<self-uri xlink:href="http://ve.scielo.org/scielo.php?script=sci_arttext&amp;pid=S0001-63652007000300020&amp;lng=en&amp;nrm=iso"></self-uri><self-uri xlink:href="http://ve.scielo.org/scielo.php?script=sci_abstract&amp;pid=S0001-63652007000300020&amp;lng=en&amp;nrm=iso"></self-uri><self-uri xlink:href="http://ve.scielo.org/scielo.php?script=sci_pdf&amp;pid=S0001-63652007000300020&amp;lng=en&amp;nrm=iso"></self-uri><abstract abstract-type="short" xml:lang="es"><p><![CDATA[RESUMEN De acuerdo a numerosos autores el tratamiento para el quiste dentígero ha sido la eliminación total del quiste incluyendo el diente involucrado aunque si la lesión se presenta en pacientes jóvenes que están en período de erupción dentaria y las características clínicas y radiográficas sugieren la presencia de un quiste dentígero, existe una alternativa de tratamiento la cual es la de mantener el diente permanente no erupcionado y enuclear completamente la lesión, siempre que se obtenga el diagnóstico definitivo histopatológico confirmatorio de quiste dentígero. Se realizó un estudio clínico en 12 pacientes de 7 a 16 años de edad con un promedio de 9,4 años, quienes fueron tratados en el Postgrado de Cirugía Bucal de la Facultad de Odontología de la Universidad Central de Venezuela (UCV). Todos los pacientes tenían las características clínicas y radiográficas similares a las del quiste dentígero, se encontraron 5 mujeres y 7 hombres, los cuales fueron sometidos a biopsia excisional, donde se les eliminó la lesión por completo, dejándoles el diente permanente no erupcionado. Las lesiones eran en la mandíbula y las imágenes radiolúcidas eran bien delimitadas al cuello dentario y menores a 2,5 cms. de diámetro. Los pacientes con quiste dentígero asociado a un tercer molar fueron excluídos del estudio. El estudio histopatológico fue confirmatorio de quiste dentígero para cada paciente y a cada uno se les realizó un seguimiento clínico y radiográfico periódico desde 2 meses hasta 7 años, con un intervalo aproximado de cada 6 meses. (1999-2006). No se encontraron imágenes radiolúcidas en ninguna de las radiografías post-quirúrgicas. Todos mostraron nueva formación ósea alrededor del diente involucrado y erupción completa de la dentición permanente. Tres(3) pacientes fueron seguidos por 7 años, cuatro(4) por 5 años, dos(2) por 3 años con 9 meses y tres(3) por 2 años y 6 meses. A dos(2) de los pacientes se les realizó tratamiento ortodóncico para corregir su maloclusión. Preservar el diente involucrado en un Quiste Dentígero al enuclear totalmente la lesión es una alternativa de procedimiento quirúrgico  con resultados clínicos y radiográficos predecibles y estables. Este tratamiento puede ser usado como un manejo quirúrgico conservador que permite la erupción normal de la dentición permanente. <abstract abstract-type="short" xml:lang="en"><p><![CDATA[ABSTRACT According to most authors the common treatment for the dentigerous cyst has been total elimination of the cyst including the tooth involved; however if the lesion is present in young patients during the eruption period, and the radiographic and clinical features are suggesting of dentigerous cyst, an alternative treatment is the maintenance of the permanent tooth and enucleation of the lesion alone, stating that ameloblastomatous changes or other odontogenic lesions have been ruled out after histopathological analysis, and a diagnosis of dentigerous cyst is confirmed. This study was conducted on 12 patients with an age range of 6 to 16 years with a mean age of 9.4 years that were treated at the Oral Surgery Postgraduate Department, Dental School, Central University of Venezuela. There were 5 females and 7 males. All lesions were mandibular and all of them were unilocular and well-defined radiolucencies measuring less than 2,5 cms in greatest diameter. All cases involved the coronal portion with cervical attachment of an unerupted permanent tooth. Third molars were excluded. Patients underwent surgery (excisional biopsy) of the cyst with preservation of the involved tooth. Histologic diagnosis of dentigerous cyst was confirmed in each case. Clinical and radiographic follow-up was performed at 6 months intervals up to 7 years (1999-2006). There were no recurrences of radiolucent lesions in any of the post-surgical x-rays. Three (3) patients were followed for 7 years, four (4) patients for 5 years, two (2) patients for 3 years and 9 moths and three (3) patients for 2 years and 6 months. The cases showed new bone formation surrounding the teeth involved and complete eruption of the permanent dentition. Two (2) of the patients had to use orthodontic appliances to correct their malocclusion. Preservation of the tooth involved in a Dentigerous Cyst and the enucleation of the total lesion is an alternative surgical procedure with predictable and stable clinical and radiographic