MÉTODOS: Se practicó la biopsia del ganglio centinela en 130 casos en 129 pacientes, entre 1998 y el 2004, con un promedio de seguimiento de 20 meses. Se usó la técnica combinada con sulfuro coloidal de tecnecio 99 no filtrado y colorante azul patente, ambos inyectados en el área peri-areolar subdérmica. Cada ganglio se seccionó longitudinalmente en fragmentos de 2 mm de espesor. Se realizó estudios de inmunohistoquímica en los ganglios negativos. En los casos en que se diagnosticó metástasis, en un segundo tiempo, se llevó a cabo una disección axilar. RESULTADOS: Obtuvimos un 96,92 % de identificación de los ganglios centinela. En el 63,49 % se identificó un ganglio y en el 36,51 % se detectó más de un ganglio. En 28 casos, se reportó enfermedad metastásica, de estos 19 fueron macrometástasis. El estudio por inmunohistoquímica sólo detectó 1 caso de micrometástasis que no fue observada con hematoxilina y eosina. CONCLUSIONES: La técnica de biopsia del ganglio centinela es eficaz y segura para su identificación. Su estudio en forma minuciosa y fuera del acto quirúrgico de la mama, la hacen altamente recomendable para el diagnóstico de micrometástasis. El estudio por inmunohistoquímica en casos negativos no ofrece beneficio importante.<hr/>OBJECTIVES: To determine the utility of the immunehistochemistry in the detection of micrometastasis in the sentinel node dissection and the necessity to perform an axillary dissection with diagnosis of micrometastasis in the sentinel node. METHODS: We practiced the sentinel node biopsy in 130 cases in 129 patients, between 1998 and the 2004, with a follow-up average of 20 months. We used a combined technique with not filtered sulfide colloidal technetium 99 and colorant blue patent, both injected in subdermal peri areolar area. Each lymphatic node was sectioned longitudinally in fragments of 2 mm of thickness. It was made immunehistochemistry studies in the negative sentinel node. In the cases in that metastasis was diagnosed, in a second time, an axillary dissection was performed. RESULTS: We obtained a 96.92 % of identification of the sentinel nodes. In 63.49 % we only identified one sentinel node and in 36.51 % we identified more than a one sentinel node. In 28 cases, metastasic disease was reported, of which, 19 were macrometastasis disease. The immunehistochemical study only detected one micrometastasis case witch one, no was detected by hematoxylin and eosin stains. CONCLUSIONS: The technique of sentinel node biopsy is effective and safe for its identification. Its study in meticulous form and outside the surgical act of the breast, they highly make it recommendable for the diagnosis of micrometastasis. The study by immunehistochemistry in negative cases does not offer important benefit. <![CDATA[<b>Uso del bisturí armónico</b><b> en la cirugía tiroidea</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822006000400003&lng=en&nrm=iso&tlng=en El propósito de este estudio es investigar la utilidad del uso del bisturí armónico en el tratamiento de la patología tiroidea. Aunado a los avances existentes en los estudios imaginológicos, sobre todo el ultrasonido, ultrasonido Doppler y, la instauración de la punción aspiración con aguja fina en los pacientes con nódulos tiroideos, el bisturí armónico permite no sólo ahorrar tiempo quirúrgico, sino que disminuye significativamente la posibilidad de complicaciones. El uso de este equipo ha evolucionado a la par del desarrollo de la cirugía laparoscópica, que ha permitido el uso de técnicas novedosas el desarrollo de la cirugía mínimamente invasiva. La tiroidectomía efectuada con bisturí armónico es un procedimiento factible, con escasas complicaciones y que permite principalmente ahorrar tiempo quirúrgico, lo cual redundará en los costos finales de la cirugía.<hr/>The purpose of this study is to investigate the utility of the use of the harmonic scalpel in the treatment of the thyroid pathology. Joined to the existent advances in the imaginologic studies, ultrasound and Doppler and setting-up of the fine-needle aspiration in patients with thyroid nodules, the harmonic scalpel only allows not to save surgical time, but rather it diminishes the possibility of complications significantly. The use of this scalpel has evolved at same time of the development of the laparoscopic surgery, which has allowed the use of technical novel the development of the minimally invasive thyroidectomy. The thyroidectomy made with harmonic scalpel is a feasible procedure, with low complications and that allows mainly