Hay una prevalencia de 1,2%, demostrada en estudios aislados, 2,5% en una serie realizada en población pediátrica y 1,92% en trabajadores de salud. Se desconoce la prevalencia y características epidemiológicas de VHC en el estado Aragua. Se presenta la experiencia de 5 años del Comité de Hepatitis del Estado Aragua (CHA). Objetivos: Caracterizar epidemiológicamente la población infectada por VHC en el Estado Aragua, tomando en cuenta género, edad, estado civil, actividad laboral, antecedente de exposición ocupacional, cirugías previas, transfusiones sanguíneas, hemoderivados, procedencia, genotipo predominante, coinfección por VHB, VIH, VEB, Parvovirus B-19. Materiales y métodos: Encuesta epidemiológica a 101 pacientes con diagnóstico serológico de VHC; rango de edad, 18 - 80 años. Resultados: 60% género masculino, edad promedio 43 años ambos géneros (31,3%), 67,7% casados, provenientes en mayor porcentaje del Municipio Girardot(27,4%). En su mayoría sin profesión definida (80,2%), predominando comerciantes y obreros. De los trabajadores del sector salud (8%), 42% con antecedente de exposición ocupacional. 81,1% asintomáticos en el momento del diagnóstico y 31,8% presentó síntomas inespecíficos. 60,2% refirió cirugías previas; 29,8% transfusiones sanguíneas y, en este grupo, 49% las recibió antes de 1992. Genotipo 1 fue predominante (33,3%); sólo 8% de la población presentó coinfección por VHB, VIH, VEB o Parvovirus B-19. Conclusiones: Predominó el genero masculino, edad promedio 43 años, casados, no profesionales, 60,2% refirió cirugías previas y, en menor porcentaje (29,8%), transfusiones sanguíneas, predominó genotipo 1 y en su mayoría fueron asintomáticos.<hr/>Venezuela presents a sub-record of the HCV infection. There is a prevalence of this infection of 1.2%, demonstrated in isolated studies, 2.5 % in a set made in a pediatric population, and 1.92 % in health care workers. The prevalence and epidemiologic characteristics of the HCV infection in the Aragua State are unknown. Here, it is reported the experience of the Hepatitis Committee in the Aragua State for 5 years. Objetives: Carry out the epidemiologic characterization of the HCV infected population of the Aragua State, regarding gender, age, civil status, address, job, history of occupational exposure, previous surgeries, blood transfusion, genotype of the virus, and coinfection with HBV, HIV, EBV or Parvovirus B-19. Materials and methods: Epidemiologic interview applied to 101 patients with serologic diagnosis of HCV, between 18 and 80 years old. Results: The 60 % of infected patients were males, the average age to both genders was of 43 years old (31.3 %), 67.7 % were married, mainly from the Giradot District (27.4 %). Most of this infected patients didnÊt have a specific profession (80.2 %), and consisted predominantly of traders and workers. Of the health care professionals (8 %), 42 % had history of occupational exposition. The 81.1 % of infected patients were asymptomatic at the time of the diagnosis, and 31.8 % showed unspecific symptoms. The 60.2 % had been previously subjected to surgery; 29.8 % to blood transfusion, and 49 % of the patients in this group received the transfusion before 1992. The virus genotype 1 was the predominant (33.3 %); and only 8 % of the infected population studied had coinfection with HBV, HIV,Parvovirus B-19 or EBV. Conclusions: in the infected population studied prevailed the male gender, an average age of 43 years old, the married condition, non professionals, 60.2 % reported a previous surgery, and a lower porcentage (29,8%), blood transfusions most of them were infected with genotype 1 virus and were asymptomatic. <![CDATA[<b>Longitud esofágica</b>: <b>estudio prospectivo en pacientes adultos con enfermedad por reflujo gastroesofágico</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0016-35032009000400005&lng=es&nrm=iso&tlng=es Objetivos: Correlacionar la longitud esofágica y de sus esfínteres, en pacientes adultos con