Abstract The increased emphasis on the role of bacteria in the initiation and progression of periodontal disease has led to great interest in the use of antibiotics and antimicrobials in therapy. Systemic antibiotics have been used extensively in periodontics for various purposes including prevention of postsurgical infection, management of juvenile periodontitis and rapidly progressive forms of disease and therapy for patients who have been non-responsive to conventional therapy. Inherent to systemic administration of antibiotics are problems such as the risk of increasing the bacterial resistance to multiple antibiotics as well as encountering potentially unpleasant side effects and problems with patient compliance. Changing concepts relative to the hypersensitivity potential of locally administered antibiotics and the rationale of targeting localized areas of periodontal destruction have led to great interest in controlled release, local delivery systems for antimicrobial/antibiotic therapy of periodontitis. This report describes the use of minocycline in a subgingival local delivery system alone or as an adjunct to scaling and root planing in periodontal disease..