saving surgical time, that which will redound in the final costs of the surgery. <![CDATA[<b>Tipificación del vph en cáncer de cuello</b><b> uterino en la población venezolana</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822006000400004&lng=en&nrm=iso&tlng=en OBJETIVOS: Realizar la tipificación del virus del papiloma humano en pacientes con cáncer de cuello uterino en la población venezolana. MÉTODOS: Se incluyeron 53 pacientes con diagnóstico de cáncer de cuello uterino entre abril de 2004 y noviembre de 2005. Se realizó tipificación del virus del papiloma humano mediante la reacción en cadena de la polimerasa para los tipos 6, 11, 16, 18, 31, 33 y 35. Se analizaron otras variables como edad, estadio y tipo histológico. RESULTADOS: La edad promedio fue 44,79 años. La mayoría de las pacientes tenían diagnóstico de cáncer localmente avanzado (43,18 % St IIb, 40,90 % St IIIb y 2,27 % St IVa). El tipo histológico predominante fue carcinoma epidermoide en 86,36 %, seguido de adenocarcinoma en 11,36 % y carcinoma adenoescamoso en 2,27 %. Se evidenció la presencia de secuencias de ADN asociados a virus del papiloma humano en 28 pacientes (52,83 %). Se identificaron secuencias de ADN asociadas a virus del papiloma humano 16 en 13 pacientes (24,52 %), a virus del papiloma humano 18 en 4 pacientes (7,54 %), virus del papiloma humano 31 en 3 (5,66 %), virus del papiloma humano 6 en 2 (3,77 %) y virus del papiloma humano 11, virus del papiloma humano 33 y virus del papiloma humano 35 en un caso respectivamente (1,88 %). CONCLUSIONES: Los resultados obtenidos en la tipificación del virus del papiloma humano en nuestras pacientes no reflejan aquellos reportados en otros países y plantean la necesidad de realizar estudios epidemiológicos a gran escala y que incluyan otros tipos de alto riesgo.<hr/>OBJECTIVES: Identify and typify human papilloma virus in patients with cancer of the uterine cervix in the venezuelan population. METHODS: We included 53 patients with diagnosis of cancer of the uterine cervix between April 2004 and November 2005. Polymerase chain reaction was used to typify human papilloma virus types 6, 11, 16, 18, 31, 33 and 35. Other variables such as age, tumor stage and histology were analized. RESULTS: Mean age was 44.79 years. The majority of patients had diagnosis of locally advanced uterine cervix cancer (43.18 % St IIb, 40.90 % St IIIb and 2.27 % St IVa). The predominant histology was epidermoid carcinoma in 83.36 % of cases, followed by adenocarcinoma in 11.36 % and adenosquamous carcinoma in 2.27 %. When human papilloma virus identification and typification was performed using PCR, the presence of DNA sequences associated with human papilloma virus were not found in 28 patients (52.83 %). DNA sequences associaed with human papilloma virus type 16 were identified in 13 patients (24.52 %), with human papilloma virus type 18 in 4 patients (7.54 %), human papilloma virus 31 in 3 (5.66 %), human papilloma virus 6 in 2 (3.77 %) and human papilloma virus 11, human papilloma virus 33 and human papilloma virus 35 in one case respectively (1.88 %). CONCLUSIONS: The results obtained with human papilloma virus identification and typification in our patient population is different from those reported in other countries. Epidemiological studies that include other high risk virus types must be performed. <![CDATA[<b>Detección de mamoglobina y MAGE-A3</b><b> en ganglios linfáticos axilares histológicamente negativos, en cáncer de mama</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822006000400005&lng=en&nrm=iso&tlng=en La detección histológica de metástasis en los ganglios linfáticos axilares es un parámetro de mayor valor pronóstico en cáncer de mama. Un 30 % de pacientes con ganglios linfáticos negativos tienen recaída en cinco años, sugiriendo que, este estudio no es suficiente para detectar todas las metástasis. En el presente estudio, se determinó la expresión de marcadores mamoglobina y MAGE-A3, en ganglios axilares histológicamente negativos de pacientes con cáncer de mama, mediante reacción en cadena polimerasa transcriptasa-reversa, a fin de detectar células tumorales aisladas o micrometástasis. La expresión mamoglobina y MAGE-A3 se detectaron en 55,6 % y 33,3 %, respectivamente. Se amplificó el ARN de mamoglobina y MAGE-A3 en ganglios histológicamente positivos en un 72,7 % y 54,5 %, respectivamente. Se relacionó la presencia o ausencia de células tumorales en ganglios negativos con tamaño tumoral, grado histológico, émbolos neoplásicos vasculares, obteniendo resultados no concluyentes, no se logró distribución equitativa de las muestras. Estos resultados muestran que la mamoglobina y MAGE-A3 son útiles para detección de células tumorales aisladas en ganglios axilares negativos.