Este estudio analizó, mediante Amplificación al Azar del ADN Polimórfico (RAPD) con cuatro oligonucleótidos (M13, AP3, T3B y R108), la variabilidad genética de 11 aislados clínicos del complejo C. parapsilosis, obtenidos en los servicios de Medicina Interna y Cirugía General de un hospital de la Ciudad de México, e identificar si los pacientes fueron infectados por el mismo genotipo. La cepa ATCC® 22019™ fue incluida en el análisis. Los aislados se identificaron por VITEK 2 Compact® y PCR. Con base en los perfiles polimórficos,se construyó un dendrograma por UPGMA y se calcularon el coeficiente de correlación cofenética(CCCr), el índice de asociación (I A) y los indicadores de diversidad genética. Todos los aislados fueron identificados como C. parapsilosis sensu stricto. El dendrograma mostró dos grupos (I y II), en el I se encontraron tres genotipos integrados por la cepa 22019 y cinco aislados asociados en su mayoría a candidiasis invasiva; el II mostró seis genotipos integrados por seis aislados asociados en su mayoría a candidiasis mucocutánea. El I A y los indicadores de diversidad genética obtenidos revelaron un sistema de reproducción recombinante. El RAPD con los oligonucleótidos M13, AP3, T3B y R108 es útil en la investigación de posibles brotes causados por C. parapsilosis y en la determinación de su variabilidad genética.<hr/>The infection caused by the Candida parapsilosis complex may occur sporadically or in the form of outbreaks, so the study of the genetic variability in clinical isolates may reveal the presence of endemic genotypes and the occurrence of horizontal transmission. This study analyzed the genetic variability of 11 clinical isolates of the C. parapsilosis complex obtained from Internal Medicine and General Surgery services of a hospital in Mexico City, using Random Amplification of Polymorphic DNA (RAPD) with four oligonucleotides (M13, AP3, T3B and R108), also evaluated whether the patients were infected by the same genotype. The strain ATCC® 22019 ™ was included in the analysis. Isolates were identified by VITEK 2 Compact® and PCR. With the polymorphic profiles, a dendrogram was constructed by UPGMA and the cophenetic correlation coefficient (CCCr), the association index (I A), and the indicators of genetic diversity were calculated. All isolates were identified as C. parapsilosis sensu stricto. The dendrogram showed two groups (I and II). Three genotypes integrated by the strain 22019 and five isolates, mostly associated with invasive candidiasis, were found in the group I. The group II showed six genotypes composed of six isolates, mostly associated with mucocutaneous candidiasis. The I A and the indicators of genetic diversity obtained, revealed a recombinant reproduction system. The RAPD with the oligonucleotides M13, AP3, T3B and R108 is useful in the investigation of possible outbreaks caused by C. parapsilosis and in the determination of their genetic variability. <![CDATA[<b>Dyslipidemia and tobacco smoking synergistically increase serum manganese</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0535-51332017000300003&lng=en&nrm=iso&tlng=en Manganese is a trace metal involved in both physiology and toxicity. The association between manganese and dyslipidemia has been scarcely revised, and results from studies in both animals and humans are inconsistent. The aim of this study was to evaluate the association between serum manganese levels and dyslipidemia, considering some manganese sources and factors that could affect its concentration, especially tobacco smoking. Serum manganese concentration in 63 volunteers was determined and their smoking habits were recorded. Dietary manganese, iron, fat and alcohol consumption was also estimated by a food-frequency questionnaire. A bivariate analysis was carried out to identify those factors affecting manganese concentration. Only dyslipidemia and smoking resulted statistically significant and thus were considered for the subsequent two-way analysis of variance, to test a possible interaction between dyslipidemia and smoking. Marginal means for serum manganese were as follows: 8.32 ± 2.14 nmol/L for nonsmokers without dyslipidemia, 9.21 ± 2.22 nmol/L for smokers without dyslipidemia, 10.21 ± 2.53 nmol/L for nonsmokers with dyslipidemia, and 14.21 ± 3.44 nmol/L for smokers with dyslipidemia. Dyslipidemia and tobacco smoking were synergistically associated with increased serum manganese. To maintain adequate manganese levels in the organism, other factors in addition to its dietary intake should be considered, for instance, lipid status and smoking habits, particularly in those conditions in which manganese accumulation is an issue.