En el 53% de las muestras procesadas se determinaron serotipos de VPH, 4 del tipo VPH 6 y 4 del tipo VPH 11, coincidiendo estos datos con lo descrito en la literatura mundial. En el resto de muestras (47%) la determinación del VPH fue negativa. No se detectó ninguno de los otros serotipos investigados ni se evidenció coinfección viral.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[The objective of this study was to determine the HPV types from tissue samples from fifteen (15) patients aged between 3 and 20 years, who had been clinically and histopathologicaly diagnosed as juvenile laryngeal papillomatosis, by means of the Polymerase Chain Reaction (PCR) test, using a compound of the genomic DNA of HPV types 6, 11, 16, 18, 31, 33 and 35, in the Otoralyngology Department of the University Hospital of Caracas, during the interval 1994-1999. In 53% of the specimen processed were positive, four (4) for HPV type 6 and four (4) for HPV type 11. These results are similar to the findings of international publications. In the remaining samples (47%) were negative for HPV. Furthermore, no other serological types investigated were found, or viral coinfection was detected.]]></p></abstract>

<body><![CDATA[<P ALIGN="JUSTIFY">__________________</P>     <P ALIGN="JUSTIFY"></P> <B>    <P ALIGN="JUSTIFY">&nbsp;</P>     <P ALIGN="JUSTIFY">&nbsp;</P>     <P ALIGN="JUSTIFY">INTRODUCCI&Oacute;N</P> </B>    <P ALIGN="JUSTIFY"></P>     <P ALIGN="JUSTIFY">La papilomatosis lar&iacute;ngea juvenil (PLJ) es una enfermedad infecciosa de la mucosa lar&iacute;ngea causada por el virus del papiloma humano (VPH) que pertenece a la familia papovaviridae constituida por un grupo de virus ADN epiteliotr&oacute;picos con gran afinidad por &oacute;rganos tubulares en contacto con el exterior tales como fosas nasales, cavidad oral, faringe, laringe, tr&aacute;quea, bronquios, es&oacute;fago, vagina, etc<SUP>(1,2,3)</SUP>. Debido a su elevado potencial de recurrencia, localizaci&oacute;n m&uacute;ltiple especialmente en laringe y conocimiento incompleto del mecanismo patog&eacute;nico, el &iacute;ndice de prevalencia de la enfermedad se mantiene elevado y a trav&eacute;s del tiempo se ha convertido en un verdadero problema terap&eacute;utico<SUP>(4,5)</SUP>.</P>     <P ALIGN="JUSTIFY">Su mayor incidencia en las dos primeras d&eacute;cadas de la vida la colocan como la lesi&oacute;n tumoral benigna de laringe m&aacute;s frecuente de la infancia. En Estados Unidos, la incidencia infantil es de 4,3 por 100.000 ni&ntilde;os con una frecuencia mayor entre los 2 y 4 a&ntilde;os<SUP>(6,7,8)</SUP>. El mecanismo de transmisi&oacute;n de la PLJ aunque todav&iacute;a no se ha determinado en forma exacta, la mayor&iacute;a de los autores est&aacute;n de acuerdo en que una de las formas m&aacute;s frecuente es debida a la ingesti&oacute;n de part&iacute;culas virales o de c&eacute;lulas infectadas durante el paso del ni&ntilde;o a trav&eacute;s del canal del parto en el momento del nacimiento, de una madre con condilomatosis genital<SUP>(9,10)</SUP>.