The Actinomyses is a disease that rarely occurs at the abdominal level. Characterized by the formation of multiple abscesses, with abundant production of grains and fibrotic tissue. We describe the case of 70 year male who had fever 39 c, epigastric pain and back pain. In laboratory tests evidenced leukocytosis 15000, ending abdominal ultrasound cyst complex in the pancreatic head and abdominal CT in the space-occupying lesion in solid lesser sac ending retroperitoneal tumor. He underwent endoscopic ultrasound finding injury approximately 8 to 10 cm, cystic type abscessed abscessed mesenchymal tumor less likely. Guided aspiration was performed by echo endoscopy obtaining abundant cellularity with dense inflammatory exudate of neutrophils, histiocytes, lymphocytes and plasma cells. Also added filamentous bacterial cells, basophils to hematoxylin-eosin, suggesting the presence of Actynomises. Colonoscopy was performed in the transverse colon found foreign body (fish bone), coinciding with the location of the abscess considering the possibility of cause and effect, is removed and placed ampicillin-sulbactam for 14 days, non-steroidal anti-inflammatory, with the resulting resolution abscess. Currently the patient is in good condition after 8 months of being treated.
