Se analizó: edad, sexo, procedencia, manifestaciones clínicas, tiempo entre el inicio de los síntomas y la hospitalización y tratamiento. De los 15 pacientes, 10 (66,67%) fueron de sexo femenino, 9 (60,0%) entre 20 y 29 años de edad. El 53,34% eran procedentes de los Municipios Mara y Páez. Las manifestaciones clínicas fueron; cefalea y rigidez de nuca 14 (93,33%), alteración de la consciencia 13 (86,67%), alteración mental 7 (46,66%), convulsión 8 (53,33%), hipertensión intracraneana 5 (33,33%), déficit Motor 3 (20,00%), alteración de pares craneales 2 (13,33%) como falso signo localizador y en 2 (13,33%) diplopía. La citoquímica del líquido cefalorraquídeo reveló hipoglucorraquia en el 100,00% de los pacientes e hiperproteinorraquia. La coloración de Ziehl Neelsen fue positiva en 1 (6,67%). 6 pacientes (40,00%) tenían más de 1 mes con TBC. Se concluye que son diversas las manifestaciones clínicas de meningoencefalitis por TBC, siendo más frecuente el síndrome meníngeo.<hr/>With the objective of describing clinical manifestations in patients with meningeal encephalitis due to tuberculosis (TBC), a descriptive, retrospective study was made by reviewing clinical histories of 15 adult patients with a diagnosis of TBC in the SAHUM neurology service from January 2002 to December 2008. Age, sex, clinical manifestations, hospitalization and treatment were analyzed. Of the 15 patients, 10 (66.67%) were female, 9 (60.0%) were between 20 and 29 years of age; 53.34% came from the Mara and Páez Municipalities. The clinical manifestations were: headache and nape of the neck rigidity, 14 (93.33%); alteration of consciousness, 13 (86.67%); alteration of mental state, 7 (46.66%); convulsions, 8 (53.33%); intracranial hypertension, 5 (33.33%); motor deficit, 3 (20.00%); alteration of cranial pairs, 2 (13.33%) as a false locator sign and diplopia, 2 (13.33%). Cytochemical study of the cerebrospinal fluid revealed hypoglycorrhachia in 100.00% of the patients. Ziehl Neelsen coloration was positive in 1 (6.67%). Six patients (40.00%) had had the disease for more than 1 month. Conclusions were that the clinical manifestations of meningeal encephalitis due to TBC are diverse, with the meningeal syndrome as the most frequent. <![CDATA[<b>Mecanismos de resistencia a antibióticos β-lactámicos en Staphylococcus aureus</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0075-52222010000100003&lng=es&nrm=iso&tlng=es S. aureus ha demostrado un gran poder de adaptación a los agentes antimicrobianos, adquiriendo paso a paso resistencia a todos los antibióticos disponibles para el tratamiento de las infecciones que ocasiona. Existen tres mecanismos de resistencia a los antibióticos β-lactámicos en S. aureus: resistencia mediada por enzimas (penicilinasas o β-lactamasas) las cuales desactivan al antibiótico; resistencia intrínseca, que no es debida a la inactivación de drogas y es responsable de la resistencia a meticilina; y la modificación de las proteínas de unión a penicilinas (PBPs). Además, S. aureus puede expresar el fenómeno de tolerancia, en el que ocurre una disociación de las acciones inhibitoria y bactericida de los antibióticos β-lactámicos. De éstos, el mecanismo más importante, es la resistencia intrínseca que es probablemente más compleja, debido a que varios factores pueden afectar también su expresión.<hr/>S. aureus has shown a great power of adaptation to antimicrobial agents, acquiring, step-by-step, resistance to all available antibiotics for treatment of the infections it causes. S.aureus has three major mechanisms of resistance to β-lactam antibiotics: enzyme mediated (penicillinaseor β-lactamase) by which the antibiotic is inactivated; intrinsic resistance,which is not due to drug inactivation and accounts for methicillin resistance;and modifications of penicillin-binding proteins (PBPs). Additionally, S. aureus can express the tolerance phenomenon, in which there is a dissociation of the inhibitoryand killing actions of â-lactam antibiotics. Of these, the most important mechanism is intrinsicresistance, which is probably more complex because several factors can affectits expression. <![CDATA[<b>Mecanismos de resistencia a Glicopéptidos en Staphylococcus