enfermedad por reflujo gastroesofágico (ERGE) con síntomas típicos y atípicos. Métodos: Se realizó estudio prospectivo, de corte transversal. Se incluyeron un total de 80 pacientes con diagnóstico clínico, endoscópico y/o pHmétrico de enfermedad por reflujo gastroesofágico referidos al Laboratorio de Motilidad Digestiva del Centro Médico Docente La Trinidad en 2006-7. Se seleccionaron dos grupos: El "A" de pacientes con ERGE y síntomas típicos y el "B" con ERGE y síntomas atípicos. Se les realizó manometría esofágica, determinándose: longitud total y cuerpo esofágico; longitud y presión de los esfínteres esofágicos y presencia de hernia hiatal. Resultados: Del total de pacientes (n=80) que se incluyeron, 58 (72,5%) en el grupo A y 22 (27,5%) en el grupo B. Se encontró un EES de menor longitud en los pacientes del grupo B comparado con el del grupo A con una longitud promedio de 1,91 cm vs 2,71 cm (p= 0,0003). No se encontraron diferencias significativas en cuanto a la longitud del EEI, longitud del cuerpo esofágico. En cuanto a la longitud total esofágica, se encontró que era más corta en el grupo B que en el A, (25,32 cm. vs. 27,34 cm. p= 0,01). La presión del EEI del grupo B fue menor que la presión del mismo en el grupo A. Conclusión: La longitud del esófago total, del EES es menor en pacientes con ERGE y síntomas atípicos que en pacientes con síntomas típicos. Los pacientes con síntomas atípicos presentan una presión disminuida del EEI y una mayor frecuencia de hernia hiatal con respecto a los pacientes con ERGE y síntomas típicos.<hr/>Objective: To correlate the length of their esophageal sphincter and, in adult patients with gastroesophageal reflux disease (GERD) with symptoms typical and atypical. Methods: We conducted prospective study, cross-sectional. We included a total of 80 patients with clinical diagnosis, endoscopic and / or pHmétrico of gastroesophageal reflux disease related to laboratory Motility Digestive Centro Medico Docente La Trinidad in 2006-7. Were selected two groups: The "A" and GERD patients with typical symptoms and the "B" with atypical symptoms and GERD. They were conducted esophageal manometry, determined: total length and esophageal body; pressure and length of the esophagus and sphincters presence of hiatal hernia. Results: Of the total patients (n = 80) that were included, 58 (72.5%) in group A and 22 (27.5%) in group B. There was a minor ESS length of patients in group B compared with Group A with an average length of 1.91 cm vs. 2.71 cm (p = 0.0003) No significant differences were found on the length ERA, esophageal body length. As for the total length esophageal, was found to be shorter in group B compared with A (25.32 cm. Vs. 27.34 cms. P = 0.01). The pressure of the ERA group B was lower than the same pressure in group A. Conclusion: The total length of the esophagus, the ESS is lower in patients with atypical symptoms and GERD in patients with typical symptoms. Patients with atypical symptoms have diminished pressure ERA and a higher frequency of hiatal hernia regarding patients with typical symptoms and GERD. <![CDATA[<b>Incidencia de linfoma gastrointestinal en el hospital militar Dr. Carlos Arvelo en el lapso 2003-2008 un período de 5 años</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0016-35032009000400006&lng=es&nrm=iso&tlng=es Los Linfomas No Hodgkin del tracto gastrointestinal son los linfomas extranodales más comunes, representando entre el 30 y 70% de esta última patología constituyendo un grupo heterogéneo de tumores con diferentes características clínicas y patológicas. El estómago está lesionado de manera predominante siendo los linfomas tipo MALT los tumores de bajo grado más frecuentes en el sistema gastrointestinal, contando con 40% de todos los linfomas gástricos primarios, manteniéndose localizados por períodos prolongados sin tratamiento. Objetivos: evaluar la epidemiología de los linfomas gastrointestinales en nuestro centro. Pacientes y métodos: Análisis retrospectivo de las historias de pacientes con diagnóstico de Linfoma Gastrointestinal. Resultados: Se presentan datos clínicos e histopatológicos de 20 pacientes con diagnóstico de Linfoma Gastrointestinal tratados en el Hospital Militar "Dr. Carlos Arvelo", Caracas, Venezuela, desde 2003 a 2008. 