La genética molecular, los ha estratificado en grupos diferentes a la clasificación morfológica. Trabajos respaldan que un grupo específico: carcinoma de “tipo basal” tiene peor pronóstico, es frecuentemente triple negativo, es decir: RE (-), RP (-), C-erbB-2(-) expresa citoqueratinas de epitelios basales, como la citoqueratina 5/6 (CK5/6, planteamos determinar los aspectos morfológicos de estos carcinomas, identificar la frecuencia en la que expresan CK5/6 por inmunohistoquímica, relacionarlos con parámetros clínicos disponibles. Se seleccionaron 91 carcinomas entre 2003 - 2008 se les realizó CK5/6. 34 (37,4 %) fueron TN-CK5/6(+) y 57 (62,6 %) fueron TN-CK5/6(-). Las características morfológicas descritas se observaron en ambos grupos, más frecuentemente en los carcinomas TN-CK5/6(+). La detección de estos tumores por inmunohistoquímica es una forma práctica, rentable de identificar a este grupo de peor pronóstico en la práctica diaria.<hr/>Breast cancer is heterogeneous disease classification parameters prognostic factors are not always predictors its clinical course. Molecular genetics are categorizing in different ways, as the well-known morphologic classification. Studies endorse specific group within this classifying system: Basal-like carcinomas, has worst prognosis; tends to be triple-negative: ER (-), RP (-), Erb-B2(-); expresses basal cytokeratins, such as cytokeratin 5/6 (CK5/6). Considered determining morphological aspects, frequency in which they express CK5/6 through immunohistochemistry, and contrasting them to existing clinical parameters. 91 carcinomas were selected between the years 2003 - 2008 and checked for CK5/6. Or the aforementioned cases, 34 (37.4 %) resulted TN-CK5/6(+) and 57 (62.6 %) resulted TN-CK5/6(-). Morphological aspects described for and carcinomas were observed in both groups, though they were more frequent in breast carcinomas TN-CK5/6(+). For this reason, detecting these tumors using immunohistochemistry is a practical, worthwhile, and convenient way to identify this group with the worst prognosis on our daily routines. <![CDATA[<b>Sarcoma de Kaposi asociado a VIH</b>: <b>Uso de doxorrubina loposomal terapia antirretroviral de alta eficicencia</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822011000100003&lng=es&nrm=iso&tlng=es No existen criterios definidos para el tratamiento del sarcoma de Kaposi asociado a infección por VIH en la era de terapia antirretroviral de alta eficacia. Hay evidencias de regresión de lesiones cutáneas solamente con terapia antirretroviral sin quimioterapia; es muy poco probable que lesiones viscerales y mucosas respondan sin administración simultánea de quimioterapia. OBJETIVO: Estudio prospectivo en pacientes Kaposi de “bajo riesgo” y “alto riesgo”, tratando de definir el momento oportuno de quimioterapia simultáneamente con administración de terapia antirretroviral. MÉTODO: 66 pacientes 28 (42 %) T0I0S0, 6 (9 %) T0I0S1, 8 (12 %) T0I1S1, 5 (8 %) T1I1S0 y 19 (29 %) T1I1S1. Todos los pacientes T1, T0I1S1 y un tercio de T0I0S1 fueron tratados con quimioterapia (4 a 8 ciclos con doxorrubicina liposomal) ninguno de los pacientes T0I0S0 recibió quimioterapia. RESULTADOS: Total de 34 respuestas clínicas, 6 (18 %) completas (3 T1I1S1, 2 T1I1S0, y 1 T0I0S1), 23 (68 %) parciales (16 T1I1S1, 3 T1I1S0, 3 T0I1S1 y 1 T0I0S1) y 5 (14 %) estabilizaron enfermedad (T0I1S1). CONCLUSIONES: Pacientes con tumor extenso (T1) requieren quimioterapia desde el diagnóstico, y administración de antirretrovirales. Pacientes con tumores localizados, con nivel células CD4 ≤ 200 /μL (I1), síntomas sistémicos requieren quimioterapia aproximadamente 2 a 3 meses después del inicio terapia antirretrovitral, ninguno de los pacientes con tumores localizados, sin síntomas sistémicos, y nivel células CD4 > 200 /μL (I0) van a requerir quimioterapia, al menos durante un promedio de 20 meses.