Con la finalidad de determinar la frecuencia de Aeromonas spp. como agente causal de enfermedades gastrointestinales, se realizó un estudio en pacientes menores de seis años de edad con síndrome diarreico agudo, asistidos en la emergencia pediátrica del Servicio Autónomo Hospital Universitario “Antonio Patricio de Alcalᔠ(SAHUAPA), en Cumaná, estado Sucre; para ello se recolectaron 130 muestras de heces durante el período de mayo a septiembre de 2002, a las cuales se les realizaron estudios microbiológicos a través de coprocultivo y susceptibilidad antimicrobiana mediante la técnica de difusión en disco. Del total de muestras analizadas, se obtuvo un porcentaje de positividad de 24,60%; de los cuales 12,50% correspondieron a Aeromonas spp. Las especies identificadas del género Aeromonas, fueron A. caviae, A. hydrophila y A. sobria; Aeromonas caviae fue la más frecuente, representando el 50,00% de las especies aisladas. La distribución por edad, ubicó las especies de Aeromonas spp. en edades comprendidas de 1 a 4 años, específicamente en el grupo de edad de 1 a 2 años se ubicaron tres de los cuatro aislamientos obtenidos. En relación a la susceptibilidad antimicrobiana, las cepas aisladas de Aeromonas spp. fueron sensibles al cloranfenicol en 100,00% y al trimetoprim-sulfametoxazol en un 75,00%; así mismo, se mostraron resistentes a cefalotina en 75,00%. Los resultados obtenidos en este trabajo de investigación, muestran que Aeromonas spp. es una bacteria de baja frecuencia y de fácil tratamiento con antimicrobianos, en el caso de pacientes que así lo requieran; sin embargo, por su repercusión clínica y microbiológica se justifica su búsqueda en muestras intestinales.<hr/>Diarrhea represents a serious public health problem, and bacteria is one of the most frequent causes of the same, which requires the need to select an effective bacteriological diagnosis. In order to determine the frequency of Aeromonas spp. as a causal agent of gastrointestinal illnesses, a study was carried out in patients under 6 years of age with acute diarrheic syndrome, who were attended in the pediatric emergency service at the “Antonio Patricio de AlcalᔠUniversity Hospital (SAHUAPA), in Cumaná, Sucre state. For this study130 stool samples were collected during the period of May to September 2002, and a microbiological study of the samples was carried out through copro-culture and the antimicrobial susceptibility test using the disk diffusion method. Of the total number of samples analyzed, a positive response was obtained in 24,60%; of which 12,5% corresponded to Aeromonas spp. The species of the Aeromonas gender that were identified were were A. caviae, A. hydrophila and A. sobria. Aeromonas caviae was the most frequent, representing 50% of the isolated species. The distribution for age placed the species of Aeromonas spp. in the 1 to 4 year age group, and more specifically in the age group of 1 to 2 years where three of the four isolates were located. In relation to antimicrobial susceptibility, all the isolates of Aeromonas spp. were sensitive to chloramphenicol at 100,00% and to trimetroprim-sulfamethoxazole at 75,00%. They showed resistant to cefalotin at 75,00%. The results obtained in this research indicate that Aeromonas spp. presented a low frequency and ease of treatment with antimicrobials; however, due to their clinical and microbial repercussion, their identification in intestinal samples is justified. <![CDATA[<b>Pathogenic Bacteria in Infections of the Respiratory Tract</b>: <b>Autonomous University Hospital Service “Antonio Patricio de Alcalá”. </b><b>Cumaná, Sucre State</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0075-52222005000100003&lng=pt&nrm=iso&tlng=pt Con el propósito de determinar la frecuencia de bacterias patógenas a nivel del tracto respiratorio en niños con edades entre 0 y 11 años que acudieron a la emergencia pediátrica del Servicio Autónomo “Hospital Antonio Patricio de Alcalá”, Cumaná estado Sucre, durante el período junio 2001 a noviembre 2002, se analizaron 144 muestras de niños con infecciones del tracto respiratorio. El diagnóstico microbiológico se realizó siguiendo los métodos clásicos para el procesamiento de muestras clínicas y la resistencia antimicrobiana por los métodos estándares del National Committee of Clínical Laboratory (NCCLS). Se encontró una mayor frecuencia de infecciones a nivel del tracto respiratorio superior (75,00%). La principal causa de consulta médica fue la faringitis (43,00%), seguido por otitis (32,00%) y neumonías (25,00%). Del total de muestras procesadas 62,00% fueron positivas para cultivo bacteriológico. El microorganismo aislado con mayor frecuencia en el tracto respiratorio superior fue Streptococcus β-hemolítico del grupo A (24,49%) y Staphylococcus aureus (16,32%) y en el tracto respiratorio inferior Klebsiella pneumoniae (10,20%). El mayor porcentaje de resistencia de los microorganismos aislados se presentó para el grupo de los β-lactámicos.