Materiales y métodos: Estudio analítico, prospectivo y de corte transversal evaluando 65 pacientes referidos a la consulta de Gastroenterología por la Consulta de Otorrinolaringología con el diagnóstico de reflujo faríngolaríngeo, del Centro Medico Docente La Trinidad en el período Enero 2013 a Noviembre 2014. Se les realizo interrogatorio evaluando síntomas típicos (regurgitación y pirosis) y síntomas atípicos (tos, carraspeo y disfonía), manometría y monitoreo del pH esofágico de 24 horas. Posteriormente se seleccionaron dos grupos a comparar: Grupo A con ausencia de porción intra-abdominal del esfínter esofágico inferior y grupo B con presencia de porción intra-abdominal del esfínter esofágico inferior. Resultados: Las variables: edad, sexo, síntomas típicos (pirosis, regurgitación), atípicos (tos, carraspeo, ronquera), presión del esfínter esofágico inferior y la motilidad del cuerpo esofágico no fueron significativas al compararse los grupos Grupo A N=17, y grupo B N=48. El monitoreo de pH esofágico de 24 horas fue estadísticamente significativo al compararse los grupos, evidenciándose que el grupo A tenía mayor porcentaje de resultados positivos para reflujos patológicos. Conclusión: La ausencia de porción intra-abdominal determinada por manometría esofágica se relaciona con la presencia de reflujos patológicos determinados por el monitoreo de pH esofágico de 24 horas, demostrando que esta puede ser un factor desencadenante de ERGE y por consiguiente de reflujo faríngolaríngeo.<hr/>Objetive: To determine the relationship between pharyngolaryngeal reflux and the absence of intra-abdominal portion of the lower esophageal sphincter. Materials and methods: Analitical, prospective study. Evaluating 65 patients referred to the consultation of the Gastroenterology from Consultation of the Otorhinolaryngology diagnosed with reflux pharyngolaryngeal of Medical Teaching Center La Trinidad period January 2013 to November 2014. They were interviewed to assess typical symptoms (heartburn and regurgitation) and atypical symptoms (cough, hawking and hoarseness), esophageal manometry and ambulatory 24 hour esophageal pH monitoring. Two groups were selected to compare: group A with no intra-abdominal portion of the lower esophageal sphincter and group B with presence of intra-abdominal portion of the sphincter upper esophageal. Results: The variables: age, gender, typical symptoms (heartburn and regurgitation) and atypical symptoms (cough, hawking and hoarseness), pressure of the lower esophageal sphinter and the motility of the esophagic body, they were not significant when compared groups. The ambulatory 24 hours esophageal pH monitoring was significant statistically when compared groups, demostrating that the group A had the highest percentage of positive results for pathological reflux. Conclusion: the absence of intra-abdominal portion determined by esophageal manometry is related to the presence of pathological reflux determined by ambulatory 24 hour esophageal pH monitoring, demonstrating that this can be a trigger factor of Gastroesophageal reflux disease and therefore pharyngolaryngeal reflux. <![CDATA[<b>Cáncer Colorectal</b>: <b>características de la expresión del oncogen K-ras</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0016-35032016000200003&lng=es&nrm=iso&tlng=es Introducción: El cáncer de colon ocupa el cuarto lugar como causa de muerte por cáncer en adultos. Objetivo: Describir características de la expresión del oncogen K-ras en pacientes con cáncer colorectal (CCR) que acudieron a consulta de Gastroenterología del Hospital Universitario de Caracas en el período enero-julio 2014. Metodología: Estudio de corte transversal, descriptivo y prospectivo. La población de estudio estuvo conformada por pacientes con diagnóstico de CCR por colonoscopia e histología con evaluación molecular del K-ras. Resultados: de 35 pacientes 57,14% fueron del sexo masculino y 42,86% del femenino con edad media de 57±17años; el 100% de la muestra por histología correspondió a ADC predominando el tipo moderadamente diferenciado (40,00%). 