<hr/>At present there are no defined criteria for treating Kaposi´s sarcoma associated with HIV in the era of highly active antiretroviral therapy. Evidence of regression of cutaneous lesions with only retroviral high efficacy therapy without need of chemotherapy; nevertheless, it is very unlikely that visceral or mucous lesions respond to retroviral without simultaneous initiation chemotherapy. OBJECTIVE: Prospective study in “low risk” and “high risk” Kaposi patients, trying to define the adequate moment for initiating the chemotherapy simultaneously with retroviral. METHOD: 66 male Kaposi patients, 28(42 %) T0I0S0, 6 (9 %) T0I0S1, 8 (12 %) T0I1S1, 5 (8 %) T1I1S0 y 19 (29 %) T1I1S1. All T1, T0I1S1 and a third of T0I0S1 patients were treated with chemotherapy (4 to 8 cycles with liposomal doxorubicin) but none of the T0I0S0 patients. RESULTS: 34 clinical responses, 6 (18) complete (3 T1I1S1, 2 T1I1S0, and 1 T0I1S1), 23 (68 %) partial (16 T1I1S1, 3 T1I1S0, 3 T0I1S1, and 1 T0I0S1) and 5 (14 %) stable disease (T0I1S1). CONCLUSIONS: All patients with extensive tumors require chemotherapy administration since the moment of diagnosis, together with retroviral high efficacy agents. Patients with localized tumors, CD4 cell levels ≤ 200 /μL), and systemic symptoms will require chemotherapy in approximately 2 to 3 months period after initiation of retroviral, while none of patients with localized tumors, without systemic symptoms, and CD4 cell levels > 200 /μL will require chemotherapy, at least in a mean 20 months follow-up period. <![CDATA[<b>Suplementación con antioxidantes no previene nefrotoxicidad por cisplatino en pacientes con cáncer cérvico uterino avanzado</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822011000100004&lng=es&nrm=iso&tlng=es OBJETIVO: En México el cáncer cérvico uterino al igual que en otros países de América representa un grave problema de salud pública. El tratamiento depende de su extensión; los estadios localmente avanzados son tratados con una combinación de quimioterapia con cisplatino y radioterapia. Ambas terapias utilizadas son consideradas oxidativas y por ello son capaces de influir en las toxicidades del propio tratamiento, el objetivo de este trabajo es determinar la eficacia de la suplementación con antioxidantes y su efecto sobre la prevención de la toxicidad renal por cisplatino. MÉTODOS: Ensayo clínico aleatorizado que incluyó a pacientes con cáncer cérvico uterino en estadios localmente avanzados cuyo tratamiento antineoplásico consistió en radioterapia y quimioterapia con cisplatino. Se asignó aleatoriamente a las pacientes a recibir un suplemento antioxidante diariamente o bien un placebo. Se determinó la función renal mediante la depuración de creatinina antes de iniciar el tratamiento y al término del mismo. Se realizaron pruebas t-Student inter e intra grupales a fin de determinar el efecto de la suplementación sobre los parámetros evaluados. RESULTADOS: No se encontraron diferencias estadísticamente significativas entre ambos grupos; en cambio, existió una disminución significativa en ambos grupos al finalizar el tratamiento. CONCLUSIONES: La suplementación con antioxidantes no es capaz de prevenir la toxicidad a nivel renal producida por la quimioterapia con cisplatino.