<hr/>In orden to determine the frequency of bacterial pathogens in the respiratory tract of children between the ages of 0 - 11 years old who attend the emergency service in the Pediatrics ward at the Autonomous Hospital “Antonio Patricio de Alcalᔠin Cumaná, Sucre state during the period from June to November 2001, 150 samples from children with infections in the respiratory tract were analyzed. The microbiological diagnosis was performed using classical methods to identify the microorganisms in clinical samples. The resistance to antibiotics was determined by the agar dilution technique according to the guidelines of the National Committee for Clinical Laboratory Standards (NCCLS). The upper respiratory tract infections (75.00%) exceeded the lower respiratory tract infections. The main cause for medical consultation was pharyngitis (43.00%), followed by otitis (32.00%) and pneumonia (25.00%), and. 62.00% the samples were positive. The microorganisms most frequenct in the upper respiratory tract were Streptococcus bbb-hemolytic group A (24.49%) and Staphylococcus aureus (16.32%) and in the lower respiratory tract Klebsiella pneumoniae (10.20%). The greater percentaje of resistance in the microorganism isolates was for the β-lactamics group. <![CDATA[<b>Biological Waste Generated in Laboratories in the Faculty of Medicine at a University Institution</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0075-52222005000100004&lng=pt&nrm=iso&tlng=pt Con el objeto de determinar el manejo de desechos biológicos generados en laboratorios de docencia, asistencia e investigación de una Facultad de Medicina, se realizó un estudio descriptivo, transversal en dos fases, la primera para determinar número, tipo y distribución de los laboratorios; la segunda consistió en la elaboración y aplicación de un instrumento de recolección de información concerniente al manejo de desechos biológicos, según lo establecido en el Decreto Nacional venezolano 2218-92. Del total de 43 laboratorios, respondieron el 81,4%, de los cuales el 71,4% genera desechos biológicos, prevaleciendo los tipos B y C, seguido de los tipos D (44%). De estos los más frecuentes fueron sangre (64%), suero (32%) y orina (28%). La recolección y almacenamiento primario más utilizado fue el tipo 1 (44%). No se apreció uniformidad en la frecuencia del transporte interno. El 56% de los laboratorios no implementan modalidad de tratamiento antes de su disposición final. Esta investigación revela que los laboratorios de la Facultad estudiada generan desechos biológicos que pueden causar efectos adversos a la salud y al ambiente si no se toman medidas correctivas en el manejo de los mismos, siendo por ello necesario implementar un sistema de gestión de desechos biológicos.<hr/>In order to determine the management of biological waste generated in teaching and research laboratories in the Faculty of Medicine, a two phase, cross-sectional, descriptive study was carried out; the first study was to determine number, type and distribution of the laboratories; and the second consisted in the elaboration and application of an instrument to collect information concerning management of biological waste, according to what is established in the Venezuelan National Decree No. 2218-92. Of the total sample of 43 laboratories, 81.4% responded, 71,4% of which generate biological waste, with a higher prevalence of types B and C, followed by type D (44%). Of these the more frequent sources were blood (64%), serum (32%) and urine (28%). The harvesting and primary storage most utilized was type 1 (44%). There does not appear to be uniformity in the frequency of internal transport. In 56% of the laboratories modality of processing before final disposition is not implemented. This research revealed that laboratories in university faculties generated biological waste that can cause adverse effects on health and the environment if corrective measures are not employed in their management, and for this reason it is necessary to implement a biological waste management system. <![CDATA[<b>Prevalence of<i> Entamoeba histolytica/Entamoeba dispar </i>in patients with Gastrointestinal Symptoms of Diarrhea and who Come from Cumaná, Sucre