Abstract

Introduction and objective: Lithiasis is the main cause of obstruction, biliary tree dilated and acute biliary pancreatitis. There are different criteria to approach patients with this pathology but the clinical presentation, labs and imaging modalities help to establish the cause of the obstruction or acute pancreatitis. We peformed a comparative study of two diagnostic modalities (echo-endoscopy(EUS) and cholangioresonance(c-RMN)) in acute biliary-pancreatic pathologies. Method: Observational, comparative and retrospective study; including 14 patients (5 men and 9 women) to whom EUS and c-RMN was performed to rule out biliary lithiasis, our gold standard was surgery, retrogradecholangiografy (ERCP) and follow-up. Results: 14 patients, Mean age 54 y.o. The diagnoses were biliary obstruction (78.57 %) and acute biliary pancreatitis (21.43 %). As diagnosis test, endoscopic ultrasound had a sensitivity, specificity; positive and negative predictive values of 100 %, in each case, likelihood ratios positive and negative 1.01 and 0.99, respectively. Cholangioresonance had a sensitivity 45 %, specificity 66,6 %; positive predictive values 83,3 %, negative predictive values 25 %, likelihood ratios positive and negative 1.36 and 0.82, respectively. Conclusions: The present study confirmed USE as an accurate and adequate procedure for the diagnosis of common gallstones.