15 hombres, 5 mujeres, con edades comprendidas entre 23 y 64 años. De ellos, 14 (70 %) se presentaron con enfermedad limitada (estadio I-II) mientras que 6 (30%) tuvieron lesión de varios órganos (hígado, páncreas, bazo, duodeno) o más de un órgano gastrointestinal, siendo clasificado como estadio IV. 11 pacientes tuvieron linfoma gástrico, 2 de páncreas, 1 hepático esplénico y de columna lumbar, 1 intestino delgado, 1 en bazo, 4 de colon, y en 5 pacientes se observaron multiples localizaciones en el sistema gastrointestinal. Dolor abdominal, hemorragia digestiva, pérdida de peso fueron los síntomas más frecuentes de presentación. El estómago fue el órgano mas frecuentemente lesionado.<hr/>Non-Hodgkin´s lymphomas (NHL) of the gastrointestinal (GI) tract are the most common extra nodal lymphomas. They represent between 30% to 70% of all extra nodal lymphomas and constitute a heterogeneous group of tumors with different clinical and pathological features. The stomach is predominantly involved. MALT type lymphomas are the most frequent low grade non-Hodgkin´s lymphomas encountered in the GI tract. They account for 40% of all primary gastric lymphomas and remain, in the majority of cases, localized for a prolonged period of time without therapy. Aim: To evaluate the epidemiology of gastrointestinal lymphomas in our Hospital. Patients and Methods: Retrospective analysis of patientÊs recorders performed at the Hospital Militar "Dr. Carlos Arvelo", Caracas, that had the diagnosis of gastrointestinal lymphoma. Results Clinical and histopathologjcal data from 20 patients presenting with GI-NHL treated from 2003 until 2008, were reviewed. 15 men, 5 women with an age range between 23 and 64 years old. Of these, 14 (70%) presented with limited disease (stage I and II), while 6 patients were found to have disease involvement of other abdominal organs (i.e., liver, pancreas, spleen, duodenum) or more than one gastrointestinal site and were therefore classified as stage IV. 11 patients presented with lymphoma in the stomach, 2 pancreatic, 1 in liver, spleen and lumbar vertebrae, 1 in the small intestine and 4 in the large bowel, while in 5 cases multiple localizations of the gastrointestinal tract were documented. Abdominal pain, gastrointestinal bleeding, weighs loose were the main presenting symptom. <![CDATA[<b>Dispepsia y patrones sonográficos de pancreatitis crónica y esteatosis diagnósticados por ultrasonido endoscópico</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0016-35032009000400007&lng=es&nrm=iso&tlng=es El término dispepsia se define como un discomfort o dolor recurrente localizado en el hemiabdomen superior, es un desorden heterogéneo que afecta más del 25-30% de la población. Existen muchas causas orgánicas posibles de dispepsia. La ultrasonografía endoscópica (USE), ha emergido como la técnica ideal entre las pruebas que evalúan la estructura pancreática ya que brinda importantes ventajas sobre las otras modalidades disponibles. Se ha descrito un número de criterios ecoendoscópicos para enfermedad pancreática. La relación entre la enfermedad pancreática y la dispepsia es desconocida. Objetivo: determinar enfermedad pancreática en pacientes con dispepsia, evidenciada en patrones de pancreatitis crónica o de esteatosis, por cambios en el USE. Resultados: Se evaluaron 119 pacientes, de los cuales 86 (72%) refirieron algún tipo de dispepsia mientras que 33 (28%) no. Un 60% de los pacientes presentó un patrón de pancreatopatía crónica, seguido de un 24 % de esteatosis pancreática, y otros diagnósticos en un 16%. De los pacientes con pancreatitis crónica un 40% presentó un patrón tumoral (maligno) y un 59% benigno, 46% areolar y 14% calcificado. El 69% de los pacientes que expresaron dispepsia del tipo intestino irritable presentaron pancreatitis crónica al estudio endosonográfico, al igual que el 57% que presentó dispepsia biliar. Conclusión: el USE debe ser considerado como un método diagnóstico no invasivo en la evaluación de pacientes con síntomas dispépticos y en el descarte de la patología pancreática.