State</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0075-52222005000100005&lng=pt&nrm=iso&tlng=pt Entamoeba histolytica es considerada un problema de salud pública debido a su alta prevalencia y las consecuencias para los individuos, especialmente niños a quienes puede causar incluso la muerte. Pocos estudios se han llevado a cabo sobre la prevalencia de estas especies en el nororiente de Venezuela, por lo que se planteó analizar 400 muestras de heces de pacientes con síntomas gastrointestinales de diarrea de ambos sexos y diferentes grupos etarios para determinar la prevalencia del complejo Entamoeba histolytica/Entamoeba dispar y su asociación con otros parásitos ademàs de su asociación con algunos parámetros epidemiològicos. Las muestras fueron analizadas por métodos coproparasitológicos usando solución salina fisiológica al 0,85%, lugol, coloración tricrómica, sangre oculta y concentración por Ritchie. A cada paciente se le aplicó una encuesta para obtener datos clínicos y epidemiológicos. Se encontró una prevalencia de 16,0% (n=64) para el complejo Entamoeba histolytica/Entamoeba dispar. Otros parásitos frecuentes fueron Blastocystis hominis 19,3% (n=77), Entamoeba coli 9,3% (n=37), Endolimax nana 8,0% (n=32), Giardia lamblia 5,8% (n= 23), Trichuris trichiura 4,0% (n=16), Ascaris lumbricoides 3,8% (n=15). Un 15,5% de los pacientes estaban poliparasitados, mientras que 37,0% mostraron un único parásito. Blastocystis hominis resultó ser el parásito mayormente asociado con complejo Entamoeba histolytica/Entamoeba dispar 10,9% (n=7). La sintomatología fue variable entre los que podemos mencionar vómitos (26,6%), nauseas (39,1%), dolor abdominal (68,8%), fiebre (28,1%) y flatulencia (65,6%). No se encontró asociación significativa entre la presencia de las amibas y el sexo (χ²=0,46, p>0,05), pero sí con la edad (χ²=16,63, p<0,05), siendo el grupo etario 10-19 años el más afectado. Las cifras de prevalencia de parásitos intestinales, especialmente para el complejo Entamoeba histolytica/Entamoeba dispar, demuestran que existe un problema importante de salud que debe ser abordado por las autoridades sanitarias del estado.<hr/>Entamoeba histolytica is considered a public health problem due to its high prevalence and its consequences for the affected individuals, and especially in children where it can cause death. Few studies of the prevalence of this parasite have been carried out in northeastern Venezuela, and for this reason feces samples from 400 patients with gastrointestinal symptoms of diarrhea, of all ages and both sexes were studied, in order to determine the prevalence of complex Entamoeba histolytica/Entamoeba dispar and its association with other parasites. Samples were analyzed copro-parasitologically using 0.85% saline physiological and lugol solution, trichromic staining, hidden blood tests and concentration tests elaborated by Ritchie. An interview was made with each participant in order to register epidemiological and clinical data. A prevalence of 16.0% (n=64) for complex Entamoeba histolytica/Entamoeba dispar was found. Other frequently observed parasites were Blastocystis hominis 19.3% (n=77), Entamoeba coli 9.3% (n=37), Endolimax nana 8.0% (n=32), Giardia lamblia 5.8% (n=23), Trichuris trichiura 4.0% (n=16), Ascaris lumbricoides 3.8% (n=15). About 15.5% of the patients showed multiparasitic infections and 37% presented only one parasite. Blastocystis hominis was the species most frequently associated with complex Entamoeba histolytica/Entamoeba dispar 10.9% (n=7). Symptoms were variable, including vomiting (26.6%), nausea (39.1%), abdominal pain (68.8%), fever (28.1%) and flatulence (65.6%). The presence of amoebas was not statistically associated with sex (χ²=0.46, p>0.05), but it was related to age (χ²=16.63, p<0.05), and the age group from 10-19 years old was the most affected. The prevalence of intestinal parasites, especially for complex Entamoeba histolytica/Entamoeba dispar, indicated that this is an important problem which needs to be addressed by sanitation authorities. <![CDATA[<b>Stingray</b>: <b>Epidemiological and Clinical Aspects. Bolívar State, Venezuela</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0075-52222005000100006&lng=pt&nrm=iso&tlng=pt Las rayas son peces venenosos que pueden producir lesiones mediante la inyección de veneno a través de una estructura punzante, al ser pisados bajo la arena. El objetivo de este estudio fue determinar la prevalencia del envenenamiento por raya (Rayismo) en el Hospital “Ruiz y Páez”, centro de referencia del estado Bolívar. Se realizó un estudio descriptivo de los casos de envenenamiento por Raya, durante