MÉTODOS: Revisión retrospectiva de las historias clínicas de pacientes con cáncer de mama sometidas a reconstrucción mamaria posterior a una mastectomía entre enero de 2000 y diciembre de 2004. Se registró edad, estadio de la enfermedad, tiempo en que se efectuó la reconstrucción, técnica utilizada y complicaciones. RESULTADOS: Un total de 146 pacientes fueron sometidas a reconstrucción mamaria, con edad promedio de 41 años. A 110 pacientes se les realizó la reconstrucción con colgajo miocutáneo de dorsal ancho (75,3 %) y a 36 pacientes con colgajo miocutáneo de recto anterior del abdomen (24,7 %). En 69,9 % de las pacientes el procedimiento de reconstrucción fue realizada en forma inmediata. El 30,8 % de las pacientes presentaron complicaciones secundarias al procedimiento de reconstrucción. La principal complicación en la reconstrucción con colgajo de dorsal ancho fue la formación de contractura capsular sintomática (28,8 %). La complicación más frecuente con el colgajo de recto anterior fue la eventración abdominal (13,3 %). CONCLUSIONES: La reconstrucción mamaria posterior a la mastectomía representa un procedimiento seguro, que puede ser realizado de manera inmediata a la mastectomía, empleando diversas técnicas quirúrgicas. El tratamiento adyuvante con radioterapia no afecta de manera significativa sus resultados estéticos, con un gran beneficio psicológico para la paciente mastectomizada. Igualmente se puede realizar en cualquier estadio sin afectar la morbilidad posoperatoria.]]></p></abstract>

In 69.9 % of patients breast reconstruction was done immediately after mastectomy. Postoperative complications due to the reconstructive procedure were seen in 30.8 % of patients. The main complication after latissimus dorsi reconstruction was a symptomatic capsular contraction (28.8 %), whereas after a rectus anterior abdominal flap, the abdominal incisional hernia was most commonly observed (13.3 %). Desde la introducci&oacute;n en 1982 de la t&eacute;cnica para la reconstrucci&oacute;n con colgajo miocut&aacute;neo del recto abdominal (TRAM) por Hartrampf y col.<sup>(1)</sup>, se han descrito muchas otras opciones entre la que se incluyen el uso de pr&oacute;tesis y expansores<sup>(2)</sup>, colgajos dermograsos, miocut&aacute;neos e injertos libres<sup>(3)</sup>.  La selecci&oacute;n del procedimiento adecuado depende de m&uacute;ltiples factores tales como: la edad del paciente, estadio de la enfermedad, h&aacute;bito tab&aacute;quico, obesidad, necesidad de tratamiento adyuvante, experiencia y capacidad t&eacute;cnica del centro donde se va a realizar la reconstrucci&oacute;n, entre otros<sup>(4)</sup>.</font></P>     <P align="JUSTIFY" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2">Similarmente el momento de la realizaci&oacute;n del procedimiento, bien sea inmediatamente posterior a la mastectom&iacute;a o de manera diferida, depende del centro donde se realice el tratamiento<sup>(5)</sup>.  Hay autores que defienden las reconstrucciones diferidas (entre 6 meses a 1 a&ntilde;o posterior a la mastectom&iacute;a), bas&aacute;ndose en la posibilidad de recurrencia local, la necesidad de tratamiento adyuvante y su posible efecto delet&eacute;reo en los colgajos y el impacto psicol&oacute;gico sobre la paciente que permaneci&oacute; un tiempo &quot;mutilada&quot; y que ahora tiene la ventaja de la reconstrucci&oacute;n.  Sin embargo, la mayor&iacute;a de los centros recomiendan que &eacute;sta se haga de manera inmediata, puesto que m&uacute;ltiples estudios sugieren que no existe diferencia en cuanto a seguimiento, recurrencia local y sobrevida libre de enfermedad, sumado a la mejor&iacute;a sobre el estatus ps&iacute;quico y la necesidad de menos intervenciones quir&uacute;rgicas.</font></P>     <P align="JUSTIFY" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2">El objetivo de este trabajo es revisar la experiencia en reconstrucci&oacute;n mamaria posterior a la mastectom&iacute;a en las pacientes tratadas con c&aacute;ncer de mama en el Instituto de Oncolog&iacute;a Luis Razetti, sus caracter&iacute;sticas y complicaciones.