;ana siguiente cuando fueron trasladados al Laboratorio. Se obtuvieron catorce muestras de tumores, cuatro fueron utilizadas para la estandarizaci&oacute;n inicial, dos se contaminaron en el traslado y ocho fueron evaluadas mediante las pruebas de sensibilidad.</P> <B>    ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Preparaci&oacute;n y realizaci&oacute;n del cultivo</P> </B>    <P ALIGN="JUSTIFY">Los fragmentos fueron colocados en una c&aacute;psula de Petri, seccionados con tijera y sometidos a digesti&oacute;n enzim&aacute;tica en una soluci&oacute;n de tripsina al 0.25% en Medio M&iacute;nimo Esencial Eagle (MEM). Estos fueron incubados a 37°C durante 20 min y luego pasados por una malla est&eacute;ril (105 mm). Las c&eacute;lulas obtenidas se centrifugaron a 3000 rpm por 5 min, fueron lavadas con medio fresco y sembradas en frascos de cultivo con 2 ml de MEM ajustado a pH 7.4 con hidr&oacute;xido de sodio, en presencia de 10% de suero fetal bovino y los siguientes antibi&oacute;ticos: 20 U/ml de penicilina cristalina, 0.01 mg/ml de gentamicina y 0.02 mg/ml de bacitracina. Los frascos de cultivo fueron cubiertos previamente con poli-L-lisina, 1 mg/ml, y lavados con MEM antes de la siembra (viabilidad del 92-96% medida por exclusi&oacute;n de azul Trip&aacute;n). El cultivo se realiz&oacute; en estufa a 37°C en atm&oacute;sfera humidificada con una concentraci&oacute;n de 5% de CO<SUB>2</SUB> y 95% de aire hasta obtener confluencia celular (15-45 d&iacute;as). </P> <B>    <P ALIGN="JUSTIFY">Pruebas de quimiosensibilidad</P> </B>    <P ALIGN="JUSTIFY">Se utilizaron (1,3-bis(2-cloroetil)-1-nitrosour&eacute;a (BCNU), (SP-4-2)diaminedicloroplatino (cisplatino, CPL) y 22-oxovincaleukoblastina (vincristina, VNC), en concentraciones de 0.045, 0.15 y 0.015 mg/frasco, respectivamente<SUP>(3,18)</SUP>. El procedimiento empleado se describe a continuaci&oacute;n: el medio se extrajo y se a&ntilde;adi&oacute; el citost&aacute;tico a frascos por duplicado en 3 ml de MEM, seguidamente se incubaron a 37°C por una hora, se extrajo el medio con el citost&aacute;tico, se realiz&oacute; un lavado suave con soluci&oacute;n salina y se procedi&oacute; a evaluar la integridad de la membrana mediante la exclusi&oacute;n de azul de Trip&aacute;n (50%). El BCNU se disolvi&oacute; previamente en 200 ml de metanol. Las c&eacute;lulas te&ntilde;idas con el colorante fueron contadas en un microscopio invertido y fueron corregidas o normalizadas con los resultados de frascos controles a los que se a&ntilde;adi&oacute; s&oacute;lo medio y se sometieron al mismo proceso de incubaci&oacute;n. Un total de 200 a 300 c&eacute;lulas fueron evaluadas por este m&eacute;todo.</P> <B>    <P ALIGN="JUSTIFY">An&aacute;lisis estad&iacute;stico</P> </B>    <P ALIGN="JUSTIFY">Se realiz&oacute; la prueba de Chi cuadrado y se consider&oacute; un valor de alfa de 0.05 para la significaci&oacute;n estad&iacute;stica.</P> <B>    <P ALIGN="JUSTIFY">RESULTADOS</P>     <P ALIGN="JUSTIFY">Pacientes</P> </B>    <P ALIGN="JUSTIFY">La edad de los catorce pacientes sometidos a cirug&iacute;a fue de 2 a 51 a&ntilde;os. Los ocho pacientes procesados se encontraban entre 41 y 51 a&ntilde;os. El 78% de los pacientes consultaron a los 6 meses o m&aacute;s despu&eacute;s de iniciar la sintomatolog&iacute;a. Los s&iacute;ntomas m&aacute;s frecuentes fueron cefalea, v&oacute;mitos, n&aacute;useas y deficiencias motoras. Dos pacientes presentaron convulsiones y alteraciones de conducta. Un paciente present&oacute; alteraci&oacute;n del control de esf&iacute;nteres, disminuci&oacute;n de la agudeza visual, deficiencias del lenguaje y disminuci&oacute;n de la memoria. Al ingreso los s&iacute;ntomas predominantes se relacionaron con el aumento de la tensi&oacute;n endocraneana, tales como cefalea, v&oacute;mitos y n&aacute;useas. Antes de la intervenci&oacute;n quir&uacute;rgica se realiz&oacute; tomograf&iacute;a axial computarizada a ocho, resonancia magn&eacute;tica nuclear a uno y ambas a cinco pacientes. Estos estudios reportaron con mayor frecuencia desplazamiento de la l&iacute;nea media, edema perilesi&oacute;n y colapso del ventr&iacute;culo ipsilateral. Se evidenci&oacute; lesi&oacute;n de ocupaci&oacute;n de espacio: frontal en cinco, parieto-occipital en tres y del III ventr&iacute;culo en dos de los casos. Los diagn&oacute;sticos anatomopatol&oacute;gicos fueron los siguientes: diez de los tumores correspondieron a astrocitomas, los cuatro restantes fueron m&eacute;duloblastoma, ependimoma, craneofaringioma y adenocarcinoma tiroideo. Los tumores 1, 3, 4, 7 y 8 fueron astrocitomas, el 2 un craneofaringoma, el 5 un m&eacute;duloblastoma y el 6 un tumor metast&aacute;sico tiroideo.