<hr/>El manganeso es un metal traza esencial involucrado tanto en procesos fisiológicos como en toxicidad. La asociación entre el manganeso y las dislipidemias se ha estudiado poco, y los resultados de estudios en animales y en humanos son inconsistentes. El objetivo de este trabajo fue evaluar la asociación entre el manganeso sérico y las dislipidemias, considerando algunas fuentes de manganeso y factores que pudieran afectar su concentración, especialmente el tabaquismo. Se determinaron las concentraciones séricas de manganeso de 63 voluntarios y se registraron sus hábitos de consumo de tabaco. Se estimó la ingesta de manganeso, hierro, grasa y alcohol mediante un cuestionario de frecuencia de consumo. Se realizó un análisis bivariado para identificar los factores que afectaron las concentraciones de manganeso; únicamente las dislipidemias y el tabaquismo resultaron estadísticamente significativos y se consideraron enel subsecuente análisis de varianza de dos vías, para examinar una posible interacción entre las dislipidemias y el tabaquismo. Las medias marginales para el manganeso sérico fueron: 8,32 ± 2,14 nmol/L para no fumadores sin dislipidemia, 9,21 ± 2,22 nmol/L para fumadores sin dislipidemia, 10,21 ± 2,53 nmol/L para no fumadores con dislipidemia, y 14,21 ± 3,44 nmol/L para fumadores con dislipidemia. Las dislipidemias y el tabaquismo se asociaron sinérgicamente con el aumento del manganeso sérico. Para mantener niveles adecuados de manganeso en el organismo, se deben tomar en cuenta factores adicionales a su consumo dietético, como el estatus lipídico y el tabaquismo, particularmente en condiciones en las que la acumulación de manganeso sea un problema. <![CDATA[<b>Chimo, a smokeless tobacco preparation, is associated with a higher frequency of type 2 diabetes</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0535-51332017000300004&lng=en&nrm=iso&tlng=en The relationship between smokeless tobacco (ST) use and type-2 diabetes (T2D) has only been reported in Swedish men, though with contradictory results. In Venezuela, chimó is the most common ST preparation. The relationship between chimó and T2D in Venezuela is unknown. The objective of the study was to evaluate the relationship between chimó use and T2D in a population with high prevalence of ST use in the Andes region of Venezuela. An observational, cross-sectional, correlational study was designed. During 2013-2014, 759 consecutive subjects aged 20 years or older were evaluated in a medical center. Anthropometric measurements and responses to a standard questionnaire were obtained. Blood glucose and lipid concentrations were measured. The mean age was 53.1 years, 58% were female, and 24.1% reported ST use. ST use was more frequent in men than women (32.9% vs. 17.7%; p < 0.001). ST users showed lower body mass index (BMI), body fat and total cholesterol, but a higher frequency of T2D, than non-ST users. Logistic regression analysis, adjusted by age, family history of T2D, and hypertriglyceridemia, demonstrated that ST use was associated with an increased odd for T2D by 77% (OR 1.77; 95% CI 1.15 - 2.72) among ST-users. In conclusion, chimó, a ST form frequently used in the Andes region of Venezuela, is associated with a higher frequency of T2D and lower fat mass. Implications of these findings are discussed.<hr/>La relación entre el uso de tabaco no inhalado (TBNI) y la diabetes mellitus tipo 2 (DM2) ha sido únicamente reportada en hombres de Suecia, con resultados contradictorios. En Venezuela, el chimó es la preparación más común de TBNI. La relación entre el chimó y la DM2 en Venezuela es desconocida. El objetivo de este estudio fue evaluar la relación entre el uso de chimó y la DM2 en una población con elevada prevalencia de uso de TBNI en la región de los Andes de Venezuela. Se diseñó un estudio observacional, transversal, tipo correlacional. Durante 2013-2014, 759 sujetos de 20 o más años fueron evaluados de forma consecutiva en un centro médico. La edad promedio fue 53,1 años, 58% fueron mujeres y 24,1% reportó uso de TBNI. El uso de TBNI fue más frecuente en hombres que en mujeres (32,9% vs. 17,7%; p < 0,001). Los consumidores de TBNI mostraron menor índice de masa corporal (IMC), grasa corporal y colesterol total, pero mayor frecuencia de DM2 que los no consumidores de TBNI. El análisis de regresión logística ajustado por edad, historia familiar de DM2 e hipertrigliceridemia, demostró que el uso de TBNI se asoció con un incremento de 77% de probabilidad de presentar DM2 (OR 1,77; 95% CI 1,15 - 2,72). En conclusión, el chimó, una forma de TBNI frecuentemente usada en la región de los Andes de Venezuela, está asociada con una mayor frecuencia de DM2 y con una menor masa grasa. Se discuten las implicaciones de este hallazgo. <![CDATA[<b>Novel prodrug derivative from lilolidine with central dopaminergic activity</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0535-51332017000300005&lng=en&nrm=iso&tlng=en La desregulación de la neurotransmisión dopaminérgica central ha sido relacionada con enfermedades neurodegenerativas, tales como la enfermedad de Parkinson y la Corea de Huntington. En las últimas décadas son muchos los compuestos con actividad dopaminérgica central que se han diseñado, sintetizado y evaluado farmacológicamente y, a pesar de estos esfuerzos, no se ha logrado obtener un fármaco efectivo capaz de mejorar o curar estas patologías. Con el fin de contribuir en esta búsqueda primordial, en el presente trabajo se describe el diseño, la síntesis y la evaluación