<hr/>OBJECTIVE: In Mexico, our country, the pathology of cervical cancer is a major public health issue, the same situation is present in other American countries. The treatment for this pathology depends on its extension; for locally advanced stages, a combination of radiotherapy and chemotherapy with cisplatin drug is common used. The both therapies are considered to be pro oxidative and this can be implied in the toxicities of the treatment. The objective of this work was to determine the efficacy of antioxidant supplementation on the prevention of cisplatin drug in the renal toxicity. METHODS: We conducted a randomized clinical trial in the patients with locally advanced stage cervical cancer whose antineoplastic treatment consisted in radiation therapy and chemotherapy with the cisplatin drug. The patients were randomly assigned to receive either an antioxidant supplement or the placebo. We assessed renal function as creatinine clearance before and after concluding the oncologic treatment. We performed inter and intragroupal t-Student tests in order to determine the effect of the antioxidant supplementation on the evaluated parameters. RESULTS: No statistically significant differences we were found between the groups; however, there was a significant decrease in renal function in the both groups after finalizing the oncologic treatment. CONCLUSIONS: The antioxidant supplementation does not prevent the renal toxicity from the cisplatin drug chemotherapy. <![CDATA[<b>Experiencia de 15 años en el manejo del cáncer de pene</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822011000100005&lng=es&nrm=iso&tlng=es OBJETIVO: Conocer características clínicas, factores pronósticos, tratamiento quirúrgico y sobrevida en pacientes con cáncer de pene en el Instituto de Oncología Dr. Miguel Pérez Carreño. MÉTODO: Estudio descriptivo, transversal, en 50 de 65 pacientes con cáncer de pene en nuestro hospital entre 1995-2009. RESULTADOS: Obtuvimos 11 pacientes evaluados entre 1995-1998,12 entre 1999-2002,15 de 2003 a 2006 y 12 de 2007 a 2009. 32 % fueron < de 50 años, 26 % entre 51-60, 12 % entre 61-70 y 9 % > de 71 años. 80 % fue carcinoma epidermoide, 2 % epidermoide con patrón sarcomatoso y 18 % verrugosos. El 66 % grado histológico GI, 32 % GII y 2 % GIII. La falectomía parcial se practicó en 44 %, esta más linfadenectomía bilateral en 18 %, emasculación en 16 %. El estadio patológico fue de 36 % para I y III 20 % para estadio IV y 8 % estadio II. La realización de vaciamientos inguinales en su mayoría fue en estadios avanzados con un 29,6 % en pT3 G1. La sobrevida global en estadio I fue 88,8 % al primer año y 50 % a 5 años, estadios IV en un año 70 % y cero en 5 años. CONCLUSIÓN: El pronóstico depende del estadio patológico, afectación ganglionar, grado histológico, los pacientes pT3 y pT4 independientemente del grado histológico tienen peor pronóstico así se describe en la literatura, la linfadenectomía es fundamental en el manejo de estos casos.<hr/>OBJECTIVES: To determine clinical characteristics, prognostic factors, surgical treatment and survival in patients with penile cancer at the Institute of Oncology Dr. Miguel Perez Carreño. METHOD: Descriptive, transversal, in 50 of 65 patients with penile cancer. RESULTS: 11 patients evaluated in the years 1995 to 1998. 12 from 1999 to 2002, 15 patients from 2003 to 2006, and 12 from 2007 to 2009. 32 % were &lt; 50 years, 26 % between 51- 60, 12 % between 61- 70 and 9 % > 71 years. 80 % were squamous cell carcinoma, 2 % with a sarcomata’s squamous and 18.% warty. 66 % was in GI histological grade, 32.% and 2 % GII GIII. The partial penectomy were performed in 44 %, bilateral lymphadenectomy in 18 %, 16 % emasculation. The pathological stage was 36 % for I and III 20 % for stage IV and 8 % stage II. The performance of inguinal dissection was mostly in advanced stages with 29.6 % in pT3 G1. Overall survival of stage I patients was 88.8 % in the first year and 50 % at 5 years, while stage IV in 70.