</P> <B>    <P ALIGN="JUSTIFY">Sensibilidad a citost&aacute;ticos</P> </B>    ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">En la <a href="#tab1">Tabla 1</a> se muestran los porcentajes de c&eacute;lulas muertas en cada una de las condiciones para los cultivos ensayados. El an&aacute;lisis de Chi cuadrado de tablas de contingencia para comparar el n&uacute;mero de c&eacute;lulas te&ntilde;idas y no te&ntilde;idas con azul de Trip&aacute;n, que incluy&oacute; todos los grupos por cada tumor (cuatro filas correspondientes a control y tres drogas, y dos columnas correspondientes a c&eacute;lulas te&ntilde;idas y no te&ntilde;idas) result&oacute; estad&iacute;sticamente significativo para todos los tumores (P &lt; 0.001, <a href="#tab2">Tabla 2</a>). El an&aacute;lisis que correspondi&oacute; a los resultados obtenidos con los citost&aacute;ticos (tres filas correspondientes a las tres drogas, y dos columnas correspondientes a c&eacute;lulas te&ntilde;idas y no te&ntilde;idas) se se&ntilde;ala en la <a href="#tab2">Tabla 2</a> y las diferencias fueron estad&iacute;sticamente significativas para los tumores 4, 5, 6 y 7. El an&aacute;lisis de Chi cuadrado del control y de cada una de las drogas para todos los tumores (ocho filas correspondientes a los ocho tumores, y dos columnas correspondientes a c&eacute;lulas te&ntilde;idas y no te&ntilde;idas) fue el siguiente: Control 23.55, P &lt; 0.01; BCNU 36.87, P &lt; 0.001; CPL 27.75, P &lt; 0.001; VNC 102.17, P &lt; 0.001. Se compararon los tumores con el mismo diagn&oacute;stico histopatol&oacute;gico que correspondieron a los astrocitomas (Tumores 1, 3 y 8), que fue el m&aacute;s frecuente y se obtuvo el siguiente resultado: Control 6.40, P &lt; 0.05; BCNU 20.29, P &lt; 0.001; CPL 10.09, P &lt; 0.01; VNC 4.41, no significativo. Los valores de Chi cuadrado y la significancia de combinaciones de dos drogas entre s&iacute; (dos filas correspondientes a las dos drogas comparadas, y dos columnas correspondientes a c&eacute;lulas te&ntilde;idas y no te&ntilde;idas) se realiz&oacute; para cada tumor y est&aacute;n reportados en la <a href="#tab3">Tabla 3</a>. Existi&oacute; una respuesta diferencial significativa para los Tumores 2, 4, 5, 6 y 7. Evidentemente no existe una preparaci&oacute;n ideal <I>in vitro</I> que reproduzca las condiciones del tumor <I>in situ</I>, pero una aproximaci&oacute;n experimental podr&iacute;a contribuir al mejor enfoque terap&eacute;utico de los tumores cerebrales, especialmente los gliomas malignos que ocurren con elevada frecuencia y cursan con deficiencias neuropsiqui&aacute;tricas graves. No s&oacute;lo interesa la sobrevida, sino la oferta de una calidad de vida adecuada. La dificultad com&uacute;n a todos los enfoques experimentales corresponde a la farmacocin&eacute;tica del tratamiento con drogas citost&aacute;ticas. Las medidas eval&uacute;an concentraciones en plasma, aunque &eacute;stas no se corresponden con las alcanzadas en el tejido, que siempre son menores. Si esto es as&iacute;, tambi&eacute;n resulta dif&iacute;cil realizar estimaciones de las concentraciones de drogas a usar en condiciones de cultivo<SUP>(3,11,19)</SUP>. En el presente trabajo se escogieron concentraciones de drogas relacionadas con dosis efectivas cincuenta se&ntilde;aladas en la literatura<SUP>(3)</SUP>, aunque consideramos que ser&iacute;a deseable realizar curvas de dosis-respuesta que pudieran constituir una mejor gu&iacute;a para el tratamiento. A pesar de los inconvenientes que pueden se&ntilde;alarse, que no son diferentes a los que podr&iacute;an relacionarse con un antibiograma de cualquier tipo, el hecho de encontrarse respuestas diferenciales a las drogas en un mismo tipo de tumor, hace que se tome en consideraci&oacute;n una quimioterapia racional y dirigida. Por ejemplo, el Tumor 4, que corresponde a un glioblastoma multiforme, present&oacute; comportamientos estad&iacute;sticamente diferentes entre el BCNU y el CPL, as&iacute; como entre el CPL y la VNC. En base a esto probablemente el cl&iacute;nico neurocirujano y el onc&oacute;logo no escoger&iacute;an el BCNU ante una mayor eficiencia del CPL y la VNC. Sin embargo, s&oacute;lo estudios prospectivos con un n&uacute;mero adecuado de pacientes podr&iacute;an determinar el valor de los resultados obtenidos en cultivo.