aureus</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0075-52222010000100004&lng=es&nrm=iso&tlng=es Los glicopéptidos (vancomicina y teicoplanina) constituyen una alternativa terapéutica en el tratamiento de infecciones severas por cepas de S. aureus resistentes a meticilina. Sin embargo, ya se han descrito dos mecanismos de resistencia en S. aureus: resistencia de bajo nivel, caracterizada por un engrosamiento anormal de la pared celular, presente en las cepas VISA y, resistencia de alto nivel mediada por el operón vanA, que provoca la sustitución de los residuos terminales D-ala-D-ala por D-ala-D-lac, disminuyendo su afinidad por el antibiótico. Esta revisión resume la historia de la aparición de la resistencia a glicopéptidos en S. aureus y considera los mecanismos que determinan la resistencia en estos organismos como base para comprender la necesidad y los potenciales roles de nuevos agentes de esta clase.<hr/>Glycopeptides (vancomycin and teicoplanin) are an alternative therapeutic in the treatment of severe infections by methicillin-resistant S. aureus strains. However, two resistance mechanisms of S. aureus have already been described: low-level resistance, characterized by an abnormal thickening of the cellular wall, present in the VISA strains, and high-level resistance, mediated by the vanA operon, which causes the replacement of D-ala - D-ala terminal residues by D-ala-D-lac, decreasing its affinity for the antibiotic. This review summarizes the history of the emergence of glycopeptide resistance in S. aureus and considers the mechanisms that determine the resistance in these organisms as a background for understanding the need and potential roles of new agents of this kind. <![CDATA[<b>Identificación de flora fúngica en una empresa procesadora de alimentos del estado Carabobo</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0075-52222010000100005&lng=es&nrm=iso&tlng=es Los hongos se encuentran difundidos en el medio ambiente y son contaminantes frecuentes de los alimentos, siendo éste, el medio más fácil y rápido de transporte. Sus efectos nocivos dependerán de las condiciones externas que se brinden para que ellos puedan producir toxinas que afecten la salud del consumidor y el deterioro de los productos finales. El objetivo fue la identificación de la flora fúngica que pudiese estar presente en una empresa procesadora de alimento. La muestra estuvo constituida por áreas y superficies de interés para la empresa: áreas de empaque, rodillos de secado, piso 1, piso 2, piso 3 y la azotea, ubicada en el estado Carabobo, Venezuela. Se tomaron un total de 19 hisopados, para ser sembrados en agar Sabouraud y 49 placas empleando la técnica de sedimentación con el mismo agar, posteriormente fueron contabilizadas las UFC/mm³. Para la identificación por género y especie de los microorganismos se emplearon diferentes métodos y técnicas como: Cinta Adhesiva Transparente, Tubo Germinativo, Microcultivos y Pruebas API 20C. Los resultados obtenidos arrojaron que el mayor número de UFC/mm³ aisladas correspondieron a Cladiosporum oxysporum, Aspergillus níger, Rhizopus Spp y a Cándida albicans.<hr/>Fungi are found spread throughout the environment and are frequent contaminants of food, since this is the fastest and easiest means for transport. Their injurious effects depend on the external conditions provided so that they can produce toxins affecting the health of the consumer and deterioration of the end products. The objective of this study was to identify the fungal flora that could be present in a food processing company. The sample consisted of areas and surfaces of interest to the company: packing areas, drying rollers, floor 1, floor 2, floor 3 and the roof, located in the state of Carabobo, Venezuela. A total of 19 swabs were taken, which were sown in Sabouraud agar and 49 plates, using the sedimentation technique with the same agar; later, the UFC/mm3 were counted. For identification of the microorganisms by genre and species, different methods and techniques were used, such as: transparent sticky tape, germinating tubes, microcultures and* API 20C tests. Results indicated that the greatest number of UFC/mm3 isolates corresponded to Cladiosporum oxysporum, Aspergillus