<hr/>The term dyspepsia is defined as a recurring discomfort or pain located in the upper hemi-abdomen, is a heterogeneous disorder that affects 25-30% of the population. There are many possible organic causes of dyspepsia. Endoscopic Ultrasonography (EUS) has emerged as the ideal technique between tests that assess pancreatic structures and, provides significant advantages over other available modes. There has been described a number of endoscopic criteria for pancreatic disease. The correlation between pancreatic disease and dyspepsia is unknown. Objective: determine pancreatic disease evidenced in chronic pancreatitis or steatosis patterns in patients with dyspepsia, due to changes in the EUS. Results: We evaluated 119 patients, of whom 86 (72%) reported some form of dyspepsia while 33 (28%) no. About 60% of patients had a pattern of chronic pancreatopathy, followed by 24% of pancreatic steatosis, and other diagnoses in 16%. Of the patients with chronic pancreatitis 40% showed a pattern tumor (malignant) and 59% mild, 46% and 14% calcified areaolar. 69% of patients expressed type dyspepsia irritable bowel showed chronic pancreatitis endosonographic study, like the 57% who presented biliary dyspepsia. Conclusion: The EUS must be considered as a non-invasive diagnostic method in evaluating patients with dyspeptic symptoms and on ruling out pancreatic disease. <![CDATA[<b>Virus del papiloma humano y lesiones papulosas de esófago</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0016-35032009000400008&lng=es&nrm=iso&tlng=es El Virus del Papiloma Humano (VPH) es un virus ADN de doble cadena, que infecta el epitelio escamoso en individuos sanos para producir verrugas y condilomas y se ha transformado en un problema de salud pública que afecta a muchos países a nivel mundial. Así mismo, causa algunos tipos de cáncer como de cuello uterino, tumores de vagina y pene; también se cree en su implicación en algunos tumores de esófago que en algunos países tiene una inusual incidencia elevada. Objetivo: correlacionar el virus de papiloma humano y las lesiones papulosas de esófago. Material y Método: estudio prospectivo, descriptivo y correlacional, conformado por 38 pacientes de ambos sexos, mayores de 18 años de edad, que asistieron al Servicio de Gastroenterología del Hospital Central Dr. Urquinaona, con síntomas de reflujo gastroesofágico y/o por hallazgo casual endoscópico presentaron lesiones papulosas de esófago, en el período de marzo-septiembre 2007. Se evaluaron desde el punto de vista clínico; realizándoles videoendoscopia digestiva superior, con toma de biopsias para estudio histopatológico e inmunohistoquimico. Resultados: la edad media de los pacientes con lesiones papulosas de esófago fue de 40,5 años. Con respecto al sexo, hubo una prevalencia del sexo femenino con un 68,4%. La clínica que prevaleció fueron los síntomas de reflujo gastroesofágico como pirosis, regurgitación, disfagia y dispepsia representados en un 57,9%. La presencia de células tópicas (coilocitos) con núcleos atípicos, se demostró en el 55,3% de pacientes asintomáticos que por hallazgo casual endoscópico presentaron lesiones papulosas de esófago. En la inmunohistoquímica se observó el VPH (+) serotipo de alto riesgo en un 39,5% de los pacientes evaluados, seguidos del 36,8% para pacientes evaluados con VPH (+) serotipo de bajo riesgo. Conclusión: existe correlación directa y proporcional entre pacientes con lesiones papulosas de esófago y VPH (+) serotipo de alto riesgo que pudieron ser corroboradas por histopatología e inmunohistoquimico.