El objetivo de este estudio fue implementar la técnica de los pozos de difusión para el estudio de la susceptibilidad de los dermatofitos. Se utilizaron un total de 100 aislados de dermatofitos (74 Trichophyton rubrum, 18 Trichophyton mentagrophytes, 1 Trichophyton tonsurans, 5 Microsporum canis y 2 Epidermophyton floccosum) provenientes de muestras clínicas aisladas e identificadas en la Sección de Micología Médica, IMT. Se empleó la técnica de los pozos de difusión con los siguientes antimicóticos: Griseofulvina, Terbinafina, Fluconazol e Itraconazol. Se aplicaron los criterios de interpretación acordados por la NCCLS. El inóculo, se estandarizó utilizando el peso como unidad. El medio Agar Staib demostró ser el más adecuado, al obtenerse un mejor crecimiento de los aislados en un 97 %. Esta es la primera vez, que se utiliza el peso como medida para cuantificar el inóculo en los dermatofitos. La mayor sensibilidad se evidenció ante Terbinafina seguido de Griseofulvina e Itraconazol. No se demostró sensibilidad alguna ante el Fluconazol. Sin embargo se requieren más estudios en los cuales se relacione la susceptibilidad “in vitro” con los resultados “in vivo”.<hr/>There are no test standarizaded for the antifungal susceptibility of dermatophytes. The aim of this study is to probe the well diffusion technique as antifungal susceptibility test for dermatophites. One hundred species of dermatophytes were isolated and identified (74 Trichophyton rubrum, 18 Trichophyton mentagrophytes, 1 Trichophyton tonsurans, 5 Microsporum canis and 2 Epidermophyton floccosum).in the Medical Micology Departament of TMI. The antifungal sensitivity of the isolates of the dermatophytes to the drugs was carried out using the well diffusion technique. The antifungal agents tested were griseofulvin, terbinafine, itraconazole and fluconazole. Results were interpreted according to NCCLS breakpoints. The inoculum suspension was standardized by weight measurement. Agar Staib was considered the most suitable, because 97% of the isolates grew on it. This is the first time the inoculum suspension of dermatophytes is standardized by weight measurement. Terbinafine was found to be the most effective drug, followed by Griseofulvin and Itraconazole. Fluconazole were not found to be effective. <![CDATA[<b>Pre-Disposition Factors for the Acquisition of Candida albicans in the Urinary Tract of Adult Patients Hospitalized in the Maracaibo University Hospital over the Period 1999-2001</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0075-52222006000200003&lng=en&nrm=iso&tlng=en Con el fin de estudiar los factores que predisponen al paciente para la adquisición de Candida albicans en el tracto urinario, se realizó una investigación de tipo retrospectiva en el Hospital Universitario de Maracaibo durante el período 1999-2001. Para tal fin se tomó una muestra de 53 pacientes: 28 que mostraron la presencia de Candida albicans (Ca+) y 25 que no la mostraron (control), con una edad promedio de 42 años. Las variables estudiadas fueron sexo, tiempo de hospitalización (TH), ingreso a UCI (IU), uso de sonda vesical (SV), tiempo de colocación de sonda vesical (TCSV), uso de antibióticos (UA), tiempo de uso de antibióticos (TUAB) y número de antibióticos (NAB). Los datos se registraron en un instrumento tipo cuestionario. Los análisis estadísticos de los resultados mostraron que en los pacientes Ca+ las variables: TH, TCSV, TUAB, NAB, IU y sexo femenino fueron más frecuentes. El análisis discriminante de las variables estudiadas permitió establecer relación estadísticamente significativa con las variables TCSV, TUAB y NAB, aunque esta última variable no tiene influencia en el modelo al analizar la ecuación discriminante. La prueba de Ji-Cuadrada indicó que hubo dependencia entre el sexo femenino, TCSV mayor a 10 días, NAB mayor o igual a 2, TUAB mayor a 14 días. La remoción temprana de la sonda vesical y el adecuado uso de antibióticos durante la hospitalización, como medidas de prevención podrían evitar la adquisición de C. albicans en la orina de pacientes hospitalizados.