</P>     <P ALIGN="JUSTIFY">El diagn&oacute;stico de la enfermedad es determinado por la cl&iacute;nica caracterizada principalmente por disfon&iacute;a, estridor, disnea progresiva y la presencia de papilomas en estructuras de la v&iacute;a a&eacute;rea-digestiva, que al crecer obstruyen la v&iacute;a a&eacute;rea <B><I>(<a href="#fig1">Fig. 1</a>)</B></I> provocando la aparici&oacute;n de insuficiencia respiratoria que muchas veces compromete la vida del paciente. El examen laringoendosc&oacute;pico, la biopsia de las lesiones y su estudio histopatol&oacute;gico determinado por la presencia de coilocitos, hiperplasia epitelial e hiperqueratosis <B><I>(<a href="#fig2">Fig. 2</a>)</B></I> confirman el diagn&oacute;stico de infecci&oacute;n por virus del papiloma humano (VPH). La microscopia electr&oacute;nica ha permitido visualizar las part&iacute;culas o inclusiones virales dentro del n&uacute;cleo como se puede ver en la <B><I><a href="#fig3">Fig. 3</a></I></B>, lo cual confirma a&uacute;n m&aacute;s el diagn&oacute;stico<SUP>(11)</SUP>.</P>     <P ALIGN="JUSTIFY"><a name="fig1"></a></P>     ]]></body>

<body><![CDATA[<P ALIGN="JUSTIFY">&nbsp;</P></DIR> </DIR>      <P ALIGN="JUSTIFY"> El n&uacute;mero total de serotipos de VPH se ha incrementado notoriamente en los &uacute;ltimos 20 a&ntilde;os, de los cuales unos 25 han sido detectados en la laringe. La tipificaci&oacute;n virol&oacute;gica de las cepas involucradas es muy importante por dos razones: 1) La recurrencia de la enfermedad es la regla y seg&uacute;n algunos estudios los tipos VPH 6 y 11 est&aacute;n relacionados con este proceso y 2) Los tipos VPH 16, 18 y 30 se relacionan con procesos de malignizaci&oacute;n especialmente de laringe<SUP>(12,13)</SUP>.</P>     <P ALIGN="JUSTIFY">Debido a que este virus no puede ser cultivado &quot;in vitro&quot; se realiza la detecci&oacute;n y tipificaci&oacute;n de los &aacute;cidos nucleicos del virus del papiloma humano (VPH) mediante m&eacute;todos moleculares de ingenier&iacute;a gen&eacute;tica uno de los cuales es la prueba de Reacci&oacute;n en Cadena de la Polimerasa (PCR) que tiene una gran especificidad<SUP>(14)</SUP>.</P>     <P ALIGN="JUSTIFY">El objetivo de este trabajo fue contribuir al estudio m&aacute;s preciso de la enfermedad papilomatosa lar&iacute;ngea juvenil en Venezuela por medio de la tipificaci&oacute;n del virus del papiloma humano seg&uacute;n el m&eacute;todo de PCR, de pacientes menores de 20 a&ntilde;os que consultaron al Servicio de Otorrinolaringolog&iacute;a del Hospital Universitario de Caracas en el per&iacute;odo 1994-1999.</P>     <P ALIGN="JUSTIFY"></P>     <P ALIGN="JUSTIFY">&nbsp;</P> <B>    <P ALIGN="JUSTIFY">MATERIALES Y M&Eacute;TODOS</P> </B>    <P ALIGN="JUSTIFY"></P>     <P ALIGN="JUSTIFY">Se realiz&oacute; estudio prospectivo y descriptivo a una poblaci&oacute;n de 15 pacientes con edades comprendidas entre los 3 y 20 a&ntilde;os de edad con el diagn&oacute;stico cl&iacute;nico e histopatol&oacute;gico de papilomatosis laringea juvenil del Servicio de ORL del HUC en el per&iacute;odo 1994 y 1999. Se efectu&oacute; la detecci&oacute;n de VPH y tipificaci&oacute;n viral por m&eacute;todos moleculares de Ingenier&iacute;a Gen&eacute;tica a especimenes cl&iacute;nicos (muestras de tejidos) utilizando la prueba de Reacci&oacute;n en Cadena de la Polimerasa (PCR) con un mezcla de los ADN gen&oacute;micos de VPH 6, 11, 16, 18, 31, 33, 35.