<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Cunninghamella bertholletiae is an unusual opportunistic pathogen which belongs to the order of the Mucorales and causes infection in immunosuppressed patients, particularly with malignant hematological diseases. We report a case of rhinosinusal mucormycosis caused by C. bertholletiae in a patient with acute myeloid leukemia. The critical factors for optimal medical management of mucormycosis are early diagnosis, reversal of predisposing factors, surgical debridement, and appropriate early antifungal therapy. However, the success of the intervention could be threatened by the delicate balance of the immune system of immunocompromised patients.]]></p></abstract>

<body><![CDATA[<p style="text-autospace: none" align="justify"><font face="Verdana"><b> <span style="font-size: 10.0pt; font-family: TimesNewRomanPS-BoldMT"> Introducción</span></b></font></p>     <p style="text-autospace: none" align="justify"><font face="Verdana"> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">La mucormicosis  es una infección fúngica oportunista e invasora causada por hongos filamentosos  aseptados, pertenecientes al orden de los Mucorales. Los factores de riesgo más  importantes asociados a esta enfermedad son: granulocitopenia, inmunosupresión,  diabetes y traumatismo. Las presentaciones clínicas son infecciones cutáneas,  rinocerebrales, pulmonares y diseminadas, estas últimas caracterizadas por angio-invasión  y necrosis con mal pronóstico. Se reporta un caso de mucormicosis  rinosinusal causado por </span><i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPS-ItalicMT">C.  bertholletiae </span></i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">en un paciente  con leucemia mieloide aguda (LMA).</span></font></p>     <p style="text-autospace: none" align="justify"><font face="Verdana"><b> <span style="font-size: 10.0pt; font-family: TimesNewRomanPS-BoldMT">Caso  clínico</span></b></font></p>     <p style="text-autospace: none" align="justify"><font face="Verdana"> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">Paciente  masculino de 33 años de edad, natural del estado Mérida, procedente de la ciudad  de Caracas, con diagnóstico de LMA subtipo M2 desde febrero del año 2002. Fue  hospitalizado en el Servicio de Medicina II del Hospital Vargas de Caracas y  tratado con citarabina (Ara-C) y mitoxantrona. El día 13 de hospitalización  inició episodio de neutropenia febril que respondió a tratamiento con cefepime y  amikacina. A los 45 días, después del primer ciclo de quimioterapia, presentó  otro episodio de neutropenia febril (500 leucocitos/&#956;L - Valor absoluto de  neutrófilos: 1/&#956;L) y rinorrea serohemática unilateral izquierda. Durante la  evaluación rinoscópica se observaron lesiones necróticas en el cornete medio  izquierdo. Los estudios tomográficos evidenciaron hipertrofia de la mucosa del  cornete y de las celdillas maxilares izquierdas. Se inició tratamiento con  Amphotec® (anfotericina B de dispersión coloidal, ABDC), a dosis de 4 mg/Kg/día,  se realizó desbridación quirúrgica y se tomó muestra de tejido para estudio  micológico.</span></font></p>     <p style="text-autospace: none" align="justify"><font face="Verdana"> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">El paciente  completó el tratamiento acumulando 4 gr de ABDC y se le realizaron dos limpiezas  quirúrgicas adicionales; las nasofibroscopias de control evidenciaron  desaparición de las lesiones. Durante el tratamiento con ABDC se inició la  segunda inducción de la quimioterapia con Ara-C y topotecan. A los 15 días  siguientes, presentó nuevo episodio de neutropenia febril. Se realizó una nueva  evaluación rinoscópica, en la cual no se evidenciaron nuevas lesiones; los  controles tomográficos fueron negativos. Se indicaron cefepime, amikacina y  anfotericina B con respuesta favorable. Tres meses y medio después de la segunda  inducción de la quiomiterapia, el paciente presentó una recaída de su enfermedad  hematológica con nuevo episodio de neutropenia febril y datos compatibles con  infección pulmonar. Las evaluaciones tomográfica de senos paranasales y  rinoscópica fueron normales, pero el paciente falleció 20 días después por shock  séptico de punto partida pulmonar. No se realizó necropsia.</span></font></p>     <p style="text-autospace: none" align="justify"><font face="Verdana"><b> <span style="font-size: 10.0pt; font-family: TimesNewRomanPS-BoldMT">Estudio  micológico</span></b></font></p>     <p style="text-autospace: none" align="justify"><font face="Verdana"> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">La muestra de  tejido fue enviada al laboratorio de micología del Hospital Vargas de Caracas,  en donde se realizaron examen directo y cultivo. En el examen microscópico de la  muestra con KOH más tinta Parker® (100X), se observaron hifas anchas de 7-15 &#956;m  de diámetro, aseptadas, ramificadas en ángulo recto (<a href="#fig1">Figura 1A</a>) y de paredes  delgadas (<a href="#fig1">Figura 1B</a>). El hallazgo de estas estructuras fúngicas permitió  establecer el diagnóstico de mucormicosis [4,7].</span></font></p>     <p style="text-autospace: none" align="center"><a name="fig1"> <img border="0" src="/img/fbpe/rsvm/v37n2/art08fig1.gif" width="426" height="370"></a></p>     