<hr/>Histological detection of metastasis in axillary lymph nodes is a current practice, and the parameter has greatest prognostic value in breast cancer. 30 % of negative lymph nodes patients at histology studies has disease relapse in five years, suggesting that this method is inadequate to identify all cases with metastasis. In this study, the expression of mammaglobin and MAGE-A3 markers were determined in histological negative axillaries lymph nodes breast cancer patients, by reverse transcriptase polymerase chain reaction. The expression mammaglobin and MAGE-A3 were detected in 55.6 % and 33.3 %, respectively. The ARN of mammaglobin and MAGE-A3 was amplified in patients with histological positive axillaries lymph nodes, obtaining an expression of 72.7 % and 54.5 %, respectively. Finally, presence or absence of tumour cells detected in negative lymph nodes were related to tumour size, histological grade, vascular tumour emboli; non conclusive results were obtaining because it wasn’t possible to obtain an equitable distribution of the samples. These results show that mammaglobin and MAGE-A3 are useful for detection of isolated tumour cells in axilar node negative. <![CDATA[<b>Tumores del esqueleto</b>: <b>20 años de experiencia (1984-2003)</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822006000400006&lng=en&nrm=iso&tlng=en Se presenta la experiencia del Servicio de Partes Blandas y Tumores Óseos del Hospital Oncológico Padre Machado, de Caracas, en el manejo de los tumores del esqueleto, por un período de 20 años (1984-2003). La evaluación de la experiencia fue retrospectiva, con un análisis objetivo estadístico lineal, presentándose el espectro de la patología atendida, y la diversidad de alternativas terapéuticas quirúrgicas utilizadas. Se reporta una incidencia preferencial de esta patología en los miembros inferiores (58,58 %), particularmente alrededor de la rodilla (fémur distal y tibia proximal). El grupo de tumores más frecuentemente observado fue el de formadores de tejido óseo (20,95 %), seguido de los formadores de tejido cartilaginoso (14,79 %), tumor de células gigantes (9,93 %), y las lesiones pseudotumorales (5,85 %). Entre los tumores formadores de tejido óseo llama la atención la alta incidencia de las variedades malignas (90,52 %); mientras que en los formadores de tejido cartilaginoso, las variedades benignas fueron las más frecuentes (69,80 %). Entre los tipos de cirugía realizadas, una proporción importante tuvo finalidad diagnóstica (58,41 %), aunque en muchas de ellas la intención fue simultánea o adicionalmente terapéutica (escisión, drenaje, relleno, etc.). Entre las cirugías con finalidad terapéutica, hay una amplia diversidad de opciones, destacándose, la posibilidad de la práctica de una cirugía preservadora, siempre que se respeten los principios oncológicos quirúrgicos. Se resalta la importancia del manejo de esta patología en centros especializados.<hr/>The experience of the Service of Soft Tissue and Bone Tumors, of the Oncology Hospital Padre Machado, Caracas, in the management of the tumors affecting the skeleton, for a period of 20 years (1984-2003), is presented. The evaluation of this experience was a retrospective revision, with an objective statistics lineal analysis, showing the spectrum of the treated pathology, and the therapeutic surgical alternatives utilized. The report shows a higher incidence of this pathology affecting the lower limbs (58.58 %), particularly around the knee joint (distal femur and proximal tibia). The most frequent type of tumor was the bone forming tumors group (20.95 %), followed by the cartilaginous tissue forming tumors group (14.79 %), giant cell tumors (9.93 %), and pseudotumoral lesions (5.85 %). Among the bone forming tumor group, called our attention the prevalence of the malignant variety (90.52 %); whereas in the cartilaginous tissue forming type, the benign options were seen more frequently (69.80 %). Looking at the performed surgeries, an important proportion had a diagnostic goal (58.41 %), although many of them allowed a therapeutic solution at the same time (resection, drainage, filling defects, etc.). Among the surgical options with a therapeutic goal, we performed a very large variety of them, proposing limb preservation when the surgical oncology principles could