28 pacientes (80%) no presentó mutación del K-ras mientras que 7 (20,00%) sí, de los cuales 6 (85,71%) reportó mutación en el codón 12 y 1 en el 13 (14,28%); en 4 de los 7 pacientes (57,14%) la mutación estuvo en el colon izquierdo. Conclusiones: la mutación del K-ras predomina en el sexo masculino con edad media de 57años estando presente en 20% de la población; la mutación en el codón 12 es más frecuente asociada al colon izquierdo y el CCR más común es el ADC bien diferenciado.<hr/>Introduction: Colon cancer ranks fourth leading cause of cancer death in adults. Objective: To describe characteristics of the expression of K-ras oncogene in patients with colorectal cancer (CRC) who attended the outpatient Gastroenterology of the University Hospital of Caracas (HUC) in the period from january-july 2014. Methodology: Transversal, descriptive and prospective court. The study population consisted of patients diagnosed with CRC by colonoscopy and histology with molecular evaluation of K-ras. Results: Of the 35 patients 57,14% were male and 42,86% female with a mean age of 57±17 years old; 100% of the sample corresponded to ADC histology predominating moderately differentiated rate (40.00%). 28 patients (80%) had no mutation of K-ras while 7 (20,00%) did, of which 6 (85,71%) reported mutation at codon 12 and 1 in the 13 (14,28%); in 4 of 7 patients (57,14%) mutation was in the left colon. Conclusions: K-ras mutation predominates in males with an average age of 57 years old and is present in 20% of the population; mutation in codon 12 is most frequently associated with the left colon and the most commom type of CRC by histology is the well differentiated ADC. <![CDATA[<b>Ictericia obstructiva benigna y maligna</b>: <b>utilidad clínica del CA 19-9</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0016-35032016000200004&lng=es&nrm=iso&tlng=es Introducción: Se estableció como objetivo determinar la utilidad clínica del marcador tumoral CA 19-9 en ictericia obstructiva de origen benigno y maligno en pacientes adultos que acuden al servicio de gastroenterología del Hospital Universitario de Caracas. Pacientes y métodos: se estableció como muestra a todos los pacientes que fueron hospitalizados en el Servicio de Gastroenterología del Hospital Universitario de Caracas con ictericia obstructiva, a los cuales se les realizó la determinación del CA 19-9 antes y siete días después del drenaje de la vía biliar a través de una colangiopancreatografía retrógrada endoscópica y se determinó la variación de los valores de CA 19-9 y si este valor permitió diferenciar entre ambas etiologías. Resultados: Los valores de CA 19-9 no se modificaron de forma significativa después del drenaje biliar en la patología benigna ni en la maligna. Se obtuvo un nuevo punto de corte para el CA 19-9 (47U/L), manteniendo la sensibilidad, pero aumentando la especificidad. Conclusiones:No se puede utilizar la variación del CA 19-9 después del drenaje biliar como elemento diagnóstico para diferenciar entre enfermedad benigna o maligna biliopancreática.<hr/>Aim: to determinate the clinical utility of the tumor marker CA 19-9 in obstructive jaundice of benign and malignant origin in adult patients who comes to the service of gastroenterology of the Caracas University Hospital.Patients and methods: was stablish as sample every patient who undergo hospitalization in the service of gastroenterology Caracas University Hospital with obstructive jaundice in which was determinate the CA 19-9 before and seven days after the drainage of the biliary pathway with endoscopic retrograde colangiopancreatography and was determinate the variation of the results of the CA 19-9 and if this result can differentiate between both etiologies. Results: the values of CA19-9 did not modified in significant way after the biliary drainage in the benign and malignant pathology. It was obtained a new cohort point of the CA 19-9 (47U/L), keeping the sensitivity, but increasing the specificity. Conclusions: it can’t be used the variation of the CA 19-9 after the biliary drainage as diagnostic element to differentiate between benign and malignant pathology.. <![CDATA[<b>Experiencia colangioscopia peroral monoperador (SPYGLASS) en patología biliar en un centro privado de tercer nivel en Venezuela</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0016-35032016000200005&lng=es&nrm=iso&tlng=es Spyglass es un colangioscopio peroral mono operador desechable que ha estado disponible