<p style="text-autospace: none" align="justify"><font face="Verdana"> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">En agar  Sabouraud dextrosa más cloranfenicol (Difco) incubado a 28 y 37 ºC se desarrolló  una colonia algodonosa blanca a las 48 horas con crecimiento ilimitado, que  posteriormente cambió a color beige [4,7] (<a href="#fig1">Figura 1C</a>). En el examen microscópico  de la colonia con azul de lactofenol se observaron hifas gruesas aseptadas, con  esporangióforo erecto y una vesícula globosa de 30-60 &#956;m de diámetro con  esporangiolas esféricas u ovales de 5-10 &#956;m de diámetro, unidas individualmente  a la vesícula mediante un pequeño esterigma (<a href="#fig1">Figuras 1D y E</a>). Estas  características fueron compatibles con el género </span><i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPS-ItalicMT"> Cunninghamella </span></i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">[4,7].</span></font></p>     ]]></body>
<body><![CDATA[<p style="text-autospace: none" align="justify"><font face="Verdana"> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">La confirmación  de género e identificación de especie se realizó en el Departamento de Micología  del Instituto Nacional de Higiene “Rafael Rangel” (INHRR) mediante pruebas  fisiológicas: 1) crecimiento en SDC a 42 ºC (positivo); 2) crecimiento en agar  Mycosel® (BBL) a 28 ºC (negativo); 3) hidrólisis de gelatina, urea y caseína  (positivas) y 4) asimilación de azúcares según el método de Huppert, con  asimilación de glucosa, maltosa, sacarosa, celobiosa, xilosa, rafinosa y  trehalosa, pero no para lactosa, melibiosa e inositol [8].</span></font></p>     <p style="text-autospace: none" align="justify"><font face="Verdana"> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">La cepa fue  identificada como </span><i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPS-ItalicMT"> Cunninghamella bertholletiae </span></i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">y fue confirmada  adicionalmente por taxónomos de la Universidad de La Rovira i Virgili, Reus,  España, en el año 2006. Se encuentra preservada en la colección de hongos del  Departamento de Micología del INHRR.</span></font></p>     <p style="text-autospace: none" align="justify"><font face="Verdana"><b> <span style="font-size: 10.0pt; font-family: TimesNewRomanPS-BoldMT">Discusión</span></b></font></p>     <p style="text-autospace: none" align="justify"><font face="Verdana"> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">La taxonomía de  los agentes causales de mucormicosis (antes denominada zigomicosis) ha sufrido  grandes cambios en la última década. Los análisis filogenéticos han confirmado  que la división Zygomycota es polifilética y los taxones clasificados en ella  fueron distribuidos en la división Glomeromycota. Esta división agrupa 4  subdivisiones de ubicación taxonómica incierta (</span><i><span style="font-size: 10.0pt; font-family: TimesNewRomanPS-ItalicMT">incertae  sedis</span></i><span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">),  siendo una de ellas la subdivisión Mucoromycotina, que alberga a su vez al orden  Mucorales que contiene 6 familias. Dentro de la familia Cunninghamellaceae, </span><i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPS-ItalicMT">C.  bertholletiae </span></i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">es la única  especie causante de mucormicosis que es clínicamente importante y se ha descrito  casi exclusivamente (98%) en pacientes inmunocomprometidos [2,4,9,10]. Esta  nueva clasificación afectó también la denominación de la enfermedad, ya que el  término zigomicosis se basaba en la división Zygomycota. Actualmente se ha  retomado el nombre de mucormicosis, que es el más conocido a nivel clínico y a  su vez ofrece estabilidad en la nomenclatura [2,10].</span></font></p>     <p style="text-autospace: none" align="justify"><font face="Verdana"> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">La forma  predominante de adquisición de </span><i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPS-ItalicMT">C.  bertholletiae </span></i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">es por vía  inhalatoria. Las esporangiosporas son pequeñas y pueden permanecer en el aire  por largos períodos de tiempo, incrementando así el riesgo de exposición. Este  hongo frecuentemente coloniza las mucosas oral, nasal y los senos paranasales;  su penetración, con la subsecuente extensión a la región rinocerebral o a  pulmón, es una forma frecuente de infección [1,2,4,5]. En el caso de este  paciente, su enfermedad de base y grado de inmunosupresión fueron los factores  que favorecieron la colonización y el desarrollo posterior de la infección  [2,4,6].</span></font></p>     <p style="text-autospace: none" align="justify"><font face="Verdana"> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">Según las  revisiones realizadas por algunos investigadores, se ha reportado un total de 44  casos de mucormicosis por </span><i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPS-ItalicMT">C.  bertholletiae </span></i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">en la literatura  inglesa (desde 1983 hasta 2011). Nueve casos correspondieron a la presentación  rinocerebral de la enfermedad y sólo en uno la forma rinosinusal fue el punto de  partida de la diseminación. Únicamente dos casos correspondieron a la  presentación localizada de la infección a nivel rinosinusal y en uno la  enfermedad se diseminó a los pulmones [4-6]. Según el índice de literatura  científica y técnica en salud de América Latina y del Caribe (LILACS, <a href="http://lilacs.bvsalud.org/es/">http://lilacs.bvsalud.org/es/</a>) y el  índice SciELO (Scientific Electronic Library Online, <a href="http://www.scielo.org/es">www.scielo.org/es</a>), sólo se ha reportado  un caso de mucormicosis rinosinusal causado por </span><i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPS-ItalicMT">Mucor </span></i><span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">spp.  (Aguad L y col. Rev Chil Infect 2004; 21:345-50), por lo tanto, y a pesar del  tiempo transcurrido, este pudiera ser el primer informe venezolano y  latinoamericano reportado en la literatura de mucormicosis rinosinusal, causado  por </span><i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPS-ItalicMT">C.  bertholletiae</span></i><span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">.  Es probable que la infección diseminara a pulmón en este paciente, pero no puede  afirmarse con seguridad ya que no se realizó la necropsia.</span></font></p>     <p style="text-autospace: none" align="justify"><font face="Verdana"> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">El diagnóstico  definitivo de la mucormicosis requiere evidencia de la invasión del tejido por  el hongo [1,3,4,7]. En este paciente, en el examen directo del tejido se  evidenciaron las hifas típicas de los Mucorales que permitieron establecer el  diagnóstico. El material clínico debe cultivarse a temperaturas entre 25-37 ºC  (Fuerza de recomendación A, nivel de evidencia II) [3]. </span><i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPS-ItalicMT"> Cunninghamella </span></i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">spp. puede  hallarse en el laboratorio como un contaminante ambiental, sin embargo, un  aislamiento de </span><i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPS-ItalicMT">C.  bertholletiae</span></i><span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">,  proveniente de cualquier muestra clínica, debe analizarse con precaución ya que  no es un contaminante ordinario. Su crecimiento a temperaturas por encima de los  40 ºC la diferencia de otra especie del género como </span><i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPS-ItalicMT">C.  elegans </span></i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">[3,4,7].</span></font></p>     <p style="text-autospace: none" align="justify"><font face="Verdana"> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">La  identificación de género y especie es importante para mejorar el conocimiento  epidemiológico de la mucormicosis (Fuerza de la recomendación A, nivel de  evidencia II) [3]. La identificación de los Mucorales mediante técnicas  micológicas convencionales es difícil, debido a que las diferentes especies del  orden comparten características morfológicas similares [3,4,7]. Las técnicas  moleculares son más confiables (Fuerza de la recomendación A, nivel de evidencia  II) [3], pero éstas no se encuentran disponibles en los laboratorios de  microbiología. Sin embargo, pruebas como la asimilación de carbohidratos  permiten una identificación precisa de estos hongos a nivel de especie (Fuerza  de la recomendación B, nivel de evidencia II) [3,7], tal y como sucedió en este  caso.</span></font></p>     <p style="text-autospace: none" align="justify"><font face="Verdana"> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">La desbridación  quirúrgica del tejido infectado es un aspecto crítico y debe realizarse lo antes  posible para limitar la diseminación de la infección [1,3]. La extirpación  repetida y oportuna del tejido necrótico proporcionó al paciente un beneficio  significativo, como pudo evidenciarse en este caso. El tratamiento para la  mucormicosis es la anfotericina B (AB) deoxicolato, pero sus formulaciones  lipídicas, como la AB liposomal (Ambisome®) y la ABDC (Amphotec®), son mejor  toleradas por el paciente y son consideradas de primera línea como monoterapia  para la enfermedad (Fuerza de la recomendación A, nivel de evidencia II) [1-4].  La dosis estándar recomendada, independientemente del agente que la produce y de  la formulación, es de 5 mg/kg y se han propuesto dosis más elevadas de hasta  7,5-10 mg/kg (Fuerza de la recomendación A, nivel de evidencia II) [1,3,4]. El  inicio temprano del tratamiento con polienos ha sido asociado con mejoras en la  tasa de supervivencia de los pacientes [2,4]. La infección por </span><i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPS-ItalicMT">C.  bertholletiae </span></i> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">posee el peor  pronóstico entre todas las infecciones causadas por los Mucorales [4], pero este  paciente respondió apropiadamente a la combinación de terapia quirúrgica  asociada a ABDC, lo que incrementó su supervivencia.</span></font></p>     <p style="text-autospace: none" align="justify"><font face="Verdana"> <span style="font-size: 10.0pt; font-family: TimesNewRomanPSMT">En el manejo  clínico de la mucormicosis los factores críticos son el diagnóstico temprano, la  reversión de los factores predisponentes (si es posible), el desbridamiento  quirúrgico y una terapia antifúngica apropiada y precoz. Sin embargo, el éxito  de la intervención podría verse amenazado por el delicado equilibrio del sistema  inmune que poseen los pacientes inmunocomprometidos.</span></font></p>     ]]></body>
