Although the incidence of gastric cancer has decreased in recent decades, a stable or slightly increasing trend has been observed among young adults. We report the case of a 24-year-old male patient referred to our center due to functional dyspepsia and weight loss, which did not improve with symptomatic treatment. An abdominal ultrasound revealed a thickened gastric wall, prompting gastroscopy. The procedure identified Bormann type IV gastric cancer, and the histopathological analysis confirmed a diffuse, poorly differentiated adenocarcinoma with signet ring cells. Consequently, a CT scan of the chest, abdomen, and pelvis was performed, showing findings suggestive of mesenteric metastasis. During the patient’s clinical course, persistent and increasingly severe abdominal pain led to celiac plexus neurolysis, resulting in pain relief. However, despite initiating chemotherapy, the patient ultimately passed away. Gastric cancer in young patients is a rare condition, often diagnosed at advanced stages. Therefore, performing gastroscopy in patients with dyspeptic symptoms unresponsive to conventional treatment can facilitate early diagnosis, enabling curative treatment and potentially improving survival outcomes.
