Biliary ascariasis is a complication of intestinal geohelminthiasis more frequent worldwide, this complication is caused by the erratic migration of the parasite into the biliary system. Occasionally, migration to the pancreatic duct also presents with acute pancreatitis. Endoscopic treatment is sometimes indicated to achieve complete removal of the parasite from the biliary or pancreatic route, being successful in 55% to 89% of the cases. We report the case of a female patient 18 years old with symptoms of abdominal pain in the right hypochondrium and fever without any other symptom or sign at physical examination. The abdominal ultrasound reports the presence of worm in situ in the gallbladder and common bile duct, the conservative medical management is proposed in the first 48 hours of hospitalization, with persistence of the symptoms, thus deciding the endoscopic management and satisfactory evolution, the patient reentresa complicated with acute pancreatitis by the same etiology at 15 days, with ineffective pharmacological treatment and with endoscopic treatment through satisfactory endoscopic retrograde cholangiopancreatography.