<hr/>For the purpose of studying the predisposition factors in patients in the acquisition of Candida albicans in the urinary tract, retrospective research was undertaken in the University Hospital in Maracaibo from 1999-2001. For such a purpose a sample of 53 patients was taken: 28 showing the presence of Candida albicans (Ca+) and 25 that did not show it (control), with an average overall age of 42. The variables studied were sex, time of hospitalization (TH), permanence in an intensive care unit -UCI- (IU), use of bladder catheter (SV), bladder catheter usage time (TCSV), use of antibiotics (UA), duration of the use of antibiotics (TUAB) and number of antibiotics used (NAB). The data was registered in a questionnaire type instrument. The statistical analyses of the results showed that in Ca+ patients, the variables: TH, TCSV, TUAB, NAB, IU and female (sex), were more frequent than the averages of each one of these variables in the Ca- group. The discriminative analysis for the same variables allowed for the establishment of statistically significant relation with variables TCSV, TUAB and NAB; although the last variable did not have any influence in the model when the discriminative equation was analyzed. The Ji-Square test indicated that there was dependence for the feminine sex, TCSV was greater at 10 days, NAB was greater or equal to 2, TUAB was greater at 14 days. Early removal of the bladder catheter and the adequate use of antibiotics during hospitalization, are practices that help to prevent the acquisition of C. albicans in hospitalized patients. <![CDATA[<b>Antiphenolic Glycolipid-1 Antibodies in Personnel in Health Centers in Contact with Hansen´S Disease Patients</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0075-52222006000200004&lng=en&nrm=iso&tlng=en La lepra es una enfermedad con diferentes formas clínicas, sin embargo, es poco el conocimiento que se tiene sobre portadores de Mycobacterium leprae (M. leprae), así como de su transmisión y los fenómenos inmunológicos que conllevan a la aparición de la enfermedad. El antígeno Glicolípido Fenólico 1 (PGL-1) es un factor de virulencia, específico del M. leprae que protege a la bacteria del ataque de los macrófago. El objetivo principal de la siguiente investigación fue detectar anticuerpos anti PGL-1 en el personal en contacto estrecho con pacientes con lepra, para conocer su grado de exposición al bacilo y su utilidad como factor de riesgo para sufrir la enfermedad. Se seleccionó un grupo de individuos (n: 52) en contacto frecuente con pacientes leprosos y un grupo control (n: 60) sin contacto demostrado. La presencia de anticuerpos séricos IgM anti PGL-1 se determinó por la técnica de ELISA. Del total analizado, se detectaron anticuerpos anti-PGL-1, en 22 muestras (42,3%) del grupo expuesto y 3 (5,0%) en el grupo control (p<0,05). No se observaron diferencias en relación al sexo (45,5% vs. 41,5%). Se encontró un predominio de la seropositividad en el personal entre 36 a 55 años, sin relación a los años de exposición. Se destaca una mayor frecuencia en el personal médico (73,3%) y un riesgo relativo de exposición al bacilo de la Lepra del 1.9 en la población expuesta. Estos resultados demuestran un mayor porcentaje de personas seropositivas al anti-PGL-1 en el personal de centros de salud, relacionado al grado de exposición al bacilo de Hansen, especialmente el personal médico. Hallazgos que sugieren riesgo incrementado de adquirir M. leprae con respecto a la población general. Se recomienda la toma de medidas de bioseguridad, así como realizar un seguimiento serológico de los casos positivos del presente estudio.<hr/>Although leprosy is a disease with a wide range of clinical manifestations, there is a lack of knowledge about the presence of carriers of Mycobacterium leprae (M leprae), as well as its transmission and the immunological phenomena that cause the onset of the illness. The glycolipid phenolic-1 antigen (PGL-1) is a M .leprae specific virulence factor, that protects the bacteria against attack by macrophages. Anti PGL-1 antibodies were detected in health center workers (physicians and paramedics) in close contact with leprosy patients to assess their degree of exposition to the bacilli and to determine their usefulness as a risk factor for developing clinical leprosy. Blood samples from 52 leprosy patients contacts and 60 healthy individuals without leprosy patients contact (control group) were colleted. The presence of IgM anti-PGL-1 antibodies was determined using the ELISA technique. It was found that 42.3% (22) of the contacts and 5.0% (3) of the healthy individuals without leprosy patient contact were positive for anti