níger, Rhizopus Spp and Cándida albicans. <![CDATA[<b>Incidencia de la infección por el virus de la hepatitis A diagnosticadas en la ciudad de Maracaibo, estado Zulia, Venezuela</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0075-52222010000100006&lng=es&nrm=iso&tlng=es La infección por el virus de la hepatitis A (VHA) sigue siendo un problema de salud pública en los países en vías de desarrollo. El objetivo de la presente investigación fue determinar la incidencia de la infección por el VHA en individuos de la ciudad de Maracaibo, estado Zulia, Venezuela. Durante el periodo comprendido entre enero 2004 a diciembre 2007 se seleccionaron 1056 pacientes en edades de 1 a 60 años de ambos sexos (media ± DS: 27,48 ± 5,24), procedentes de los municipio Maracaibo y San Francisco ubicados en la ciudad de Maracaibo estado Zulia, Venezuela. La presencia de anticuerpos anti-IgM contra el virus de la hepatitis A se determinó mediante métodos convencionales de micro partículas enzimoinmunoanalisis (MEIA). Los resultados encontrados indican una seropositividad en 73 (35,1%), 76 (28%), 66 (24,3%), 35 (20,3%) individuos de las zonas Norte, Sur, Este, Oeste respectivamente, correspondientes al municipio Maracaibo y 31 (23,3%) correspondiente al municipio San Francisco. La mayor prevalencia fue observada en pacientes del sexo femenino menor de 20 años (47,3%), y en pacientes del sexo masculino menor de 10 años (53,7%), siendo el signo clínico más frecuente la ictericia (95,7%). Las deficientes medidas de saneamiento ambiental y sanitario fueron factores aparentemente determinantes en la diseminación del VHA.<hr/>Hepatitis A virus (HAV) infection continues to be a public health problem in developing countries. The aim of this study was to determine incidence of the hepatitis A virus infection in a population from Maracaibo, Venezuela. From January, 2004 until December, 2007, this study was carried out on 1056 individuals, whose ages were between 1-60 years (average ± DS: 27.48 ± 5.24 years), coming from the Maracaibo and San Francisco municipalities, Venezuela. The IgM antibody against HAV was determined by the ELISA technique. The overall incidence was 26.6%. The North, South, East and West areas of the Maracaibo Municipality and the San Francisco Municipality in the city of Maracaibo showed incidences of 73 (35.1%), 76 (28%), 66 (24.3%), 35 (20.3%) and 31(23.3%), respectively. The highest incidences were observed in females under 20 years (47.3%) and males under 10 years (53.7%). The most frequent clinical sign was jaundice in 95.7% of patients. Deficient sanitary conditions and hygiene habits seemed to be the main risk factors in spreading HAV infection. <![CDATA[<b>Diagnóstico diferencial de virus hepatótropos y no hepatótropos en pacientes atendidos en el Laboratorio Regional de Referencia Virológica</b>: <b>Maracaibo-Venezuela, año 2007</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0075-52222010000100007&lng=es&nrm=iso&tlng=es La hepatitis viral representa un problema de salud pública a escala mundial, causada por diferentes agentes etiológico como el virus de la hepatitis A, B, C, D, E, F o G. Sin embargo, se han reportado casos relacionados con la enfermedad hepática causados por virus no hepatótropos como el Citomegalovirus (CMV) y el Virus Epstein Barr (VEB). El objetivo del presente estudio fue establecer el diagnóstico diferencial de virus hepatótropos y no hepatótropos en pacientes con síntomas y signos sugestivo de infección hepática, de la ciudad de Maracaibo-Venezuela. Año 2007. Se recolectaron 268 muestras de sangre de pacientes procedente de diferentes centros de salud de la ciudad de Maracaibo, que acudieron al Laboratorio regional de Referencias Virológica. La presencia de anticuerpos de tipo IgM, para el virus de hepatitis A (VHA), B (VHB), C (VHC), CMV y VEB fue detectada mediante la prueba de ELISA. Los resultados obtenidos indican una alta incidencia del VHA 118/268(44,03%), seguido del VHB 5/268(1,86%) y ausencia del VHC. Para virus no hepatótropos, el 4,48% (12/268) fueron positivos para el VEB y el 9,7% (26/268) para CMV. El 39,92% (107/268) son considerados negativos para los virus antes mencionados. En este estudio se señala que los virus CMV y VEB suelen estar comprometidos con daño hepático y deben ser consideradas como posibles agentes etiológicos ante cualquier caso de hepatitis.