</font></P> <B>    <P align="justify" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2">M&Eacute;TODOS</font></P>     <P align="justify" style="word-spacing: 0; line-height: 100%"></B><font face="Verdana" size="2">Se realiz&oacute; un estudio retrospectivo, revisando las historias cl&iacute;nicas de las pacientes con c&aacute;ncer de mama que fueron sometidas a reconstrucci&oacute;n posterior a una mastectom&iacute;a, en el Instituto de Oncolog&iacute;a Luis Razetti, entre enero de 2000 hasta diciembre de 2004.</font></P>     <P align="JUSTIFY" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2">Se determin&oacute; edad, estadio de la enfermedad, tiempo en que se efectu&oacute; la reconstrucci&oacute;n, t&eacute;cnica utilizada, y complicaciones posoperatorias.</font></P> <B>    <P align="justify" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2">RESULTADOS</font></P>     ]]></body>
<body><![CDATA[<P align="justify" style="word-spacing: 0; line-height: 100%"></B><font face="Verdana" size="2">Un total de 146 pacientes fueron sometidas a reconstrucci&oacute;n mamaria por el Servicio de Cirug&iacute;a Pl&aacute;stica del Instituto de Oncolog&iacute;a Luis Razetti entre enero de 2000 y diciembre de 2004.  La edad de las pacientes vari&oacute; desde los 25 hasta los 61 a&ntilde;os con un promedio de 41 a&ntilde;os.  Quince pacientes ten&iacute;an diagn&oacute;stico de carcinoma <I>in situ</I> (10,3 %), 10 (6,8 %) c&aacute;ncer de mama estadio I, 70 (47,9 %) pacientes estadio II, 46 (31,5 %) estadio III y 5 (3,4 %) estadio IV.</font></P>     <P align="JUSTIFY" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2">Las dos t&eacute;cnicas quir&uacute;rgicas utilizadas para estas pacientes fueron el colgajo miocut&aacute;neo de dorsal ancho y de recto anterior del abdomen (TRAM).  En el caso del TRAM todos los colgajos fueron bipediculados.  A 110 pacientes se les realiz&oacute; la reconstrucci&oacute;n con dorsal ancho (75,3 %) y a 36 pacientes con TRAM (24,7 %).</font></P>     <P style="word-spacing: 0; line-height: 100%" align="justify"><font face="Verdana" size="2">En el 69,9 % de los pacientes el procedimiento de reconstrucci&oacute;n fue realizada en forma inmediata (102 pacientes).  En el restante 30,1 % (44 pacientes) se realiz&oacute; el procedimiento reconstructivo de manera tard&iacute;a, entre 1 a 10 a&ntilde;os posterior a culminar el tratamiento adyuvante, con un promedio de 3,14 a&ntilde;os.</font></P>     <P align="JUSTIFY" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2">De las pacientes a quienes se les realiz&oacute; la reconstrucci&oacute;n con dorsal ancho, en 85 pacientes se realiz&oacute; de manera inmediata y en 25 de manera diferida.  De las pacientes reconstru&iacute;das con colgajo TRAM, 17 fueron realizadas de manera inmediata y 19 en forma tard&iacute;a.</font></P>     <P align="JUSTIFY" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2">El 30,8 % de las pacientes presentaron complicaciones secundarias al procedimiento de reconstrucci&oacute;n.  La principal complicaci&oacute;n posoperatoria (28,8 % del total de complicaciones) fue la formaci&oacute;n de contractura capsular sintom&aacute;tica en la reconstrucci&oacute;n con colgajo miocut&aacute;neo de dorsal ancho.  La complicaci&oacute;n m&aacute;s frecuente en los pacientes que se reconstruyeron con colgajo tipo TRAM fue la eventraci&oacute;n abdominal (13,3 %).  Hubo necrosis del colgajo en 6,6 % de los pacientes, 2 con dorsales y 1 con TRAM.  Las complicaciones inmediatas del procedimiento, independientemente del tipo de colgajo utilizado, fue epidermiolisis (22,2 %), seromas (13,3 %) y celulitis (6,6 %).  Seis de 105 pacientes con pr&oacute;tesis como parte de la reconstrucci&oacute;n presentaron extrusi&oacute;n y rechazo de la misma. En nuestra revisi&oacute;n la mayor&iacute;a de las pacientes (65 %) se encontraban en estadios tempranos de la enfermedad (estadios 0, I y II).  