% and zero in five years. CONCLUSION: Penile cancer prognosis depends on the pathological stage, lymph node involvement and histological grade, pT3 and pT4 patients regardless of histological grade are the ones who have a worse prognosis as described in the literature, therefore, is central lymphadenectomy handling these cases. <![CDATA[<b>Anemia hemolítica autoinmune como manifestación inicial de linfoma hodgkin</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822011000100006&lng=es&nrm=iso&tlng=es Se presenta el caso de una paciente de 66 años de edad, a la que se le diagnostica anemia hemolítica, la cual fue refractaria al tratamiento y requirió esplenectomía. Además presenta adenomegalias inguinales, cuya biopsia determina infiltración parcial por células linfoides B CD20+, con atipia, y CD30+, con factor de proliferación alto; en médula ósea se constata incremento de linfocitos T. Cuatro meses después, consulta por la aparición de adenopatías inguinales y axilares, de las cuales la biopsia reveló enfermedad de Hodgkin variante esclerosis nodular, y en médula ósea se evidenció infiltración por la enfermedad linfoproliferativa. Si bien es infrecuente la asociación entre anemia hemolítica y linfoma Hodgkin, debe tenerse en cuenta para llegar a la búsqueda oportuna de su causa y al diagnóstico de un probable proceso linfoproliferativo subyacente.<hr/>In this work we report and study a case of 66 year old woman, whit diagnosis of hemolytic anemia, which was refractory to treatment and she required splenectomy. The patient presented inguinal lymphadenopathy which biopsy has determined a partial infiltration of B-cells CD20 + and CD30 + with atypia and high growth factor. The bone marrow biopsy informed an increased number of T lymphocytes. Four month later, the patient complained due to the appearance of inguinal and axillaries lymph nodes, which biopsy revealed a nodular sclerosis Hodgkin lymphoma. The bone marrow biopsy showed infiltration by lymphoproliferative disease. Although the association between hemolytic anemia and Hodgkin lymphoma is less frequent, this fact should be taken into account in searching its cause and reaching the diagnosis of a probable underlying lymphoproliferative process. <![CDATA[<b>Linfoma no Hodgkin secundario a inmunosupresión por transplate renal tratado con quimioterapia</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822011000100007&lng=es&nrm=iso&tlng=es OBJETIVO: Los síndromes linfoproliferativos posterior a trasplante renal incluyen un grupo de enfermedades linfoides que por definición se presentan después del mismo en un órgano sólido o en la médula ósea y están en relación con la terapia inmunosupresora administrada. La probabilidad de desarrollar una neoplasia maligna en un receptor de trasplante renal seguido a lo largo de 17 años, es de un 14 % para cánceres no cutáneos, de un 47 % en los carcinomas de piel y un 55 % para cualquier tipo de cáncer. MÉTODO: En este trabajo se describe un caso poco común de un paciente de 40 años con insuficiencia renal crónica secundario a hipertensión arterial en hemodiálisis, quien se le realizó trasplante renal de cadáver en el 2007, presentando alteración de función renal con masa palpable en fosa ilíaca derecha, en tratamiento con inmunosupresores. RESULTADOS: El examen histopatológico de la biopsia reportó: desorden linfoproliferativo posterior a trasplante con inmunohistoquímica positivo para CD20, CD79 en células linfoides B y CD45 en células linfoides T. Estudios de extensión metástasis en mediastino concluyéndose como: linfoma no Hodgkin inmunofenotipo B CD20+ estadio IV extra nodal renal con metástasis en mediastino superior mayo/2009 IPI score alto riesgo secundario a trasplante renal. Recibió tratamiento con R-CHOP obteniéndose respuesta oncológica completa, demostrado por CT-PET, actualmente libre de enfermedad, en controles nefrológicos. CONCLUSIÓN: Se debe vigilar cualquier alteración clínica, ya que un rechazo de trasplante se puede confundir con una patología oncológica.