<hr/>Viral hepatitis is a public health problem worldwide, caused by different etiologic agents, such as hepatitis A, B, C, D, E, F or G. However, cases have been reported related to liver disease caused by hepatotropic viruses, such as cytomegalovirus (CMV) and Epstein Barr Virus (EBV). The aim of this study was to establish the differential diagnosis of hepatotropic and non-hepatotropic viruses in patients with symptoms and signs suggestive of liver infection in the city of Maracaibo, Venezuela, 2007. 268 blood samples were collected from patients at different health centers in the city of Maracaibo, who recurred to the Regional Viral Reference Laboratory. The presence of IgM antibodies to hepatitis A virus (HAV), B (HBV), C (HCV), CMV and EBV was detected by ELISA. The results indicate a high incidence of HAV 118/268 (44.03%), followed by HBV 5/268 (1.86%) and the absence of HCV. For non-hepatotropic viruses, 4.48% (12/268) were positive for EBV and 9.7% (26/268) for CMV. For the above-mentioned viruses, 39.92% (107/268) are considered negative. This study suggests that CMV and EBV viruses are often involved with liver damage and should be considered as possible etiologic agents for any case of hepatitis. <![CDATA[<b>Comportamiento epidemiológico de la rabia humana en el estado Zulia, Venezuela, durante el período de vigilancia</b>: <b>enero 1993 - junio 2009</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0075-52222010000100008&lng=es&nrm=iso&tlng=es La rabia representa un importante problema de salud pública en el estado Zulia, constituyendo en Venezuela la entidad con mayor incidencia de casos en animales y humanos, registrándose 25 muertes humanas por rabia durante el período enero 1993 - junio 2009. Describir su comportamiento epidemiológico durante este período constituye nuestro objetivo. Estudio epidemiológico realizado: observacional, descriptivo. La muestra incluyó todos los casos confirmados como rabia humana en la entidad durante este lapso (n = 25), con edades entre 2-57 años, de ambos sexos y procedentes de 7 municipios con incidencia. Datos tomados de los informes de casos de rabia de la Dirección Regional de Epidemiología. Los resultados están expresados en porcentajes, promedios, intervalos de confianza, tasa de letalidad y distribuciones según edad, sexo y municipio de ocurrencia. La mayor incidencia ocurrió en varones (68%). El grupo entre 2-10 años resultó mayormente afectado (60%). El 80% casos no efectuó consulta post exposición no recibiendo inmunoprofilaxia oportuna. La Inmunofluorescencia directa fue utilizada para confirmación diagnóstica. El conocimiento sobre las formas de transmisión de rabia es crucial para prevenirla; el tiempo entre la inoculación viral y la invasión neural es quizá el único período para una inmunoprofilaxia efectiva.<hr/>Rabies represents a major public health problem in the State of Zulia, constituting the entity with the highest incidence of animal and human cases in Venezuela, registering 25 human deaths by rabies from January, 1993 - June, 2009. The objective of this study is to describe its epidemiological behavior during this period; the epidemiological study was observational and descriptive. The sample included all cases confirmed as human rabies in the entity during this period (n = 25); they were between 2 and 57 years of age, from both sexes and 7 municipalities. Data was taken from the reports of rabies cases in the Regional Epidemiology Direction. The results are expressed in percentages, means, confidence intervals, mortality rate and distributions according to age, sex and municipality of occurrence. The highest incidence was in males (68%).The group between 2-10 years was principally affected (60%). Eighty per cent of the cases did not seek post-exposure consultation and did not receive timely prophylaxis. The direct fluorescent antibody test was used for diagnostic confirmation. Knowledge about forms of rabies transmission is crucial for its prevention; the time between viral inoculation and neural invasion is perhaps the only period for an effective prophylaxis.