el período comprendido entre Enero de 1996 y Julio de 2003. Un total de 24 casos fueron registrados (3,8 casos /año). La edad media de los afectados fue 26,2+16,1 años. El 91,7% eran de sexo masculino; y las ocupaciones más frecuentes eran: pescadores (33,3%), estudiantes (25%) y agricultores (16,7%). El 70,8 % procedía del Estado Bolívar. La herida se localizó en los miembros inferiores en todos los casos, con dolor (100%), eritema (91,6%), sangramiento (41,6%), necrosis y ulceración (29,2%). A todos los pacientes se le realizó asepsia y antisepsia de la herida, sólo el 33,4% requirió limpieza quirúrgica de la herida. Todos evolucionaron satisfactoriamente. El rayismo constituye un problema de salud en el estado Bolívar, del que tienen que ocuparse los médicos que ejercen en la región.<hr/>Sstingrays are poisonous fish that can produce lesions by means of injecting poison to the victim through a penetrative structure. The aim of this study was to determine the prevalence of stingray poisoning in the “Ruiz y Páez” Hospital, the principal hospital in Bolívar state. It was a descriptive, retrospective study. We reviewed data as to stingray poisoning cases during the period between January 1996 and July 2003. Twenty-four cases were registered (3.8 cases / year). Mean age of those affected was 26.2±16.1 years, 91.7% were masculine. The most frequent occupations were: fishermen (33.3%), students (25%) and farmers (16.7%), and 70.8% came from Bolivar State. The wounds were located in the lower members in all the cases, with local pain (100%), erythema (91.6%), hemorrhage (41.6%), necrosis and ulceration (29.2%). All patients were treated with asepsis and antisepsis of the wound, only 33.4% required surgical cleaning of the wound. All evolved satisfactorily. Poisoning by stingrays constitutes a reality in the Bolívar state, and the physicians that serve this region have to see to this problem and to prepare to adequately manage it. <![CDATA[<b>Reactivation of the Chagasic Infection in Wistar Rats in Gestation</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0075-52222005000100007&lng=pt&nrm=iso&tlng=pt En este trabajo investigamos en ratas Wistar crónicamente infectadas con Trypanosoma cruzi, la reactivación de la infección durante la gestación y después del parto, mediante un estudio parasitológico, inmunológico, histopatológico e inmunohistoquímico. Los resultados mostraron un control de las parasitemias patentes y/o subpatentes; títulos elevados de anticuerpos específicos anti-T. cruzi, detectándose en las ratas gestantes una disminución en los niveles de IgG y un incremento significativo de la IgM (P<0,05) a partir de la primera semana de gestación; ausencia de formas flageladas de T. cruzi en el líquido amniótico. La histopatología reveló, persistencia parasitaria a nivel de las fibras musculares cardíacas y de la musculatura lisa del útero grávido; establecimiento de una discreta miocarditis y miositis con características de cronicidad; moderado infiltrado inflamatorio, sin parasitismo en placentas y cerebro. El estudio inmunohistoquímico demostró la presencia de un nido de amastigotes en el músculo esquelético y abundantes depósitos antigénicos en las secciones de corazón, útero, cerebro y placentas, lo que demuestra la presencia de tripomastigotes en esos tejidos. Finalmente, se discuten los factores que pudieran inducir la reactivación de la infección por T. cruzi en ratas Wistar gestantes.<hr/>Wistar rats with chronic Trypanosoma cruzi infection were subjected to a parasitological, immunological, histopathological and immunohistochemical examination for a reactivation of the infection during gestation and after giving birth. Results showed a control of patent and/or sub-patent parasitemias; high concentrations of specific anti-T. cruzi antibodies, a moderate reduction in IgG levels and a significant increase in IgM (P<0.05) after the first week of gestation; and absence of flagellate forms of T. cruzi in the amniotic fluid. Histopathology showed parasitic persistence in cardiac muscle fibers and in the uterus, mild myocarditis and myositis with signs of chronicity; moderate inflammatory infiltrate, without parasitism in the placenta stroma or brain. Immunohistochemical examination showed the presence of a nest of amastigotes in the skeletal muscle with abundant antigen deposits in sections of the heart, uterus, brain and placenta, revealing the presence of trypomastigotes in the tissue. Finally, the factors that might lead to a reactivation of T. cruzi infection in gestating Wistar rats are discussed. <![CDATA[<b>Treatment of Vertebrate