</P>     <P ALIGN="JUSTIFY"></P>     ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY">&nbsp;</P> <B>    <P ALIGN="JUSTIFY">RESULTADOS</P> </B>    <P ALIGN="JUSTIFY"></P>     <P ALIGN="JUSTIFY">En los pacientes evaluados, 7 fueron del g&eacute;nero masculino y 8 del femenino. La mayor incidencia se observ&oacute; en los grupos etarios de 0 a 10 a&ntilde;os, con una edad promedio de 9 a&ntilde;os, una m&iacute;nima de 3 a&ntilde;os y una m&aacute;xima de 20 a&ntilde;os <B><I>(<a href="#fig4">Fig. 4</a>)</B></I>. Se determin&oacute; el tipo serol&oacute;gico en el 53% de las muestras procesadas: 4 con serotipo 6 y 4 con serotipo 11. No se estableci&oacute; serotipo de VPH en el 47% de los pacientes <B><I>(<a href="#tabla1">Tabla 1</a>)</B></I>, sin que esto quiera decir que no exista la enfermedad, sino que el serotipo puede corresponder a uno diferente de los investigados. </P>     <P ALIGN="JUSTIFY"><a name="fig4"></a></P> <DIR> <DIR>  <I>    <P ALIGN="center"><b>Figura 4:</b> Papilomatosis Lar&iacute;ngea Juvenil. <body><![CDATA[<P ALIGN="JUSTIFY">VPH negativo</FONT></TD> <TD WIDTH="34%" VALIGN="TOP"> <FONT SIZE=3>    <P ALIGN="CENTER">7</FONT></TD> <TD WIDTH="27%" VALIGN="TOP"> <FONT SIZE=3>    <P ALIGN="CENTER">47</FONT></TD> </TR> </TABLE> </CENTER>  <FONT SIZE=3>    <P ALIGN="center"><I><b>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Tabla 1:</b> Papilomatosis lar&iacute;ngea juvenil tipificaci&oacute;n viral. </I></P>     <P ALIGN="JUSTIFY">&nbsp;</P>     <P ALIGN="JUSTIFY">&nbsp;</P> <B>    <P ALIGN="JUSTIFY">DISCUSI&Oacute;N</P> </B>    <P ALIGN="JUSTIFY"></P>     <P ALIGN="JUSTIFY">Lograr el control de la papilomatosis respiratoria recurrente ha sido el deseo de todos los investigadores dedicados al estudio de la enfermedad, tratando a trav&eacute;s de numerosos estudios encontrar un m&eacute;todo de diagn&oacute;stico espec&iacute;fico y efectivo que ayude a lograr un tratamiento eficaz que controle completamente la recurrencia de la infecci&oacute;n viral.</P>     <P ALIGN="JUSTIFY">Hasta los a&ntilde;os 80, el diagn&oacute;stico de la papilomatosis se bas&oacute; en la cl&iacute;nica y los hallazgos histopatol&oacute;gicos, pero el avance tecnol&oacute;gico de la medicina ha permitido demostrar la etiolog&iacute;a viral y la presencia de &aacute;cidos nucleicos de VPH en las lesiones papilomatosas. Dado que hasta la fecha el virus del papiloma humano no ha podido ser cultivado, se han introducido m&eacute;todos moleculares de ingenier&iacute;a gen&eacute;tica, uno de ellos de PCR que permite la tipificaci&oacute;n de los diferentes virus de VPH, lo cual es importante por el potencial oncog&eacute;nico detectado en ciertos serotipos y que han sido encontrados en laringe<SUP>(15)</SUP>. En este estudio se procesaron por ensayo de PCR, 15 muestras de tejido de laringe de pacientes de ambos g&eacute;neros, entre 3 y 20 a&ntilde;os de edad, lo cual permiti&oacute; el despistaje y tipificaci&oacute;n de ADN viral en el 53% de las muestras, permitiendo hacer el diagn&oacute;stico de certeza y la determinaci&oacute;n del tipo viral de cada caso. El 47% de las muestras procesadas no revelaron presencia de ADN viral, lo cual no descarta la infecci&oacute;n por VPH debido a que s&oacute;lo fueron utilizados los seis genomas clonados disponibles en Venezuela los cuales son 6, 11, 16, 18, 31, 33, 35, pudi&eacute;ndose inferir que en las muestras con resultado negativo pudiera estar presente un tipo de VPH no investigado. Los tipos de VPH detectados en las muestras del estudio fueron el 6 y el 11 (4 de cada tipo) lo cual coincide con lo reportado en la literatura mundial para la papilomatosis lar&iacute;ngea juvenil.</P>     ]]></body>