<hr/>At world-wide level, it is a problem of public health that affects to many countries The Virus of Papilloma Human (VPH) is a virus DNA of double chain, that infects epitelio flaky in individuals healthy to produce warts and condilomas. Also, causes another type of cancer besides the one of uterine neck: tumors of vagina and penis; it is believed in his implication in some tumors of esophagus, disease that in some countries has an unusual high incidence. The main objective of study was correlational the human papilloma virus and papulosas injuries of esophagus. Materials y Mhetods: prospective, descriptive and corelational study, conformed by 38 patients of both sexes, majors of 18 years of age, that attended the Service of Gastroenterology of the Central Hospital Dr. Urquinaona, with ebb tide symptoms of gastroesofágico ebb tide and/or by endoscopic accidental finding they presented/displayed papulous injuries of esophagus, in the period of March-September 2007. They were evaluated from the clinical point of view; realising digestive videoendoscopia to them superior, with taking of biopsies for histopatológico and inmunohistoquímico study. Results: the average age of the patients with papulous injuries of esophagus was of 40.5 years. With respect to sex, there was a prevalence of the feminine sex of a 68.4% in relation to the masculine one. The clinic that prevailed was the symptoms of gastroesofágico ebb tide like pirosis, regurgitation, disfagia and dispepsia represented in a 57.9%. The presence of topical cells (coilocitos) with atypical nuclei, demonstrated in 55.3% of patients asintomát that by endoscopic accidental finding they presented/displayed papulous injuries of esophagus. In the findings inmunohistoquímico it was demonstrated that the VPH (+) serotype of high risk appeared in 39.5% of the evaluated patients, followed of 36.8% for patients evaluated with VPH (-) serotype of low risk. Conclusion: it exists direct and proportional correlation between patients with papulous injuries of esophagus and VPH (+) serotype of high risk that could be corobbery by histopathology and immunohistochemistry. <![CDATA[<b>Grosor de la pared vesicular en pacientes con hipertensión portal</b>: <b>Utilidad clínica</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0016-35032009000400009&lng=es&nrm=iso&tlng=es Introducción: El engrosamiento de la pared vesicular se halla poco frecuente en el ultrasonido abdominal de pacientes con cirrosis hepática e hipertensión portal. En este trabajo se determinó cual es su importancia en relación con la presencia de varices esofágicas y otras variables. Pacientes y Métodos: Estudio prospectivo y comparativo con un grupo de pacientes con Cirrosis hepática e hipertensión portal y otro control sin Enfermedad hepática. Se realizaron pruebas hepáticas, ultrasonido abdominal, doppler de la vena porta y endoscopia digestiva superior. Resultados: Se evaluaron 25 pacientes: 14 casos y 11 controles, edad promedio 46,28 + 17,47 vs 38,27 +10,63. Las diferencias entre ellos fueron: Diámetro de vena porta 10,78 + 2,54 vs 8,63 + 1,80 mm, p=0,02; Albúmina sérica 3,16 + 0,83 vs 4,13 + 0,48 gms., p=0,003 y grosor de pared vesicular 3,51 + 1,45 vs 1,77 + 0,45mm, p=0,0009). La sensibilidad del engrosamiento de la pared vesicular para el diagnóstico de varices esofágicas resultó 80 %, especificidad 93,3 %, Valor predictivo positivo 89 % y valor predictivo negativo 88 %. Se encontró asociación entre grosor de la pared vesicular, albúmina (r=0,5347, p=0,005) y el diámetro de la porta (r=0,4105, p= 0,041). No hubo diferencias en cuanto a Velocidad sistólica máxima p=0,13; Velocidad diastólica final p=0,22; ¸ndice de resistencia p=0,15, Flujo sanguíneo portal: p=0,94. No se logró demostrar relación entre el Grosor de pared vesicular y el Child Pugh (r=0,25, p= 0,386). Conclusiones: Existe relación entre el grosor de la pared vesicular y la presencia de varices esofágicas en los pacientes con cirrosis hepática.