</P>     <P ALIGN="JUSTIFY">Una estandarizaci&oacute;n estricta es imprescindible para el buen manejo y comparaci&oacute;n adecuada de los cultivos primarios de c&eacute;lulas de gliomas<SUP>(20)</SUP>. Sin lugar a dudas los sistemas de cultivo celulares son la herramienta experimental m&aacute;s usada para comprender la biolog&iacute;a de los gliomas<SUP>(5)</SUP>. La duraci&oacute;n del cultivo determina las caracter&iacute;sticas celulares del mismo, por ejemplo, c&eacute;lulas positivas a la prote&iacute;na ac&iacute;dica fibrilar glial son frecuentes en cultivos tempranos, sin embargo, existe gran variaci&oacute;n en la expresi&oacute;n de marcadores celulares espec&iacute;ficos con respecto al tiempo de permanencia en cultivo<SUP>(20)</SUP> y tambi&eacute;n en relaci&oacute;n a la interacci&oacute;n de factores tr&oacute;ficos y c&eacute;lulas gliales<SUP>(21)</SUP>. Entre los marcadores bioqu&iacute;micos se han se&ntilde;alado evidencias de relaci&oacute;n de efectos de la sustancia P y la secreci&oacute;n de interleucinas por el tumor<SUP>(22)</SUP>, de la presencia de una prote&iacute;na liberadora de dopamina<SUP>(21)</SUP>, y sobre la secreci&oacute;n de prostaglandina E<SUB>2</SUB> por c&eacute;lulas de glioblastoma que puede regular las respuestas inmunol&oacute;gicas desencadenadas por el tumor<SUP>(23)</SUP>. Aunque la significaci&oacute;n de estos hallazgos no est&aacute; clara hasta el presente, la duraci&oacute;n del cultivo puede resultar en diferencias de la respuesta a los citost&aacute;ticos, dado la heterogeneidad temporal que se puede encontrar. Una manera de reducir esta diferencia es mediante el establecimiento de criterios en la estandarizaci&oacute;n, que en el presente estudio correspondi&oacute; a la confluencia celular.</P>     <P ALIGN="JUSTIFY">El uso de cultivos a corto plazo de material derivado de tumores se utiliza para la identificaci&oacute;n de nuevas drogas citot&oacute;xicas<SUP>(8,24,25,26,27)</SUP>, esto debido a la relativa resistencia de los gliomas malignos a la quimioterapia. Si estas preparaciones son &uacute;tiles para el mapeo de nuevas drogas, como una prueba biol&oacute;gica de sensibilidad apreciable, y realmente la combinaci&oacute;n de drogas en quimioterapia antitumoral se realiza a ciegas, un enfoque racional de la misma no dejar&iacute;a de tener utilidad, y podr&iacute;a hacerse a un costo relativamente alcanzable. Los cultivos de c&eacute;lulas derivadas de astrocitomas tambi&eacute;n han sido empleados en el estudio de sensibilidad a radiaciones<SUP>(28)</SUP>. De manera que la comprensi&oacute;n de la diferenciaci&oacute;n celular glial mediante avances en t&eacute;cnicas inmunol&oacute;gicas, de cultivo de tejido, y de biolog&iacute;a molecular ha contribuido al desarrollo de la neurooncolog&iacute;a<SUP>(19)</SUP>.</P>     <P ALIGN="JUSTIFY">Con respecto a la sensibilidad diferencial de quimoterap&eacute;uticos conocidos, algunos estudios en l&iacute;neas celulares se&ntilde;alan, por ejemplo, que el CPL ejerce su efecto antitumoral por la inhibici&oacute;n del activador de plasmin&oacute;geno del tipo uroquinasa, sin embargo, el BCNU no tiene ese efecto<SUP>(29)</SUP>, por lo que el CPL podr&iacute;a tener m&aacute;s propiedades antiinvasivas que el BCNU, al menos en las l&iacute;neas estudiadas. Esta evidencia indica m&aacute;s caminos interesantes de estudio diferencial sobre drogas citost&aacute;ticas en preparaciones <I>in vitro</I>.</P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">Como ha sido previamente considerado<SUP>(11)</SUP>, el valor predictivo de estos ensayos presenta dificultades, tales como la heterogeneidad celular en el tumor y por ende en los cultivos, y las modificaciones inherentes al estudio <I>in vitro</I>, que no reproduce la situaci&oacute;n <I>in situ</I>. Se ha considerado que, m&aacute;s que la sensibilidad a drogas, estas pruebas son &uacute;tiles para determinar la resistencia a las mismas.
