The pro inflammatory cytokines are involved in the pathogenesis of acute pancreatitis. The OBJECTIVE of this study was to determine the profile of the mediators of inflammation of the patients with acute pancreatitis (PA) at the admission to the Emergency Room of the Hospital Universitario "Antonio Maria Pineda" in Barquisimeto, Lara State, Venezuela during the period February 2002 and January 2004. MATERIALS AND METHODS: serum levels of the tumor necrosis factor (TNF), interleukin (IL-1), interleukin 6 (IL-6) and interleukin 8 (IL8), were determined in patients with mild and severe acute pancreatitis within 72 hours of the onset of symptoms and within the 48 hours following their admission. The cytokines were determined using an experimental test of immunosorbent assay (ELISA), (Quantikine; R&D Systems, Minneapolis, MN). The diagnosis of severity was done before the presence of local or systemic complications. RESULTS: A total of 97 patients with acute pancreatitis were selected for the study, 67 women, 30 men, 20.6% of the patients were between the ages15-25; 22.6 % between 26-36; 20.6% between 37-47 and 19,6% between 48-58. The most frequent etiology was choledocholithiasis. 52,57% of the cases were mild and 47,42% severe. The general average of TNF was 56,48 pg with SD of 38,13; of IL-1 it was of 17.36 with SD of 14,51; IL-6 average of 68.36 with SD of 85,82; IL-8 average of 103,03 with SD of 242,08. The serum levels of TNF, IL-6, IL-8, were significantly higher in patients with severe acute pancreatitis, not therefore the IL-1. The IL-6 levels were elevated in 64,9% of patients with pancreatitis. Making it the best prognosis indicator of all.