Sin embargo, un grupo considerable de pacientes (45 % restante) se encontraban dentro de los estadios m&aacute;s avanzados (III y IV), lo que demuestra la factibilidad t&eacute;cnica del procedimiento en este grupo de pacientes, similar a los resultados descritos en la literatura<sup>(6)</sup>.</font></P>     <P align="JUSTIFY" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2">La realizaci&oacute;n de procedimientos reconstructivos en tipo y n&uacute;mero var&iacute;a ampliamente de un centro a otro, desde un 5 % a 10 % en pa&iacute;ses  como  Australia  e  Inglaterra,  hasta  un 30 % de las pacientes mastectomizadas en EE.UU<sup>(7,8)</sup>.  A pesar de las m&uacute;ltiples t&eacute;cnicas descritas<sup>(3)</sup>, en nuestro centro s&oacute;lo realizamos dos de ellas: el colgajo miocut&aacute;neo de dorsal ancho y el TRAM.  La t&eacute;cnica m&aacute;s utilizada fue la primera representando un 75 % de los casos casi siempre acompa&ntilde;ada de la colocaci&oacute;n de una pr&oacute;tesis en el mismo acto quir&uacute;rgico.  En las &uacute;ltimas d&eacute;cadas se ha incrementado a nivel mundial el uso del TRAM como m&eacute;todo de reconstrucci&oacute;n por sus m&uacute;ltiples ventajas<sup>(5)</sup>, sin embargo, en nuestro centro s&oacute;lo representa la t&eacute;cnica usada en el 25 % de las pacientes.</font></P>     <P align="JUSTIFY" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2">El momento de la cirug&iacute;a reconstructiva es otro tema controversial en la literatura<sup>(9-11)</sup>.  Inicialmente se propon&iacute;a que &eacute;sta deb&iacute;a ser realizada de manera tard&iacute;a b&aacute;sicamente por las probabilidades de recurrencia local, y la necesidad de tratamiento adyuvante.  Sin embargo, en los &uacute;ltimos a&ntilde;os se ha demostrado que esto no afecta de manera importante los resultados de la reconstrucci&oacute;n<sup>(5)</sup>.  As&iacute; se prefiere la reconstrucci&oacute;n inmediata versus la tard&iacute;a, siempre individualizando a cada una de las pacientes y escogiendo el procedimiento m&aacute;s adecuado para ella.  En nuestra revisi&oacute;n en el 69,9 % de los casos la reconstrucci&oacute;n se realiz&oacute; en el mismo acto quir&uacute;rgico de la mastectom&iacute;a en comparaci&oacute;n con un 30,1 % de procedimientos diferidos desde 1 hasta 10 a&ntilde;os posterior al tratamiento completo.</font></P>     ]]></body>
<body><![CDATA[<P align="JUSTIFY" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2">La reconstrucci&oacute;n inmediata puede proporcionar un gran beneficio psicol&oacute;gico a la paciente mastectomizada y, por ello, disfruta actualmente de una gran popularidad.  Sin embargo, se asocia a un porcentaje mayor de complicaciones, tales como necrosis tisular, dehiscencia de suturas, seroma e infecci&oacute;n.  Estas complicaciones est&aacute;n relacionadas principalmente con el adelgazamiento de los tejidos preservados, tensi&oacute;n a la que son suturados y realizaci&oacute;n de dos procedimientos quir&uacute;rgicos en el mismo acto operatorio.  En este caso, las complicaciones se presentaron en el 30,8 % de los pacientes, siendo la m&aacute;s frecuente la formaci&oacute;n de c&aacute;psula alrededor de la pr&oacute;tesis, seguida de la epidermiolisis.</font></P>     <P align="JUSTIFY" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2">Por &uacute;ltimo, el porcentaje de recurrencia local  en  las  diferentes  series  oscila  entre  3 % - 15  %<sup>(12,13)</sup>.   En  este  estudio  reportamos  un 3,4 %, correspondi&eacute;ndose con los datos revisados de la literatura.</font></P>     <P align="JUSTIFY" style="word-spacing: 0; line-height: 100%"><font face="Verdana" size="2">Concluimos que la reconstrucci&oacute;n mamaria posterior a la mastectom&iacute;a con sus diferentes t&eacute;cnicas representa un procedimiento seguro y que se debe implementar en todos los centros en que se manejen pacientes con c&aacute;ncer de mama.