desde principios de 2007 para patología biliar. Objetivo: Demostrar la experiencia en Venezuela usando sistema Spyglass en pacientes con patología biliar. Pacientes y Métodos: Estudio prospectivo (febrero2012-mayo2015), Policlínica Metropolitana.50 spyglass fueron realizados, representando 7,7% del volumen de CPRE biliares en el mismo período (50/650). Se incluyeron 46 pacientes (45 hombres, 1 mujer), edad media 62,6 años (24-92 años). Indicaciones: coledocolitiasis (20), estenosis (23), tumor (2), descarte Neo entre 2 prótesis (1), prótesis trans stent (1). Un mono operador a través de un duodenoscopio terapéutico realizó colangioscopia spyglass. Sedación por anestesiología en quirófano y antibiótico profiláctico fue administrada. Resultados: Spyglass fue realizada en 46 pacientes (50 procedimientos).Hallazgos: litiasis (20 pacientes), estenosis benigna (10), estenosis maligna (13), colangitis esclerosante (1), prótesis trans stent (1). De los pacientes con litiasis, el clearance ductal posterior a litotripsia holmiun láser se logró en el 85% después de una sesión, y el 15 % en un segundo intento.Spyglass confirmó el diagnóstico de malignidad en 13/21 (62%), el diagnóstico se estableció por combinación de características visuales y biopsias directas.En 7/21 (33,33%) cambio diagnóstico: benigno (6), colangitis esclerosante (1) y en uno no se pudo establecer diagnóstico. La sospecha de benignidad se confirmó en 3/4 pacientes. Pancreatitis, microperforación retroperitoneal y fístula pancreática se reportó en un paciente (2,17%) resuelto con tratamiento médico y quirúrgico. Conclusiones: Spyglass es una técnica útil en estenosis sospechosa de benignidad o malignidad, difirió el diagnóstico, modificando la conducta. Es una técnica adyuvante de la CPRE en el manejo de cálculos.<hr/>Spyglass is a disposable peroral cholangioscope monoperator that has been available since early 2007 for biliary disease. Objective: To demonstrate the experience in Venezuela using Spyglass system in patients with biliary disease. Patients and Methods: Prospective study (february2012-may2015), Polyclinic Metropolitana.50 spyglass were performed, representing 7.7% of the volume of bile ERCP in the same period (50/650). 46 patients (45 males, 1 female), mean age 62.6 years (24-92 years) were included. Indications: choledocholithiasis (20), stenosis (23), tumor (2), dicard neo between two prothesis (1), trans prosthesis stent (1) .A monoperator through a therapeutic duodenoscope made cholangioscopy spyglass. Sedation for anesthesiology and prophylactic antibiotic was administered. Results: Spyglass was performed in 46 patients (50 procedures) .Findings: lithiasis (20 patients), benign stricture (10), malignant stricture (13), sclerosing cholangitis (1), trans prosthesis stent (1). Of patients with stone disease, ductal clearance holmium laser after lithotripsy was achieved in 85% after a session, and 15% in a second attempt.Spyglass confirmed the diagnosis of malignancy in 13/21 (62%), the diagnosis was established by combination of visual features and directs biopsies.En 7/21 (33.33%) change diagnosis: benign (6), sclerosing cholangitis (1) and one diagnosis could not be established. The suspicion was confirmed benignity in 3/4 patients. Pancreatitis, retroperitoneal microperforation and pancreatic fistula was reported in one patient (2.17%) resolved with medical and surgical treatment. Conclusions: Spyglass is a useful technique for suspected benign or malignant stricture, diagnosis differed modifying behavior. Spyglass is a useful adjuvant to ERCP in the management of difficult stone disease. <![CDATA[<b>Influencia de la microbiota intestinal en la enfermedad hepática crónica. Su rol en el hepatocarcinoma</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0016-35032016000200006&lng=es&nrm=iso&tlng=es El incremento de la traslocación de bacterias intestinales es característico en pacientes con enfermedad hepática crónica y favorece la inflamación y la fibrosis del hígado.1-3 Se ha implicado a la microbiota intestinal en la patogénesis y progresión de enfermedades hepáticas como Enfermedad Grasa Hepática No alcohólica, Esteatohepatitis no alcohólica, Colangitis Esclerosante Primaria, Enfermedad hepática por Falla intestinal y el Hepatocarcinoma.