PGL-1 antibodies (p <0.05). There were no significant differences related to sex among the studied groups (45.5% vs 41.5%). A higher frequency of sero-positive individuals were found in health centers workers of 36 to 55 years of age, especially medical personnel (73.3%), regardless of years of exposition to the same. A relative risk factor of 1.9 was found in the individuals in close contact with leprosy patients. These results show a higher sero-positive level among health centers workers, especially physicians, which is related to their degree of exposition to the Hansen bacilli. These findings suggest that they have an increased risk of developing clinical leprosy in comparison with the general population. It is recommended that proper basic bio-safety measures be taken as well as follow-up of the sero-positive cases found. <![CDATA[<b>Efficacy of Dexamethasone as a Complementary Therapy in Acute Bacterial Meningitis Due to Streptococcus pneumoniae in adults</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0075-52222006000200005&lng=en&nrm=iso&tlng=en La meningitis bacteriana aguda por S. pneumoniae en adultos presenta elevada tasa de morbimortalidad. Los cortiocosteroides no son usados de forma rutinaria, aquí la terapia complementaria con dexametasona se administró a 6 pacientes que recibieron penicilina sódica y cloranfenicol. Los resultados sugieren que mejora la evolución clínica. Se recomienda continuar el estudio con más pacientes.<hr/>Acute bacterial meningitis due to S. pneumoniae in adults has a high morbidity rate. Corticosteroids are not used routinely. Here, complementary therapy with dexamethasona was used in 6 patients who received penicillin plus chloramphenicol. The results showed there was clinical improvement. The continuation of this study with more patients is recommended. <![CDATA[<b>Species of Candida in Oral Cavity in Underfed Children</b>: <b>A Case of Candida dubliniesis</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0075-52222006000200006&lng=en&nrm=iso&tlng=en En este estudio se evaluó la incidencia de las especies de Candida de la cavidad oral sana de una población infantil desnutrida. Diversas especies de Candida fueron encontradas, C. albicans fue aislada en 29.4%, posteriormente se identificó una como C dubliniensis por PCR usando cebadores específicos para esta especie.<hr/>In this study the incidence of the species of Candida in the healthy oral cavities of an infantile population suffering from malnutrition was studied. Diverse species of Candida were described, C. albicans was isolated from 29.4%, one was subsequently identified as C. dubliniensis by PCR using specific primers for this species. <![CDATA[<b>Improvement of the Student Performance in Tropical Medicine with the Problem Based Learning Method in the Universidad del Zulia, Facultad de Medicina, Universidad del Zulia</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0075-52222006000200007&lng=en&nrm=iso&tlng=en El aprendizaje basado en problemas (ABP) es una metodología de enseñanza aprendizaje que se basa en la andragogía y el constructivismo. Comparamos el rendimiento escolar en los cinco períodos semestrales previos al año 2000, con los cinco períodos anuales a partir del 2000. Se demostró estadísticamente una mejoría del rendimiento.<hr/>The problem based learning method (ABP) is an educational method of learning that is based on andragogy and constructivism. Student perfomance in the five semesters prior to the year 2000 were compared with the five annual periods beginning in the year 2000. Improvement of student performance was statistically proven. <![CDATA[<b>Balantidiasis in an Amerindian child from the Sierra de Perijá-Venezuela</b>: <b>A Case Report</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0075-52222006000200008&lng=en&nrm=iso&tlng=en Se reporta el caso de una paciente indígena de la etnia Yucpa de la Sierra de Perijá-Venezuela, con cuadro de diarrea por Balantidium coli. El diagnóstico se realizó mediante el examen directo de las heces. Se le indicó tratamiento con Metronidazol. La evolución fue satisfactoria.<hr/>A case of balantidiasis in an Amerindian Yukpa child with diarrhea due to Balantidium coli was reported in the Sierra de Perijá-Venezuela. The diagnosis was made with direct stool examination. The initial management included Metronidazol, with a satisfactory outcome.
