En el tratamiento de las fracturas diafisarias se han empleado numerosas técnicas, desde el tratamiento ortopédico pasando por la osteosíntesis con placas y clavos endomedulares. El objetivo del tratamiento es promover la consolidación, la movilización precoz y restauración de la función, conservando la longitud, rotación, y ejes mecánicos de la extremidad. Elegir el implante depende de la personalidad de la fractura. Los clavos con pernos de bloqueo proximal y distal permiten controlar las inestabilidades axiales o rotacionales. Se identificaron 132 historias clínicas, la información se tabuló mediante formato de recolección de datos y los resultados se analizaron y reportaron en tablas y gráficos. Esta revisión pone en evidencia que las fracturas diafisarias con una gran frecuencia afectan a la población adulta joven 68.1%, con predominio del sexo masculino 86%, quienes se encuentran expuestos principalmente a hechos violentos, en el 50.7% de los casos estuvo comprometido el fémur, el 68.9% ameritó tratamiento quirúrgico a cielo abierto y el 53.7% más de dos horas de cirugía, requiriendo hasta treinta días de hospitalización en el 75.7% de los casos.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[ABSTRACT: In the treatment of the fractures diafisarias numerous techniques have been used from the orthopedic treatment going by the osteosíntesis with badges and nails endomedulares. The objective of the treatment is to promote the consolidation, precocious mobilization and to restore the function, conserving the longitude, rotation, and the mechanical axes of the extremity. To choose the one implants it depends on the personality of the fracture, the nails with fasteners of blockade proximal and distal allow to control the axial uncertainties or rotacionales. You identificaron 132 clinical histories, the information is tabulated by means of format of gathering of data, the results were analyzed and they reported in charts and graphics. This revision puts in evidence that these fractures frequently affect the population adult young 68.1%, with prevalence of the sex masculine 86% who you/they are exposed mainly to violent facts, in 50.7% of the cases it was committed the femur, 68.9% needed surgical treatment to open sky and the 53.7% more than two hours of surgery, 75.7% required up to thirty days of hospitalization.]]></p></abstract>

<body><![CDATA[<P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">Fecha de Recepci&oacute;n: 11/06/2003 Fecha de Aprobaci&oacute;n: 13/04/2004</font></P> <B>    <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">INTRODUCCI&Oacute;N</font></P> </B>    <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">&nbsp;&nbsp;&nbsp; En el tratamiento de las fracturas diafisarias se han empleado numerosas t&eacute;cnicas desde tratamiento ortop&eacute;dico (enyesados r&iacute;gidos, f&eacute;rulas funcionales), pasando por la osteos&iacute;ntesis con placas y clavos endomedulares<sup>(1-12)</sup>. En los &uacute;ltimos a&ntilde;os ha aumentado notablemente el n&uacute;mero de implantes disponibles para la osteos&iacute;ntesis mediante el enclavado endomedular. Los distintos implantes se diferencian en cuanto a dise&ntilde;o (ranurado o liso, s&oacute;lido o canulado; di&aacute;metro peque&ntilde;o o grande; bloque est&aacute;tico o din&aacute;mico), a material (acero o titanio), y a t&eacute;cnica quir&uacute;rgica (con fresado progresivo, fresado &uacute;nico o sin fresado). Sus indicaciones respectivas se solapan considerablemente entre s&iacute;.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">&nbsp;&nbsp;&nbsp; Elegir el implante depende de la personalidad de la fractura, los clavos con pernos de bloqueo proximal y distal permiten controlar las inestabilidades axiales o rotacionales<sup>(13,14,15)</sup>, el objetivo del tratamiento es promover la consolidaci&oacute;n, movilizaci&oacute;n precoz y restauraci&oacute;n de la funci&oacute;n, conservando la longitud, rotaci&oacute;n y los ejes mec&aacute;nicos de la extremidad<sup>(16,17)</sup>.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">&nbsp;&nbsp;&nbsp; Consideramos fracturas de la di&aacute;fisis humeral, la situada entre 5 cm por debajo del reborde superior de la cabeza humeral a 5 cm por encima de la superficie articular distal del h&uacute;mero<sup>(1)</sup>. Constituye entre el 1.29% y el 1.5% de las fracturas de los miembros, siendo m&aacute;s frecuentes en la uni&oacute;n del tercio medio con el distal<sup>(2)</sup>, ya que en este punto el h&uacute;mero cambia de una arquitectura cil&iacute;ndrica a una aplanada. El mecanismo m&aacute;s frecuente es el traumatismo directo sobre el miembro superior sobre todo en accidentes de tr&aacute;nsito. El tratamiento en general es conservador (yeso colgante de Caldwell, f&eacute;rula braquial de coaptaci&oacute;n en ¨U¨, ortesis funcionales) no siendo necesario realizar una reducci&oacute;n perfecta de los fragmentos, tolerando angulaci&oacute;n anterior no mayor de 20° y un varo menor de 30°, tampoco una aposici&oacute;n del 100% de los fragmentos. Dentro de las posibilidades del tratamiento quir&uacute;rgico, destaca el enclavado endomedular, fijaci&oacute;n con placa y tornillos y la fijaci&oacute;n externa.