Objective: The objective of the present paper is to report the results of the national experience in pediatric renal transplantation in Venezuela during the last 25 years. Methods: 268 patients with end-stage renal disease whom received 275 renal transplants during the period 1982-2005 were included. Treatment protocols, graft origins, rejection frequency, mortality causes and patient and graft survival were analyzed. Results: Mean age was 11.1 +/- 7.3 years. 70% of the grafted kidneys were obtained from cadaveric donors and 30% from alive related donors. The mortality causes were: Severe infections 34%, hemodynamic and cardiovascular complications 22%, metabolic complications 17%, hematological complications 12% and other causes 8%. The causesof graft lost were: chronic rejection 60%, thrombosis of renal vessels 20%, reccurence of the primary renal disease in the graft 13%, and acute tubular necrosis 7%. The actuarial survival of patients was: 98% at 6 months, 98% at 1 year, 90% at 3 years and 85% at 5 years. There was no significant difference between patients with live donor's transplants and patients with cadaveric kidneys (p> 0.05). The actuarial survival of grafts (lst transplants) was: 97% at 6 months, 90% at one year, 80% at 3 years and 70% at 5 years. Conclusion: These results confirm the validity of kidney transplantation programs for the rehabilitation of the uremic child in Venezuela, with satisfactory actuarial survival.