be respected. The importance of treating this pathology in specialized centers is underlined. <![CDATA[<b>Pseudotumor inflamatorio de pulmón</b><b> en preescolar femenina</b>: <b>A propósito de un caso</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822006000400007&lng=en&nrm=iso&tlng=en INTRODUCCIÓN: El pseudotumor inflamatorio es una lesión benigna inflamatoria caracterizada en el aspecto histológico por una mezcla de fibroblastos, histiocitos, linfocitos y células plasmáticas. Relativamente rara, con localización tamaño, y sintomatología variable, que puede simular la presencia de un tumor maligno. En la infancia se presenta como una masa intrapulmonar. CASO CLÍNICO: Se describe caso de paciente preescolar femenina de 3 años de edad, quien consultó a nuestro centro por presentar fiebre, disfagia y tos de 3 meses de evolución. Se evidenció en estudios imaginológicos de tórax, imagen densa en base de hemitórax izquierdo que impresionó como empiema tabicado. Se le realizó toracotomía izquierda evidenciándose tumoración intercisural, adherida fuertemente a pared torácica, diafragma y proyectada a mediastino posterior. Además lesiones satélites en pleura parietal. Se toma muestra de lesión principal y resección de las satélites para estudio histopatológico, la cual reportó pseudotumor inflamatorio. DISCUSIÓN: El pseudotumor inflamatorio es una patología tumoral benigna, que se sospecha en niños con infecciones respiratorias a repetición, refractaria a tratamiento convencional y con hallazgos imaginológicos compatibles con lesión ocupante de espacio a nivel pulmonar. Se puede observar su relación con el virus de Epstein-Baar. El diagnóstico es sólo posible por biopsia, y el tratamiento de elección es resección quirúrgica. Sin embargo, en este caso se evidenció aparente mejoría dada por la disminución de la masa tumoral con el tratamiento con antiinflamatorios no esteroideos.<hr/>INTRODUCTION: The pseudo inflammatory tumor is a benign lesion characterized in the histological aspect by a mixture of fibroblasts, histiocytes, lymphocytes and plasmatic cells. Relatively rare, with location, size and variable simptomatology that it can simulate the presence of a malignant tumor. In the childhood it appears like an intrapulmonary mass. CLINICAL CASE: A case of female infant patient of 3 years old is described, that consult to our center to display fever, dysphagia and cough of 3 months of evolution. In radiological images of thorax were viewed a dense image in the left thorax base that impresses septate emphyema. A left thoracotomy was made viewed an intercysural tumor like, adhered strongly to thoracic wall, diaphragm and projected to later mediastinum, and in addition, satellites injuries in parietal pleural. An incisional sample of main lesion was taking and complete resection of the satellites to histopathology study, which reported pseudo inflammatory tumor. DISCUSION: Pseudo inflammatory tumor is a benign tumoral pathology that is suspected in children with repetition respiratory infection refracted to conventional treatment and with image findings compatible with tumor at pulmonary level. Its relation with the virus of Epstein-Baar can be observed. The diagnosis is only possible by biopsy, and the election treatment is the surgical resection. Although in this case the improvement was viewed by diminution of tumor mass with use of non steroidal anti inflammatory drugs. <![CDATA[<b>Tumor neuroectodérmico primitivo</b><b> de maxilar</b>: <b>Presentación de un caso</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822006000400008&lng=en&nrm=iso&tlng=en El tumor neuroectodérmico primitivo periférico forma parte de la familia de tumores sarcoma de Ewing representando el 5 % de los mismos, en maxilar superior es infrecuente. Se reporta este caso clínico de un adolescente masculino, de 15 años referido a nuestro centro con pérdida de peso, tumoración en cuadrante superior izquierdo de cavidad bucal, de 6 cm de diámetro, que ocupa el 50 % de la misma, sobre infectada, con áreas de necrosis. La biopsia reportó: tumor de células redondas indiferenciadas. Se observó rápido crecimiento tumoral que triplica su tamaño en 8 días, protruyendo por cavidad oral, la ocupa en totalidad, tendiente a obstrucción de la vía aérea; ameritó traqueotomía y gastrostomía de emergencia. Los estudios de inmunohistoquímica concluyen: tumor neuroectodérmico primitivo (inmunorreactivo a CD99, enolasa neuroespecífica y cromogranina), recibe dos ciclos de ciclofosfamida, vincristina, actinomicina D, ifosfamida