farmacológica del derivado de la lilolidina, clorhidrato de 6-(2-aminoindanil)-N-(2,4,5,6-tetrahidro-1H-pirrolo[3,2,1-ij]quinolina) 4 (MAIL), con el propósito de restablecer la homeostasis de la transmisión dopaminérgica en la enfermedad de Parkinson y/o la Corea de Huntington. Para ello, se utilizaron las diferentes estrategias nigroesclásicas y heterocíclicas enmarcadas en la síntesis orgánica a través de una ruta convergente. Asimismo, se realizó la evaluación farmacológica preliminar, al determinar el comportamiento estereotipado en ratas Sprague-Dawley cuando les fue administrado el derivado de la lilolidina, por las vías ICV (intracerebroventricular) e IE (intraestriatal). Los resultados obtenidos del compuesto 4 mostraron una acción central como agonista, a través de mecanismos dopaminérgicos.<hr/>Central dopaminergic neurotransmission deregulation has been related with neurodegenerative sicknesses, like Parkinson’s disease and Huntington’s Chorea. During the last decades, many compounds with dopaminergic activity have been designed, synthesized and pharmacologically evaluated, but despite all these efforts, an effective drug able to improve or cure these pathologies has not been achieved. With the purpose to contribute in this essential search, this work describes the design, synthesis and pharmacological evaluation of the lilolidine derivative, 6-(2-aminoindanyl)-N-(2,4,5,6-tetrahydro-1H-pyrrolo[3,2,1-ij]quinoline hydrochloride) 4 (MAIL), with the purpose of restoring the homeostasis of dopaminergic transmission in Parkinson’s disease and/or Huntington’s Chorea. To perform that, different organic synthesis classic and heterocyclic strategies were employed through a convergent route. Also, a preliminar pharmacological evaluation was done, where the stereotyped behavior of Sprague-Dawley rats was studied after the ICV (intracerebroventricular) and IE (intrastriatum) administration of the lilodine derivative. The results obtained of compound 4 showed a central dopaminergic agonist activity through dopaminergic mechanisms. <![CDATA[<b>Phenylketonuria of late diagnosis and associated mutations in an Ecuadorian family</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0535-51332017000300006&lng=en&nrm=iso&tlng=en La fenilcetonuria es un error innato del metabolismo, producido por mutaciones en el gen de la fenilalanina hidroxilasa. Se describe el caso de un adolescente de 15 años con diagnóstico tardío de fenilcetonuria, quien presenta retardo mental severo, convulsiones e hipopigmentación. En este estudio se realizó el diagnóstico molecular de fenilcetonuria y se detectó la mutación p.R252W en homocigosis en el gen que codifica para la fenilalanina hidroxilasa. La presencia de esta variante nos permitió inferir la falta de respuesta a la terapia con sapropterina, medicamento que actúa como cofactor de la enzima, por la ausencia de actividad enzimática residual reportada para esta variante. Debido al retraso psicomotor del paciente, se decidió aplicar terapia lúdica y fortalecimiento muscular a través de la intervención fisioterapéutica; sin embargo, no se observó una mejoría permanente al aplicar este tratamiento, motivado por la falta de continuidad.<hr/>Phenylketonuria is an inborn error of metabolism due to mutations on the phenylalanine hydroxylase gene. We described the case of a 15 years old-adolescent with late diagnosis of phenylketonuria, who presents severe mental retardation, convulsions and hypopigmentation. In this study, the molecular diagnosis of phenylketonuria was performed, detecting p.R252W mutation in homozygous state on the phenylalanine hydroxylase gene. The presence of this variant allowed us to infer the lack of response to drug therapy with sapropterina which works as an enzyme cofactor, due to the absence of residual enzymatic activity reported for the p.R252W variant. Physical therapy was applied through playful therapy and muscular strengthening, because of the psychomotor retardation present in the patient. The failing in continuing with the physical therapy program stopped the patient´s improvement. <![CDATA[<b>Utility of proteomic analysis in the search for biomarkers and therapeutic targets in metabolic alterations associated with obesity</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0535-51332017000300007&lng=en&nrm=iso&tlng=en La obesidad es una enfermedad compleja y multifactorial, caracterizada por un aumento de grasa corporal que puede ser ocasionado por un desequilibrio entre la ingesta de alimentos y el gasto energético. En el proceso de aumento de peso intervienen factores como la susceptibilidad genética, factores ambientales y el estilo de vida. Está bien documentado que la obesidad aumenta el riesgo de padecer numerosas enfermedades y trastornos metabólicos como la resistencia a la insulina, diabetes tipo 2, hipercolesterolemia, enfermedades cardiovasculares, hígado graso, inflamación de bajo grado, algunos tipos de cáncer y trastornos sicológicos. Debido al incremento de la obesidad y sus comorbilidades en los últimos años en la población mundial, los gastos médicos