A representative case report of a woman with a chronic, progressive, hepatitis C, who underwent long-term treatment with combined pegylated interferon plus ribavirin, and resorted to multiple cycles of filgrastim to recover a severe, recurring granuloytopenia caused by antiviral therapy itself, and to maintain an effective dosage of anti-HCV antivirals, developed an extensive and severe cutaneous psoriasis, which improved only after specific cyclosporin treatment. From a pathogenetic point of view, in our case it remains extremely difficult to distinguish the role of pegylated interferon from that of the accompanying ribavirin, from that of the frequently administered granulocyte growth factor (filgrastim), since all mentioned drugs were administered concurrently during many months, and according to the existing literature evidences, all of them have a potential to induce psoriasis as a potential untoward effect in subjects suffering from chronic hepatitis. Cyclosporin treatment obtained a stable remission of this last severe cutaneous complication, but the efforts to contain the progression of the underlying evolutive hepatitis C were blunted by the difficult-to-treat genotype 1 HCV infection, and the frequent need to lower drug dosages and/or to interrupt antiviral therapy, because of myelotoxic and later cutaneous complications prompted by anti-HCV therapy itself. <![CDATA[<b>Evaluación del Diklason<sup>®</sup> ampollas en el dolor agudo</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-02642010000100003&lng=es&nrm=iso&tlng=es Antecedentes: El dolor agudo es definido como dolor de reciente comienzo y probablemente de duración limitada que se relaciona de manera causal y temporal con un daño o enfermedad. Los antiinflamatorios no esteroideos modulan el dolor periférico por reducción de la producción de prostaglandinas y de los neuropéptidos. Ejemplos típicos de dolor agudo son: el dolor post-operatorio, lumbago, dolor por injuria de partes blandas o hueso y cólico nefrítico. Este tipo de dolor es incrementado por la ansiedad, el catastrofismo, neuroticismo y la depresión. El diclofenac potásico ha demostrado ser un fármaco seguro y efectivo en el manejo del dolor agudo. Material y Método: Se realizó un estudio abierto, prospectivo y multicéntrico en 220 pacientes con diagnóstico de dolor agudo, de intensidad severa y moderada a los cuales se les suministró una dosis IM o IV de diclofenac potásico (Diklason®) de 75 mg y se midió la intensidad del dolor mediante las siguientes escalas: VAS 0-100, PID, SPID, Pain Relief y TOTPAR, medicación de rescate y escala clínica de cambio. La inocuidad se evaluó por interrogatorio directo de efectos adversos. Las evaluaciones se realizaron al inicio, a las 0.5, 1, 2, 4, 6, 8 y 12 h de la administración de la dosis. Resultados: Se produjo un descenso importante en los niveles de VAS desde los 30 min. de tratamiento y este descenso fue significativo entre los períodos de evaluación hasta las 8 horas, el efecto persistió hasta las 12 horas. El 90,45% de los pacientes comenzaron a sentir alivio del dolor a los 30 min. Todas las escalas evaluadas mostraron cambios significativos en los períodos evaluados desde los primeros 30 min. hasta las 8 horas (SPID), 12 h (SPID, PR, y TOTPAR). En el 92,72% de los pacientes los resultados fueron adecuados: excelentes (56,3%) y buenos (36,36%). Se presentaron 6 efectos adversos leves. Conclusiones: El diclofenac potásico (Diklason®) resulta rápido, efectivo y duradero en el manejo del dolor agudo de diferentes etiologías.<hr/>Background: Acute pain is defined as a pain of recent onset and probably limited duration which is associated with a causal and temporal damage or disease. NSAIDs modulate peripheral pain by reducing prostaglandin and neuropeptides production. Typical examples of acute pain include post operatory, lumbago, bone or soft tissue injuries and, renal colic. This type of pain is increased by anxiety, catastropic events, neuroticism and depression. Diclofenac potassium is proved to be safe and effective in management acute pain. Materials and methods: We conducted a multicenter; prospective, open study in 220 patients with acute pain (moderate to severe intensity) which supply with IM or IV dose of diclofenac potassium (Diklason®) 75 mg, the pain intensity was measured