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">&nbsp;&nbsp;&nbsp; La di&aacute;fisis femoral se extiende desde el troc&aacute;nter menor hasta el inicio del ensanchamiento metafisario distal a la altura del tub&eacute;rculo aductor. Presenta una ligera incurvaci&oacute;n de concavidad posterior, en su tercio central la di&aacute;fisis femoral es pr&aacute;cticamente cil&iacute;ndrica, en su cara posterior existe un refuerzo estructural, la l&iacute;nea &aacute;spera para contrarrestar las tensiones en compresi&oacute;n producidas por la mencionada concavidad. En cuanto al mecanismo de la lesi&oacute;n requiere un impacto de alta energ&iacute;a para fracturarse por ser extremadamente resistente.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">&nbsp;&nbsp;&nbsp; Conviene recordar que la fractura diafisaria de f&eacute;mur es la lesi&oacute;n m&aacute;s frecuente en el politraumatizado, desempe&ntilde;ando un importante papel pron&oacute;stico su r&aacute;pida estabilizaci&oacute;n. El tratamiento definitivo es la estabilizaci&oacute;n inmediata con enclavado endomedular, placas atornilladas<sup>(6)</sup>.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">&nbsp;&nbsp;&nbsp; La fractura de la tibia es la fractura diafisaria m&aacute;s frecuente del organismo<sup>(3)</sup>. Diversas caracter&iacute;sticas anat&oacute;micas de este hueso hacen que tanto el pron&oacute;stico como el tratamiento de sus fracturas sean en ocasiones especialmente complejos. Tiene una secci&oacute;n transversa triangular, la cavidad medular es m&aacute;s infundibuliforme que la femoral, lo que supone una cierta desventaja para la estabilidad proporcionada por los enclavados intramedulares. Presenta en su tercio inferior una moderada concavidad medial, cuyo restablecimiento tras una fractura es importante tanto est&eacute;tica como biomec&aacute;nicamente. La cara externa es subcut&aacute;nea por lo que explica la gran frecuencia de fracturas abiertas.</font></P>     <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">&nbsp;&nbsp;&nbsp; El enclavado endomedular a foco cerrado se utiliza actualmente para la mayor parte de las fracturas, ya que respeta buena parte del potencial biol&oacute;gico de consolidaci&oacute;n, disminuye el riesgo de contaminaci&oacute;n directa al no abrir el foco, permite la movilizaci&oacute;n y el apoyo precoz.</font></P> <B>    <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">MATERIALES Y M&Eacute;TODOS</font></P> </B>    ]]></body>
<body><![CDATA[<P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">&nbsp;&nbsp;&nbsp; Se hizo una revisi&oacute;n de tipo retrospectivo en el Hospital Vargas de Caracas-Venezuela durante el periodo comprendido entre el 01 de enero de 1999 y el 30 de octubre de 2001. Se identificaron 132 historias cl&iacute;nicas de aquellos pacientes que ingresaron por emergencia con el diagn&oacute;stico de fracturas diafisarias de h&uacute;mero, f&eacute;mur o tibia, tratadas con el criterio por la t&eacute;cnica de enclavado endomedular bloqueado y se revisaron en forma exhaustiva. Se tabul&oacute; la informaci&oacute;n mediante formato de recolecci&oacute;n de datos (distribuci&oacute;n por sexo, edad, localizaci&oacute;n de la fractura, miembro afectado, causas de la fractura, tracci&oacute;n esquel&eacute;tica, t&eacute;cnica quir&uacute;rgica, tiempo de cirug&iacute;a, d&iacute;as de hospitalizaci&oacute;n, distribuci&oacute;n por a&ntilde;o). Los resultados se analizaron y reportaron en tablas y gr&aacute;ficos.</font></P> <B>    <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">RESULTADOS</font></P> </B>    <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">&nbsp;&nbsp;&nbsp; Un total de 132 pacientes (pac.), con predominio del sexo masculino 86% sobre el femenino 14 % <B>(<a href="#fig1">Figura 1</a>)</B>, relaci&oacute;n 6.3/1. Las fracturas se distribuyeron de la siguiente manera: el f&eacute;mur 67 casos (50.7%), la tibia 45 casos (34.6%) y 20 casos de h&uacute;mero (15.1%) <B>(<a href="#fig2">Figura 2</a>)</B>, siendo el lado izquierdo el m&aacute;s frecuentemente afectado (62.1%). <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">DISCUSI&Oacute;N</font></P> </B>    <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">&nbsp;&nbsp;&nbsp; Los resultados de esta revisi&oacute;n ponen en evidencia que estas fracturas afectan frecuentemente a la poblaci&oacute;n adulta joven 68.1%, con predominio del sexo masculino 86% quienes se encuentran expuestos principalmente a hechos violentos (herida por arma de fuego, colisi&oacute;n y arrollamiento)<sup>(7)</sup>. En nuestra revisi&oacute;n, la fractura diafisaria de f&eacute;mur fue la m&aacute;s frecuente 50.7% de los casos, pero reportes indican que la fractura de la tibia es la fractura diafisaria m&aacute;s frecuente del organismo por sus caracter&iacute;sticas anat&oacute;micas<sup>(5)</sup>. El enclavado endomedular a foco cerrado se utiliza actualmente para la mayor parte de las fracturas ya que respeta buena parte del potencial biol&oacute;gico de consolidaci&oacute;n, disminuye el riesgo de contaminaci&oacute;n directa al no abrir el foco, permitiendo la movilizaci&oacute;n y el apoyo precoz, pero en nuestro caso amerit&oacute; tratamiento quir&uacute;rgico a cielo abierto en el 68.9% de los casos debido al tiempo de espera preoperatorio por diversas causas.</font></P> <B>    <P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">REFERENCIAS BIBLIOGR&Aacute;FICAS</font></P>  </B>    <!-- ref --><P ALIGN="JUSTIFY"><font face="Times New Roman" size="3">1.