erogados para su tratamiento representan un problema grave para los sistemas de salud pública. El análisis proteómico a gran escala, es una herramienta potente y prometedora para el descubrimiento de biomarcadores tempranos y para la comprensión de los mecanismos moleculares que subyacen a las alteraciones metabólicas asociadas con la obesidad. No obstante, es imprescindible considerar las limitaciones técnicas y el análisis e interpretación de los hallazgos proteómicos, para avanzar en la comprensión funcional integral de la dinámica del proteoma ligado a la obesidad. Adicionalmente, los abordajes con un enfoque proteómico, permitirán el desarrollo de nuevas terapias preventivas, así como el descubrimiento de agentes terapéuticos potenciales en el tratamiento de disfunciones metabólicas. El objetivo de esta revisión es analizar las contribuciones más recientes de la proteómica en la búsqueda de biomarcadores relacionados con la obesidad.<hr/>Obesity is a complex and multifactorial disease characterized by an increase in body fat that can be caused by an imbalance between food intake and energy expenditure. In the process of weight gain, factors such as genetic susceptibility, environmental factors and lifestyle are involved. It is well documented that obesity increases the risk of numerous diseases and metabolic disorders such as insulin resistance, type 2 diabetes, hypercholesterolemia, cardiovascular disease, fatty liver, low grade inflammation, some types of cancer and psychological disorders. Due to the increase in obesity and its comorbidities in recent years at the global level, medical expenses incurred for its treatment represent a serious problem for public health systems. Large-scale proteomic analysis is a powerful and promising tool for the discovery of early biomarkers and for the understanding of the molecular mechanisms underlying the metabolic alterations associated with obesity. Nevertheless, it is essential to consider the technical limitations and the analysis and interpretation of the proteomic findings, to advance in the integral functional understanding of the dynamics of the proteome linked to obesity. In addition, approaches with a proteomic viewpoint will allow the development of new preventive therapies, as well as the discovery of potential therapeutic agents in the treatment of metabolic dysfunctions. The aim of this review is to analyze the most recent contributions of proteomics in the search for biomarkers related to obesity. <![CDATA[<b>New vision of plantar fasciitis treatment in athletes</b>: <b>Utility of functional training by ski</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0535-51332017000300008&lng=en&nrm=iso&tlng=en La fascitis plantar es un síndrome degenerativo que se produce como resultado de traumas repetidos en el origen de ésta, en el calcáneo. Suele presentarse en atletas y corredores, aunque también aparece en la población general, afectando aproximadamente a un 10% en ambos casos. Aunque su etiología no es del todo clara, es probable que la causa no sea única y haya diversos factores que contribuyan a su aparición. Estos son tales como el aumento de peso, el exceso de ejercicio físico o el calzado inadecuado, entre otros. La fascitis plantar se caracteriza por dolor en la región inferior del talón, en la planta del pie, que es especialmente intenso en los primeros momentos de la mañana al andar o después de un período de inactividad física o tras una bipedestación prolongada. A lo largo del día el dolor va disminuyendo, pero volverá si se lleva a cabo la actividad de levantamiento de peso. El tiempo de recuperación o resolución de esta patología es prolongado y existen varios métodos que pueden ayudar en su tratamiento, todos ellos diríamos que convencionales. Esta revisión plantea el tratamiento de la fascitis plantar a través del entrenamiento funcional de esquí, dadas las características biomecánicas que comporta este deporte.<hr/>Plantar fasciitis is a degenerative syndrome that occurs because of repeated traumas in the fascia origin on the calcaneus. It usually occurs in athletes and runners, although it also appears in the general population, affecting approximately 10% in both cases. Although its etiology is not entirely clear, it is likely, not due to a unique cause since there are several factors that contribute to its appearance. These are: weight gain, excessive physical exercise or inadequate footwear, etc. Plantar fasciitis is characterized by pain in the lower region of the heel, in the sole of the foot, which is especially intense in the first moments of the morning when walking, after a period of physical inactivity or after prolonged standing. Throughout the day the pain diminishes, but will return if weight-lifting activity is carried out. The recovery time or resolution of this pathology is prolonged and there are several conservative methods that may help in the treatment of plantar fasciitis. Therefore, in this review we propose the treatment of plantar fasciitis through functional ski training, given the biomechanical characteristics of this sport.