results. This treatment could be used as a conservative management for the dentigerous cyst allowing the normal eruption of the permanent dentition. Según Regezi y col en 1999 y Marx y col en el 2003, tiende a tener una predilección ligera por el sexo masculino que va de 1.6 : 1. Generalmente es asintomático a menos que se infecte secundariamente pero puede producir inflamación y expansión ósea. <sup>(1-2 -3)</sup></font></span></p>     <p class="MsoNormal" style="tab-stops: 396.0pt; word-spacing: 0; line-height: 100%; margin-right: .15pt; margin-bottom: 0" align="justify"><span lang="ES-VE" style="mso-ansi-language:ES-VE"><font face="Verdana" size="2">Radiográficamente consiste en una radiolucidez bien circunscrita, unilocular aunque a veces podría ser multilocular, relacionada con la corona de un diente no erupcionado o en desarrollo y que puede desplazar a los dientes vecinos.<sup>(2-3)</sup></font></span></p>     ]]></body>
<body><![CDATA[<p class="MsoNormal" style="tab-stops: 396.0pt; word-spacing: 0; line-height: 100%; margin-right: .15pt; margin-bottom: 0" align="justify"><font face="Verdana" size="2"><span lang="ES-VE" style="mso-ansi-language:ES-VE">El tratamiento más común es la completa eliminación quirúrgica de la lesión y dependiendo de si el diente permanente está involucrado más allá de la región cervical o si es de gran tamaño que al eliminar la lesión, el diente está dentro del tejido a extirpar, será extraído, de otra forma se puede ser más conservador y dejar el diente involucrado pero la cirugía debe ser realizada con mucha seguridad de haber eliminado todo el tejido lesional para evitar recidivas o complicaciones post-quirúrgicas. Algunos autores han reportado el tratamiento quirúrgico conservador al dejar el diente permanente involucrado.<sup> (4-5-6-7-8-9)</sup></span></font></p>     <p class="MsoNormal" style="tab-stops: 396.0pt; word-spacing: 0; line-height: 100%; margin-right: .15pt; margin-bottom: 0" align="justify"><font face="Verdana" size="2"><span lang="ES-VE" style="mso-ansi-language:ES-VE">El no ser tratado adecuadamente y a tiempo pudiera ocasionar, transformación de ameloblastoma, desarrollo de carcinoma epidermoide y/o carcinoma mucoepidermoide.<sup> (1-2)</sup></span></font></p>     <p class="MsoNormal" style="tab-stops: 396.0pt; word-spacing: 0; line-height: 100%; margin-right: .15pt; margin-bottom: 0" align="justify"><span lang="ES-VE" style="mso-ansi-language:ES-VE"><font face="Verdana" size="2">El propósito de esta investigación clínica fue demostrar la eficacia del seguimiento postquirúrgico periódico que permitió demostrar que el tratamiento quirúrgico conservando el diente permanente no erupcionado involucrado con el quiste dentígero es exitoso. Estableciendo que se pueden mantener los dientes no erupcionados asociados con quistes dentígeros de pequeño a mediano(menos de 2,5 cms) tamaño que sólo hayan abarcado hasta el cuello de la corona del diente involucrado.</font></span></p>     <p class="MsoNormal" style="tab-stops: 396.0pt; word-spacing: 0; line-height: 100%; margin-bottom: 0" align="justify"><b style="mso-bidi-font-weight: normal"><span lang="ES-VE" style="mso-ansi-language:ES-VE"><font face="Verdana" size="2">PACIENTES Y METODOS</font></span></b></p>     <p class="MsoNormal" style="tab-stops: 396.0pt; word-spacing: 0; line-height: 100%; margin-bottom: 0" align="justify"><span lang="ES-VE" style="mso-ansi-language:ES-VE"><font face="Verdana" size="2">Este estudio fue conducido en 12 pacientes tratados en el Postgrado de Cirugía Bucal de la Facultad de Odontología de la UCV con edades comprendidas entre 7 y 16 años, promedio de 9,4 años de edad, de los cuales 5 fueron mujeres y 7 hombres, lo cual coincide con la literatura revisada.<sup> (1-2-3-4) </sup>Todas las lesiones estaban localizadas en la mandíbula, mostrando diferentes grados de expansión ósea, la cual se confirmó con radiografías oclusales y todas eran asintomáticas. En las radiografías panorámicas se observaron imágenes radiolúcidas bien delimitadas a la región cervical de dientes permanentes no erupcionados, midiendo menos de 2,5 cms de diámetro. Se excluyeron los pacientes que presentaron quistes dentígeros asociados a terceros molares no erupcionados. Los pacientes fueron sometidos a eliminación quirúrgica de la lesión (biopsia excisional) preservando el diente involucrado ya que en ningún caso el tejido que lo rodeaba lo movía y el resultado histopatológico reportó quiste dentígero lo que confirmó el diagnóstico clínico en todos los pacientes estudiados, por lo cual se les hizo un seguimiento clínico y radiográfico contínuo que comenzó a los 2 meses y luego del primer año cada 6 a 12 meses hasta por 7 años. (1999-2006)</font></span></p>     <p class="MsoNormal" style="tab-stops: 396.0pt; word-spacing: 0; line-height: 100%; margin-bottom: 0" align="justify"><b