Abstract

A 17 years old female patient consulted referring a progressive increase of size in her superior hemiabdomen accompanied with abdominal pain since September 2005. Abdominal Ultrasound (11-01-2006) reported in the left hepatic lobe, a lobulated tumor of a mixed echogenicity pattern of 9.4 x 7.3 cm with displacement of organs from the medial line, with a positive Doppler signal and free fluid in cavity. The patient had normal alphaphetoprotein levels and negative markers for viral hepatitis. On the 25/01/2006 she presented hemodynamic instability and accentuation of the abdominal pain and a decision to take her to the operating room was taken, performing a left hepatectomy (left lobectomy) with the following findings: 1500 milliliters of hemoperitoneum, a hepatic tumor that included 90 % of the left lobe of 30x25x25 cm. of diameter, multilobulated, very vascular. It was concluded as a stage I Fibrolamellar Hepatocarcinoma. After 4 months the patient is symptom free, without evidence of disease. We discuss clinical characteristics, treatment and prognosis of Fibrolamellar Hepatocarcinoma.