Osteomielitis with Candida spp. using Caspofungina</b>: <b>The Presentation of a Case Study and a Bibliographical Review</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0075-52222005000100008&lng=pt&nrm=iso&tlng=pt Introducción: La osteomielitis Vertebral (OV) por especies de Candida no albicans es una forma infrecuente de osteomielitis, sin embargo el aumento de los pacientes inmunosuprimidos y las técnicas invasivas han aumentado su incidencia. La resistencia de estos Hongos a los antifungicos del tipo de los azoles como el Fluconazol es mayor que en Candida albicans. Metodo: Se reporta el caso de un paciente masculino de 75 años, Diabético con antecedente de prostatectomia con anestesia por bloqueo epidural, que 3 semanas después presenta fiebre y dolor lumbar irradiado a piernas. Resultado: El diagnóstico de OV fue realizado por Resonancia Magnética (R.M) de columna lumbosacra que reportó lesiones osteolíticas en L4 y L5 y en el disco intervertebral. Se practicó drenaje quirúrgico, obteniéndose muestra para cultivo que reportó Candida spp., se inicio tratamiento con FLUCONAZOL 400 mg. E.V. día. Después de 15 días, el paciente persistía con sintomatología, Velocidad de Sedimentación Globular (VSG) elevada, Proteína C Reactiva positiva y secreción purulenta por la herida quirúrgica. Se reporta nuevamente OV por R.M. de columna lumbosacra y absceso paravertebral. Se somete el paciente a nueva limpieza quirúrgica donde se aísla nuevamente Candida spp. en cantidad abundante. En vista del fracaso clínico y microbiológico se inicia terapia con Caspofungina 50 mg. E.V día. Luego de 10 días de tratamiento, el paciente se vuelve asintomático con VSG de 5 mm y Proteína C reactiva negativa, por lo que se cambio el tratamiento a Itraconazole 200 mg. V.O. día por 3 meses; se practicó R.M de control que reportó fusión de L4-L5, sin evidencias de osteomielitis. Conclusiones: En la literatura revisada existen pocos casos de OV debido a Candida spp. posterior a bloqueo epidural y este es el único en nuestro país. El porcentaje de resistencia de especies Candida no albicans al FLUCONAZOL es bajo pero cuando aparece se deben utilizar alternativas como la Anfotericina B y la Caspofungina. En este caso se decidió utilizar la Caspofungina por el daño renal subyacente al paciente diabético. Este caso demuestra que la Caspofungina puede ser una alternativa valida en el tratamiento de la OV por Candida no albicans.<hr/>Introduction: Vertebral osteomyelitis (VO) by non-albicans Candida species is an infrequent type of osteomyelitis, however, its incidence has increased due to the increasing number of immunosuppressed patients and invasive procedures. Candida spp is more frequently resistant to azole antifungal drugs, such as fluconazole, than Candida albicans. Methods: We report one case of a 75-year-old diabetic male, with low back pain radiated to both legs, tenderness, and fever 3 weeks after an epidural anesthesia for prostatectomy. Results: VO diagnosis was supported by lumbosacral magnetic resonance imaging (RMI), which showed osteolitic lesions in the L4-L5 spine and the intervertebral disk. Surgical drainage was performed, with a sample for culture yielding Candida spp. The patient was treated with on I.V fluconazole 400 mg per day. Symptoms persisted 15 days later; the erythrocyte sedimentation rate (ESR) was elevated, the C-reactive protein was positive and there was purulent secretion in the surgical wound. A new lumbosacral spine MRI still showed vertebral osteomyelitis and a paravertebral abscess. The patient underwent a new surgical debridement and the sample for culture yielded a heavy growth of Candida spp. In view of the clinical and microbiologic treatment failure, the patient was placed on iv caspofungin 50 mg daily. The patient became asymptomatic after 10 days of treatment; the ESR decreased to 5 mm and the C-reactive protein was negative; therefore, treatment was switched to 200 mg of itraconazole for 3 months. A follow-up MRI showed a L4-L5 spine fusion with no evidence of osteomyelitis. Conclusions: Few cases of epidural blockage related to vertebral osteomyelitis was found in the reviewed literature and this is the only case due to Candida. The percentage of fluconazole-resistant C. tropicalis isolates is low but when no-albicans Candida fluconazole resistance emerges, alternatives such as amphotericin B and caspofungin have to be used. Caspofungin was chosen in this case due to the patient’s underlying renal injury. This case demonstrates that caspofungin can be an alternative for the treatment of Candida spp vertebral osteomyelitis.