<hr/>OBJECTIVE: The posterior transplant lymph syndrome proliferative disorders include a group of renal lymphoid diseases by definition are present after a solid organ transplant or in bone marrow and are related to immunosuppressive therapy administered. The likelihood of developing a malignancy renal transplant recipient followed over the 17 years is 14 % for the non-skin cancers, 47 % in the carcinomas of the skin and 55 % for any type of cancer. METHOD: In this work we describes a rare case of a 40 year old patient with chronic renal failure secondary to arterial hypertension in hemodialysis, who underwent cadaveric renal transplant in 2007, the patient presented impaired renal function with palpable mass in right iliac fosse, in treatment with immune suppressants. RESULTS: The histopathology examination of the biopsy reported, posterior transplant with lymphoproliferative disorder with immunohistochemistry positive for CD20, CD79 on B lymphoid cells and CD45 on T lymphoid cells, the extension studies concluding that the mediastinal metastases were: Non-Hodgkin’s lymphoma immunophenotyping B extranodal CD20 + stage IV renal with metastases in the superior mediastinum IPI score May/2009 classified how secondary to high risk renal transplantation. The patient was treated with R-CHOP we obtained a oncology complete response, as demonstrated by CT-PET, and currently free of disease, in nephrology controls. CONCLUSION: It should monitor any clinical manifestation in the treatment of this kind of patients and served as a rejection of transplant can be confused with metastatic brain tumor. <![CDATA[<b>Incidentaloma en evaluación pretrasplante de donante vivo</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822011000100008&lng=es&nrm=iso&tlng=es OBJETIVO: Los incidentalomas adrenales, oscilan alrededor del 2 % - 13 %. Afecta a ambos sexos, con discreta preponderancia femenina, especialmente hacia el final de la tercera y cuarta décadas. La etiología incluye: tumores benignos y malignos de todas las zonas de la corteza y de médula suprarrenal, las metástasis a las glándulas suprarrenales, así como enfermedades infiltrativas. MÉTODO: Paciente de sexo femenino, 47 años de edad, asintomática, sin antecedentes patológicos conocidos, en evaluación preoperatoria para trasplante renal donante vivo relacionado, la angiografía por tomografía evidencia tumoración en glándula suprarrenal izquierda. Se concluye evaluación pretrasplante sin ninguna contraindicación por lo cual se decide realizar adrenalectomía laparoscópica, biopsia extemporánea y posteriormente nefrectomía laparoscópica para trasplante renal donante vivo relacionado. RESULTADOS: Se extrae glándula suprarrenal izquierda de 4 cm x 3 cm x 2 cm de consistencia firme, al corte se observan áreas hemorrágicas y de necrosis. En vista de no ser concluyente el informe de la biopsia extemporánea se difiere nefrectomía. El resultado definitivo de la biopsia reportó feocromocitoma. DISCUSIÓN: El 5 % de los incidentalomas son feocromocitomas y el 25 % de los feocromocitomas son hallazgos de algún estudio. La clínica puede ser variada, y no guarda una clara relación con el tamaño, localización o aspecto histológico del tumor. Los síntomas más comunes son: cefalea, diaforesis y palpitaciones. También pueden tener poca expresividad clínica o ser asintomáticos, circunstancia que ocurre en un 20 % de los casos aproximadamente.<hr/>OBJECTIVE: The adrenal carcinoma accounts for 0.02.% of cancers. In adrenal incidentalomas, the figures range around 2 % - 13 %. It affects both sexes, with a slight female preponderance, especially towards to the end of the third and the fourth decades. The etiology of incidentalomas includes the benign and the malignant tumors of all parts of the cortex and medulla, the adrenal metastasis, and the infiltrative diseases. METHOD: Describe a female patient, 47 years old, asymptomatic, with no known pathological conditions in preoperative evaluation for living-related donor kidney transplantation, evidenced angio tomography left adrenal gland tumor. Pre transplant