style="mso-bidi-font-weight: normal"><font face="Verdana" size="2"><span lang="EN-US" style="mso-bidi-font-size: 10.0pt">RESULTADOS</span></font></b></p>     <p class="MsoNormal" style="tab-stops: 396.0pt; word-spacing: 0; line-height: 100%; margin-bottom: 0" align="justify"><font face="Verdana" size="2"><span lang="ES-VE" style="mso-ansi-language: ES-VE">No hubo recurrencia de imágenes radiolúcidas en ninguna de las radiografías postquirúrgicas durante el seguimiento periódico de los pacientes. Tres (3) pacientes fueron seguidos por 7 años y uno de ellos tenía la lesión más abajo de la región cervical del primer molar inferior permanente no erupcionado y ahora está completamente en oclusión pero tuvo que ser ayudado con ortodoncia. Todos los casos mostraron buena formación ósea alrededor del diente involucrado con la lesión y completa<span style="mso-spacerun: yes">&nbsp; </span>erupción de la dentición permanente. Los dientes erupcionados mostraron adecuada vitalidad pulpar y llegaron a ocluir correctamente. Dos (2) de los pacientes necesitaron tratamiento ortodóncico para corregir su maloclusión sin extracciones.</span></font></p>     <p class="MsoNormal" style="tab-stops: 396.0pt; word-spacing: 0; line-height: 100%; margin-bottom: 0" align="justify"><b style="mso-bidi-font-weight: normal"><font face="Verdana" size="2"><span lang="ES-VE" style="mso-bidi-font-size: 10.0pt; mso-ansi-language: ES-VE">CONCLUSIONES</span></font></b></p>     <p class="MsoNormal" style="tab-stops: 396.0pt; word-spacing: 0; line-height: 100%; margin-bottom: 0" align="justify"><font face="Verdana" size="2"><span lang="ES-VE" style="mso-ansi-language: ES-VE">- Preservar el diente involucrado asociado a un Quiste Dentígero en pacientes pediátricos al momento de enuclear completamente la lesión es una alternativa de tratamiento quirúrgico conservador con resultados radiográficos y clínicos tangibles, predecibles y estables.</span></font></p>     <p class="MsoNormal" style="tab-stops: 396.0pt; word-spacing: 0; line-height: 100%; margin-bottom: 0" align="justify"><font face="Verdana" size="2"><span lang="ES-VE" style="mso-ansi-language: ES-VE">- El tamaño de la lesión y la ubicación limitada a la corona del diente permanente no erupcionado involucrado, a pesar de que en un caso se usó el criterio per operatorio basado en la no movilidad y fuerte unión ósea a las paredes alveolares remanentes al diente involucrado, mientras se enucleaba toda la lesión alrededor del mismo, pensando en no dañar su futura dentición permanente que le permitiría una adecuada y completa oclusión.</span></font></p>     <p class="MsoNormal" style="tab-stops: 396.0pt; word-spacing: 0; line-height: 100%; margin-bottom: 0" align="justify"><font face="Verdana" size="2"><span lang="ES-VE" style="mso-ansi-language: ES-VE">- Es muy importante contar con un diagnóstico histopatológico definitivo que confirme la ausencia de cambios amelobásticos del epitelio de la lesión, ya que de lo contrario tendríamos que repetir el procedimiento quirúrgico siendo más agresivos y removiendo el diente involucrado que se dejó.</span></font></p>     <p class="MsoNormal" style="tab-stops: 396.0pt; word-spacing: 0; line-height: 100%; margin-bottom: 0" align="justify"><font face="Verdana" size="2"><span lang="ES-VE" style="mso-ansi-language: ES-VE">- En todos los casos, los padres fueron bien informados de la importancia del control radiográfico periódico durante un largo tiempo.</span></font></p>     <p class="MsoNormal" style="tab-stops: 396.0pt; word-spacing: 0; line-height: 100%; margin-bottom: 0" align="justify"><font face="Verdana" size="2"><span lang="ES-VE" style="mso-ansi-language: ES-VE">- Los resultados epidemiológicos encontrados en cuanto a sexo, edad, características clínicas y radiográficas de los pacientes diagnosticados con Quiste Dentígero, reflejan una similar coincidencia con los reportes de la literatura revisada.</span></font></p>     <p class="MsoNormal" style="tab-stops: 396.0pt; word-spacing: 0; line-height: 100%; margin-bottom: 0" align="justify"><font face="Verdana" size="2"><span lang="ES-VE" style="mso-ansi-language: ES-VE">- Este tratamiento conservador del quiste dentígero ocasionalmente incluyó el uso de ortodoncia.</span></font></p>     <p class="MsoNormal" style="tab-stops: 396.0pt; word-spacing: 0; line-height: 100%; margin-bottom: 0" align="justify"><font face="Verdana" size="2"><span lang="ES-VE" style="mso-ansi-language: ES-VE">- Es muy relevante el marcar la importancia del manejo multidisciplinario en éste estudio clínico, ya que los cirujanos bucales, patólogos bucales, ortodoncistas y odontopediatras deben trabajar en constante comunicación, en equipo y es reponsabilidad del cirujano mantener la evaluación postquirúrgica contínua de sus pacientes e informarle a sus colegas sobre el seguimiento de los mismos.</span></font></p>     ]]></body>