y etoposido, con progresión de la enfermedad y deterioro del estado físico, por lo que recibe radioterapia 6 000 cGy con cisplatino, lográndose condiciones favorables a la cirugía: Maxilectomía bilateral con preservación del piso de la órbita. El estudio anatomopatológico reportó tejido fibromixoide con abundante vascularización sin evidencia de malignidad, márgenes libres. Actualmente, 7 meses sin evidencia de enfermedad en espera de prótesis palatina para la restitución de deglución funcional. En localizaciones infrecuentes debe el tratamiento ser multidisciplinario e individualizado, incluyendo las tres modalidades terapéuticas (quimioterapia, radioterapia y cirugía).<hr/>Periferical neroectodermic primary tumor is a part of the sarcoma Ewing family and represents 5 % of them. The superior maxillary location is not frequent, we report these clinical case of a male teenage of 15 years old sent to our hospital with loss weigh, tumor in the oral cavity in the left superior quadrant of the oral cavity, with a diameter of 6 cm, that it occupies 50 % of the same one with sobreinfection, with necrosis. The biopsy reported: tumor of undifferentiated round cells. We observed a growth of the tumor that triples its size in 8 days, it occupies it in totality, with tendency to obstruction of the air way, needed emergency traqueostomy and gastrostomy. The inmunohistoquimical studies reveal primary neuroectodermic tumor (inmunoreactive to CD99 enolasa neurospecific and cromagine), received two cycles of ciclofosfamide, vincristine, actinomicyn D, ifosfamida and etoposid, with disease progression and depletory of his physical status; receive radiation therapy 6 000 cGy with platinum, obtaining favorable condition to surgery: A bilateral maxilectomy with preservation of the floor of the orbit. The anatomopathological study report fibromixoide weave a lot of vascularization without evidence of malignity, free margins. Actually, 7 months without evidence of disease waiting for platinum prosthesis to restituted the functional deglution. In the inusual localizations, the treatment has to be multidisciplinary, individualized, and include the three treatment modalities (Quemotherapy, radiation therapy and surgery). <![CDATA[<b>Carcinoma epidermoide en condilomatosis vulvoperineal gigante</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822006000400009&lng=en&nrm=iso&tlng=en La condilomatosis vulvoperineal es una entidad muy frecuente en mujeres en edad reproductiva; recomendándose realizar la tipificación del virus papiloma humano, dada la relación de algunos subtipos con el carcinoma epidermoide a nivel ginecológico. Se presenta el caso de una paciente femenina de 39 años que consultó por presentar una lesión exofítica, verrugosa que ocupa labios mayores y menores de la vulva, clítoris, introito vaginal y región perianal; de la cual se toma biopsia incisional reportando papilomatosis por virus papiloma humano, sin evidencia de atipias. Se lleva a quirófano realizando dermovulvectomía, cuya biopsia definitiva reportó carcinoma epidermoide invasor bien diferenciado, focal, con bordes resección libres; se completa con disección inguinal bilateral superficial y profunda sin evidencia de lesión metastásica ganglionar. Se realizó tipificación viral por reacción de cadena de polimerasa resultando VPH 16. A 20 meses de seguimiento, no hay evidencia clínica ni citológica de lesión recurrente. Es importante realizar la tipificación viral de estas lesiones por su relación con el carcinoma epidermoide de la vulva. Se realizó una revisión de la literatura.<hr/>The vulvoperineal condylomatosis it’s a very frequent disease in reproductive age young women; it’s recommended to realize the viral typification, because the relation between some human papillomavirus subtypes with gynaecology epidermoid cancer. We present the case of a 39 years old female patient who consulted to present a exofitic injury, warty that occupies the majora and minora labia of the vulva, clitoris, vaginal vestibule and perianal region; from which incisional biopsy was taken, reporting papilomatosis by human papillomavirus, without evidence of atypias. Patient went to operating room to receive a dermovulvectomy, whose definitive biopsy reported invasive epidermoid carcinoma well differentiated, focal, with free resection margins; it is completed with superficial and deep bilateral inguinal dissection without evidence of metastasic nodal involment. Viral tipificación by reaction of polymerase chain reaction was made being VPH 16. To 