by the following scales: VAS 0-100, PID, SPID, Pain Relief and TOTPAR, rescue medication and clinical scale changes. The safety was assessed by direct questioning side effects. Evaluations were performed at baseline at 30 minutes, 1, 2, 4, 6, 8, and 12 hours after dosage. Results: There was a significant decrease in VAS levels from 30 minutes of treatment and this decrease was significant between the evaluation periods up to 8 hours, the effect persisted for 12 hours. In 90.45% of patients began to feel pain relief within 30 minutes. All scales evaluated showed significant changes in the periods evaluated from the first 30 minutes up to 8 hours (SPID), 12 h (SPID, PR, and TOTPAR). In 92.72% of results were suitable: excellent (56.3%) and good (36.36%). We found 6 mild adverse effects. Conclusions: Diclofenac potassium (Diklason®) is quick, effective and durable in the management of acute pain of various etiologies. <![CDATA[<b>Evaluación del Diclofenac Potásico Diklason<sup>®</sup>,</b> <b>ampollas en el dolor agudo postoperatorio</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-02642010000100004&lng=es&nrm=iso&tlng=es Tradicionalmente, el dolor postoperatorio no ha sido bien controlado, el dolor es una manifestación externa de la respuesta al estrés de la cirugía, el objetivo del tratamiento del dolor postoperatorio es eliminarlo, reducirlo y aliviar también la sensación de malestar del paciente, con un mínimo de efectos secundarios o adversos y con métodos más económicos. Los AINEs tienen un valor importante como alternativa analgésica no opioide postoperatoria y pueden utilizarse con seguridad si se respetan las contraindicaciones y se administran dosis más apropiadas. Materiales y Métodos: Se realizó estudio abierto, prospectivo, multicéntrico en 165 pacientes con diagnóstico de dolor postoperatorio moderado a severo, es decir, VAS 60 a 100 mm, a los cuales se les administró una dosis IM o IV de diclofenac potásico (Diklason®) de 75 mg y en los cuales, posteriormente, se midió la intensidad del dolor con las escalas VAS 0-100, PID, SPID, máxima PID, Pain Relief y TOTPAR. Las evaluaciones fueron realizadas al inicio, a los 30 min, 1, 2, 4, 6, 8 y 12 h. posteriores a la administración de la dosis evaluada. El interrogatorio directo de los efectos adversos evaluó la inocuidad. Resultados: Se observó una disminución evidente en los niveles de VAS desde los 30 min de administración de diclofenac potásico; este descenso fue significativo en todos los períodos evaluados y el mismo se mantuvo hasta las 12 h post administración del mismo. El 81,21% de los pacientes experimentaron mejoría del dolor desde los 30 min post dosis. Se evidenciaron cambios significativos en la intensidad del dolor en todos los períodos estudiados y en todas las escalas evaluativas del dolor. La opinión de los pacientes acerca del alivio del dolor fue muy buena y buena en el 79.39% (44,24 muy buena y 35,15% buena) El 73% de los pacientes no ameritó terapia analgésica de rescate. Solo se observó una tasa de fracasos en el 1,81% de los pacientes y solo un efecto adverso leve fue reportado. Conclusiones: Para el manejo del paciente con dolor post-operatorio, el uso de diclofenac potásico (Diklason®), resulta seguro, rápido, duradero y efectivo.<hr/>Traditionally, postoperative pain has not been well controlled, pain is an outward manifestation of stress response to surgery, the goal to treat postoperative pain is to eliminate, reduce and relieve and the patient with a minimum of side effects or adverse economic and methods. NSAIDs have significant value as an alternative of non-opioid analgesic in postoperative pain and can be safely used if contraindications are respected and lower doses are appropriate. Materials and Methods: This a multicentric, open, prospective study. A 165 patients were colected with diagnosis of moderate to severe postoperative pain to severe or VAS 60 to 100 mm, which were administered either IM or IV dose of diclofenac potassium (Diklason®) 75 mg; the pain intensity was measured by VAS 0-100, PID, SPID, PID, Pain Relief and TOTPAR scales. The reviews were conducted at baseline, 0.5, 1, 2, 4, 6, 8 and 12 h after administration of doses evaluated. Direct examinations of the side effects, evaluate safety. Results: An evident reduction on pain levels was