evaluation is concluded with no contraindication for which it was decided to perform laparoscopic adrenelectomy, extemporaneous biopsy and then laparoscopic nephrectomy for livingrelated donor renal transplantation. The left adrenal gland is removed from 4 cm x 3 cm x 2 cm firm consistence, the court observed hemorrhagic areas and necrosis. In view of the report not be conclusive of extemporaneous biopsy is deferred nephrectomy. The final results of the biopsy reported pheochromocytoma. DISCUSION: The 5 % of the incidentalomas are pheochromocytomas and in 25 % of pheochromocytomas are findings of a study. The clinic can be varied, and bear no clear relation to the size, location or histology of the tumor. The most common symptoms are: Headache, diaphoresis and palpitations. They may also have little clinical expression or be asymptomatic, a circumstance that occurs in 20 % of all cases. <![CDATA[<b>Tratamiento preservador del cáncer de la mama</b>: <b>Dos años de experiencia</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822011000100009&lng=es&nrm=iso&tlng=es OBJETIVO: Experiencia de nuestra institución en tratamiento preservador del cáncer de mama, analizando cirugía preservadora en términos de recidiva local, sobrevida global y libre de enfermedad. MÉTODOS: Estudio descriptivo, retrospectivo durante 2004-2005, en pacientes sometidos a tratamiento preservador: la realización de mastectomía parcial oncológica con márgenes negativos, evaluación quirúrgica de axila mediante técnica del ganglio centinela o disección axilar, el tratamiento adyuvante con radioterapia, y quimioterapia en los casos que tuviesen indicación. RESULTADOS: 132 cirugías conservadoras. Edad media 51,49 años, Tipo histológico más frecuente fue carcinoma ductal infiltrante (70,06 %), (85,94 %) fueron estadio I y II. La cirugía que más se realizó fue la mastectomía parcial oncológica más disección axilar (80 pacientes, 61,87 %). 10,6 % recibieron neoadyuvancia con quimioterapia. El promedio de ganglios obtenidos fue de 11,37, el 62,6 % estado patológico axilar 0. 50 % expresaron receptores de estrógeno y progestágenos positivos el 13,17 % sobre expresaron el Her2neu. 66,66 % recibió quimioterapia adyuvante, 79,84 % radioterapia adyuvante. 56,58 % terapia hormonal adyuvante, seguimiento 39,89 meses promedio. 88,88 % vivas libres de enfermedad, 2,22.% vivas con recaída local, 5,92 % viva con metástasis, 1,48.% viva con recaída local y metastásica 1,07 % muerta por enfermedad. El intervalo libre de enfermedad promedio fue de 25,8 meses y la sobrevida global a los 5 años de 98,24 %. CONCLUSIONES: El tratamiento preservador es una terapia primaria efectiva en casos de cáncer de mama estadio I y II.<hr/>OBJECTIVE: Presented the experience in our institution in the conservative treatment of breast cancer, we analyzed the surgery, local recurrence, and the global super life, and the disease free survival. METHOD: These are a descriptive and retrospective study during 2004-2005 in patients underwent conservative treatment: Realize of partial oncology mastectomy with negative margins surgical evaluation of axillaries sentinel node technique or axillaries dissection. The adjuvant treatment with radiation therapy and chemotherapy when there are indicated in the patients. RESULTS: We realize 132 conservative surgery, median ages of patients were 51.49 histological type more frequent was ductal infiltrante carcinoma (70.06.%), (85.94 %) were stage I and II. The surgical more frequent were partial oncology mastectomy with axillaries dissection (80 patients, 61.87 %).10.6 % received adjuvant chemotherapy. The average of finding nodes was 11.37, the 62.6 % pathological axillaries state were 0.50.% expression positive estrogen and progestagen receptors 13.17 % super expression of Her2neu. 66.66 % received adjuvant chemotherapy, 79.84.