20 months of fall up, there are not clinical or cytological evidence of recurrent lesions. It is important to make the viral tipificación of these lesions by its relation with the epidermoid carcinoma of the vulva. A revision of literature was made. <![CDATA[<b>Linfoma T primario colónico</b>: <b>Reporte de un caso y revisión de la literatura</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822006000400010&lng=en&nrm=iso&tlng=en Los linfomas del tracto gastrointestinal son el tipo más frecuente de linfomas extraganglionares y de estos el 15 % a 20 % son de localización intestinal. Los linfomas colónicos primarios son extremadamente raros, comprenden 0,2 % a 0,6 % de todas las neoplasias malignas de colon. Pueden ser B o T. Los tipo T son menos comunes y pueden estar o no asociados a enteropatía celiaca, enfermedad de Crohn y/o SIDA, siendo más frecuente en hombres adultos en una proporción de 2:1. Son de evolución lenta, clínicamente debutan con dolor abdominal asociado a cuadros obstructivos, seguidos de diarrea, hemorragia, perforación, o masa abdominal. El tratamiento es controvertido, en vista de que solamente el 30 % de los pacientes que van a cirugía tienen la firme presunción de linfoma. Presentamos el caso de un paciente masculino de 62 años, quien consultó por dolor abdominal en hipocondrio izquierdo de 30 días de evolución asociado a anorexia, náuseas, vómitos y melena. Los estudios de extensión gastrointestinales reportaron dentro de lo normal, en la tomografía axial computarizada abdominal se evidenció tumoración en hipocondrio izquierdo que comprimía extrínsecamente al colon descendente; siendo intervenido quirúrgicamente hallándose una gran adherencia entre epiplón mayor, peritoneo, mesocolon, ángulo esplénico, yeyuno y mesenterio, en el cual, se evidenció tumoración de 15 cm de diámetro. Se realizó una colectomía parcial y resección intestinal más anastomosis T-T, con buena evolución posoperatoria.<hr/>Lymphomas of the gastrointestinal tract are the most frequent type of primary extranodal lymphomas and of these, the 15 % to 20 % are of intestinal location. The primary colonic lymphomas is extremely rare, they include 0.2 % to 0.6 % of all the colonic malignant neoplasm. They can be B or T. Intestinal T-cell lymphomas are much less common and can be or no associate to enteropathy celiac, Crohn’s disease and/or AIDS. They are more frequent in adult men in a proportion of 2:1. They are of slow evolution, clinically they make debut with abdominal pain associate to obstructive squares, followed by diarrhea, hemorrhages, perforation or abdominal mass. The treatment is controverted, in view of which only 30 % of the patients who go to surgery have the firm presumption of lymphoma. We present the case of a 62 year-old male patient who consulted by abdominal pain in left hypochondriac of 30 days of evolution associated to anorexia, sick feeling, vomits and manes. The gastrointestinal extension studies reported normal; the Computerized Axial Tomography showed a tumor in left hypochondriac that compressed extrinsically the descending colon. The patient was under surgery finding in a great adherence among great omentun, peritoneum, mesocolon, splenic flexure, jejunum and mesentery surgically, a 15 cm diameter tumor in jejune and great omentun. Los linfomas colónicos primarios son extremadamente raros, comprenden 0,2 % a 0,6 % de todas las neoplasias malignas de colon. Pueden ser B o T. Los tipo T son menos comunes y pueden estar o no asociados a enteropatía celiaca, enfermedad de Crohn y/o SIDA, siendo más frecuente en hombres adultos en una proporción de 2:1. Son de evolución lenta, clínicamente debutan con dolor abdominal asociado a cuadros obstructivos, seguidos de diarrea, hemorragia, perforación, o masa abdominal. El tratamiento es controvertido, en vista de que solamente el 30 % de los pacientes que van a cirugía tienen la firme presunción de linfoma. Presentamos el caso de un paciente masculino de 62 años, quien consultó por dolor abdominal en hipocondrio izquierdo de 30 días de evolución asociado a anorexia, náuseas, vómitos y melena. Los estudios de extensión gastrointestinales reportaron dentro de lo normal, en la tomografía axial computarizada abdominal se evidenció tumoración en hipocondrio izquierdo que comprimía extrínsecamente al colon descendente; siendo intervenido quirúrgicamente hallándose una gran adherencia entre epiplón mayor, peritoneo, mesocolon, ángulo esplénico, yeyuno y mesenterio, en el cual, se evidenció tumoración de 15 cm de diámetro.