observed from the early 30 min of diclofenac potassium administration. This reduction were significant in all evaluated periods and persisted until 12 hours post administration dose. The 81,21% of the patients experienced improvement in pain relief from the 30 min post dose. Significant changes on pain intensity in all periods and all evaluative pain scales studied were showed. The patients opinion about pain relief was very good and good in 79.39% (44.24 very good and 35.15% good) of them. No rescue analgesic therapy was used in the 73% of patients. A single failure rate of 1.81% was observed and 1 only slight adverse effect was reported. Conclusions: The use of diclofenac potassium (Diklason®), for handling postoperative pain demostrated was safe, effective, fast, and durable. <![CDATA[<b>Ensayo clínico sobre la bioequivalencia clínica de una dosis</b> <b>de CLOPIDOGREL LETI CRAVID<sup>®</sup> (CLOP-L) 75 mg. comprimidos frente</b> <b>a CLOPIDOGREL SANOFI PLAVIX<sup>®</sup> (CLOP-S) 75 mg. tabletas administradas</b> <b>en una dosis diaria por 7 días en voluntarios sanos</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-02642010000100005&lng=es&nrm=iso&tlng=es Antecedentes Los pacientes sometidos a procedimientos de intervención coronaria percutánea, al igual que en pacientes con enfermedad coronaria, deben recibir en forma indefinida tratamiento con ácido acetilsalicílico (AAS) y Clopidogrel. El desarrollo de nuevos productos a base de Clopidogrel a menores costos ayudan a evitar la discontinuación prematura de la terapia antiplaquetaria; entre estos productos desarrollados tenemos la marca Cravid® de Laboratorios LETI S.A.V. y esta debe comparar su efectividad e inocuidad con el patrón internacional de Clopidogrel marca Plavix® de Laboratorios Sanofi Aventis. Métodos: Se realizó un estudio: prospectivo, comparativo, cruzado, aleatorizado, en voluntarios sanos. Cada grupo recibió 1 comprimido de Clopidogrel Leti, CLOP-L o Clopidogrel Sanofi, CLOP-S de 75 mg. en una sola dosis al día durante 7 días continuos. Después de un período de lavado de 7 días recibieron el segundo tratamiento. Se midió la agregación plaquetaria al inicio de cada período y a los 7 días de tratamiento mediante agregometría óptica, con un agregómetro Óptico Modelo 490-2D marca Cronolog, con sistema de autocalibración que trabajó con plasma rico en plaquetas. Lectura 0-100% de paso de luz. Resultados: En ambos grupos se produjo un descenso importante en la agregabilidad plaquetaria a los 7 días de tratamiento de más del 50 % independiente del reactivo de ADP (Helena y Cronolog) utilizado para agregar (P < 0.05). Conclusiones: La relación de las medias e IC del porcentaje de agregación obtenida con las dos diferentes marcas comerciales de ADP se encontraron entre el 80 y 125%, por lo cual se concluye que ambas marcas de Clopidrogrel son bioequivalentes y por lo tanto, son perfectamente intercambiables.<hr/>Background The undergoing percutaneous coronary intervention procedures, as in patients with coronary disease should receive treatment indefinitely with acetylsalicylic acid (ASA) and Clopidogrel. Developing new products based on lower costs clopidogrel to help prevent premature discontinuation of antiplatelet therapy, among these products we have developed the brand Cravid® of LETI S,A.V. Laboratories® and this should compare their effectiveness and safety with the international standard Clopidogrel brand Plavix® of Sanofi Aventis Laboratories. Methods: We conducted a study: a prospective, comparative, cross-randomized, in healthy volunteers. Each group received 1 tablet of Clopidogrel Leti, CLO-L or Clopidogrel Sanofi-LCLOP-S 75 mg. in a single dose daily for 7 days. Followed for 7 day-washout period before administration of second treatment. Platelet aggregation was measured at the beginning of each period and 7 days of treatment by agregometría optics. With a agregómetro Optical Model 490-2D. Results: In both groups there was a decrease in platelets add to the 7 days of treatment for more than 50% independent of the reagent ADP (Helena and cronolog) used to add (P <0.05). Conclusions: The mean and CI at 90%, obtained with two different trademarks of ADP were between 80 and 125%, which was concluded that both brands are bioequivalent and are therefore perfectly interchangeable.