% adjuvant radiation therapy 56.58 % adjuvant hormonal therapy. Average vigilance was 39.89 months. 88.88 % alive free disease, 2.22 % alive with local recurrence, and 5.92 % alive with metastases. 1.48 % alive with local and metastasis recurrence 1.07 % dead for disease. The free interval of disease was 25.8 month and the global super life to 5 years 98.24 %. CONCLUSIONS: The conservative treatment is a primary and effective in breast cancer therapy for stage I and II. <![CDATA[<b>Conocer la sociedad</b>]]> http://ve.scielo.org/scielo.php?script=sci_arttext&pid=S0798-05822011000100010&lng=es&nrm=iso&tlng=es OBJETIVO: Experiencia de nuestra institución en tratamiento preservador del cáncer de mama, analizando cirugía preservadora en términos de recidiva local, sobrevida global y libre de enfermedad. MÉTODOS: Estudio descriptivo, retrospectivo durante 2004-2005, en pacientes sometidos a tratamiento preservador: la realización de mastectomía parcial oncológica con márgenes negativos, evaluación quirúrgica de axila mediante técnica del ganglio centinela o disección axilar, el tratamiento adyuvante con radioterapia, y quimioterapia en los casos que tuviesen indicación. RESULTADOS: 132 cirugías conservadoras. Edad media 51,49 años, Tipo histológico más frecuente fue carcinoma ductal infiltrante (70,06 %), (85,94 %) fueron estadio I y II. La cirugía que más se realizó fue la mastectomía parcial oncológica más disección axilar (80 pacientes, 61,87 %). 10,6 % recibieron neoadyuvancia con quimioterapia. El promedio de ganglios obtenidos fue de 11,37, el 62,6 % estado patológico axilar 0. 50 % expresaron receptores de estrógeno y progestágenos positivos el 13,17 % sobre expresaron el Her2neu. 66,66 % recibió quimioterapia adyuvante, 79,84 % radioterapia adyuvante. 56,58 % terapia hormonal adyuvante, seguimiento 39,89 meses promedio. 88,88 % vivas libres de enfermedad, 2,22.% vivas con recaída local, 5,92 % viva con metástasis, 1,48.% viva con recaída local y metastásica 1,07 % muerta por enfermedad. El intervalo libre de enfermedad promedio fue de 25,8 meses y la sobrevida global a los 5 años de 98,24 %. CONCLUSIONES: El tratamiento preservador es una terapia primaria efectiva en casos de cáncer de mama estadio I y II.<hr/>OBJECTIVE: Presented the experience in our institution in the conservative treatment of breast cancer, we analyzed the surgery, local recurrence, and the global super life, and the disease free survival. METHOD: These are a descriptive and retrospective study during 2004-2005 in patients underwent conservative treatment: Realize of partial oncology mastectomy with negative margins surgical evaluation of axillaries sentinel node technique or axillaries dissection. The adjuvant treatment with radiation therapy and chemotherapy when there are indicated in the patients. RESULTS: We realize 132 conservative surgery, median ages of patients were 51.49 histological type more frequent was ductal infiltrante carcinoma (70.06.%), (85.94 %) were stage I and II. The surgical more frequent were partial oncology mastectomy with axillaries dissection (80 patients, 61.87 %).10.6 % received adjuvant chemotherapy. The average of finding nodes was 11.37, the 62.6 % pathological axillaries state were 0.50.% expression positive estrogen and progestagen receptors 13.17 % super expression of Her2neu. 66.66 % received adjuvant chemotherapy, 79.84.% adjuvant radiation therapy 56.58 % adjuvant hormonal therapy. Average vigilance was 39.89 months. 88.88 % alive free disease, 2.22 % alive with local recurrence, and 5.92 % alive with metastases. 1.48 % alive with local and metastasis recurrence 1.07 % dead for disease. The free interval of disease was 25.8 month and the global super life to 5 years 98.24 %. CONCLUSIONS: The conservative treatment is a primary and effective in breast cancer therapy for stage I and II.
