The purpose of the present study was to determine the prevalence of oral leukoplakia in HIV/AIDS patients, attending the Infectious Disease Center Dra Elsa La Corte, Faculty of Dentistry, Central University of Venezuela and to establish the relation to tobacco use, CD4+ cell count and viral load. 75 individuals, age ranging 20-55 years, 61 male (81.3%) and 14 female (18.7%), with a positive diagnosis of HIV/AIDS infection were selected. A detailed clinical examination, incisional biopsy, histopathological diagnosis, CD4+ cell count using flow citometry and viral load by PCR, were performed. Our results showed the evidence of oral leukoplakia in a 29 % (22/75) subjects, being the most common site the buccal mucosa, 81.1% (18/22), and 40.9% (9/22) presented some degree of epithelial dysplasia. Interestingly, 54.54% (12/22) of the leukoplakia cases in HIV individuals, exhibited viral load values > 30.000 copies/mm3 independently from the CD4+cell count. It is noteworthy, that 59% of the evaluated patients (13/22). It is noteworthy, that 59% of the evaluated patients (13/22) were under antirretroviral therapy either biotherapy or HAART. Of these 30.7% (4/13) exhibited some degree of epithelial dysplasia, in contrast to 55,5% (5/9) of the nontreated patients that presented epithelial dysplasia. We may concluded from the present investigation, that a 29% of the evaluated cases showed oral leukoplakia observing a close relationship to tobacco use, viral load and antirretroviral therapy.