<hr/>Objetive: The thickening of the gall bladder wall is little common in the abdominal ultrasound of patients with liver cirrhosis and portal hypertension. In this work it was determined its importance in relation with the presence of esophageal varices and other variables. Patients and Methods: A prospective and comparative study was made (cases and controls). A group of patients with liver Cirrhosis and portal hypertension and another control without liver Disease. Were made Liver tests, abdominal ultrasound, Doppler of the porta vein and upper endoscopy. Results: Evaluated 25 patients; 14 cases patients, and 11 patients controls, mean age 46.28 ± 17.47 vs 38.27 ± 10, 63. Differences between them were: Vein diameter porta 10.78 + 2.54 vs 8.63 + 1.80mm, p=0,02; Seric albumin 3.16 + 0.83 vs. 4.13 + 0.48 gm, p=0,003 and gall bladder wall thickness 3.51 + 1.45 versus 1.77 + 0,45mm, p=0,0009. The sensitivity of the thickening gall bladder wall for the diagnosis of esophageal varices was of 80%, specificity 93.3%, predictive value positive 89% and predictive value negative 88%. Association was found between thickness of the gall bladder wall and albumin (r=0,5347, p=0,005) and the diameter of the porta (r=0,4105, p= 0.041). There was not differences respect to speed systolic Maximum p=0,13; diastolic speed final p=0,22; resistance index p=0,15; blood Flow portal: p=0,94. No profit to demonstrate between the Thickness of gall bladder wall and the Child Pugh (r=0, 25, p= 0.386).Conclusions: Relationship betwen the gall bladder wall thickening and the presence of esophageal varices in patients with cirrhosis of the liver and portal hypertension. <![CDATA[<b>Tratamiento con fenestración</b>: <b>Reporte de un caso y revisión</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0016-35032009000400010&lng=es&nrm=iso&tlng=es The congenital liver cysts may be unique, multiple or diffusely distributed in the liver parenchyma ("polycystic liver"). In polycystic liver, the number, size cysts may be associated with polycystic kidney, with the severity of renal disease, which affects the prognosis. The pain and progressive increase in the abdominal cavity, associated with the growth of cysts worsen the quality of life coming to incapacitate the patient. Medical treatment consists of conservative amendments to the diet, AIN, use of antihypertensive and diuretics if developed hypertension. Others include the use of percutaneous drainage led by ultrasound, fenestration, partial hepatectomy and eventually liver transplantation and / or liver-kidney. The following case is a patient of 39-year-old female, with polycystic kidney and liver who does not respond to conservative medical treatment; presenting diffuse persistent abdominal pain, breathlessness on moderate efforts to limit the activity of daily living and incapacity to work, who was practiced remove the wall and fenestration of liver cysts with satisfactory postoperative evolution and improvement of the quality of life. <![CDATA[<b>Herencia dominante de malformación anorrectal. </b><b>Estudio en una familia </b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0016-35032009000400011&lng=es&nrm=iso&tlng=es Las Malformaciones anorrectales (MAR) son un grupo de malformaciones congénitas caracterizadas por la falta de unión entre la fosa anal y el conducto anorrectal. Afecta al varón con una frecuencia ligeramente mayor que en la mujer. Se presentan en uno de cada 4000 a 5000 recién nacidos vivos. El riesgo estimado de una pareja de tener otro hijo con una MAR es de alrededor del 1 %. Se ha descrito en algunos casos un tipo de herencia autosómica recesiva, pero la recurrencia en la hermandad es por lo general baja. Presentamos un caso de neonato masculino con diagnóstico antenatal de megacolon congénito Vs MAR quien a la exploración física se evidencia ano no permeable y se le realiza invertograma con hallazgo de MAR alta, por lo cual se realizó colostomía derivativa, y en un segundo tiempo quirúrgico se diagnostica malrotación intestinal.
